        FLAVONE INHIBITS VASCULAR CONTRACTION BY DECREASING PHOSPHORYLATION OF THE MYOSIN PHOSPHATASE TARGET SUBUNIT

        <P>SUMMARY</P><OL TYPE='1'><li level='1'>Flavonoids modulate vascular tone through an endothelium-dependent or -independent mechanism. Although a few mechanisms for endothelium-independent relaxation have been suggested, such as interference with protein kinase C or cAMP or cGMP phosphodiesterase, the inhibition of Ca<SUP>2+</SUP> release from intracellular stores or Ca<SUP>2+</SUP> influx from extracellular fluids, the mode of action of flavonoids remains elusive.</LI><li level='1'>We hypothesized that treatment with flavone inhibits vascular smooth muscle contraction by decreasing the phosphorylation of the myosin phosphatase target subunit (MYPT1).</LI><li level='1'>Rat aortic rings were denuded of endothelium, mounted in organ baths and contracted with U46619, a thromboxane A<SUB>2</SUB> analogue.</LI><li level='1'>Flavone dose-dependently inhibited the U46619-induced contractile response and myosin light chain (MLC<SUB>20</SUB>) phosphorylation. At 10<SUP>−7</SUP> mol/L, U46619 induced vascular contraction with the concomitant phosphorylation of MYPT1 at Thr855, but not at Thr697. Incubation with flavone (100 or 300 µmol/L) for 30 min attenuated the phosphorylation of MYPT1<SUB>Thr855</SUB>, but not MYPT1<SUB>Thr697</SUB>.</LI><li level='1'>It is concluded that treatment with flavone inhibits vascular smooth muscle contraction by decreasing the phosphorylation of the MYPT1. These results suggest that flavone causes endothelium-independent relaxation through, at least in part, the inhibition of p160 Rho-associated coiled-coil-containing protein kinase (ROCK) signalling.</LI></OL>

      • Pitfall of anti-adhesion material in clinical follow up of patients after surgical treatment

        목적: Anti-adhesion adjuvant materials were usually applied during operations to reduce risks of postoperative adhesion around operation-fields. 방법: In the current cases, we used (MacroPore Biosurgery, Inc, San Diego, California, USA) as anti-adhesion material, which was grossly or microscopically identical to foreign body in explorative mass excision 결과: On base of clinical follow up with standard interval`s imaging study, 6 cases among these 102 pateints had positive lesion-mimicking of metastasis or local recurrence, which should be ruled out cancer lesion by histologic confirmation. Tumor markers were within normal range in all six patients. Laparoscopic exploration and biopsy was performed in all six patients. Histologic confirmation showed recurrence in one case and foreign body reaction without malignancy in the remained, five cases (5/102, 4.9%) . 결론: To avoid any confusion during clinical follow up of patients with gynecologic malignancies, we suggest avoiding application of the anti- adhesion adjuvant materials which can have a chance of postoperative foreign body reaction.

        Flavone Attenuates Vascular Contractions by Inhibiting RhoA/Rho Kinase Pathway

        Our previous study demonstrated that flavone inhibits vascular contractions by decreasing the phosphorylation levelof the myosin phosphatase target subunit (MYPT1). In the present study, we hypothesized that flavone attenuates vascular contractions through the inhibition of the RhoA/Rho kinase pathway. Rat aortic rings were denuded of endothelium, mounted in organ baths, and contracted with either 30 nM U46619 (a thromboxane A2 analogue) or 8.0 mM NaF 30 min after pretreatment with either flavone (100 or 300ՌM) or vehicle. We determined the phosphorylation level of the myosin light chain (MLC<sub>20</sub>), the myosin phophatase targeting subunit 1 (MYPT1) and the protein kinase C-potentiated inhibitory protein for heterotrimeric myosin light chain phophatase of 17-kDa (CPI17) by means of Western blot analysis. Flavone inhibited, not only vascular contractions induced by these contractors, but also the levels of MLC<sub>20</sub> phosphorylation. Furthermore, flavone inhibited the activation of RhoA which had been induced by either U46619 or NaF. Incubation with flavone attenuated U46619- or NaF-induced phosphorylation of MYPT1<sup>Thr855</sup> and CPI17<sup>Thr38</sup>, the downstream effectors of Rho-kinase. In regards to the Ca<sup>2+</sup>-free solution, flavone inhibited the phosphorylation of MYPT1<sup>Thr855</sup> and CPI17<sup>Thr38</sup>, as well as vascular contractions induced by U46619. These results indicate that flavone attenuates vascular contractions, at least in part, through the inhibition of the RhoA/Rho-kinase pathway.

      • Laparoscopic adenomyomectomy under transient occlusion of uterine arteries with an endoscopic vascular clip: Single surgeon`s experience

        목적: To introduce the safe and effective surgical technique of laparoscopic adenomyomectomy with transient occlusion of uterine arteries (TOUA) in patients with symptomatic uterine adenomyoma. 방법: In a prospective case study, we examined all cases of laparoscopic adenomyomectomy with TOUA performed by a single surgeon at Ulsan University Hospital, Ulsan, Korea, between May 2011 and February 2014. Surgical outcomes included operative time, intraoperative injury of blood vessels, nerves, and pelvic organs, as well as intraoperative blood loss. We assessed the degree of improvement in dysmenorrhea and menorrhagia and the recurrence of adenomyomic lesions by ultrasonography at the 6-month follow-up after laparoscopic adenomyomectomy with TOUA. 결과: Seventy-one women who were refractory to medical treatment or who wanted surgical treatment for preserving their uterus underwent laparoscopic adenomyomectomy with TOUA using an endoscopic vascular clip. The mean age was 42.9 ± 5.35 years (mean age ± standard deviation). The mean diameter of the adenomyomas was 3.2 ± 2.16 cm. The mean operation time, and hospital stay were 84.09 ± 31.48 minutes and 5.42 ± 1.54 days, respectively. The mean estimated blood loss was 184.35 ± 134.25 mL, and no injury to the uterine arteries or pelvic nerves occurred. No cases of conversion to a laparotomy or major complications occurred. At the 6-month follow-up, complete remission of dysmenorrhea and menorrhagia occurred in 94.5% and 86.5% of patients, respectively. The mean follow-up period is 18.7 months. During this period, recurrence rate is 5 cases (7%) and one of these 5 recurred cases was undertaken the operation (laparoscopic hysterectomy). 결론: Laparoscopic adenomyomectomy with TOUA could be a safe and effective surgical method for women with symptomatic uterine adenomyoma who want to preserve their fertility.

        Monochorionic twin delivery after conservative surgical treatment of a patient with severe diffuse uterine adenomyosis without uterine rupture

        A 31-year-old nulliparous woman with severe diffuse uterine adenomyosis, which replaced nearly the whole uterine myometrium, visited our hospital due to severe dysmenorrhea, menorrhagia, and a desire to have a baby. The patient had a history of two spontaneous abortions. Laparotomic adenomyomectomy with transient occlusion of uterine arteries (TOUA) was performed safely and the patient tried in vitro fertilization and achieved a intrauterine twin pregnancy after recovery time of the operation. At 31+6 weeks of gestation, a male neonate baby weighing 1,620 g and a male neonate baby weighing 1,480 g were born by transverse lower segment cesarean delivery. There was no complication after the operation. The babies were discharged after receiving routine neonatal intensive care for neonatal respiratory distress syndrome. Adenomyomectomy with TOUA technique would be an option for conservative surgical treatment in patients with severe diffuse whole uterine adenomyosis. This is the first report of twin pregnancy after diffuse whole uterine adenomyomectomy with TOUA.

      • Biaxial Chain Growth of Polyolefin and Polystyrene from 1,6-Hexanediylzinc Species for Triblock Copolymers

        <P>Synthesis of polyolefin (PO)-based block copolymers is of immense research interest. In this work, we report a strategy for the construction of polystyrene (PS)-<I>block</I>-PO-<I>block</I>-PS, a useful thermoplastic elastomer, directly from olefin and styrene monomers. Multinuclear zinc species Et[Zn(CH<SUB>2</SUB>)<SUB>6</SUB>]<SUB><I>a</I></SUB>ZnEt were prepared through successive additions of BH<SUB>3</SUB> and Et<SUB>2</SUB>Zn to 1,5-hexadiene. Poly(ethylene-<I>co</I>-propylene) chains were biaxially grown from the −(CH<SUB>2</SUB>)<SUB>6</SUB>- units in Et[Zn(CH<SUB>2</SUB>)<SUB>6</SUB>]<SUB><I>a</I></SUB>ZnEt via “coordinative chain transfer polymerization (CCTP)” using the pyridylaminohafnium catalyst. PS chains were subsequently grown in one pot from the generated polymeryl-Zn sites by subsequent introduction of the anionic initiator Me<SUB>3</SUB>SiCH<SUB>2</SUB>Li·(pmdeta) (pmdeta, pentamethyldiethylenetriamine) and styrene monomers. The fraction of the extracted PS homopolymer grown from the Me<SUB>3</SUB>SiCH<SUB>2</SUB> sites was low (homo-PS (g)/total PS (g), 15-22%). The gel permeation chromatography (GPC) curves shifted evidently after styrene polymerization, and change in the molecular weight (Δ<I>M</I><SUB>n</SUB>, 39-56 kDa) was approximately twice the homo-PS <I>M</I><SUB>n</SUB> (20-23 kDa), in accordance with attachment of the PS chains at both ends of the PO chains. Transmission electron microscopy analysis of the thin films showed segregation of the PS domains in the PO matrix to form spherical or wormlike rippled structures depending on the PS content. The prepared triblock copolymers exhibited elastomeric properties in the cyclic tensile test, similar to the commercial PS-<I>block</I>-poly(ethylene-<I>co</I>-1-butene)-<I>block</I>-PS.</P> [FIG OMISSION]</BR>
