        Doppler ultrasound를 이용한 교정적 치아 이동 시 치수 혈류량의 변화의 측정 : 예비실험

        교정적 치아 이동 시 지속되는 압력에 의해 혈류변화가 발생하게 되며, 이러한 변화양상에 있어서 선학들의 연구 방법 및 그 결과의 다양성이 존재하였다. 본 연구에서는 Doppler ultrasound를 이용하여, 교정치료 전과 교정치료 시작 3주, 6주 후 치수 혈류량의 연속적인 변화를 알아보고자 하였다. 만 15세 이상 환자 18명을 연구대상으로 Doppler ultrasound를 이용하여 상, 하악 6전치 중 경도의 총생(2 mm 미만)을 보이는 치아(총생측정: required space-available space)와 인접치아를 포함한 3개의 치아를 대상으로 치수 혈류량의 변화를 알아보았다. 연구결과 부위별(상, 하악), 치아별, 기간에 따른 치수 혈류량의 변화는 교정 치료 시작 전과 시작 후 3주, 6주 혈류량에 유의한 차이가 없었다. 또한 치수 생활력 상실의 내재적 위험성을 비교하고자 교정 치료 시작 전 치아별로 치수혈류량을 비교하였을 때, 상악에서는 측절치, 하악에서는 견치에서 통계학적으로 유의한 차이는 나타나지 않았지만, 모든 항목에서 적은 값을 나타냈다 (p > 0.05). 본 연구의 결과는 이후에 진행될 Doppler ultrasound의 치아이동유형, 환자의 연령을 고려한 세부적인 실험 시 방법론적인 기초 자료로서 뿐만 아니라, 교정치료 시 치수 생활력의 상실에 대한 참고 자료가 될 수 있을 것이다. Objective: This study was to change of pulp blood flow among maxillary and mandibular anterior tooth with mild crowding and adjacent teeth using Ultrasound Doppler graphy. Methods: The change of pulp blood flow was measured three times using Ultrasound Doppler graphy; before the attachment of brackets, after 3 week, and after 6 week. The sample consists of 15 year old eighteen patients. Results: Before the attachment of brackets, after 3 weeks, and after 6 weeks, there were no significant differences in the change of pulp blood flow in each part (maxilla and mandible) and each tooth according to period. In addition, to compare internal dangerousness of loss of the pulp vitality, when pulp blood flow is compared in each tooth before orthodontic treatment, there were no statistically significant differences in maxillary lateral incisor and mandibular canine but it showed low values in all measurement items (p > 0.05). Conclusions: Results of this study can be not only methodological preliminary data in further study such as tooth movement type of Ultrasound Doppler graphy and particular study considered the patient age, but also reference materials for the loss of pulp vitality in orthodontic treatment.

        과두과증식을 동반한 안면비대칭 환자에서 과두절제술의 적용과 효과

        과두과증식은 하악의 3차원적인 골격적인 비대가 일어나는 병적인 상태이다. 이러한 과두과증식의 원인은 호르몬의 작용, 외상, 감염, 유전, 태아기 때의 요인, hypervascularity 등의 요인으로 알려져 있다. 과두과증식의 진단 시 가장 중요한 것은 과두과증식 상태가 아직도 활성화 상태인지 판단하는 것이다. 이러한 과두과증식의 상태에 따라서 안면비대칭 환자의 치료는 이환측 과두의 성장 부위를 제거하는 과두절제술을 시행하는 방법, 성장이 완료될 때까지 기다렸다가 통상적인 악교정 수술만을 시행하거나 과두절제술을 병행하는 시술방법이 있다. 과두과증식의 활성화 상태를 판단하는 것은 치료 안정성에 매우 중요한 요인이며, bone scan이나 주기적인 3차원 컴퓨터 단층촬영이나 정모두부방사선사진 등을 통하여 확인할 수 있다. 본 보고에서는 과두과증식을 동반한 안면비대칭 환자를 과두절제술을 이용하여 개선한 증례를 소개하고자 한다. Condylar hyperplasia is a pathologic condition showing 3-dimensional skeletal hyperplasia of the mandible. The reason for condylar hyperplasia is not yet known, but the effects of hormone, trauma, infection, genetics, fetal condition, and hypervascularity are known as possible reasons. When we diagnose a patient as having condylar hyperplasia, it is important to decide if it is in progress or not. Treatment for facial asymmetry due to condylar hyperplasia are decided accordingly, including condylectomy, that is removal of growth site of the affected condyle, and conventional orthognathic surgery only or condylectomy with orthognathic surgery after the completion of growth. Therefore, it is important to determine the growth state of condylar hyperplasia in treatment stability. This is verified through bone scan and regular check-ups with 3D CT or PA cephalogram. This case report introduces an improved case of facial asymmetry with condylectomy together with orthognathic surgery.

        TALP-35 purified from human term placenta is known to increase microtubule polymerization and stabilize the polymerized microtubule. To examine the effect of TALP-35 on immune system this study was performed. MTT assay was performed to investigate the effect of TALP-35 on the proliferation of RAW264.7 cells. TALP-35 dose dependently suppress the proliferation of RAW264.7 cells at high concentration (above 1 μM) in unstimulated cells, in case of 10 μM TALP-35 treated cells the suppression was 25% but in stimulated cells it was only 15%. Cosedimentation assay was carried out to investigate whether TALP-35 can bind to tubulin of RAW264.7, monocyte/macrophage lineage of mouse, and polymerize it. TALP-35 polymerize the tubulin of RAW264.7 cells and sedimented in dose-dependent manner. To investigate the effect of TALP-35 on the expression of iNOS protein western blotting was performed. The expression level of iNOS was decreased dose dependently in high concentration of TALP-35 treatment. To examine the activity of iNOS, secreted NO was determined by method based on Griess reaction. Interferon-γ and LPS-stimulated production of NO from RAW264.7 cells was decreased dose dependently above 0.1 μM concentration of TALP-35 and 50% is decreased at 10μM of it. This study shows TALP-35 can control cytokine induced-iNOS expression therefore it might control inflammatory diseases.

        CIIA negatively regulates the Ras–Erk1/2 signaling pathway through inhibiting the Ras-specific GEF activity of SOS1

        <P>Son of sevenless 1 (SOS1) is a Ras-specific guanine-nucleotide-exchange factor (GEF) that mediates intracellular signaling processes induced by receptor tyrosine kinases. In this study, we show that CIIA (also known as VPS28) physically associates with SOS1 and thereby inhibits the GEF activity of SOS1 on Ras, which prevents the epidermal growth factor (EGF)-induced activation of the Ras–Erk1/2 pathway. Furthermore, CIIA inhibited cyclin D1 expression, as well as DNA, synthesis in response to EGF. Intriguingly, CIIA failed to inhibit the Ras-specific GEF activity of Noonan-syndrome-associated SOS1 mutants (M269R, R552G, W729L and E846K). Taken together, our results suggest that CIIA functions as a negative modulator of the SOS1–Ras signaling events initiated by peptide growth factors including EGF.</P>

        CIIA functions as a molecular switch for the Rac1-specific GEF activity of SOS1

        <▼1><P>CIIA mediates the TGF-β–induced activation of SOS1–Rac1 signaling and cell migration.</P></▼1><▼2><P>Son of sevenless 1 (SOS1) is a dual guanine nucleotide exchange factor (GEF) that activates the guanosine triphosphatases Rac1 and Ras, which mediate signaling initiated by peptide growth factors. In this paper, we show that CIIA is a new binding partner of SOS1. CIIA promoted the SOS1–Rac1 interaction and inhibited the SOS1–Ras interaction. Furthermore, CIIA promoted the formation of an SOS1–EPS8 complex and SOS1-mediated Rac1 activation, whereas it inhibited SOS1-mediated activation of Ras. Transforming growth factor β (TGF-β) up-regulated the expression of CIIA and thereby promoted the association between CIIA and SOS1 in A549 human lung adenocarcinoma cells. Depletion of CIIA in these cells by ribonucleic acid interference inhibited the TGF-β–induced interaction between SOS1 and EPS8, activation of Rac1, and cell migration. Together, these results suggest that CIIA mediates the TGF-β–induced activation of SOS1–Rac1 signaling and cell migration in A549 cells. They further show that CIIA functions as a molecular switch for the GEF activity of SOS1, directing this activity toward Rac1.</P></▼2>

        Full Arch Restoration through Orthognathic Surgery after Implantation on the Patients with Mandibular Prognathism and Loss of Posterior Teeth: A Case Report

        In case of loss of many teeth due to dental caries or periodontal disease, improvement of masticatory function and aesthetics can be obtained through implant treatment. However, if the patient does not have a normal intercondylar relationship, it is difficult to achieve an ideal occlusal relationship with only prosthetic treatment. In particular, oral reconstruction with orthodontic treatment or orthognathic surgery is necessary for patients with mandibular prognathism. However, if the posterior occlusion collapses due to severe caries or periodontal disease, orthognathic surgery may be difficult. The occlusal vertical stop is very important for the stability of the mandibular position during occlusal reconstruction through orthognathic surgery. The patient in this case had posterior occlusion collapsed due to the caries of a large number of posterior teeth, and showed mandibular prognathism and long face. We planned a full arch restoration with orthognathic surgery and extracted the hopeless teeth. To secure the vertical stop required for orthognathic surgery, the implant was placed before surgery. After the orthognathic surgery and the final prosthesis application, the results were satisfactory for the improvement of the aesthetics and the restoration of the masticatory function.

        韓國人에서 重合酵素反應으로 分析한 DXS52 遺傳座位의 遺傳的 多樣性

        The DXS52 locus was studied in a Korean population sample(n=142) for its allele and genotype frequencies and applicability to identity and paternity testing, using polymerase chain reaction with electrophoresis of PCR products and subsequent detection of allelic fragments by ethidium bromide staining. In Korean population, 15 alleles with the frequency range of 0.319-0.005 are detected in the DXS52 locus, and the highest allele frequency among them is found to be allele 1(0.319). The correlation coefficient between gene frequencies detected in female in mali is 0.9334, indicating that the gene frequency in males follows that of females in Korean population. This locus studied meets Hardy-Weinberg expectation : there is good agreement between the observed and expected values under Hardy-Weinberg equilibrium. The genotypes 1-12(0.118) are mostly frequent found among the 29 genotypes detected in Korean female, but the 91 genotypes are not found. The observed heterozygosity for this locus is 0.809, while the unbiased expected estimate of heterozygosity is 0.827 calculated by the expectation of Hardy-Weinberg equilibrium(p>0.05). The power of discrimination(PD) and power of exclusion(PEX) calculated for this locus is 0.9770(97.70%) and is 0.7068(70.67%), respectively. Thus, the allelic and genotypic data estimated for DXS52 locus can be used as the database of PCR-based DNA profile to human identity testing.

      • CIIA negatively regulates the EGF-activated EGFR-Grb2-SOS-Ras signaling by targeting the son of senvenless 1 (SOS1)

        We have recently reported a novel anti-apoptotic protein named CIIA(a CAD inhibitor that interacts with ASK1) that antagonizes both ASK1- and CAD-mediated processes. To further understand the function of CIIA, we searched for CIIA-interacting proteins by using a pull-down assay. This in vitro binding analysis revealed that CIIA directly interacts with son of sevenless 1(SOS1), the guanine nucleotide exchange factor(GEF) of Ras. Furthermore, endogenous Ciia physically associated with endogenous SOS1 in intact cells, and this interaction was enhanced after cells were exposed to epidermal growth factor(EGF). Overexpressed CIIA blocked EGF-induced Ras activation, thereby inhibiting the EGF-initiated stimulation of the ERK pathway. CIIA, by binding to the Ras interacting domain of SOS1, interfered with the interaction between SOS1 and Ras. Finally, CIIA suppressed the EGF-induced stimulation of DNA synthesis in MDCK cells. Taken together, these results suggest that CIIA functions as a negative regulator of the EGFR-Grb2-SOS-Ras signaling events.

        증례 : 한방비만치료 후 초음파 영상에서 비알코올성 지방간의 호전을 보인 환자 치험 1례

        Objectives The purpose of this study was to investigate the clinical effect of oriental obesity therapy on obese patient with non-alcoholic fatty liver disease(NAFLD). Methods The patient was treated from November 17, 2008 to May 15, 2009 for obesity treatment. And the patient had intensive care through admission treatment from November 17, 2008 to November 29, 2008. Electrolipolysis, auricular acupuncture, herbal medicine, low calorie diet and aerobic exercise were done during the treatment period. Results His body weight decreased from 102.8kg to 82.7kg. The BMI decreased from 35.2kg/m2 to 28.9kg/m2. NAFLD effectively improved on sonography from grade Ⅱ~Ⅲ to grade Ⅰ~Ⅱ after oriental obesity therapy. AST decreased from 65 to 19. ALT decreased from 152 to 18. Oriental obesity therapy could be effective to treat obese patient with NAFLD. Conclusions The obese patient with NAFLD improved on sonography by oriental obesity therapy.
