        A National Consensus Survey for Current Practice in Brain Tumor Management I: Antiepileptic Drug and Steroid Usage

        Background: The Guideline Working Group of the Korean Society for Neuro-Oncology (KSNO) conducted a nationwide questionnaire survey for diverse queries faced in the treatment of brain tumors. As part I of the survey, the aim of this study is to evaluate national patterns of clinical practice about antiepileptic drug (AED) and steroid usage for management of brain tumors. Methods: A web-based survey was sent to all members of the KSNO by email. The survey included 9 questions of AED usage and 5 questions of steroid usage for brain tumor patients. All questions were developed by consensus of the Guideline Working Group. Results: The overall response rate was 12.8% (54/423). Regarding AED usage, the majority of respondents (95.2%) routinely prescribed prophylactic AEDs for patients with seizure at the peri/postoperative period. However, as many as 72.8% of respondents prescribed AED routinely for seizure-naive patients, and others prescribed AED as the case may be. The duration of AED prophylaxis showed wide variance according to the epilepsy status and the location of tumor. Levetiracetam (82.9%) was the most preferred AED for epilepsy prophylaxis. Regarding steroid usage, 90.5% of respondents use steroids in perioperative period, including 34.2% of them as a routine manner. Presence of peritumoral edema (90.9%) was considered as the most important factor determining steroid usage followed by degree of clinical symptoms (60.6%). More than half of respondents (51.2%) replied to discontinue the steroids within a week after surgery if there are no specific medical conditions, while 7.3% preferred slow tapering up to a month after surgery. Conclusion: The survey demonstrated the prevailing practice patterns on AED and steroid usage in neuro-oncologic field among members of the KSNO. This information provides a point of reference for establishing a practical guideline in the management of brain tumor patients.

        Effect of seaweed addition on enhanced anaerobic digestion of food waste and sewage sludge

        To investigate the effect of seaweed (SW) addition on anaerobic co-digestion of food waste (FW) and sewage sludge (SS), batch experiments were conducted at various substrate concentrations (2.5, 5.0, 7.5, and 10.0 g volatile solids (VS)/L) and mixing ratios ((FW or SS):SW = 100:0, 75:25, 50:50, 25:75, and 0:100 on a VS basis). The effect of SW addition on FW digestion was negligible at low substrate concentration, while it was substantial at high substrate concentrations by balancing the rate of acidogenesis and methanogenesis. At 10 g VS/L, CH₄ production yield was increased from 103 to 350 mL CH₄/g VS by SW addition (FW:SW = 75:25). On the other hand, SW addition to SS enhanced the digestion performance at all substrate concentrations, by providing easily biodegradable organics, which promoted the hydrolysis of SS. k<SUP>hyd</SUP> (hydrolysis constant) value was increased from 0.19 to 0.28 d<SUP>-1</SUP> by SW addition. The calculation showed that the synergistic CH₄ production increment by co-digesting with SW accounted for up to 24% and 20% of total amount of CH4 production in digesting FW and SS, respectively.

        Application of a combined approach involving classical random mutagenesis and metabolic engineering to enhance FK506 production in Streptomyces sp. RM7011

        <P>FK506 production by a mutant strain (Streptomyces sp. RM7011) induced by N-methyl-N'-nitro-N-nitrosoguanidine and ultraviolet mutagenesis was improved by 11.63-fold (94.24?mg/l) compared to that of the wild-type strain. Among three different metabolic pathways involved in the biosynthesis of methylmalonyl-CoA, only expression of propionyl-CoA carboxylase (PCC) pathway led to a 1.75-fold and 2.5-fold increase in FK506 production and the methylmalonyl-CoA pool, respectively, compared to those of the RM7011 strain. Lipase activity of the high FK506 producer mutant increased in direct proportion to the increase in FK506 yield, from low detection level up to 43.1?U/ml (12.6-fold). The level of specific FK506 production and lipase activity was improved by enhancing the supply of lipase inducers. This improvement was approximately 1.88-fold (71.5?mg/g) with the supplementation of 5?mM Tween 80, which is the probable effective stimulator in lipase production, to the R2YE medium. When 5?mM vinyl propionate was added as a precursor for PCC pathway to R2YE medium, the specific production of FK506 increased approximately 1.9-fold (71.61?mg/g) compared to that under the non-supplemented condition. Moreover, in the presence of 5?mM Tween 80, the specific FK506 production was approximately 2.2-fold (157.44?mg/g) higher than that when only vinyl propionate was added to the R2YE medium. In particular, PCC expression in Streptomyces sp. RM7011 (RM7011/pSJ1003) together with vinyl propionate feeding resulted in an increase in the FK506 titer to as much as 1.6-fold (251.9?mg/g) compared with that in RM7011/pSE34 in R2YE medium with 5?mM Tween 80 supplementation, indicating that the vinyl propionate is more catabolized to propionate by stimulated lipase activity on Tween 80, that propionyl-CoA yielded from propionate generates methylmalonyl-CoA, and that the PCC pathway plays a key role in increasing the methylmalonyl-CoA pool for FK506 biosynthesis in RM7011 strain. Overall, these results show that a combined approach involving classical random mutation and metabolic engineering can be applied to supply the limiting factor for FK506 biosynthesis, and vinyl propionate could be successfully used as a precursor of important methylmalonyl-CoA building blocks.</P>

        Role of TAZ in Lysophosphatidic Acid-Induced Migration and Proliferation of Human Adipose-Derived Mesenchymal Stem Cells

        Transcriptional co-activator with a PDZ-binding motif (TAZ) is an important factor in lysophosphatidic acid (LPA)-induced promotion of migration and proliferation of human mesenchymal stem cells (MSCs). The expression of TAZ significantly increased at 6 h after LPA treatment, and TAZ knockdown inhibited the LPA-induced migration and proliferation of MSCs. In addition, embryonic fibroblasts from TAZ knockout mice exhibited the reduction in LPA-induced migration and proliferation. The LPA1 receptor inhibitor Ki16425 blocked LPA responses in MSCs. Although TAZ knockdown or knockout did not reduce LPA-induced phosphorylation of ERK and AKT, the MEK inhibitor U0126 or the ROCK inhibitor Y27632 blocked LPA-induced TAZ expression along with the reduction in the proliferation and migration of MSCs. Our data suggest that TAZ is an important mediator of LPA signaling in MSCs in the downstream of MEK and ROCK signaling.

        Background The Guideline Working Group of the Korean Society for Neuro-Oncology (KSNO) conducted a nationwide questionnaire survey for diverse queries faced in the treatment of brain tumors. As part II of the survey, the aim of this study is to evaluate the national patterns of clinical practice for patients with diffuse midline glioma and meningioma. Methods A web-based survey was sent to all members of the KSNO by email. The survey included 4 questions of diffuse midline glioma and 6 questions of meningioma (including 2 case scenarios). All questions were developed by consensus of the Guideline Working Group. Results In the survey about diffuse midline glioma, 76% respondents performed histologic confirmation to identify H3K27M mutation on immunohistochemical staining or sequencing methods. For treatment of diffuse midline glioma, respondents preferred concurrent chemoradiotherapy with temozolomide (TMZ) and adjuvant TMZ (63.8%) than radiotherapy alone (34.0%). In the survey about meningioma, respondents prefer wait-and-see policy for the asymptomatic small meningioma without peritumoral edema. However, a greater number of respondents had chosen surgical resection as the first choice for all large size meningiomas without exception, and small size meningiomas with either peritumoral edema or eloquent location. There was no single opinion with major consensus on long-term follow-up plans for asymptomatic meningioma with observation policy. As many as 68.1% of respondents answered that they would not add any adjuvant therapies for World Health Organization grade II meningiomas if the tumor was totally resected including dura. Conclusion The survey demonstrates the prevailing clinical practice patterns for patients with diffuse midline glioma and meningioma among members of the KSNO. This information provides a point of reference for establishing a practical guideline in the management of diffuse midline glioma and meningioma.

        Numerical Modeling Study for the Analysis of Transient Flow Characteristics of Gas, Oil, Water, and Hydrate Flow through a Pipeline

        This study presents the development of a four-phase, four-fluid flow pipeline simulator to describe simultaneous flow of gas, oil, water, and hydrate through a pipeline. The model has been equipped with a phase behavior model and hydrate equilibrium model to efficiently estimate thermodynamic and hydrodynamic properties of multicomponent mixtures. The governing equations are formulated for describing the physical phenomena of mass, momentum, and heat transfers between the fluids, and the wall. The equations are solved by utilizing the implicit finitedifference method on the staggered-grid system which can properly describe the boundary conditions as well as phase appearance or disappearance. The developed pipeline simulator has been validated against the field data presented by a previous investigator, and their matches are found to be relatively excellent. The model also has been applied to a multi-component, four-phase flow system in order to examine the transient flow characteristics in pipeline. Also, the potential and the location of hydrate formed in the pipeline have been studied by analyzing the flow characteristics. As a result, it was found that a pipeline system flowing gas, oil, water, and hydrate could be optimized by systematically investigating the hydrodynamic variables for the prevention of hydrate formation.

        The Interpretation of DST Data for Donghae - 1 Gas Field , block VI - 1 , Korea

        Donghae-1 gas field is located in Ulleung basin at offshore Ulsan, Korea, and its recoverable reserve is expected to be 170 to 200 BCF (Billion cubic feet). The held was confirmed to have potential gas and condensate reserves from an exploration well in 1998 and two appraisal wells in 1999. This field consists of five zones, with an average reservoir depth of about 7,000 to 8,000 ft. In this study, we have performed an analysis of Gorae V DST (Drillstem test) #2 for testing B4 zone which has the biggest reserves and Gorae V-1 DST #2 for testing B3 and B4 zones simultaneously among DST data achieved in a total of 11 zones at three wells. The pressure and flow rate recorded from two tested zones were used to obtain the reservoir characteristics and the well productivity. For pressure transient test data, we carried out the analysis of reservoir permeability, skin factor, wellbore storage effect and barrier effect by using the Horner plot and type curve matching methods. Also, with the deliverability test data, we estimated the absolute open flow which is the maximum flow rate of the gas well, and extracted the correlations representing production rate with reservoir pressure. According to the analysis, Gorae V DST #2 of B4 zone has a permeability and skin factor of 37 and (Millidarcy), 4.54, and Gorae V-1 DST #2 of B3 and B4 zones has 23 and and 21.0, respectively. It was also found that the wellbore storage effect was not significant for the two wells tested. From the deliverability test analysis, the AOF (Absolute open flow) of the Gorae V DST #2 is 152.8 MMSCFD (Million standard cubic feet per day), and that of the Gorae V-1 DST #2 is calculated to be 68.2 MMSCFD.

As part III of the survey, the aim of this study is to evaluate the national patterns of clinical practice for patients with brain metastasis and primary central nervous system lymphoma (PCNSL). Methods: A web-based survey was sent to all members of the KSNO by email. The survey included 7 questions of brain metastasis and 5 questions of PCNSL, focused on the management strategies in specific situations. All questions were developed by consensus of the Guideline Working Group. Results: In the survey about brain metastasis, respondents preferred surgical resection with adjuvant treatment for patients with a surgically accessible single brain metastatic lesion less than 3 cm in size without extracranial systemic lesions. However, most respondents considered radiosurgery for surgically inaccessible lesions. As the preferred treatment of multiple brain metastases according to the number of brain lesions, respondents tended to choose radiotherapy with increasing number of lesions Radiosurgery was mostly chosen for the brain metastases of less than or equal to 4. In the survey about PCNSL, a half of respondents choose high-dose methotrexate-based polychemotherapy as the first-line induction therapy for PCNSL. The consolidation and salvage therapy showed a little variation among respondents. For PCNSL patients with cerebrospinal fluid dissemination, intrathecal chemotherapy was most preferred. Conclusion: The survey demonstrates the prevailing clinical practice patterns for patients with brain metastasis and PCNSL among members of the KSNO. This information provides a point of reference for establishing a practical guideline in the management of brain metastasis and PCNSL.

        Dermatomyositis (DM) is an idiopathic infiammatory myopathy with characteristic cutaneous manifestations such as heliotrope rash on eyelids and Gottron`s papule. The myopathy in DM shows proximal and often symmetric muscle involvement in the extremities (hip, thigh and shoulder girdle) but rarely affects paraspinal muscles. Here, we report the case of a 43-year old man diagnosed as DM presenting predominant involvement of psoas and paraspinal muscles. Case: A 43-year-old man presented with diffi culty of supine to sitting position and lower back pain for 3 months. On physical exams, he presented scaling, erythematous, itchy cutaneous rashes on dorsum of hands, scalp, and face, especially eyelids. He did not sit on his own in supine position and showed only a low grade muscle weakness (grade 4/5) on lower proximal extremities. Laboratory fi ndings confi rmed the muscle enzyme elevation: CPK 595 U/L (N<170), aldolase 17.8 U/L (N<7.6), serum myoglobin 442 ng/mL (N: 14-106), and LDH 1039 U/L (N: 240-480). Other laboratory fi ndings were ESR 39 mm/hr (N<10), and CRP 1.63 mg/dL (N<0.5). ANA and antibodies were negative. EMG showed polyphasic motor unit potential and increased insertional activity on psoas, paraspinal and gluteus medius muscles. Positron emission tomography demonstrated increased 18FDG uptake in psoas and paraspinal muscles but no 18FDG uptake in proximal muscles of upper and lower extremities. MRI showed hyperintense signals on T2-weighted and gadolinium-enhanced T1-weighted images on psoas and paraspinal muscles. Paraspinal muscle biopsy revealed diffuse infi ammatory endomysial and perivascular infi ltrates, which were compatible with infi ammatory myopathy. Hewas diagnosed as DM with predominant involvement of paraspinal and psoas muscle involvement. He was treated with high dose of steroid and immunosuppressants and showed decreased muscle enzymes and improved muscular weakness.
