        This research on organic acid and its component ratio (H_(AC) : H_(pt) : H_(Bu)) mainly produced in acid formation steps of anaerobic treatment process. From comparing the above experimental results with digestion efficiency of acetic acid used as substrates, I can find the conclusion as follows : 1. Removal efficiency for component ratios of organic acid is 64.3-98.6%, 59.6-98.2 %, respectively, varied with HRT of 3-20 days. 2. For l0days of HRT, in order to investigate the effects of component ratio of organic acid, when its component ratio(H_(ac) : H_(pt) : H_(bu)) is 100 : 0 : 0, 50 : 25 : 25, 28 : 38 : 34 and 0 : 50 : 50(as COD), respectively, effluent concentration is 464mg/l, 666mg/l, 832mg/l and 1,148mg/l, respectively. 3. Component ratio of menthane gas for organic acids represents 55-70%, with variation o: HRT, 3-20days. 4. When the component ratios of organic acid(H_(ac) : H_(pt) : H_(bu)) is 50 : 25 : 25 and 23 : 38 : 34(as COD), methane production per COD removal for HRT of 3-20days is 0.27~0. 32, 0.21~0.30. 5. From the mass balance relationship, when the component ratios of organic acid is 50 : 25 : 25 and 28 : 38 : 34(as COD), methane production ratio, varied with HRT of 3days, 5days, l0days, 15days and 20days in 49.6%, 66.4%, 77.5%. 85.1% and 89.7%, respectively and 39.1%, 56.3%, 68.3%, 76.3%, 83.9%, respectively.

      • 부신 결핵에 의한 Addison씨병 1예

        Addison's disease is a rare primary adrenal insufficient disorder resulting from chronic deficiency of adrenal cortical hormones. Clinical manefestations are generalized weakenss, weight loss, hyperpigmentation (especially sun exposed area and mucous membrane), hypotension, hyponatremia, hyperkalemia, gastrointestinal symptoms (involving anorexia, nausea, vomiting, abdominal pain). A 34-year-old woman has experienced slowly progressive generalized weakenss and skin pigmentation, anorexia, nausea, vomiting with ascites and diffuse abdominal pain. On the time of admission, her main clinical manifestations were anorexia, nausea, vomiting, fatigue, generalized weakness, amenorrhea, hair loss, diffuse abdominal pain revealed as Addison's disease due to bilateral adrenal tuberculosis. Her adrenal insufficient symptoms were recovered with the replacement of adrenocortical hormones and antituberculous medications. After treatment, Her skin pigmentation was decreased and menstruation was reappeared. Here we experienced one cases of Addison's disease with tuberculous peritonitis.

      • Neisseria elongata subsp. nitroreducens 에 의한 감염성 심내막염 1 예

        Neisseria elongata subsp. nitroreducens는 Neisseria elongata의 세 가지 아종 중에서 인체에 병원균으로 작용하는 세균으로서 주로 심내막염을 일으키며 드물기는 하지만 그 증례 수가 점차 증 가하고 있으며 국내에서는 아직 보고된 바가 없었다. 저자들은 이전에 심질환의 병력이 없던 36세의 남자에서 치과치료 후 발생한 N. elongata subsp. nitroreducens에 의한 심내막염을 경험하여 문헌고찰과 함께 보고하는 바이다. N. elongata subsp. nitroreducens에 의한 심내막염은 경과 중 판막 파괴를 많이 초래하여 판막치환술이 필요했던 경우가 많았던 것의로 보고되고 있으나 본 증례에서는 4주간의 항균제 치료만으로 합병증 없이 호전되었다. Neisseria elongata subsp. nitroreducens differs from the other subspecies of N. elongata in additional reduction of nitrate without gas formation. Like most Neisseria, species except Neisseria meningitidis and Neisseria gonorrheae, N. elongata subsp. nitroreducens is usually classified in the group of non-pathogenin Nisseria. However, N. elongata subsp. nitroreducens, usual commensal of nasopharynx, can cause serious infections, usually endocarditis. To date, infective endocarditis with N. elongata subsp. nitroreducens has not been reported in Korea. We report a case of 36 year-old Korean male with Neisseria elongata subsp. nitroreducens endocarditis who was treated with antimicrobial agents without complication.

        Heterogeneity in liver histopathology is associated with GSK-3β activity and mitochondrial dysfunction in end-stage diabetic rats on differential diets

        While liver histopathology is heterogeneous in diabetes, the underlying mechanisms remain unclear. We investigated whether glycemic variation resulting from differential diets can induce heterogeneity in diabetic liver and the underlying molecular mechanisms. We generated end-stage non-obese diabetic model rats by subtotal-pancreatectomy in male Sprague- Dawley rats and ad libitum diet for 7 weeks (n = 33). The rats were then divided into three groups, and fed a standard- or a low-protein diet (18 or 6 kcal%, respectively), for another 7 weeks: to maintain hyperglycemia, 11 rats were fed ad libitum (18AL group); to achieve euglycemia, 11 were calorierestricted (18R group), and 11 were both calorie- and proteinrestricted with the low-protein diet (6R group). Overnightfasted liver samples were collected after the differential diets together with sham-control (18S group), and histology and molecular changes were compared. Hyperglycemic-18AL showed glycogenic hepatopathy (GH) without steatosis, with the highest GSK-3β inactivation because of Akt activation during hyperglycemia; mitochondrial function was not impaired, compared to the 18S group. Euglycemic-18R showed neither GH nor steatosis, with intermediate GSK-3β activation and mitochondrial dysfunction. However, euglycemic-6R showed both GH and steatosis despite the highest GSK-3β activity and no molecular evidence of increased lipogenesis or decreased ApoB expression, where mitochondrial dysfunction was highest among the groups. In conclusion, heterogeneous liver histopathology developed in end-stage non-obese diabetic rats as the glycemic levels varied with differential diets, in which protein content in the diets as well as glycemic levels differentially influenced GSK-3β activity and mitochondrial function in insulin-deficient state.         Preoperative Carbohydrate Antigen 19-9 Levels Can Predict Stage and Survival Rate in Patients with Colorectal Cancer

        Background/Aims: The purpose of this study was to determine whether preoperative carbohydrate antigen 19-9 (CA 19-9) levels can predict the stage of diseases or survival rate in patients with resectable colorectal cancer (CRC). Methods: A total of 247 patients who underwent curative resections at Kyung Hee University Hospital at Gangdong between 2006 and 2011 were enrolled. We investigated the correlations of preoperative serum levels of CA19-9 with clinicopathological features of CRC. Receiver operative curve was constructed for evaluating the efficiency of the serum CA 19-9 levels in the stratifying stage of CRC. Survival analysis was performed with Kaplan-Meire method and log-rank test. Cox regression analysis was used for the multivariate analysis for survival. Results: Abnormal level of serum CA 19-9 (>37 IU/mL) were associated with advanced T stage (P<0.001), N stage (P=0.002) and TNM stage (P<0.001) in patients with resectable CRC. Furthermore, abnormal level of serum CA 19-9 were related with vascular invasion (P=0.002) and lymphatic invasion (P=0.026). The area under the curve was 0.75 (95% confidence interval [CI] 0.67-0.83) for T4 stage CRC and 0.680 (95% CI 0.61-0.75) for TNM stage III CRC. In patients with TNM stage III CRC, a preoperative CA 19-9 higher than 60 IU/mL (P=0.033) and presence of vascular invasion (P=0.002) were identified as significant predictors of survival rate on multivariate analysis. Conclusions: In patients with resectable CRC, preoperative CA 19-9 correlates with T stage, N stage and TNM stage of disease. Serum CA 19-9 >60 U/mL was an independent predictor of survival rate in the patients with TNM stage III CRC. (Intest Res 2013;11:184-190)

        Clinical Characteristics and Outcomes of Diverticulitis by Physician`s Specialty

        Background/Aims: Several factors affecting the severity and outcomes of diverticulitis have been reported, but there is little research on physician specialty related with this disease. Therefore, we evaluated the clinical characteristics and outcomes of diverticulitis depending on physician`s specialty. Methods: Medical records of 239 patients, who had been hospitalized with first-diagnosed acute colonic diverticulitis at Kyung Hee University Hospital in Gang Dong (Seoul, Korea) from June 2006 to December 2012, were retrospectively analyzed. The patients were classified according to whether they had been managed by gastroenterologists or not. Clinical characteristics and treatment outcomes were compared between two groups. Results: Of these 239 patients, 38 (15.9%) patients were treated by a gastroenterologist and 201 (84.1%) patients by a non-gastroenterologist. Clinical characteristics such as age, gender, body mass index, comorbidity, medication, laboratory results, recurrence and complication were not significantly different between two groups. However, right-sided diverticulitis predominated in the nongastroenterologist group (79% vs . 91%, P =0.028). From the sub-group analysis of uncomplicated diverticulitis, intravenous antibiotics was used for a shorter period of time by gastroenterologists than non-gastroenterologists (3.3±1.9 days vs . 4.4±2.8 days, P =0.032). Multivariate logistic regression analysis showed that the 3 day administration of intravenous antibiotics significantly depended on the physician`s specialty (odds ratio 7.984, 95% confidence interval 1.990-32.043, P =0.003). Conclusions: The results suggest that the duration of intravenous antibiotics for treating uncomplicated colonic diverticulitis was shortened by gastroenterology specialists without increasing operation or recurrence.         그레이브스병 환자의 항 갑상선제 투여에 따른 Soluble Intercellular Adhesion Molecule-1 ( ICAM-1 , CD54 ) 의 변화

        Intercellular Adhesion Molecule-1(ICAM-1) plays an important role in a variety of inflammatory and immune-mediated mechanisms, including lymphocyte recruitment and targeting, antigen presentation and recognition, and lymphocyte cytotoxicity. In order to study the changes of soluble ICAM-1 and relationship to the immune mechanism of Graves' disease, we performed the measurement of a soluble form of ICAM-1 in sera from patients with Graves' disease before and after treatment with antithyroid drugs using a highly sensitive ELISA method. Our results were as followed. 1) The serum levels of soluble ICAM-1 in patients with Graves' disease before treatment were significantly elevated than normal controls(p$lt;0.001). 2) The serum levels of soluble ICAM-1 in patients with Graves' disease after treatment significantly decreased after treatment(p$lt;0.001) but not as low as normal controls. 3) The serum levels of soluble ICAM-1 molecule in patients with ophthalmopathy were not elevated compared to patients with no ophthalmopathy(p$gt;0.2). 4) The serum levels of soluble ICAM-1 showed no significant correlation with serum titers of anti-thyroperoxidase antibody and anti-thyroglobulin antibody, serum T_3, T_4, TSH and goiter size in patients with Graves' disease. In conclusion, the soluble ICAM-1 levels reflect the activity of autoimmune reaction and might be used as a index of efficacy of antithyroid drug treatment of Graves' disease(J Kor Soc Endocrinol 10: 45-51, 1995).

        H₂ Filter for Time Delay Systems

        An H₂ filter is derived for time delay systems, where there are time delay terms in the state and in the output. A method to compute the H₂ norm of time delay systems is proposed. Based on the H₂ norm computation method, an H₂ filter design is formulated as a nonlinear optimization problem.

        <P>A large number of thioredoxins (Trxs), small redox proteins, have been identified from all living organisms. However, many of the physiological roles played by these proteins remain to be elucidated. We isolated a high M(r) (HMW) form of h-type Trx from the heat-treated cytosolic extracts of Arabidopsis (Arabidopsis thaliana) suspension cells and designated it as AtTrx-h3. Using bacterially expressed recombinant AtTrx-h3, we find that it forms various protein structures ranging from low and oligomeric protein species to HMW complexes. And the AtTrx-h3 performs dual functions, acting as a disulfide reductase and as a molecular chaperone, which are closely associated with its molecular structures. The disulfide reductase function is observed predominantly in the low M(r) forms, whereas the chaperone function predominates in the HMW complexes. The multimeric structures of AtTrx-h3 are regulated not only by heat shock but also by redox status. Two active cysteine residues in AtTrx-h3 are required for disulfide reductase activity, but not for chaperone function. AtTrx-h3 confers enhanced heat-shock tolerance in Arabidopsis, primarily through its chaperone function.</P>
