        임신중 뇌동맥류의 외과적 치료 : 증례보고 A Case Report

        A multiparous 28-year-old woman, suffering ruptured intracranial aneurysm during pregnancy is presented. Clipping of the aneurysm was performed in 32 weeks of gestational age. Three weeks after operation, she delivered vaginally without complication. Delivery may be managed according to obstetrical indication following surgical correction of the aneurysm. The authors reviewed the literature, and discussed the neurosurgical and obstetric management of such patients.

        뇌동맥류 수술시 Fenestrated Clip의 사용에 대하여 : Report of Four Cases

        Some aneurysms with peculiar shapes, large sized or in unusual location cannot be obliterated by ordinary methods. We present four cases using two kinds of fenestrated clips. There were 4 aneurysms in the 4 patients : 3 saccular and 1 fusiform aneurysm. Two aneurysms were located in anterior communicating artery, one left carotico-opthalmic artery, and one distal middle cerebral artery. All of the aneurysms were successfully obliterated. Our results suggest that the various shape of fenestrated clips may be benefit for clipping difficult aneurysms, which would not be clipped with ordinary one.

        다발성 두개강내 뇌동맥류 : 9개의 다발성 뇌동맥류 환자의 증례보고 A Case Report of Patient with Nine Multiple Intracranial Aneurysms

        One case of multiple intracranial aneurysms is presented. On carotid angiogram, nine aneurysmal sacs including a giant aneurysm of left middle cerebral artery were detected on the right distal anterior cerebral artery, anterior communicating artery, bifurcation of both internal carotid artery, both middle cerebral artery, and left posterior communicating artery. The authors reviewed the literature, and discussed the incidence, determining the site of rupture, and the operative methods of multiple intracranial aneurysms.

        외상성 뇌동맥류 : 천측두 동맥에 발생한 외상성 가성동맥류 1예 A Case of Traumatic False Aneurysm of the Superficial Temporal Artery

        A case of iatrogenic false aneurysm of the superficial temporal artery is presented. Direct injection of Urograffin into the pulsatile mass showed aneurysmal dilatation, 3×2㎝ sized, of the superficial temporal artery. Aneurysm excision is indicated to reduce the risk of hemorrhage from the subsequent head trauma, to relieve headache, and for any cosmetic defect. The authors have reviewed the literature, and discussed the incidence, classification, pathogenesis, clinical and angiographic diagnosis, differential diagnosis, and treatment.

      • 완전 방실차단을 동반한 급성 심근염

        급성 심근염환자에서 발행한 완전 방실차단을 영구 심장박동 조율기로 치료한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Myocarditis, an inflammatory process involving the myocardal wall, may be caused by most infectious agents. Irrespective of its etiology, it presents commonly with evidence of heart failure, hypotension, and various electrocardiographic abnormalities. Patient with myocarditis may be asymptomatic or may have a rapidly progressive fatal disease. Medical management of patients with myocarditis includes specific therapy for underlying infection and control of the complication of myocarditis such as congestive heart failure and arrhythmia. Recently we had experienced myocarditis with complete atrioventricular block in 37-year-old man. Although he had shown complete recovery of heart failure, he had shown complete atrioventricular block, so we inserted permanent pacemaker. We report it with review of the literature.

      • 다공질 실리콘층을 이용한 습도센서의 개발

        A humidity sensor using porous silicon layer as humidity-sensing material is developed. Its structure was designed monolithically to be compatible to the integrated circuit technology, and the porous silicon layer was formed on p-type wafers with the low resistivity of 0.01 Ωcm to be stable mechanically. Proto-type sensors were fabricated using processes such as localized formation of porous silicon, oxidation of porous silicon layer, and etching of oxidized porous silicon layer. The dependence of capacitance on relative humidity was measured at room temperature, where the capacitance increased from 0.3 nF to 1nF for the relative humidity of 55 to 90% over.

      • 척추 유합술에서 자가 이식골과 동종 이식골의 비교 : 증례보고 A Case Report

        척추 유합술에 있어서 자가 골 이식이 가장 좋은 방법으로 알려져 있지만 수술 시간과 실혈량이 늘어나고 공여부의 통증 및 감염의 위험등 단점으로 동종 골 이식도 많이 사용되어지고 있다. 이러한 동종 이식골의 효용성에 대해서 많은 논란이 있어온 것은 사실이나 여러 변수들, 즉 나이, 성별, 흡연력, 수술의 종류등에 따른 영향을 완전히 배제시키지 못하여 순전히 동종 이식골과 자가 이식골간의 비교가 어려웠다. 저자들은 척추관 협착증으로 후방 감압술 및 후방 기기 고정술, 후외방 골 이식술을 받은 환자에게서 우측은 자가 이식골, 좌측은 동종이식골로 골 이식을 시행한 후 4년 추시에서 자가 골 이식한 부위는 견고한 골유합 소견을 보였으나 동종골 이식 부위는 이식 골이 모두 흡수되는 소견을 체험하여 같은 조건하에서 자가 이식골이 동종 이식골보다 골유합체의 형성에 있어서 더 우수하다고 생각한다. Posterior decompression, spinal instrumentation and bone grafting are the standard operative treatment method of spinal stenosis and other low back disorders. Although autograft bone is considered the standard in performing fusion of the bone, allografts are recommended as an alternative bone graft agent becaue of the lack of complications at the donor site such as infection, increased blood loss and prolonged operative time Autografts have the other limitations because of inadequate quality and quantity. Many investigators have compared autografts and allografts in posterior spinal fusion, and there are proponents as well as opponents of allograft use. Numerous factors are thought to affect fusion, including age, sex, nutritional status, associated disease, type of fusion, length of fusion, types of instrumentation and smoking status. The validity of comparison studies between allografts and autografts is questionable when these variables are not controlled. We describe a man who had a spinal stenosis at the level of L4-5 and L5-S1. He was operated with posterior decompression, spinal instrumentation and underwent a posterolateral bone graft, comparing autografts and allografts in the same patient. At 4-year follow up, autografts have a solid union but allografts are resorbed completely.

      • 탱자(Poncirus tripoliata RAF)잎으로부터 Mitochondrial DNA의 분리

        Differaitial centrifugation, sucrose(30%, 60%) and percoll(13.5%, 21% and 45%) density gradient centrifugation were used to effectively isolate mitochondria and mitochondrial DNA (mt DNA) from the leaves of the genus Poncirus (Pondrus trifoliata RAF.) The results showed that percoll density gradient centrifugation provided a convinient and effective method for the preparation of clean mitochondria. The purity of prepared mt DNA was increased by the removal of the nuclear DNA by DNase treatment to the crude mitochondrial suspension mt DNA on the agarose gel was shown extra fluorescent background in case of isolation by differential centrifugation and more cleared band included slightly smeared patterns for sucrose density gradient, and highly paint band from percoll density gradient was appeared to the best results of the above three methods. In addition to chromosomal mt DNAs, two short mt plasmid-like elements were detected in the genus Poncirus. These elements consistently co-purified with the chromosomal mt DNAs and is widely distributed among natrual Poncirus trifoliata RAF. populations. But these elements required proteinase K treatment for mobility in agarose gels.

      • FET형 반도체 마이크로센서 개발(Ⅲ)

        현재 의료진단, 화학공정의 모니터링이나 환경공학적 감시 및 제어 등의 분야에서 사용되고 있는 기존의 센서는 고가이며 용적이 클 뿐만 아니라 분석시간이 길고 사용하기 까다로운 것 등 여러 가지 문제점이 있다. 또한 측정환경에 영향을 주지 않을 만큼 충분히 작으며 빠른 분석시간을 가진 센서를 필요로 하고 있다. 본 연구에서는 기존의 센서들의 난점을 극복할 수 있는 새로운 형태의 FET형 전해질(electrolyte : H^+, K^+, Ca^2+, Na^+)센서소자 및 분석 시스템, 용존가스(O_2, CO_2)센서의 개발을 중점적으로 추진하였으며, FET형 압력센서, 습도센서 둥의 개별 FET형 센서에 관한 기초연구도 병행하였다. The conventional sensors have many problems such as high cost, large dimension, long analysis time and troublesome handling to apply to the fields of medical diagnosis, monitoring of chemical process and environmental monitoring/control. The main objects of this research are to develope a new FET type electrolyte(H^+, K^+, Ca^2+, Na^+)sensors, analysis system, and dissoved gas(O_2, CO_2)senors that can overcome the problems of the conventional sensors, and parallel basic researches on FET. type sensors such as pressure and humidity are also in progress.

        현재 의료진단, 화학공정의 모니터링이나 환경공학적 감시 및 제어 등의 분야에서 사용되고 있는 기존의 센서는 고가이며 용적이 클 뿐만 아니라 분석시간이 길고 사용하기 까다로운 것 등 여러 가지 문제점이 있다. 또한 측정환경에 영향을 주지 않을 만큼 충분히 작으며 빠른 분석시간을 가진 센서를 필요로 하고 있다. 본 연구에서는 기존의 센서들의 난점을 극복할 수 있는 새로운 형태의 FET형 전해질(electrolyte : H^+, K^+, Ca^2+, Na^+)분석 시스템, 용존가스(O_2, CO_2)분석 시스템의 개발을 중점적으로 추진하며, FET형 압력센서, 습도센서, 방사형 온도센서 등의 개별 FET형 센서에 관한 연구도 병행하고자 한다. Recently various kinds of sensors have been developed, being applicated to their own purpose. There are lots of difficulties to apply them to measurements in which the real-time monitoring is required without disturbing the surrounding environment. FET type semiconductor sensors, fabricated by the semiconductor integration technologies, have many advantages for their miniaturization, standardization, mass-production and in vivo/in situ monitoring. They also hold a very proper configuration for multi-functional sensors or integrated smart sensors, and wide availability by forming various kinds of physical or chemical sensing materials onto their sensing gates.
