        A Long Way from Transfer to Transition: Challenges for Pediatric and Adult Nephrologists

        Significant advances in the diagnosis and medical care of children with chronic kidney disease (CKD) are major reasons for the better survival rates of children and adolescents with CKD than the survival rates reported in previous decades. These patients are reaching adulthood, and therefore require a transition to adult medical care. This transition phase is well-recognized to be associated with considerably increased morbidities and medical problems, such as non-adherence, graft loss after transplantation, and loss to follow-up. Low adherence increases morbidity and medical complications and contributes to poorer qualities of life and an overuse of the health care system. However, these tragic outcomes may be avoidable through a structured and well-defined transition program. In the last decade, there has been increasing interest to resolve these medical and psychological problems that occur during the transfer of young adult patients from pediatric to adult renal units. The aims of a successful transition from pediatric to adult medical care include enhancing the individual development of better health-competence and stabilizing, or even improving, the state of health. This review will focus on various aspects of the transition phase of adolescents who have CKD or who underwent kidney transplantation from pediatric to adult nephrology care.

      • Femtosecond X-ray Absorption Spectroscopy at a Hard X-ray Free Electron Laser: Application to Spin Crossover Dynamics

        <P>X-ray free electron lasers (XFELs) deliver short (<100 fs) and intense (∼10<SUP>12</SUP> photons) pulses of hard X-rays, making them excellent sources for time-resolved studies. Here we show that, despite the inherent instabilities of current (SASE based) XFELs, they can be used for measuring high-quality X-ray absorption data and we report femtosecond time-resolved X-ray absorption near-edge spectroscopy (XANES) measurements of a spin-crossover system, iron(II) tris(2,2′-bipyridine) in water. The data indicate that the low-spin to high-spin transition can be modeled by single-exponential kinetics convoluted with the overall time resolution.         냉전과 구동독의 성립 1945~1955

        Lemke beobachtet die Ursachen der deutschen Spaltung im Umfeld des Kalten Krieges unter nationalen und internationalen Aspekten. Er geht von einer nach dem Zweiten Weltkrieg noch weitgehend ambivalenten Lage (Jalta und Potsdam) in der deutschen Frage und der Moglichkeit fur eine begrenzte alliierte Nachkriegskooperation aus. Sie habe sich zunachst auch in der Besatzungspraxis gezeigt, sei aber zunehmend von Disharmonie und Konfrontation verdrangt worden. Zum einen hatten Spannungen in den internationalen Beziehungen negativ auf das VerhaItnis der Weltmachte in der deutschen Frage eingewirkt; zum anderen habe die UdSSR durch ihre Politik in der SBZ und deren schrittweise Sowjetisierung einen ostdeutschen Sonderweg eingeleitet. Er zerstorte gewachsene politische und gesellschaftliche Strukturen im ostlichen Teil Deutschlands und fuhrte durch die Ubertragung sowjetischer Modelle auf die SBZ zu einer kommunistischen Systemveranderung, die mit der Entwicklung in den westlichen Besatzungszonen unvereinbar war. Vor allem die USA gingen ab erwa Mitte 1947 angesichts sowjetischer Kompromissunfahigkeit auf einen antikommunistischen Gegenkurs, der sich weltweit in die Politik der Eindammung des sowjetischen Einflusses einordnete. Parallel dazu begann die 1946 durch die Zwangsvereinigung von ostzonaler KPD und SPD entstandene SED mit einer inneren Herrschaftssicherung, die nichtkonforme Krafte aus Politik und Gesellschaft der Ostzone ausschaltete und den antidemokratischen Umbruch cler ostcleutschen Gesellschaft mit sowjetischer Hilfe durchsetzte.

        Reoperation in Acquired Involutional Ptosis

        Postoperative upper lid asymmetry is a common problem following ptosis surgery. Recently we performed multiple ptosis operative procedures to correct lid asymmetry in the management of a patient with bilateral acquired upper eyelid ptosis. The patient’s eyelids were corrected successfully after five procedures. We retrospectively reviewed the treatment of this patient with unsatisfactory results. The medical literature was reviewed for further insight into the common problem of reoperation after ptosis surgery.

      • Retinal degeneration triggered by inactivation of PTEN in the retinal pigment epithelium.

        <P>Adhesion between epithelial cells mediates apical-basal polarization, cell proliferation, and survival, and defects in adhesion junctions are associated with abnormalities from degeneration to cancer. We found that the maintenance of specialized adhesions between cells of the retinal pigment epithelium (RPE) requires the phosphatase PTEN. RPE-specific deletion of the mouse pten gene results in RPE cells that fail to maintain basolateral adhesions, undergo an epithelial-to-mesenchymal transition (EMT), and subsequently migrate out of the retina entirely. These events in turn lead to the progressive death of photoreceptors. The C-terminal PSD-95/Dlg/ZO-1 (PDZ)-binding domain of PTEN is essential for the maintenance of RPE cell junctional integrity. Inactivation of PTEN, and loss of its interaction with junctional proteins, are also evident in RPE cells isolated from ccr2(-/-) mice and from mice subjected to oxidative damage, both of which display age-related macular degeneration (AMD). Together, these results highlight an essential role for PTEN in normal RPE cell function and in the response of these cells to oxidative stress.</P>
