급성백혈병의 화학요법에서 조혈간세포 보충요법

1. 목적 급성백혈병의 화학요법에 있어서 관해유도 요법시에 보다 더 강력한 항암화학요법을 시행하고 골수억제의 회복을 위하여 말초혈액에서 채취한 조혈모세포의 투여로 그 효과를 증대시키고자 시도하였다. 2. 방법 관해유도 화학요법후에 골수억제시기로부터 회복단계로서 백혈구감소증이 호전도면서 단핵세포가 증가될 때에 말초혈액에서 조혈간세포(stem cell)로 인정되고 있는 CD34+세포의 함유량이 1% 이상되는 시기를 찾아내고 그 때부터 평균 4차례의 단핵구를 분리채집을 하여 -74。C에 냉동보관하였다가 제2차 화학요법후에 해동시켜서 정맥주입하였다. 3. 결과 대상 급성백혈병환자는 6명에 23회의 조혈간세포를 함유한 단핵구 채집을 시행하였고 유효한 단핵구의 채집은 4례에서 가능하였고 2례에서는 유효한 채집이 못되었다. 채집효율은 39.0-74.5%로서 평균 49.8%였다. 채집 후에 혈액학적 변화는 혈색소의 약간 감소 (평균 1.34g/dl)와 혈소판의 감소 (평균20.25%)를 나타내었다. 해동 단핵구를 투여할시에 일과성인 불쾌감, 기침, 흉부압박감, 호흡곤란 등이 있었다. 4. 결론 자가골수이식에서 자기말초혈액조혈간세포이식으로 관심이 이행되는 현시점에 자가조혈간세포를 채집하여 수혈하는 기술이 확립되는 기초를 이루었다고 본다. 이번 대상들은 재발된 금성백혈병들에 시행하였으나, 향후에는 초발시에 제일차 관해유도 화학요법 과정에 시도해야할 것으로 생각한다. Object: Peripheral blood stem cells(PBSC) were collected after remission induction chemotherapy and reinfused after intensified chemotherapy in order to increase the chemotherapeutic efficacy. Method: Collection of mononuclear cells (MNC) was started when CD34+ cells above 1% and WBC above 1,000/ul with mononuclear cell percentage above 30%, the collection procedures were done 2-6 time in each patient of total 6 cases. The collected MNCs were stored in - 750C and reinfused after thawing in the water bath at 370C Results: Twenty three collection procedures from the 6 cases of acute leukemia had made effective MNC collection in 4 cases and mean collection efficiency was 49.8%(from 39.0% to 74.5%). Hematological changes after the procedure were mean reduction of hemoglobin 1.34g/dl and mean 20.25% of decreased platelet count. There were noticed transient symptoms of discomfort, coughing, chest tightness and dyspnea in association with the infusion of stored cells. Conclusion: The basic process for the peripheral blood stem cell transplantation (PBSCT) was accomplished with this supportive care for the consolidation chemotherapy of acute leukemia. Effective PBSCT would be warrented with more qualified process and high does chemotherapy. (Korean J Blood Transfus 6(2) : 41~47,1995)

부신피질 자극호르몬 단독결핍 1례

부신피질 자극호르몬 단독결핍은 국내에서 7례만이 보고된 희귀 질환이며 병인 및 경과 등 아직 불분명한 측면이 많다. 이는 질환지체의 희귀성도 있지만 치료후 자연경과에 대한 장기적인 추적검사가 이루어진 경우가 드물기 때문이기도 하다. 저자들은 비특이적인 전신증상을 주소로 내원한 폐경기후의 56세 여자 환자에서 자가면역질환의 증거없이 고프로릭틴혈증을 보인 부신피질 자극호르몬 단독결핍 1례를 경험하고 1년간의 부신피질호르몬 대치요법후 추적검사를 실시하였기에 문헌 고찰과 함께 보고하는 바이다. Isolated ACTH deficiency is a rare disorder which shows secondary adrenal insufficiency with normal states of other pituitary hormones. A 56-year-old postmenopausal woman complaining general weakness and intermittent mental confusion presented negligible serum cortisol and ACTH level with hyperprolactinemia and mild hyponatremia. Rapid ACTH stimulation test did not show significant increase of serum cortisol level. Combined pituitary stimulation test showed negligible response of cortisol and ACTH, and slightly decreased response of prolactin and gonadotropin. But the responses of TSH and GH were normal. There was no evidence of organic pituitary of hypothalamic lesion in brain MRI and antithyroid autoantibody was not detected in serum. After receiving 12 months of glucocorticoid replacement therapy, she skipped the medicine for 3 weeks and we performed follow-up endocrinologic tests. Serum ACTH and cortisol level was also negligible and prolonged ACTH simulation test showed normal response of serum cortisol. CRF stimulation test showed no response of serum ACTH. Basal prolactin level was decreased to near normal and combined pituitary stimulation test showed normal response of prolactin with slightly blunted response of TSH and GH. We concluded that the case was nonautoimmune origined isolated ACTH deficiency which was accompanied by hyperprolactinemia and resultant deficiency of gonadotropin, so we report this case with review of literatures.

림프종 환자에서의 갈륨, TI-201 그리고 Tc-99m MIBI 섭취의 비교

Purpose; Ga-67 scintigraphy has been used for the evaluation of tumors, especially lymphoma. Recently, Tl-201 and Tc-99m MIBI were also used to tumor imaging. Tl-201 and Tc-99m MIBI had better physiologic characteristics than Ga-67, so we studied 32 biopsy proven lymphoma patients (male 24, female 8, mean age 46 years) with Ga-67, Tl-201 or Tc-99m MIBI and compared the scan findings. Materials and Methods: Twenty-three of 32 patients were injected 74-111 MBq (2-3 mCi) of Tl-201, before chemotherapy and imaged with dual-headed SPECT (Prism 2000, Picker, USA) at 30 minutes after injection. Delayed images were obtained after 3 hr in 8 patients. Twenty seven of 32 patients were injected 740 MBq (20 mCi) of Tc-99m MIBI and imaged at 30 minutes after injection. 111-185 MBq (3-5 mCi) of Ga-67 was injected in 12 patients and imaged at 48 and 72 hours after injection. Twenty eight patients were diagnosed as non-Hodgkin's lymphoma and others were Hodgkin's lymphoma. Results: Twenty patients were positive on Tl-201 scan and 3 patients showed negative findings. One of these 3 patients, Tc-99m MIBI and Ga-67 scan were positive. Twenty two patients were positive on Tc-99m MIBI scan and 5 patients showed negative findings. One of these 5 patients, Tl-201 was positive and 2 were positive on Ga-67 scan. Ten of 12 patients showed positive findings on Ga-67 scan. The sensitivity of these agents were 83.3%, 87.0% and 81.5% for Ga-67, Tl-201 and Tc-99m MIBI, respectively. The sensitivity was highest in Tl-201 scan, but there were no significant differences among three tests. In this study, there was no significant difference of uptake ratios between early and delayed images of Tl-201. Conclusion: Scintigraphy with Tl-201 and Tc-99m MIBI in lymphoma patients have similar sensitivity with Ga-67.

Mature cystic teratomas comprise 10% of all ovarian tumors and show the highest frequency of incidence in women under 20 years of age. Torsion of ovarian cyst is the most common complication in young female and lower abdominal pain, peritonitis, necrosis and shock are outcomes that require emergency treatment. Generally, gross finding of mature cystic teratoma shows that most of the entire ovary is replaced by multiple cystic masses, and rarely only small portion of the ovary is changed into cystic mass. We report a case of torsion of mature cystic teratoma in an 11 year old girl with a brief review of literature.

The nature of the signals collected by an SEM(Scanning Electron Microscope) in order to form images are all dependent on the detector used to collect them, and the quality of an acquired image is strongly influenced by detector performance. Therefore, the development of detector with high performance is very important in pulling up the resolution of SEM This study presents the secondary electron detector for use in scanning electron microscope, electric circuit and I/V conversion circuit for driving that detector.

工業系 中等敎員의 資格硏修成果에 관한 調査硏究

The aims of this study is to survey the achievements of the in-service training which was executed by the In-Service Teacher Training Institute of Chungnam National University during the summer vacation in 1984. After analyzing the responses to the questionnaire which were collectted from 435 technical teachers attending the in-service training, we reached following conclusions as suggestions. 1) It is desirable to establish a committee to decide the levels and contents of the major subjects under the in-service teacher training institute. 2) Lessening the numbers of major subjects and extending the hours of each subjects will make effective results on in-service technical teacher training. 3) It will be better to give a chance to teachers to take option in the major study field. 4) The Ministry of Education should make a plan to give a course on computer programming to technical teachers regularly.

영상압축표준과 MPEG-2 기술

So we have investigated the current state-of-the-art image compression technology, and also studied MPEG-2 core technology thoroughly.

이질균의 진단용 단클론 항체 개발에 관한 연구

이질균의 항원구조는 매우 복잡하기 때문에 Shigella 균종 중에서 Shigella flexneri를 선택하여 이질균의 항원 특성을 규명하고자 본 실험을 시도하였다. 그래서 본 교실에서 분리 계대해 온 S. flexneri의 균체 항원을 2회에 걸쳐 마우스의 복강내로 주사하였다. 면역된 마우스의 비장에서 임파구를 분리하여 polyethylene glycol을 이용하여 골수종 세포인 P3X63-Ag8.653과 융합하였다. HAT 배지에서 융합 세포를 선택하였다. 무한대 희석법에 의하여 융합 세포의 클로닝을 실시하였으며 항체의 생산은 간접 효소면역흡착법을 이용하여 확인하고 다음과 같은 결과를 얻었다. 1. 세포 골수종 P3X63-Ag8.653의 평균 분열 주기는 대수 증식기에서 12시간이었다. 2. 융합이 실시된 20개의 plate 중에서 융합 세포가 자란 것은 15 well이었다. 3. 융합 세포가 형성된 15 well에서 항체 형성이 검증된 well은 3개였다. 4. 효소면역흡착법을 통해 알아본 각 well에서 생성된 면역글로불린의 class는 2 well에서 IgG, 1 well에서 IgM이었다. 5. 클로닝 후 IgM의 생성이 확인된 세포 배양 상청액을 이용하여 다른 장내세균의 균체 항원과 반응을 시켰으나 반응이 나타나지 않았다. The antigenic structure of Shigella is very complex. So, production of monoclonal antibodies against Shigella flexneri which had been the most prevalent Shigella spp. in Korea was tried to investigate the characteristics of shigella antigens. Heat-killed cellular antigens of S. flexneri which had been cultivated in our laboratory were injected intraperitoneally into BALB/c mice twice at intervals. Spleen lymphocytes of immunized mice were fused with the myeloma P3X63-Ag8.653 cells using polyethylene glocol. Only hybridized cells grew in HAT medium. Supernatants from cultures were assayed for S. flexneri-specific antibodies by the indirect enzyme-linked immunosorbent assay. Hybridomas were cloned by limiting dilution. 1. The mean doubling time of P3X63-A8.653 myeloma cells was 12 hours. 2. Only 15 wells out of 20 plates cell fusions performed showed the growth of hybridized cells in the selecive medium. 3. S. flexneri-specific antibodies were generated from 3 wells of those 15 wells, checked by indirect ELISA. 4. Supernatants from 3 wells had different classes of immunoglobulins : IgG from 2 wells and IgM from 1 well. 5. After cloning, supernatant from SFB 11 checked as Ig M with the strongest visuality was not reacted with cellular antigens of other strains of Enterobacteriaceae.
