        요추부 후방 골유합술시 제4요추 분절동맥의 손상에 의해 발생된 후복막하 혈종의 코일 색전술 치험 1례

        In spine sugery, iatrogenic vascular injury is unusual, and most common vascular injury is said to be a tear of the left common iliac artery. We report a case of retroperitoneal hematoma from lumbar segmental artery injury after posterior lumbar fusion and pedicle screw fixation. This 60-year-old woman presented with 6 months history of low back pain and radiating pain to the left lower leg. She complained of claudication after 10 meters walking due to spondylolisthesis and spinal stenosis at L2-L5 level. She underwent multiple total laminectomy, posterior lumbar inter-body fusion, and transpedicular screw at L2-L5. During operation there was not evidence of complications such as bleeding. She complained of abdominal pain on post-operation I day and showed tenderness on left upper quadrant. Hemoglobin was 8.4 g/dL on postoperative laboratory examination. Abdominal CT scan revealed retroperitoneal hematoma. We performed angiography for evaluation of hematoma and found extravasation of dye at segmental artery of L4 level. We embolized the bleeding artery with coil successfully. Her postoperative abdominal pain and tenderness were relieved after then and discharged without pain or neurological deficit.

        임상 ; 특발성 혈소판 감소성 자반증으로 진단된 산모의 임신 및 분만에 관한 임상양상

        목적: 특발성 혈소판 감소성 자반증은 출혈 경향으로 인해 분만이라는 특수한 상황을 겪게되는 산모나 신생아에게 출혈 위험의 가능성이 있으므로 이에 대한 진료지침이 필요하다. 방법: 특발성 혈소판 감소성 자반증 산모 19명에게 분만된 22예의 증례를 의무 기록을 열람하여 후향적으로 분석하였다. 특발성 혈소판 감소성 자반증 산모의 출혈 증상의 유무, 임신 전과 임신 중, 분만 시의 평균 혈소판 수, 치료 방법, 분만 방법, 분만 관련 합병증의 유무를 조사하였으며, 신생아의 혈소판 수와 출혈 합병증 발생 유무를 조사하였다. 결과: 산모는 임신 기간중 부신피질 호르몬 치료, 면역글로불린 정주, 혈소판 수혈 등의 치료를 받았으며 특히 분만에 임박해서는 혈소판 수혈을 통해 혈소판 수치를 50,000/μL 이상으로 유지하려는 경향을 보였다. 분만 방법은 질식 분만이 10예(45%), 제왕 절개가 12예(55%)에서 시행되었다. 분만과 관련된 합병증은 모든 산모에서 발생하지 않았다. 특발성 혈소판 감소성 자반증 산모에서 출생한 17명의 신생아 혈소판 수치의 중앙값은 220,000/μL (범위, 59,000~315,000/μL)이었다. 분만 직후 신생아에서 150,000/μL 이하의 혈소판 감소증을 보인 경우는 4예(23%)에서 관찰되었으나 심각한 출혈 합병증은 한 건도 발생하지 않았다. 결론: 특발성 혈소판 감소성 자반증 환자의 임신과 분만은 출혈 부작용 없이 모두 안전하게 이루어졌으며, 신생아에서도 출혈 부작용은 없었던 점으로 미루어 보아 특발성 혈소판 감소성 자반증 환자에서의 임신과 출산은 산모와 신생아 모두에게 대부분 안전하게 시술될 수 있음을 알 수 있었다. Objective: Idiopathic thrombocytopenic purpura (ITP) frequently occurs in young women. Consequently, clinicians often give medical treatment to pregnant women who are diagnosed with ITP. This study might help to make a clinical guidelines for obstetrical ITP patients and their infants. Methods: We retrospectively investigated the medical charts of 19 cases of deliveries and 22 neonates which from mothers with the diagnosis of ITP during pregnancy from March 1998 to March 2007. Result: Corticosteroid treatment was administrated in 13 cases, high-dose immunoglobulin therapy in 3 cases, and concentrated platelet transfusion in 17 cases during their antenatal care. Ten (45%) vaginal deliveries and twelve (55%) cesarean sections were performed. There were no obstetrical complications associated with their ITP and only four infants with platelet counts below the 150,000/μL were reported after birth. However there were not any signs and symptoms of neonatal complications resulting from their maternal ITP. Conclusion: Our results demonstrate that mothers with ITP can successfully deliver healthy infants in most case. Although maternal and fetal bleeding may occur, such a fatal complication is uncommon.

        Physiological Status Assessment of Locomotive Engineer During Train Operation

        In this study, physiological status of locomotive engineers were measured through EEG, ECG, EDA, PPG and respiration signals from 6 subjects to evaluate their arousal status during train operating. Existence of tunnels and mechanical vibration of train using 3-axes acceleration sensors were recorded simultaneously and were correlated with operator's physiological status. As the result of the analyzed subjects' physiological signals, mean SCR was increased in the section where more body movement is required. The RR interval was decreased before and after train stop due to the higher level of mental tension. The intensity of beta wave of EEG was found to be higher before and after train stop and tunnel section due to the increased mental arousal and tension. Therefore, it is expected that the outcomes of the physiological signals explored in this study can be utilized as the quantitative assessment methods for the arousal status to be used for sleepiness prevention system for vehicles operators which can greatly contribute to public transportation system safety.

        Nucleotide Sequence Analysis of the DNA Fragment Which Is Able to Revert Adel Mutation in Saccharomyces cerevisiae

        ◎DE1 gene encodes a phosphoribosylamino imidazole succinocarboxylamide synthe◎ which converts phosphoribosylamino imidazole carboxylate to phosphoribosyl suc◎ocarboxamide aminoimidazole in de novo pathway of adenine biosynthesis. We cloned ◎DE1 gene by the complementation of ade1 mutation in yeast and localized ADE1 gene in cloned plasmid by constructing the several subcloned derivatives. There are two separate DNA fragments which are related to complement the ade1 mutation, the large DNA fragment designated as ADE_L1 and the smaller DNA fragment designated as ADE_S1. The restriction map of ADE_L1 gene exactly coincides with the restriction map of ADE1 gene analyzed by several group. However, the smaller DNA fragment seems to have a novel function involved in complementing ade1 mutation without ADE_L1 DNA fragment, but the recombinant plasmid carrying the ADE_S1 gene shows weaker transformability than that of ADE_L1 gene. To identify the role of the smaller DNA fragment in adenine biosynthesis, we determined its nucleotide sequence by the methods of dideoxy DNA sequencing with PUC plasmids derivatives.

        Primary biliary cirrhosis(PRC) is a chronic progressive disease, more often affecting women, characterised by progressive destruction of the small intrahepatic bile ducts with portal inflammation leading to fibrosis and cirrhosis. There is a close association between PBC and antibodies to antimitochondrial antibodies(AMA). The pathogenesis of PBC remains uncertain. The disease has been considered as an example of autoimmunity. Estimates of disease prevalence vary between 20 and 240 cases per milion per year. PBC is a extremely rare liver disease in Asia. There is no definitive drug treatment, liver transplantation is indicated for patients with intractable symptoms and for end-stage disease. The authors have experienced a case of PBC in 66 year-old female who suffered from lethargy, pruritus and dry mouth since 2 years ago. We report a case of PBC with a review of literatures.

      • 고주파 전기 수술기 개발

        오늘날 환자의 수술은 간편하고, 통증이 없이 이루어질 수 있는 방향으로 의약과 의료기가 개발되고 있다. 대표적인 의료기기 중 하나가 전기 수술기라 할 수 있다. 이러한 수술기는 기존의 아날로그 전기 회로를 사용하여 시스템의 부피가 크고 무거운 단점이 있다. 본 연구에서는 대규모 집적도가 가능한 디지털 형 고주파 전기 수술기를 개발하였다. 개발된 고주파 전기 수술기는 부피가 작고 무게가 가벼운 특징을 갖고 있다. 개발된 수술기의 실험결과에서는 5가지의 다른 디지털 파형을 발생시킬 수 있음을 보여주며, 수술의 종류에 따라 특정 고주파 전기 파형이 사용된다. Recently many kinds of medical drugs and instruments have been developed to perform the surgical operation without pain. One of the instruments would be high frequency electro-surgical instrument. Conventional electro-surgical instruments has been producted with analog-typed circuits, and thus they are heavy and bulky. In this paper we shows a high frequency electro-surgical instrument based on digital electronic circuits. The developed instrument has small bulky and light weight. And the experimental results show that the new instrument can generate five kinds of electrical wave. According to the kind of surgical operations, one of the high frequency electrical waves are applied.

        Effects of Repeated Citalopram Treatments on Chronic Mild Stress-Induced Growth Associated Protein-43 mRNA Expression in Rat Hippocampus

        Although growth associated protein-43 (GAP-43) is known to playa significant role in the regulation of axonal growth and the formation of new neuronal connections in the hippocampus, there is only a few studies on the effects of acute stress on GAP-43 mRNA expression in the hippocampus. Moreover, the effects of repeated citalopram treatment on chronic mild stress (CMS)-induced changes in GAP-43 mRNA expression in the hippocampus have not been explored before. To explore this question, male rats were exposed to acute immobilization stress or CMS. Also, citalopram was given prior to stress everyday during CMS procedures. Acute immobilization stress significantly increased GAP-43 mRNA expression in all subfields of the hippocampus, while CMS significantly decreased GAP-43 mRNA expression in the dentate granule cell layer (GCL). Repeated citalopram treatment decreased GAP-43 mRNA expression in the GCL compared with unstressed controls, but this decrease was not further potentiated by CMS exposure. Similar decreases in GAP-43 mRNA expression were observed in CA1, CA3 and CA4 areas of the hippocampus only after repeated citalopram treatment in CMS-exposed rats. This result indicates that GAP-43 mRNA expression in the hippocampus may differently respond to acute and chronic stress, and that repeated citalopram treatment does not change CMS-induced decreases in GAP-43 mRNA expression in the GCL.
