      • GG-21 : Pitfall of anti-adhesion material in clinical follow up of patients after surgical treatment

        목적: Anti-adhesion adjuvant materials were usually applied during operations to reduce risks of postoperative adhesion around operation-fields. 방법: In the current cases, we used (MacroPore Biosurgery, Inc, San Diego, California, USA) as anti-adhesion material, which was grossly or microscopically identical to foreign body in explorative mass excision 결과: On base of clinical follow up with standard interval`s imaging study, 6 cases among these 102 pateints had positive lesion-mimicking of metastasis or local recurrence, which should be ruled out cancer lesion by histologic confirmation. Tumor markers were within normal range in all six patients. Laparoscopic exploration and biopsy was performed in all six patients. Histologic confirmation showed recurrence in one case and foreign body reaction without malignancy in the remained, five cases (5/102, 4.9%) . 결론: To avoid any confusion during clinical follow up of patients with gynecologic malignancies, we suggest avoiding application of the anti- adhesion adjuvant materials which can have a chance of postoperative foreign body reaction.

      • Transfer of ultrathin molybdenum disulfide and transparent nanomesh electrode onto silicon for efficient heterojunction solar cells

        <P><B>Abstract</B></P> <P>Two-dimensional transition-metal dichalcogenides (TMDCs) are very promising for photovoltaic (PV) applications due to their excellent light absorption properties and appropriate bandgap energy, Although multifunctional applications of TMDCs in photovoltaic devices have been achieved, the photovoltaic conversion efficiency under 1 sun is still very low with small active area because of their inexpedient high sheet resistance and limitation of synthesis techniques. In this study, we demonstrate uniform synthesis of 4-in. wafer-scale MoS<SUB>2</SUB> thin films by thermal decomposition of solution precursors. The solar cells are fabricated by transferring n-MoS<SUB>2</SUB> thin films on p-Si substrates to form p-n heterojunctions and then transferring Au nanomeshes prepared in a novel surface treatment as transparent top electrodes onto MoS<SUB>2</SUB>. The circular n-MoS<SUB>2</SUB>/p-Si heterojunction solar cell exhibited a power conversion efficiency of 5.96% at a diameter of 0.3 in. and proved to be easily scalable to 1-in. diameter with 5.18% efficiency. To the best of our knowledge, the solar cells of this study are the most efficient and the largest in all types of solar cells based on TMDC reported so far. Finally, based on finite difference time-domain simulation, we proposed a strategy for implementing n-MoS<SUB>2</SUB>/p-Si heterojunction solar cell with efficiency higher than 15% by introducing optimal doping control of n-MoS<SUB>2</SUB> and efficient anti-reflection layers.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The large scale, uniform (4-in.)         음성문제 자각정도 평가지 개발

        The purpose of this study was to investigate the correlation between VHI (voice handicap index) and K-SAIVP (Korean Self-Awareness Index of Voice Problem). The K-SAIVP questionnaire includes 14 items to check self-perceived voice problems. These items were developed based on modifications of the VHI-30 items suggested by speakers with voice problems. One-hundred-and-eleven healthy participants (22 male and 89 female speakers) without voice problems by self reports and clinicians’ observation were enrolled in this survey. Fifty-four professional voice users and 57 non-professional voice users were classified among all participants. Pearson’s correlation was used for the analysis of the relation between three different areas of VHI and K -SAIVP in male, female, and all participants.The results were as follows. First, in K-SAIVP, there was a significant correlation within all sub-items of K-SAIVP. K-SAIVP and 〈SS: self-perceived severity〉 (1 item) and 〈QOL: quality of life by voice〉 (one item) (r=0.528, r=0.530, p<.01) in the female group. In the male group, K-SAIVP showed a strong correlation with 〈SS〉, 〈QOL〉, and 〈SOV: self satisfaction of voice〉 in K-SAIVP questionnaire, ranging from r=0.708 to 0.920. And 14 sub-items and total scores of K-SAIVP showed a significant correlation (r=0.237 to r=0.816, p<.01). Second, the K-SAIVP and VHI showed a significant correlation, ranging from r=0.691to 0.869 (p<.01). The relation between 14 sub-items of K-SAIVP and total scores in each sub-area and overall scores of VHI were significantly correlated (r=270 to r=779, p<.01). This preliminary study of developing a questionnaire for self-awareness index of voice problems in K-SAIVP showed it can be a meaningful tool in clinical settings. However, it needs to be studied more in respect of validity and reliability.

        발화 과제 유형에 따른 청지각적 평가와 기기적 평가의 상관성 연구

        The purpose of this study was to know the relationship between auditory-perceptual ratings and acoustic and aerodynamic evaluations according to sustained vowels and continuous speech. Fourty-one speakers' sustained vowel and reading samples with voice problems(palsy, polyp, MTD, nodules) were used for auditory perceptual judgement with 10-point scale. Five acoustic measures and two aerodynamic measures were extracted from sustained vowels. The results showed overall the significant correlation between continuous speech and sustained vowels(/a/, /i/, /u/) and GRBAS with r=.453~r=.758( p<0.01). The correlation between G, R, B in GRBAS and continuous speech was higher than sustained vowels(r=.758, r=.472, r=702, p<0.01). In the relation with acoustic evaluation, continuous speech had a weak correlation with jitter value only(r=.540, p<0.01). But three sustained vowels were mildly correlated with Fo, jitter, shimmer, NHR (r=-.432∼.697( p<0.01)) except for SFF. In correlation with aerodynamic evaluation, continuous speech and three sustained, both showed significant correlation from r=.352( p<0.05) to r=.627( p<0.01) except for perceptual judgement of sustained vowel for /u/ and subglottic pressure value.

        Monochorionic twin delivery after conservative surgical treatment of a patient with severe diffuse uterine adenomyosis without uterine rupture

        A 31-year-old nulliparous woman with severe diffuse uterine adenomyosis, which replaced nearly the whole uterine myometrium, visited our hospital due to severe dysmenorrhea, menorrhagia, and a desire to have a baby. The patient had a history of two spontaneous abortions. Laparotomic adenomyomectomy with transient occlusion of uterine arteries (TOUA) was performed safely and the patient tried in vitro fertilization and achieved a intrauterine twin pregnancy after recovery time of the operation. At 31+6 weeks of gestation, a male neonate baby weighing 1,620 g and a male neonate baby weighing 1,480 g were born by transverse lower segment cesarean delivery. There was no complication after the operation. The babies were discharged after receiving routine neonatal intensive care for neonatal respiratory distress syndrome. Adenomyomectomy with TOUA technique would be an option for conservative surgical treatment in patients with severe diffuse whole uterine adenomyosis. This is the first report of twin pregnancy after diffuse whole uterine adenomyomectomy with TOUA.

      • GG-08 : Laparoscopic adenomyomectomy under transient occlusion of uterine arteries with an endoscopic vascular clip: Single surgeon`s experience

        목적: To introduce the safe and effective surgical technique of laparoscopic adenomyomectomy with transient occlusion of uterine arteries (TOUA) in patients with symptomatic uterine adenomyoma. 방법: In a prospective case study, we examined all cases of laparoscopic adenomyomectomy with TOUA performed by a single surgeon at Ulsan University Hospital, Ulsan, Korea, between May 2011 and February 2014. Surgical outcomes included operative time, intraoperative injury of blood vessels, nerves, and pelvic organs, as well as intraoperative blood loss. We assessed the degree of improvement in dysmenorrhea and menorrhagia and the recurrence of adenomyomic lesions by ultrasonography at the 6-month follow-up after laparoscopic adenomyomectomy with TOUA. 결과: Seventy-one women who were refractory to medical treatment or who wanted surgical treatment for preserving their uterus underwent laparoscopic adenomyomectomy with TOUA using an endoscopic vascular clip. The mean age was 42.9 ± 5.35 years (mean age ± standard deviation). The mean diameter of the adenomyomas was 3.2 ± 2.16 cm. The mean operation time, and hospital stay were 84.09 ± 31.48 minutes and 5.42 ± 1.54 days, respectively. The mean estimated blood loss was 184.35 ± 134.25 mL, and no injury to the uterine arteries or pelvic nerves occurred. No cases of conversion to a laparotomy or major complications occurred. At the 6-month follow-up, complete remission of dysmenorrhea and menorrhagia occurred in 94.5% and 86.5% of patients, respectively. The mean follow-up period is 18.7 months. During this period, recurrence rate is 5 cases (7%) and one of these 5 recurred cases was undertaken the operation (laparoscopic hysterectomy). 결론: Laparoscopic adenomyomectomy with TOUA could be a safe and effective surgical method for women with symptomatic uterine adenomyoma who want to preserve their fertility.

        An Experimental Study of Supersonic Dual Coaxial Free Jet

        A supersonic dual coaxial jet has been employed popularly for various industrial purposes, such as gasdynamic laser, supersonic ejector. noise control and enhancement of mixing. Detailed characteristics of supersonic dual coaxial jets issuing from an inner supersonic nozzle and outer sonic nozzles with various ejection angles are experimentally investigated. Three important parameters, such as pressure ratios of the inner and outer nozzles, and outer nozzle ejection angle, are chosen for a better understanding of jet structures in the present study. The results obtained from the present experimental study show that the Mach disk diameter becomes smaller, and the Mach disk moves toward the nozzle exit, and the length of the first shock cell decreases with the pressure ratio of the outer nozzle. It was also found that the highly underexpanded outer jet produces a new oblique shock wave, which makes jet structure much more complicated. On the other hand the outer jet ejection angle affects the structure of the inner jet structure less than the pressure ratio of the outer nozzle, relatively.

        Study of Moist Air Flow Through the Ludwieg Tube

        The time-dependent behavior of unsteady condensation of moist air through the Ludwieg tube is investigated by using a computational fluid dynamics (CFD) work. The two­dimensional, compressible, N avier-Stokes equations, fully coupled with the condensate droplet growth equations, are numerically solved by a third-order MUSCL type TVD finite-difference scheme, with a second-order fractional time step. Baldwin- Lomax turbulence model is employed to close the governing equations. The predicted results are compared with the previous experiments using the Ludwieg tube with a diaphragm downstream. The present computations represent the experimental flows well. The time-dependent unsteady condensation charac­teristics are discussed based upon the present predicted results. The results obtained clearly show that for an initial relative humidity below 30% there is no periodic oscillation of the condensation shock wave, but for an initial relative humidity over 40% the periodic excursions of the condensation shock occurs in the Ludwieg tube, and the frequency increases with the initial relative humidity. It is also found that total pressure loss due to unsteady condensation in the Ludwieg tube should not be ignored even for a very low initial relative humidity and it results from the periodic excursions of the condensation shock wave.

        A sciatic nerve injury as a complication of the position appears to be rare. There are several reports on sciatic nerve injuries that occurred as complications of the lithotomy position. We present two cases of an opposite sciatic nerve injury after the fixation of a femur shaft fracture. Two patients complained of hyperesthesia of the right foot and a foot drop after surgery. Nerve conduction and electromyographic studies were performed, which revealed a sciatic nerve injury. The patients were treated with physiotherapy and an ankle-foot orthotic. In the case 1, patient had completely recovered ten months after surgery. However, in case 2, the patient still had residual hyperesthesia and weakness two months after surgery when he was lost to follow-up. The mechanism of nerve injury in these patients was a stretching of the nerve. Tension was placed on the nerve as a result of the external rotation of the thigh with the hip and knee joints flexed.         Electro-optical properties of vertically aligned polymer network liquid crystals for normally transparent light shutters

        <P>This study examined the electro-optical properties of black-background, normally transparent polymer network liquid crystal cells suitable for light shutters applicable to transparent flexible displays. The cells were fabricated using a polymer induced phase separation method from the mixture of negative liquid crystal, reactive-mesogen and black dye. A parametric study for key material and process parameters was carried out to determine the parameter dependence of the E-O properties and to achieve high E-O performance. The E-O properties were strongly dependent on RM concentration and UV exposure intensity, not on UV exposure energy. The optimal conditions for high E-O performance are described.</P>
