Optimal anastomotic technique in rectal surgery to prevent anastomotic leakage

Complications after colorectal surgery remain inevitable, and anastomotic leakage is one of the most severe and potentially fatal complications. Generally, anastomotic leakage is associated with severe peritonitis, the need for emergency reoperation, and an increased mortality rate. Additionally, particularly after rectal cancer surgery, it has a negative impact on long-term outcomes, including postoperative anorectal function, local recurrence, and survival. To prevent anastomotic leakage, understanding the characteristics of each anastomotic technique and establishing a stable anastomotic procedure are important. Transanal total mesorectal excision (TaTME) is a relatively new advanced surgical access technique for pelvic dissection and facilitates different anastomotic techniques without the need for transabdominal rectal transection. Especially, stapled anastomosis in TaTME, also known as double purse-string circular stapled anastomosis or the single stapling technique (SST), has gained much attention as an alternative to the conventional double stapling technique (DST). In this article, we describe the DST, SST, and hand-sewn anastomosis as anastomotic techniques after rectal surgery, focusing mainly on the differences between conventional anastomotic techniques and SST in TaTME. Furthermore, the blood flow evaluation method for the reconstructive colon before anastomosis, which is extremely important in anastomotic leakage prevention regardless of the anastomotic type, is also described.

Study Design: An open-label, non-randomized prospective study. Purpose: Teriparatide (TPTD) is known to be an antiosteoporotic agent that may accelerate the healing of fractures. This study was designed to investigate the effect of once-weekly TPTD administration on vertebral stability and bony union after acute osteoporotic vertebral fracture (OVF). Overview of Literature: Once-weekly TPTD administration can lead to early vertebral stability and promote bony union of fractured vertebrae in patients with severe osteoporosis. Methods: Forty-eight subjects with acute OVF were assigned to receive activated vitamin D3 and calcium supplementation or once-weekly subcutaneous injection of TPTD (56.5 µg) in combination with activated vitamin D3 and calcium supplementation for 12 weeks. Vertebral stability was assessed using lateral plain radiography. Vertebral height at the anterior location (VHa) and the difference in VHa {ΔVHa=VHa (supine position)−VHa (weight-bearing position)} were measured at baseline and 12 weeks after starting treatment. Bony union was defined as the absence of a vertebral cleft or abnormal motion (ΔVHa >2 mm). Results: Although not significant, ΔVHa, indicating vertebral stability, tended to be lower in the TPTD group at 12 weeks (p=0.17). As for subjects with severe osteoporosis, ΔVHa at 12 weeks was significantly lower in the TPTD group than in the control group (mean ΔVHa: control group, 3.1 mm (n=15); TPTD group, 1.4 mm (n=16); p=0.02). The rate of bony union was significantly higher in the TPTD group than in the control group (control group, 40%; TPTD group, 81%; p=0.03). Conclusions: Once-weekly TPTD administration may facilitate early bony union after acute OVF accompanied by severe osteoporosis.
Human Behavior recognition Using Regression Models

This paper proposes a method for human behavior recognition by estimating the human state, i.e., position and orientation, using regression models. In the method, human silhouette in video images is detected by background sub traction technique, and the upper part of human silhouette is used for extracting the image feature. Linear regression technique is introduced to create a model that associates the image feature with human state. Human state estimation from the currently observed image is being performed through this model. Experiments are conducted on indoor space where an Omni Directional Vision(ODV) sensor is installed to the ceiling of crossing hall way. The feasibility and accuracy of our method is discussed through the experimental results.

Aging is a natural process where changes occur in multiple tissues, resulting in aesthetic and functional effects. Aging eyelids lead to a tired or sleepy appearance. Progressive changes continue throughout life; these may eventually affect the ocular surface and cause visual impairment. Objective is to report the pathophysiology of the common age-related eyelid disorders and the current management concepts. This article presents the pathophysiology and the surgical procedures of eyelid disorders, such as blepharoptosis, dermatochalasis, entropion, ectropion, baggy eyelids, epicanthus, and lateral canthal aging. In general, most agerelated eyelid changes are corrected due to its to its associated cosmetic and functional problems. Management is based on anatomic factors contributing to the eyelid disorder. Conclusion is the choice of surgical procedure is based on the exact localisation of the eyelid pathology.

Colours in our daily life give us impressions and induce various emotions. The emotions induced from the colour are made in our brain, and the colour emotions are greatly influenced by consciousness. It is difficult to know the level of the colour emotions. In our studies, some numerical expression of the colour emotions including seasonal colours has been tried. As colours induce our emotions, the colours influence human activities and behaviours. Recently, "colour marketing" relating to consumers" purchase motivations has been paid the attention. We investigated some influences of colour to the consumers" purchase motivation through some questionnaire surveys. This paper was summarized the cross-cultural comparison of colour emotion and its numerical expression, in addition, also summarized the influence of colour to the consumers" purchase motivation, with using some published papers of our previous studies.
