        Sarcopenia and Post-Operative Morbidity and Mortality in Patients with Gastric Cancer

        Purpose: Surgical resection for gastric adenocarcinoma is associated with significant post-operative morbidity and mortality. The aim of this study was to assess the prognostic significance of sarcopenia in patients undergoing resection for gastric adenocarcinoma with respect to post-operative morbidity and survival. Materials and Methods: A retrospective analysis was conducted on a cohort of consecutive patients who underwent surgical resection for gastric adenocarcinoma between 2008 and 2014. Patient demographics, radiological parameters, and pathological data were collected. OsiriX software (Pixmeo) was used to measure skeletal muscle area, which was normalized for height to calculate skeletal muscle index. Results: A total of 56 patients (41 male, 15 female; mean age, $68.4{\pm}11.9years$) met the inclusion criteria. Of these, 36% (20 of 56) of the patients were sarcopenic pre-operatively. Both sarcopenic and non-sarcopenic patient groups were equally matched with the exception of weight and body mass index (P=0.036 and 0.001, respectively). Sarcopenia was associated with a decreased overall survival (log-rank P=0.003) and was an adverse prognostic predictor of overall survival in multivariate analysis (hazard ratio, 10.915; P=0.001). Sarcopenia was a predictor of serious in-hospital complications in multivariate analysis (odds ratio, 3.508; P=0.042). Conclusions: In patients undergoing curative resection for gastric cancer, there was a statistically significant association between sarcopenia and both decreased overall survival and serious post-operative complications. The measurement and reporting of skeletal muscle index on pre-operative computed tomography should be considered.

        설교에 관한 고전적 지혜를 효과적으로 배우기 위해 우리가 찾아가야 할 스승은 바로 아우구스티누스(Augustinus) 성인이다. 설교는 궁극적으로 설득 하는 것과 관련 있음을, 곧 사람을 움직여 행동하게 하는 것, 아니 그보다는 하느님의 활동에 응답하게 하는 것과 관련 있음을 아우구스티누스는 잘 이해하고 있었다. 하지만 그는 설득이 단순한 한 단계 절차가 아니라는 것 또한 이해하고 있었다. 고전적 수사학 전통의 지혜를 통해 그는 설득하는 기술이 가르치고, 즐겁게 하고, 감동시키기라는 세 가지 요소를 포함하고 있다는 것을 배웠다. 따라서 효과적인 설교란 전인적(全人的)인 설교로서, 가르치고 밝혀주고 교정하고 설명함으로써 마음에 호소하는 설교이다. 상상력을 붙잡고 진리를 제시하는 데에 즐거움을 가미하여 그 진리를 기억하도록 하는 설교며, 탄원하고 꾸짖고 자극하는 말들을 통해 의지를 행동으로 옮기도록 재촉하는 설교이다. 이처럼 삼분화된 고전적 스케마는 모든 설교를 위한 확고한 기초를 제공해 주는데, 그것은 인간을 설득하는 기술과 관련해 수 세기를 거쳐 진실했던 것은 계속해서 오늘날에도 진실하기 때문이다. 새 복음화는 그리스도교의 깊은 뿌리를 간직하면서도 세속화의 증가로 심각한 신앙의 위기를 겪고 있는 문화를 향해 예수 그리스도의 복음을 ‘다시 제시’하도록 우리에게 요청한다.

      • Understanding Expert Knowledge

        The strength and conditioning coach has been identified as a key individual responsible for training and preparing athletes for the physical and mental demands of various sports. However, despite their acknowledged importance in athlete development, little is known about the knowledge possessed by these specialist coaches and the ways in which they have developed this knowledge allowing them to coach at the elite level. The central aim of this study was to identify the ways in which a group of elite, National level Rugby League strength and conditioning coaches acquired and developed their coaching knowledge throughout their careers. Individual semi-structured interviews were conducted with seven Rugby League strength and conditioning coaches currently coaching at the elite level. The results revealed that these coaches developed their knowledge through a range of formal, non-formal and informal learning situations including tertiary education, coaching clinics/seminars/conferences, workplace learning, practical experience, previous athletic experiences, reading and researching, constant observations and mentoring. These findings not only re-enforce the importance of knowledge in effectively coaching strength and conditioning at the elite level of Rugby League, but also highlight the necessity for strength and conditioning coaches to immerse themselves in numerous learning situations to continually develop and expand on their knowledge.

        Computed Tomography Assessment of Intestinal Gas Volumes in Functional Gastrointestinal Disorders

        Background/Aims Many patients with functional gastrointestinal disorders (FGIDs) rank sensations of bloating and distension among their most debilitating symptoms. Previous studies that have examined intestinal gas volume (IGV) in patients with FGIDs have employed a variety of invasive and imaging techniques. These studies are limited by small numbers and have shown conflicting results. The aim of our study was to estimate, using CT of the abdomen and pelvis (CTAP), IGV in patients attending FGID clinic and to compare IGV in patients with and without FGID. Methods All CTAP (n = 312) performed on patients (n = 207) attending a specialized FGID clinic over 10-year period were included in this study. Patients were classified into one of 3 groups according to the established clinical grading system, as organic gastrointestinal disorder (OGID, ie, patients with an organic non-functional disorder, n = 84), FGID (n = 36) or organic and functional gastrointestinal disorder (OFGID, ie, patients with an organic and a functional disorder, n = 87). Two independent readers blinded to the diagnostic group calculated IGV using threshold based 3D region growing with OsiriX. Results Median IGVs for the FGID, OGID, and OFGID groups were 197.6, 220.6 and 155.0 mL, respectively. Stepwise linear regression revealed age at study, gender, and calculated body mass index to predict the log IGV with an r2 of 0.116, and P < 0.001. There was a significant positive correlation between age and IGV in OGID (Spearman’s = 0.253, P = 0.02) but this correlation was non-significant in the other groups. Conclusions Although bloating is a classic symptom in FGID patients, IGV may not be increased compared with OGID and OFGID patients. (J Neurogastroenterol Motil 2012,18:419-425)

        Transcriptome and Proteome Profiling of Primary Human Gastric Interstitial Cells of Cajal Predicts Pacemaker Networks

        Background/AimsInterstitial cells of Cajal (ICC) are specialized gastrointestinal (GI) pacemaker cells required for normal GI motility. Dysfunctions in ICC have been reported in patients with GI motility disorders, such as gastroparesis, who exhibit debilitating symptoms and greatly reduced quality of life. While the proteins, calcium-activated chloride channel anoctamin-1 (ANO1) and the receptor tyrosine kinase (KIT), are known to be expressed by human ICC, relatively little is known about the broad molecular circuitry underpinning human ICC functions. The present study therefore investigates the transcriptome and proteome of ANO1-expressing, KITlow/CD45–/CD11B– ICC obtained from primary human gastric tissue. MethodsExcess human gastric tissue resections were obtained from sleeve gastrectomy patients. ICC were purified using fluorescence-activated cell sorting (FACSorting). Then, ICC were characterized by using immunofluorescence, real-time polymerase chain reaction, RNA-sequencing and mass spectrometry. ResultsCompared to unsorted cells, real-time polymerase chain reaction showed the KITlow/CD45–/CD11B– ICC had: a 9-fold (P < 0.05) increase in ANO1 expression; unchanged KIT expression; and reduced expression for genes associated with hematopoietic cells (CD68, > 10-fold, P < 0.001) and smooth muscle cells (DES, > 4-fold, P < 0.05). RNA-sequencing and gene ontology analyses of the KITlow/CD45–/CD11B– cells revealed a transcriptional profile consistent with ICC function. Similarly, mass spectrometry analyses of the KITlow/CD45–/CD11B– cells presented a proteomic profile consistent with ICC activities. STRING-based protein interaction analyses using the RNA-sequencing and proteomic datasets predicted protein networks consistent with ICC-associated pacemaker activity and ion transport. ConclusionThese new and complementary datasets provide a valuable molecular framework for further understanding how ICC pacemaker activity regulates smooth muscle contraction in both normal GI tissue and GI motility disorders.
