Validation of the oxford classification of iga nephropathy: A Single-Center Study in Korean Adults

Background/Aims: The recently published Oxford classification of IgA nephropathy (IgAN) proposed a split system for histological grading, based on prognostic pathological features. This new classification system must be validated in a variety of cohorts. We investigated whether these pathological features were applicable to an adult Korean population. Methods: In total, 69 adult Korean patients with IgAN were analyzed using the Oxford classification system at Soonchunhyang University Hospital, Seoul, Korea. All cases were categorized according to Lee`s classification. Renal biopsies from all patients were scored by a pathologist who was blinded to the clinical data for pathological variables. Inclusion criteria were age greater than 18 years and at least 36 months of follow-up. We excluded cases with secondary IgAN, diabetic nephropathy combined other glomerulopathies, less than 36 months of follow-up, and those that progressed rapidly. Results: The median age of the patients was 34 years (range, 27 to 45). Mean arterial blood pressure was 97 ± 10 mmHg at the time of biopsy. The median follow-up period was 85 months (range, 60 to 114). Kaplan-Meier analysis showed significant prognostic predictions for M, E, and T lesions. A Cox proportional hazard regression analysis also revealed prognostic predictions for E and T lesions. Conclusions: Using the Oxford classification in IgAN, E, and T lesions predicted renal outcome in Korean adults after taking clinical variables into account.
천연 생약재 열수 및 알코올 추출물의 어병 세균에 대한 항균력

감초 외 48종의 천연 생약제로부터 열수 및 알콜 추출액을 제작하여 19 균주 어병 세균을 대상으로 항균력을 조사한 결과, 알콜 추출액은 22종, 열수 추출액은 16종이 어병 세균에 대하여 항균력을 나타내며 이 중 13종의 생약재는 알콜 및 열수 추출액 모두 8㎜ 이상의 저지대를 나타내어 항균력이 있는 것으로 나타났다. 열수 추출물에서 항균력을 나타내는 약재 중 그람 음성균에 감수성이 양호한 약재는 저지대 8 ㎜ 이상을 나타내는 애엽, 개옻나무, 연교, 지유, 파엽, 대황 및 황금이었으며, 그람 양성균에 항균력이 양호한 약재는 삼지구엽초, 육계 및 보골지 등 이었다. 그리고 오매, 황련, 계혈등, 상백피, 오배자 및 오미자는 저지대가 8㎜ 이상으로 그람 음성,양성균 모두에 항균력이 있었다. 알콜 추출물중에서 그람 음성균에 항균력을 나타내는 것은 백작약, 오매, 선모 및 황금 등으로 저지대가 8㎜ 이상으로 측정되었으며, 그람 양성균에 항균력을 나타내는 것은 감초, 계혈등, 단삼, 상백피, 육계 및 보골지 등으로 저지대가 8㎜ 이상이었다. 또한 그람 음성, 양성균에 모두 8㎜ 이상의 저지대를 나타내는 것은 애엽, 개옻나무, 황련, 지유, 오배자 및 오미자 등이었다. 그러나 이 중에서 오배자 열수 추출물의 어병 세균에 대한 발육 저지대가 32㎜로 그람 음성, 양성균 모두에 가장 뛰어난 항균력을 나타내어 다른 생약재와 다른 광범위 생약재인 것으로 나타났다. 애엽, 황금, 지유, 오매, 황련 및 오배자 열수 ·알콜 추출물은 tetracycline에 내성을 나타내는 균주에 감수성을 나타내었다. Hundreds of medicinal herbs have been using for the purpose of diseases treatment and immune enhancement for human being and other animals including fishes. Among them, 49 species of medicinal herbs were selected and tested for antibacterial activities against 19 strains of fish pathogenic bacteria in different 4 species. The 49 medicinal herbs were extracted by water and ethyl alcohol. The extracts were freeze dried and some paper discs from the extracts were prepared for the evaluation of antibacterial activity. The tested pathogenic bacteria were 5 strains of Edwardsiellu tarda, 5 strains of Vibrio sp., 4 strains of Lactococcus gawieae, 1 strain of Lactococcus raffinose. 1 strain of Streptococcus parauberius, and 3 strains of Streptococcus iniue. The Galla rhois (Obaeja), Gaeonnamu and Hwangleyon showed antibacterial activities on both gram negative and gram positive fish pathogenic bacteria. The Youkgae, Sangbaekpi, Bogolji and Gamcho showed very effective antibacterial activities on gram positive pathogens while Jiyu, Aeyoeb and Yeonkyo showed very effective on gram negative pathogens.

<▼1><P/><P>Mammalian target of rapamycin (mTOR) signaling is a core pathway in cellular metabolism, and control of the mTOR pathway by rapamycin shows potential for the treatment of metabolic diseases. In this study, we employed a new proximity biotin-labeling method using promiscuous biotin ligase (pBirA) to identify unknown elements in the rapamycin-induced interactome on the FK506-rapamycin binding (FRB) domain in living cells. FKBP25 showed the strongest biotin labeling by FRB–pBirA in the presence of rapamycin. Immunoprecipitation and immunofluorescence experiments confirmed that endogenous FKBP25 has a rapamycin-induced physical interaction with the FRB domain. Furthermore, the crystal structure of the ternary complex of FRB–rapamycin–FKBP25 was determined at 1.67-Å resolution. In this crystal structure we found that the conformational changes of FRB generate a hole where there is a methionine-rich space, and covalent metalloid coordination was observed at C2085 of FRB located at the bottom of the hole. Our results imply that FKBP25 might have a unique physiological role related to metallomics in mTOR signaling.</P></▼1><▼2><P>A proximity biotin-labeling method revealed that FKBP25 is a strong rapamycin-induced interaction partner of FK506-rapamycin binding (FRB). This interaction was supported by the crystal structure of the ternary complex of FKBP25−rapamycin−FRB.</P></▼2>

In mammals, puberty is a dynamic transition process from infertile immature state to fertile adult state. The neuroendocrine aspect of puberty is started with functional activation of hypothalamus-pituitary-gonadal hormone axis. The timing of puberty can be altered by many factors including hormones and/or hormone-like materials, social cues and metabolic signals. For a long time, attainment of a particular body weight or percentage of body fat has been thought as crucial determinant of puberty onset. However, the precise effect of high-fat (HF) diet on the regulation of hypothalamic GnRH neuron during prepubertal period has not been fully elucidated yet. The present study was undertaken to test the effect of a HF diet on the puberty onset and hypothalamic gene expressions in immature female rats. The HF diet (45% energy from fat, HF group) was applied to female rats from weaning to around puberty onset (postnatal days, PND 22-40). Body weight and vaginal opening (VO) were checked daily during the entire feeding period. In the second experiment, all animals were sacrificed on PND 36 to measure the weights of reproductive tissues. Histological studies were performed to assess the effect of HF diet feeding on the structural alterations in the reproductive tissues. To determine the transcriptional changes of reproductive hormone-related genes in hypothalamus, total RNAs were extracted and applied to the semi-quantitative reverse transcription polymerase chain reaction (RT-PCR). Body weights of HF group animals tend to be higher than those of control animals between PND 22 and PND 31, and significant differences were observed PND 32, PND 34, PND 35 and PND 36 (p<0.05). Advanced VO was shown in the HF group (PND p<0.001) compared to the control (PND ). The weight of ovaries (p<0.01) and uteri (p<0.05) from HF group animals significantly increased when compared to those from control animals. Corpora lutea were observed in the ovaries from the HF group animals but not in control ovaries. Similarly, hypertrophy of luminal and glandular uterine epithelia was found only in the HF group animals. In the semi-quantitative RT-PCR studies, the transcriptional activities of KiSS-1 in HF group animals were significantly higher than those from the control animals (p<0.001). Likewise, the mRNA levels of GnRH (p<0.05) were significantly elevated in HF group animals. The present study indicated that the feeding HF diet during the post-weaning period activates the upstream modulators of gonadotropin such as GnRH and KiSS-1 in hypothalamus, resulting early onset of puberty in immature female rats.

PURPOSE: The aim of this study was to investigate the clinical performance of whole-body F-FDG PET/Dixon–volume-interpolated breath-hold examination (Dixon-VIBE), T1-weighted, and T2-weighted MRI protocol in patients with colorectal cancer. PATIENTS AND METHODS: A total of 59 patients with colorectal cancer were enrolled in this study. Each patient had one of the following clinical conditions: initial stage before therapy, stage after neoadjuvant therapy, suspicious colorectal liver metastases, and colorectal liver metastases after chemotherapy. Fourteen patients had primary colorectal cancer, whereas 38 patients had a total of 132 hepatic lesions, 53 lesions existed before chemotherapy, and 79 lesions appeared after chemotherapy. The primary stage and metastases images were obtained using our PET/Dixon-VIBE/T1/T2 MRI protocol and were analyzed by 2 nuclear medicine physicians. Diagnostic accuracy was compared with contrast-enhanced MRI images, which were based on surgical pathology results. RESULTS: The sensitivity of our imaging protocol for primary colorectal cancer was 100% (14/14). T and N stage both showed 92.9% (13/14) accuracy. Of all 132 hepatic lesions, 115 metastatic lesions were analyzed, and 17 benign lesions were excluded (6 were during pretreatment cases, and 11 were during posttreatment cases). In pretreatment metastatic lesions (n = 47), the sensitivities of our protocol and dedicated MRI were 95.7% (45/47) and 100% (47/47), respectively. In posttreatment lesions (n = 68), sensitivities of our protocol and dedicated MRI were 75% (51/68) and 91.2% (62/68), respectively. CONCLUSIONS: Whole-body PET/Dixon-VIBE/T1/T2 MRI protocol is clinically useful for TNM staging and chemonaive hepatic metastasis in colorectal cancer.

Purpose: To identify the clinical characteristics of SRY-negative male patients and genes related to male sex reversal, we performed a retrospective study using cases of 46,XX testicular disorders of sex development with a review of the literature. Materials and Methods:SRY-negative cases of 46,XX testicular disorders of sex development referred for cytogenetic analysis from 1983 to 2013 were examined using clinical findings, seminal analyses, basal hormone profiles, conventional cytogenetic analysis and polymerase chain reaction. Results: Chromosome analysis of cultured peripheral blood cells of 8,386 individuals found 19 cases (0.23%) with 46,XX testicular disorders of sex development. The SRY gene was confirmed to be absent in three of these 19 cases (15.8%). Conclusion: We report three rare cases of SRY-negative 46,XX testicular disorders of sex development. Genes on autosomes and the X chromosome that may have a role in sex determination were deduced through a literature review. These genes, through differences in gene dosage variation, may have a role in sex reversal in the absence of SRY.
