      • Efficacy of Long-Term Tenofovir-Based Therapy in Chronic Hepatitis B Patients with Previous Nucleos(t)ide Analogues Treatment Experience

        Aims: Tenofovir disoproxil fumarate (TDF) is considered as the preferred treatment option for chronic hepatitis B (CHB) patients with treatment failure or resistance to prior nucleos(t)ide analogues (NAs) treatment. We investigated the efficacy of long-term TDF-based therapy in CHB patients with previous NAs-experience. Methods: This study included total 251 patients who had previous history of NAs therapy and were treated with TDF mono (n=173) or TDF combined with other NA (n=78) from August 2012 to March 2017. Virologic response (VR) was defined as undetectable serum HBV DNA by PCR (<20 IU/mL). Results: Mean age of patients was 49.3 years, median duration of TDF therapy was 27.2 months, 75.7% were HBeAg-positive, and median HBV DNA was 3.7 log10IU/mL. The cumulative rates of VR were 188/244 (77.0%), 180/211 (85.3%), and 146/161 (90.7%) at 12, 24, and 36 months, respectively. Multivariate analysis showed that body mass index (OR 0.77, 95% CI 0.61-0.95, p=0.0189) and duration of TDF therapy (OR 1.09, 95% CI 1.02-1.18, p=0.0221) was significantly associated with VR. TDF monotherapy, HBeAg-positivity, platelet count, serum albumin was associated with VR in the univariate analysis, but not significant in the multivariate analysis. In relation to renal safety, patients showed renal impairment (7, 3.0%), mild hypophosphatemia (15, 7.2%), severe hypophosphatemia (1, 0.4%). Conclusions: Long-term TDF-based therapy demonstrated highly effective in viral suppression and relatively favorable renal safety in CHB patients with previous NA-experience. The body mass index and duration of TDF therapy was independent factors associated with VR.

      • Mutation in PMR1, a Ca^2+ -ATPase in Golgi, Confers Salt Tolerance in Saccharomyces cerevisiae by Inducing Expression of PMR2, an Na^+ -ATPase in Plasma Membrane

        Sodium tolerance in yeast is enhanced by continuous activation of calcineurin, a Ca^2+/calmodulin-dependent protein phosphatase that is required for modulation of the Na^+ efflux mechanism. We isolated several salt-tolerant mutations with the treatment of ethylmethane sulfonate under high salt stress. One of the mutations was mapped in the PMR1 gene. Pmr1p, the P-type Ca^2+ -ATPase in the Colgi apparatus, regulates a cytosolic Ca^2+ level in various responses. Cytosolic Ca^2+ concentration in the pmr1 mutant is highly maintained, and thus calcineurin is activated continuously. The treatment of FK506, a specific inhibitor of calcineurin, abolishes the salt-tolerant phenotype of the pmr1 mutant. Activates caicineurin induces the expression of PMR2, encoding the P-type Na^+-ATPase, through the specific transcription factor, Tcn1p/Crzlp. Also, experssion of the PMR2::lacZ reporter gene in the pmr1 higher than that in wild type. We propose that the pmr1 mutation confers salt tolerance through continuous activation of calcineurin and that Pmr1p might act as a major Ca^2+-ATPase under high salt stress.

      • Comparison of Virologic Response and Renal Safety of Long-Term Antiviral Therapy with Tenofovir and Entecavir in Naive Patient with Chronic Hepatitis B

        Aims: Tenofovir (TDF) entecavir (ETV) are considered as the preferred treatment options for treatment-naïve chronic hepatitis B (CHB) patients. We compared the virologic response and renal safety of long-term TDF versus ETV therapy in naïve CHB patients. Methods: This retrospective study included total 432 patients who were treated with TDF (n=205) or ETV (n=227) from August 2012 to March 2017. Virologic response (VR) was defined as undetectable serum HBV DNA by PCR (<20 IU/mL). Results: Mean age of patients, sex, baseline serum levels of AST, ALT, creatinine, and HBV DNA were not significantly different between TDF and ETV groups. The cumulative rates of VR between TDF and ETV groups were 71.6% vs. 61.8% (p=0.477), 88.1% vs. 79.6% (p=0.058), and 84.8% vs. 83.7% (p=0.966) at 1, 2, and 3 years, respectively. The cumulative rates of genotypic resistance between TDF and ETV groups were 0% vs. 0%, 0% vs. 1.5% (p=0.404), and 0% vs. 2.2% (p=0.447) at 1, 2, and 3 years, respectively. Incidences of renal impairment and hypophosphatemia during treatment up to 3 years were not significantly different between two groups. Multivariate analysis showed that HBeAg-positivity (OR 0.27, 95% CI 0.12-0.58, p=0.0012) and serum HBV DNA at 1 year 2000 IU/mL (OR 0.09, 95% CI 0.03-0.25, p<0.0001) were significantly associated with VR. Conclusions: Long-term TDF and ETV treatments appear to have similar virologic response and renal safety in naïve CHB patient. However, long-term ETV therapy might to be associated with genotypic resistance in 2.2% up to 3 years, while none of patients on TDF therapy did.

The conclusions were as follows: 1. This study measure of each time a record to considerably lower. The need of rest times is enough for after the exercise load. 2. A subject, the number of heart beating increase right after the exercise. 3. Sports massage be enforced administration of medicine, experimental groups, the necessary time of fatigued recovered after the exercise load 22.5 minutes. 4. Sports massage a method of treatment athletic sports treatment's effect on condition and fatigued recovery after the exercise. As a result of analyzing the change of pulse frequency after motion and the arrival time of pulse frequence at the time of rest by synthesizing the above study result, in accordance with the pulse frequency at the time of rest showed the remarkable difference. And, the arrival time of pulse frequency at the time of rest showed the reduction of 2 times or so after the execution in comparison with the time before the execution of sports massage. It can be seen that the sports massage after motion had much influence on the recovery by this. And, though there are various methods for recovering the fatigue after the motion in the point that the fatigue my be recovered within short time and the self-condition maybe maintained by executing the sports massage during match or season, it is deemed that this study result will give the important suggestion point.

      • 일측 요관폐쇄에 의한 실험적 수신증에서 신조직내 MCP-1 및 TNF-α유전자 발현

        Early cellular and molecular derangements have been suggested as potential pivotal factors for the development of renal injury, such as interstitial fibrosis after the experimental hydronephrosis. Several recent studies have been demonstrating that one of the initial events taking place in the progressive renal injury process is the mononuclear cell infiltration in the glomeruli and tubulointerstitiuim. In order to evaluate the renal light microscopic findings as well as the renal cortical MCP-1 and TNF-α gene expressions which are modulating inflammatory process and the recruitment of mononuclear cells in many experimental models of renal injury, Sprague-Dawley rats underwent unilateral ureteral obstruction (UUO) and sham operation under the thiopental sodium anesthesia(50 mg/kg intraperitoneal injection). Three and 7 days after surgery, rats were sacrificed. By the competitive RT-PCR, the levels of TNF-α and MCP-1 mRNA in renal cortical tissues were measured. The magnitude of mononuclear cell infiltration and interstitial widening were evaluated by standard point counting method. The level of MCP-1 gene expression was significantly increased in UUO group compared to sham group at 3 and 7 days after surgery(UUO 3 days 8.99 ± 0.45, UUO 7 days 11.2 ± 0.26, sham 3 days 1.00 ± 0.08, sham 7 days 0.94 ± 0.05, p < 0.01, p < 0.01, mean ± SEM, respectively). The level of MCP-1 mRNA at 7 days after surgery was significantly higher than that at 3 days in UUO group. The TNF-α gene expression level of UUO group was also significantly higher than that of sham group at 3 and 7 days after surgery(UUO 3 days 1.73 ± 0.16, UUO 7 days 2.03 ± 0.21, sham 3 days : 1.00 ± 0.06, sham 7 days 1.09 ± 0.03, p < 0.01, p < 0.01, respectively). But there was no significant difference between the levels at 3 and 7 days after surgery in UUO group(p>0.05). On the light microscopic examination, the relative volume of interstitium (RVI) and the total count of infiltrated mononuclear cells(MCC) at 3 and 7 days after surgery in UUO group were also significantly higher than those of sham group(RVI ; UUO 7 days 14.9±1.0%,sham 6.0±0.7%, MCC; UUO 7days 34.0± 1.6, sham 3.7± 0.6, p < 0.01, p < 0.01, respectively). In conclusion, we speculated that the early up-regulation of MCP-1 and TNF-α genes of renal cortical tissue in this experimental hydronephrosis model is closely related to the mononuclear cell infiltration and tubulointerstitial fibrosis.

        The purpose of this study is to investigate research on the adaptation of elderly couple and this study is the preliminary research for more scientific and wider studies on the adaptation of elderly couple. For the analysis, 19 articles which had been published from 1986 to 1999 were investigated by research contents, research methods, and research findings. The major findings are as following : (1) The variables with affect the adaptation of elderly couple are demegraphic fators, individual factors, couple relationships factors and family relationships factors. (2) For the adaptation of elderly couple, economic status, health condition and emotional relationships with their children have to be improved. It is difficult to generalize the research findings because of the unexguisteness of methodology, the difference of the subject, and the disproportion of research area.

      • 구두 굽 높이가 여성의 동적균형에 미치는 영향

        The purpose of this study was to investigate the effect of heel height on women's dynamic balance. Seven female subjects, aged 19-22 years, volunteered to participate for the study. The subject was asked to lean forward, backward and sideways and to reach as far as possible without moving the feet or bending at the hip, in a plane parallel with the measuring tape attached on the wall at shoulder height. Maximal antero, posterior, and lateral distance reached were measured to evaluate a dynamic balance. Subjects were tested in bare feet and 4 pairs of shoes. Shoes were women's dress pumps identical except for heel height. Heel heights were 3, 5, 7, and 9 cm. The results indicated that increasing heel height did not influence the maximum reach distance in the backward and sideway direction. However, the maximum reach distance in the forward direction decreased significantly with increasing heel hight. Post-hoc analysis revealed that the reach distance was significantly smaller in the 7 and 9 cm heel height. These findings suggest that heel height has a detrimental effect on women's dynamic balance. The poor balance derives from a decrease of postural stability in the forward direction. A critical heel height for producing balance hazards locates between 5 and 7 cm. Key words:heel height, dynamic balance, maximum reach distance.

      • 유산소운동이 비만중년여성의 신체구성 및 혈중지질성분에 미치는 영향

        After to know effect that Eeorobikundongpeurogeuraem gets in middle age women's body composition and hole discontinuance quality rain exercise ago and exercise, body composition weight, sieve local amount, the sieve local rate, several local amount, hole discontinuance quality TC, TG, LDL-C, HDL-C etc. result following conclusion that analyze sounding get . Weight after more than exercise before exercise 2.88 kg decrease (p < 0.01) . The sieve local rate decreased 3.35% after exercise more than before exercise (p < 0.01) . Sieve local amount after more than exercise before exercise 3.2 kg decrease (p <0.01) . Several local sieve gained 2.9 kg after exercise more than before exercise but did not show a car that keep in mind as statistical.
