        Currently, in the limited cases of early gastric cancer (EGC), endoscopic mucosal resection (EMR) is used for curative treatment. However, its completeness is still controversial. The purpose of this study was to review the suggested indications of EMR. We retrospectively analyzed 234 patients with EGC who underwent curative (RO) gastrectomy. The enrolled cases were further classified into mucosal and submucosal types, and mucosal EGC were further classified according to the size, gross type and histologic pattern. 133 patients had mucosal cancers and 101 patients submucosal. The overall incidence of positive lymph node was 14.1% (33/234), the incidence of mucosal cancer was 8. 3% (11/133), which was significantly lower than that of submucosal, 22% (22/101). Of 133 patients with mucosal EGC, 90 were smaller than 2㎝ in size and the incidence of lymph node metastasis was 5.6% (5/90), compared to 43 of larger than 2㎝ with the incidence of 14% (6/43). According to the gross type classification of mucosal cancer, incidence of lymph node metastasis in protruded type (EGC type Ⅰ, Ⅱa, Ⅰ+Ⅱa) was 15.4% (4/26), nat type(EGC type Ⅱb) 2. 9% (1/35), depressed type (EGC type Ⅱc, Ⅲ) 8.3% (6/72). Lymph node metastasis in differentiated type of mucosal cancer was 6.9% (5/72) and undifferentiatcd type 9.8% (6/61). In this study, there was no single indication free from lymph node metastasis, It is thought that the selection of EMR for the curative treatment of EGC should be considered carefully and more clinical studies with long-term follow up are necessary for EMR to be generally acceptable.

        A study on the Byungtae Ron(病能論) of the So Moon(素問), one of the classic book of oriental medicine, was carried out analytically for the right comments. The principal ideas obtained summarized as follows; Neng(能) is similar to tae(熊) in the pronunciation and the meaning of a chinese character. Byungtae(病能) means the state of the symptoms and the pulse condition of the diseases, etc. It deals with ethological factor, mechanism of disease, the condition of an illness, method of diagnosis, rule of treatment, technique of treatment of seven diseases and taking this seven diseases as a example, mainly discuss the technique and significance of clinical observation and the pathogenesis and condition of an illness, so It named 「Byungtaeron(病能論)」 In this chapter it shows we can grasp condition of illness well only if we classify the pathogenesis and condition of an illness in details. this viewpoint provides the foundation of development in oriental diagnostics. It places emphasis on pulse feeling in this chapter, so it describes pulse condition of various kinds of diseases in details. It also presents how to compare pulse feeling like comparison between Inyoungmaek(人迎脈, arterial carotid communes, the pulsation of which is easily felt beside Adam's apple) and Ch'onkumaek(寸口脈, the radial artery on the wrist where the pulse is routinely taken for diagnosis), between right and left Ch'onkumaek(寸口脈), between Yangmyungsangdong (陽明常動) and Taeyangbudong(太陽不動), Soyangbudong(少陽不動), etc. In this chapter it explains pulse feeling occupies a high position in the diagnosis of diseases in 『Huangdi's internal Classic(黃帝內經)』 period and the technique of pulse feeling is already plentiful and minute. In this chapter it presents 「Tongbyungyich(同病異治, to apply different methods of treatment to the same kind of disease)」 theory, taking tangible syndromes as examples. this is prominent viewpoint of determination of treatment based on the differentiation of symptoms and signs and much influence development of later ages' medicine. On the ground of oriental medical theory, it may bring about different symptoms in the same disease according to stage of disease, the seasons, the regions, the constitution of patient, therefore surely adopt different technique of treatment, this differentiation of symptoms and signs is an important character in oriental medicine.

Using various tools with differentgeometry, relationships between tool geometry parameter (rake angle, clearance angle, length of cutter) and cutting process (cutting force, surface accuracy, surface roughness) have been studied.         Post-stroke Aphasia as a Prognostic Factor for Cognitive and Functional Changes in Patients With Stroke: Ischemic Versus Hemorrhagic

        Objective To investigate the comprehensive outcomes in aphasic patients, including their cognitive and functional status after ischemic or hemorrhagic stroke. It also aimed to clarify whether aphasia is a prognostic factor for cognitive and functional improvements in stroke patients. Methods Sixty-seven ischemic or hemorrhagic stroke patients in the subacute stage who had been diagnosed with aphasia using the Korean version of Frenchay Aphasia Screening Test (K-FAST) were included in the study. Forty-six stroke patients without aphasia were used as controls. All patients were examined with the Korean version of the Western Aphasia Battery (K-WAB). Cognitive and functional assessments of the patients including the Korean version of Mini-Mental State Examination (K-MMSE), and the Korean version of Modified Barthel Index (K-MBI) were performed during admission and 4 weeks after the initial assessments. Results The initial and follow-up total K-MMSE and K-MBI scores were significantly lower in aphasic patients than in non-aphasic controls. The K-WAB scores highly correlated with the total K-MMSE scores at the follow-up stage in all aphasic stroke patients. The K-WAB scores moderately correlated with the follow-up scores of the K-MBI in ischemic stroke patients but not in hemorrhagic stroke patients. Conclusion Aphasia influences the cognitive and functional status of stroke patients and has a greater impact on cognitive improvement. Aphasia severity can be one of the prognostic factors for cognitive status in aphasic patients with stroke.

      • Development of HDF Brower for the Utilization of EOC Imagery

        HDF (Hierarchical Data Format) is the standard file format for storing and distributing EOC data. It was created to help scientists reduce the time they were spending trying to convert data sets to familiar formats and spend more time to analyze data. HDF is a physical file format that allows storage of many different types of scientific data including images, multidimensional data arrays, record oriented data, and point data. EOC images will be distributed to the public or many user groups including government, public corporations, academic or research institutes. KARI will offer the online service to the users through Internet. Some application, like generation of Digital Elevation Model (DEM) needs a secondary data such as satellite ephemeris data, attitude data to process the EOC imagery. EOC imagery with these secondary data will be stored in a file of Hierarchical Data Format (HDF) file format. By the lack of softwares supporting HDF file format, the public users may not access EOC data without difficulty. In this paper, we develop an HDF browser which visualizes all the objects stored in a HDF file. It has a user-friendly interfaces through which they can access various type of objects in a file and extract any type of objects and stores them externally. In addition to the normal utilities browsing the contents of HDF file, it can handle the EOC data specially.

        Retrobulbar Hematoma in Blow-Out Fracture after Open Reduction

        Retrobulbar hemorrhage, especially when associated with visual loss, is a rare but significant complication after facial bone reconstruction. In this article, two cases of retrobulbar hematoma after surgical repair of blow-out fracture are reported. In one patient, permanent loss of vision was involved, but with the other patient, we were able to prevent this by performing immediate decompression after definite diagnosis. We present our clinical experience with regard to the treatment process and method for prevention of retrobulbar hematoma using a scalp vein set tube and a negative pressure drainage system.

        Clinical Follow-up on Sagittal Fracture at the Temporal Root of the Zygomatic Arch: Does It Need Open Reduction?

        Background The zygoma is a major portion of the midfacial contour. When deformity occurs in this area, a reduction should be conducted to correct it. If a sagittal fracture at the temporal root of the zygomatic arch occurs, this also requires reduction, but it is difficult to approach due to its anatomical location, and the possibility of fixation is also limited. Thus, the authors attempted the reduction of sagittal fracture by two- or three-point fixation and the Gillies approach without direct manipulation. The preoperative and postoperative results of the patients were evaluated. Follow-up was performed to establish a treatment guideline. Methods A retrospective study was done with 40 patients who had sagittal fractures at the temporal root of the zygomatic arch from March 2009 to June 2012. Only two- or three-point fixation was performed for the accompanying zygomatic-orbital-maxillary fracture. The Gillies approach was used for complex fractures of the zygomatic arch, while the temporal root of the zygomatic arch was only observed without reduction. Preoperative and postoperative computed tomography and X-ray scans were performed to examine the results. Results The result of the paired t-test on preoperative and postoperative bone gap differences, the depression level, and the degree of temporal protrusion showed a marked decrease in the mean difference at a 95% confidence interval. The results were acceptable. Conclusions In the treatment of sagittal fractures at the temporal root of the zygomatic arch, it is acceptable to use indirect reduction and non-fixation methods. This leads to a satisfactory aesthetic and functional outcome.
