        강원남부 폐광지역 개발기금 사업의 경제적 효과 : 2001~2021년간의 기금사업을 중심으로

        본 논문은 2001년부터 2021년 기간의 강원남부 폐광지역 4개 시군을 대상으로 전개된 폐광지역개발기금 투자의 경제적 파급효과를 실증적으로 분석함을 목적으로 한다. 이 기간 동안 연차별 투자 건수 1,220개로 구성된 691개의 재원 사업이 이행되었다. 분석대상 기간 동안 연차별로 투자된 재원의 합계는 총 1조 6,180.5억원이며, 지역산업연관분석을 통해 실증적으로 경제적 파급효과를 도출하는 것이 본고의 목적이다. 폐광지역개발기금의 투자금액의 면에서 보면, 대체로 생산유발효과가 상대적으로 낮은 3차산업 부문에 대한 재원의 비중이 큰 것으로 나타났다. 그러나 다른 한편으로 고려해야 할 점은 3차산업 부문의 취업유발효과가 이 지역에서 타 산업에 비해 높다는 점이다. 이러한 산업구조적 특성을 토대로 향후 폐광지역개발기금 사업의 경제적 효과를 최적화시키기 위해서는 유발계수 값의 분포를 고려한 재원의 배분 계획을 마련할 필요가 있다. 본고의 추정결과는 향후 폐광지역 개발기금의 합리적인 배분을 위한 실증적 기초 자료로서의 역할을 할 것으로 기대된다. This study aimed to analyze the economic effects of the closed mine area development fund on the southern part of the Gangwon provinces during the period 2001~2021. In this time frame, 691 financial projects comprising 1,220 items in the annual base were carried out and the total amount of investment was 1,618.05 billion Won. The main goal of this study was to derive empirical evidence about the economic effect of this fund through the regional industrial input-output tables. An analysis of the distribution of financial resources available to the specific fields revealed that the portion of funds invested in the tertiary industry sector has a low production inducement effect, which was not comparable with other industries. However, the movement of the employment inducement coefficient of tertiary industries led by the services sector in this area is comparatively high. Based on the regional character of the industrial structure, it is necessary to make a deliberate funds distribution plan taking the movement of the above inducement coefficients into consideration, to optimize the economic effect of the closed mine area development fund. The empirical output derived from this study would assist policymakers in establishing an effective distribution strategy for the closed mine area development fund in the future.

        Role of Recombinant PnTx2-6 Protein as a Mediator of Vasodilation in Blood Vessels

        The venome of Phoneutria nigriventer spider has been shown to have side effects including severe painful erections that last for hours. PnTx2-6, a toxin from P. nigriventer spider venom, modulates voltage gated Na+ channels and activation of nitric oxide (NO) production. NO is essential for the regulation of blood flow and pressure. Therefore, PnTx2-6 is expected to be effective not only for erectile dysfunction but also for cardiovascular diseases. A previously has reported cDNA clone for PnTx2-6 toxin, which was expressed in E. coli cytoplasm. We created the same clone and expressed it in a bacterial expression system. PnTx2-6 increased the genes expression of superoxide dismutase 1, glutathione peroxidase 1, and sulfiredoxin 1. We hypothesized that recombinant PnTx2-6 may indirectly regulate blood flow and pressure, resulting in NO production in human umbilical vein endothelial cells (HUVEC). These data suggest differential regulation of the vascular ageing process, which may contribute to the anatomic heterogeneity of atherosclerosis. The results of this study may be used for the emergency treatment of sudden cardiovascular disease caused by ageing.

      • Prevalences of actinic skin lesions and comparison of single and multiple lesions in patients with actinic keratosis: A retrospective study of 61 patients

        Background: Photoaged skin is characterized by actinic skin lesions such as wrinkling, elastosis, and premalignant and malignant lesions. Actinic keratosis (AK) is one of the most common precancerous lesions and multiple lesions of actinic keratosis are one of the risk factors of skin cancer. There have been only a few studies to investigate prevalences of actinic skin lesions and compare characteristics between single and multiple lesions in patients with AK. Objectives: The aim of this study was to evaluate prevalences of actinic skin lesions and compare characteristics between single and multiple lesions in patients with AK in Koreans. Methods: We retrospectively reviewed the medical records and biopsy specimens of 61 patients. Results: Frequencies of deep wrinkle, solar elastosis, cutis rhomboidalis nuchae, and senile purpura were higher in the multiple lesions group. The ratio of males to females was 1 : 3.43 and the mean age was 79 years in the multiple lesions group, which were higher than those were in the single lesion group (1 : 1.14, 72 years). The mean duration was longer in the multiple lesions group. Sun protective behavior was more frequent in the single lesion group and occupation of farmer was more common in the multiple lesions group. There was no statistically significant clinicopathologic difference between the two groups. Conclusion: This study would improve our understandings of the characteristics of actinic keratosis with both single and multiple lesions.

      • Efficacy of low-dose intravenous immunoglobulin for treating chronic urticaria

        Background: The several treatment of chronic urticaria (CU) have been studied in several trials, but there have been few studies on the effect of low-dose IVIG. Objectives: The authors investigated the quality of life(CU-Q2oL), urticaria severity score(USS), urticaria activity score(UAS), and Skindx29 of the Korean patients with CU before the IVIG treatment, and assessed the quality of life and clinical outcome after IVIG treatment. Methods: A total six patients who suffered from persistent CU despite conventional treatments were administered 150mg/kg/month of IVIG at every 4 weeks. All of patients were asked to complete a questionnaire about the effect of CU of their QOL and assessed the clinical symptoms of CU at each treatment. Results: All of patients completed the medication at least 3 months. All of patients reported improvement of the symptoms and QOL after treatment of low-dose IVIG. In all patients, the UAS and USS score were improved from 5.6 to 2.8, and from 45.7 to 31, respectively. The CU-Q2oL score and Skindex29 score were measured from 69.0 to 48.6 and from 85.4 to 65.6, respectively. CU-Q2oL and Skindex29 had a statistically significant correlation. In addition, UAS had a statistically significant correlation with USS. There was a statistically significant correlation between CU-Q2oL and USS. Conclusion: This study shows that IVIG may be an efficacious treatment option for recalcitrant patients with CU.

        Explainable machine learning using perioperative serial laboratory results to predict postoperative mortality in patients with peritonitis-induced sepsis

        Purpose: Sepsis is one of the most common causes of death after surgery. Several conventional scoring systems have been developed to predict the outcome of sepsis; however, their predictive power is insufficient. The present study applies explainable machine-learning algorithms to improve the accuracy of predicting postoperative mortality in patients with sepsis caused by peritonitis. Methods: We performed a retrospective analysis of data from demographic, clinical, and laboratory analyses, including the delta neutrophil index (DNI), WBC and neutrophil counts, and CRP level. Laboratory data were measured before surgery, 12–36 hours after surgery, and 60–84 hours after surgery. The primary study output was the probability of mortality. The areas under the receiver operating characteristic curves (AUCs) of several machine-learning algorithms using the Sequential Organ Failure Assessment (SOFA) and Simplified Acute Physiology Score (SAPS) 3 models were compared. ‘SHapley Additive exPlanations’ values were used to indicate the direction of the relationship between a variable and mortality. Results: The CatBoost model yielded the highest AUC (0.933) for mortality compared to SAPS3 and SOFA (0.860 and 0.867, respectively). Increased DNI on day 3, septic shock, use of norepinephrine therapy, and increased international normalized ratio on day 3 had the greatest impact on the model’s prediction of mortality. Conclusion: Machine-learning algorithms increase the accuracy of predicting postoperative mortality in patients with sepsis caused by peritonitis.

        Annual Change in Fungal Concentrations and Allergic Sensitization Rates to Alternaria and Cladosporium in Korea During the Period 1998–2022

        Purpose: Atmospheric fungi are associated with respiratory allergies in humans, and some fungal spores can cause allergic diseases. Environmental and biological factors influence the concentrations of atmospheric spores. In this study, we evaluated the climate change-induced annual variations in fungal spore concentrations and allergic sensitization rates in the Seoul Metropolitan Area over a period of 25 years. Methods: Fungal spores and pollen were obtained from Hanyang University Seoul and Guri Hospitals; they were identified and counted for 25 years (1998-2022). The study participants included patients who underwent tests for allergic diseases in both hospitals. Their allergenic sensitization rates were determined via allergic skin prick and serum tests, after which their sensitization rates to allergenic fungi and pollens were calculated. The daily climatic variables were obtained from the Korea Meteorological Administration. Results: The total annual atmospheric fungal concentrations decreased in both areas during the period. Simultaneously, we recruited 21,394 patients with allergies (asthma, 1,550; allergic rhinitis, 5,983; and atopic dermatitis, 5,422) from Seoul and Guri Hospitals for allergenic fungal sensitization evaluations over the period. The allergenic fungal sensitization rates decreased annually in both areas over that time `+(Alternaria [3.5%] and Cladosporium [4.4%] in 1998; Alternaria [0.2%] and Cladosporium [0.2%] in 2022). In contrast, the annual pollen concentrations increased with the sensitization rates to pollen in children. Conclusions: The atmospheric fungal concentrations decreased annually, with allergic sensitization rate decreasing over the period of 25 years. Allergenic fungal sporulation could decrease with climate changes, such as desertification and drought. Extended monitoring periods and further large-scale studies are required to confirm the causality and to evaluate the impact of climate change.

        Basal cell carcinoma (BCC) has been described as the most common malignant skin tumor usually seen in head and neck region. It has various histological variants and constitues about 65% of epithelial tumors. Known variants of BCC include as cystic, nodular, adenoid, morpheaform and miscellaneous forms. Of these, adenoid type BCC is regarded as a low grade malignancy compared to other types. A recent study reported the adenoid BCC incidence of 1.3%. Adenoid type of BCC has no site predilection and have been reported at various sites including forehead, inner canthus of eye, chin, trunk and extremities. A 78-year-old woman presented with a slow growing, pigmented, 0.8cm sized nodule located on the philtrum for seven years, along with an ulcerative hemorrhagic erosion lesion and rolled up edges. Skin biopsy specimens were taken by punch biopsy. Multiple cyst-like structures with strands of basaloid cells presenting with lacelike pattern, and mucoid-patterned stroma were seen. On immunohistochemical staining, SMA, CK19, C-kit were found negative while CD 10, CK5/6, p63 and p53 were positive. Also, Ki-67 was 70 %. After 18 days, excisional biopsy with skin-fat composite graft was taken from philtrum and left side of cheek. Thereafter, patient was diagnosed with adenoid BCC based on histological finding and was treated with wide biopsy with skin-fat composite graft for cosmetic aspect. Herein we report a rare case of adenoid BCC.

      • [P349] A case of squamous cell carcinoma arising from epidermal cyst on buttock

        Epidermal cysts are common benign tumor of the skin. They are slow growing, round, firm intradermal or subcutaneous tumor and occur most commonly on the trunk, face and scalp. Interestingly, when an epidermal cyst ruptures and its contents are released into the dermis, foreign body reaction occurs. As a result, multinucleated giant cells may form a keratin granuloma and lead to a pseudocarcinomatous hyperplasia, which is thought to be a stimulant of squamous cell carcinoma (SCC) formation. From a review of the literature, malignant transformation of epidermal cysts is very rare and its incidence rate is known to be 0.011 -0.045%. An 82-year-old woman presented with painful, 2cm-sized subcutaneous nodule on left buttock for two years. Excisional biopsy specimens were taken from the skin lesion on buttock. In microscopic view, numerous keratinous materials were released from ruptured cysts. The specimen was consisted of well-differentiated squamous cells with atypical nucleus and mitotic figures. In mid dermis, horn pearl and the numerous multinucleated giant cells were also seen. On immunohistochemical staining, CK5/6 were positive. Also, Ki-67 and p53 were largely confined to basal portion of epithelium. The patient was diagnosed with a rare case of SCC arising from epidermal cyst based on histological finding and was later treated with complete excisional biopsy. Herein we report a rare case of SCC arising from epidermal cyst.

        Estimates of the Number of Workers Exposed to Diesel Engine Exhaust in South Korea from 1993 to 2013

        Background: The aim of this study was to estimate the number of workers exposed to diesel engine exhaust (DEE) by industry and year in the Republic of Korea. Method: The estimates of workers potentially exposed to DEE in the Republic of Korea were calculated by industry on the basis of the carcinogen exposure (CAREX) surveillance system. The data on the labor force employed in DEE exposure industries were obtained from the Census on Establishments conducted by the Korea National Statistical Office from 1993 to 2013. The mean values of prevalence rates adopted by EU15 countries were used as the primary exposure prevalence rates. We also investigated the exposure prevalence rates and exposure characteristics of DEE in 359 workplaces representing 11 industries. Results: The total number of workers exposed to DEE were estimated as 270,014 in 1993 and 417,034 in 2013 (2.2% of the total labor force). As of 2013, the industry categorized as "Land transport" showed the highest number of workers exposed to DEE with 174,359, followed by "Personal and household services" with 70,298, "Construction" with 45,555, "Wholesale and retail trade and restaurants and hotels" with 44,005, and "Sanitation and similar services" with 12,584. These five industries, with more than 10,000 workers exposed to DEE, accounted for 83% of the total DEE-exposed workers. Comparing primary prevalence rates used for preliminary estimation among 49 industries, "Metal ore mining" had the highest rate at 52.6%, followed by "Other mining" with 50.0%, and "Land transport" with 23.6%. Conclusion: The DEE prevalence rates we surveyed (1.3-19.8%) were higher than the primary prevalence rates. The most common emission sources of DEE were diesel engine vehicles such as forklifts, trucks, and vans. Our estimated numbers of workers exposed to DEE can be used to identify industries with workers requiring protection from potential exposure to DEE in the Republic of Korea.
