A 21-year-old male was presented with sudden headache, fever, petechiae and neck stiffeness. The diagnosis of meningococcal meningitis was confirmed by examination of cerebrospinal fluid. The clinical symptoms of the illness were improved after treatment of antibiotics. However the paient developed generalized edema, oliguria, azoternia, and heavy proteinuria in the recovery phas of illness. Low serum C3 level was also noted. A kidney biopsy was perfromed and showed the features of postinfectious glomerulonephritis and typical subepithelial humps on electron-microscopic examination. His sysmptoms and laboratory findings were improved, and C3 level re turned to normal range after conservative treatment. We suggest that a complement deficiency should be reled out in patients of glomerulonephritis developed during the recovery phase of meningococcal men ingitis. C3 nephritic factor detection and renal biopsy should be carefully considered in these patients. (Korean J Nephrol 2003;23(3):321-325
C-밴드 완전 편파 측정용 스캐터미터 시스템 보정 기술 검증

본 논문에서는 미소 뮬러 행렬 보정법(differential Mueller matrix calibration technique: DMMCT)을 이용하여 C-밴드의 지상운용용 HPS(Hongik Polarimetric Scatterometer) 시스템을 보정한 연구 결과를 선보인다. 스캐터미터 시스템 보정을 위해 금속구를 이용하여 안테나 주빔에서의 편파 측정한 안테나 패턴을 챔버에서 측정하고, DMMCT를 이용하여 지상운용용 HPS의 왜곡 정도를 분석하여 정확하게 보정한다. 단일 편파 시스템과는 다르게 완전 편파 시스템에서는 후방 산란 계수뿐만 아니라 위상 변수들도 중요하다. 보정된 지상운용용 HPS를 이용하면 논, 맨땅, 작물밭 등의 지표면에 대한 정확한 Mueller 행렬을 얻을 수 있으므로, 후방 산란 계수뿐만 아니라 동일 편파와 교차 편파의 위상 변수들도 정확하게 측정하게 된다. 보정의 정확도는 산란 모델을 이용해 계산한 후방 산란 계수와 측정한 후방 산란 계수의 비교로 확인할 수 있다. 따라서 측정으로 쟁기질 한 밭의 편파 응답특성을 얻었으며, 이 결과를 이론 모델로 얻은 편파 응답 특성과 비교하여 보정 기술을 검증하였다. This paper presents a study on the calibration of a C-band HPS(Hongik Polarimetric Scatterometer) system using the DMMCT(Differential Mueller Matrix Calibration Technique). For calibration of the polarimetric scatterometer system, a fully-polarimetric antenna pattern(magnitudes and phase-differences) of the antenna main-beam is measured using a conducting sphere at anechoic chamber. The polarimetric scatterometer system could be accurately calibrated after retrieving its distortions using the DMMCT. Unlike a single-polarimetric system, in a fully-polarimetric system, not only backscattering coefficients but also phase differences are important parameters. This calibrated HPS system can be used to measure accurate Mueller matrices of bare soil surfaces, rice paddies, and vegetation fields. The phase-difference parameters as well as the backscattering coefficients for co- and cross-polarizations can then be obtained. The accuracy of calibration was verified by comparing the measured backscattering coefficients with a scattering model. The measured polarization response of a plowed bare field was also compared with the polarization response which was synthesized using a polarimetric scattering model for verifying the calibration technique.

<B>Abstract</B><P>Amlodipine has a potential to reduce clopidogrel bioactivation through the cytochrome P450 3A4 enzyme in vivo, but the clinical impact of this interaction remains controversial. This randomized, open-label, two-period, crossover study was performed to evaluate the influence of amlodipine on the haemostatic profiles of high-risk patients during clopidogrel treatment. We recruited 40 Asian patients (Male/Female: n = 36/4) receiving clopidogrel (75 mg/day), aspirin (100 mg/day) and rosuvastatin for at least 6 months following percutaneous coronary intervention. Patients were randomly assigned to receive either 5 mg daily amlodipine or not for 2 weeks, and then were crossed over to the other treatment for 2 weeks. Haemostatic measurements were conducted with the VerifyNow assay and thromboelastography (TEG). Primary endpoint was P2Y12 Reaction Units (PRU) during on- versus off-amlodipine treatment. The on-amlodipine strategy showed higher level of PRU compared with the off-amlodipine strategy (176.8 ± 75.4 vs. 150.7 ± 65.5 PRU; ∆mean: 26.1 PRU; ∆95% confidence interval [CI]: 4.5-47.7 PRU; p = 0.019). Platelet-fibrin clot strength measured by TEG was lower during on- versus off-amlodipine treatment (7,712 ± 1,889 vs. 8,559 ± 2,174 dyne/cm2; ∆mean: -847 dyne/cm2; ∆95% CI: -1,632 to -62 dyne/cm2; p = 0.035). After amlodipine discontinuation, 27 patients (67.5%) showed a decrease in PRU, which was associated with ‘PRU ≥ 160 on-amlodipine’ in multivariate analysis (odds ratio: 62.014; 95% CI: 2.302-1670.328; p = 0.014). In conclusion, amlodipine increases platelet reactivity and decreases platelet-fibrin clot strength during clopidogrel treatment. In addition, the effect of amlodipine discontinuation on clopidogrel responsiveness is associated with on-amlodipine platelet reactivity.</P>

A 67-year-old male was admitted to the hospital for evaluation of incidentally detected anemia and mild azotemia. Urinalysis showed no abnormal finding and 24 hr urine protein amount was clinically insignificant (270 mg/day). Urine and serum protein electrophoresis were negative for a monoclonal spike. However, urine and serum immunoelectrophoresis demonstrated the presence of monoclonal free K light chains. Renal biopsy showed the features of chronic tubulointerstitial disease and on the immunofluorescence studies, K light chain was in a linear pattern in basement membranes of glomeruli and tubules. Work-up for multiple myeloma including bone marrow biopsy showed results compatible with multiple myeloma. Treatment was started with vincristine, adriamycin and doxorubicin at monthly interval for three months followed by autologus peripheral blood stem cell transplantation. At follow -up 5 months after autologus peripheral blood stem cell transplantation, the patient is well with a serum creatinine of 2.3-2.6 mg/dL and 24 hr urine protein of 200-350 mg.

The 316LVM stainless steel that is widely used in surgical implant has been studied. The objective of this study is to develop the domestic production of the surgical implant materials. In the work, the metalllirgical phenomena, physical and chemical properties and biocompatibility of the materials are investigated. According to the experimental observation, corrosion resistance is strongly depended on the -ferrite structure and passive film, and mechanical properties are mainly depended on the cold reduction ratio. The -ferrite structure is minimized in the 16.651 Cr and 14%Ni contents, and yield strength is 104 kg/mm$^2$ at 45% cold reduction. Biocompatibility is excellent in the mouse body test for six weeks.
