N/A The therapeutic requirements of patients with non-erosive reflux disease (NERD) are similar to those with erosive esophagitis. The pharmacological action mechanism of prokinetics is quite different; domperidone is a peripheral dopamine D2-antagonist and cisapride is a HT4-agonist. This study was performed to evaluate the therapeutic effect of these two different prokinetics in patients with NERD. Methods: 178 patients, with heartburn and/or regurgitation, without reflux esophagitis were enrolled and divided into 2 groups by randomization code. In this prospective multicenter trial, 178 patients (93 patients in cisapride group, 85 patients in domperidone group) received 10 mg of cisapride three times a day or 10 mg of domperidone three time a day for 2 or 4 weeks. Symptom assessment was performed in each patients before treatments, 2 and 4 weeks after treatment. Results: Of the 133 patients available for final analysis, 65 were allocated to the cisapride group and 68 to the domperidone group. After 2 weeks treatment, heartburn was reduced in 81.1% of cisapride group, 56.7% of domperidone group (p<0.05) and regurgitation was reduced in 89.7% of cisapride group, 77.7% of domperidone group. After 4 weeks treatment, heartburn was reduced in 94.3% of cisapride group, 88.7% of domperidone group and this difference was not significant. The proportion of adverse events in cisapride group was 9.4% and was 5.5% in domperidone group. Conclusions: Cisapride tartrate was more effective in relieving heartburn in NERD patients than domperidone maleate after 2 week treatment. However, this superior effect dose not persist longer than 2 weeks.(Korean Journal of Gastrointestinal Motility 2002;8:3-13)

Bullous pemphiogid (BP) is a chronic vesicular disorder and an autoimmune disease which has an autoantibodies to hemidesmosime in the keratinocyte. The authors report a case of gastrointestinal bleeding caused by BP involved with esophagus. A seventy year old man visited our clinic due to melena for a week. He had a quadriplegia due to fracture of cervical spine and was on therapy with corticosteroid and immunomodulatory drug for the diagnosis of BP. On endoscopic examination, multiple bullae filled with blood at lower esophagus was revealed and there was no other bleeding focus in stomach and duodenum. Medical treatment for BP with supportive care was done. The patient died due to sepsis caused by pneumonia. BP is common in elderly or poor conditioned patients. We need to consider the invasion of esophagus in case of gastrointestinal bleeding in patients with BP.
가공조제가 Styrene-Butadiene Rubber 배합고무의 유변특성 및 기계적 특성을 미치는 영향

공정조제의 성분 및 함량이 styrene-butadiene rubber (SBR) 배합고무의 유변학적 특성과 기계적 물성에 미치는 영향을 조사하였다. 공정조제로 지방산, 지방산 유도 금속염, 에스테르, 알코올, 아미드 성분을 단독 혹은 혼합한 5종의 상용제품을 선택하였다. 공정조제 첨가에 따른 무니점도와 전단점도의 감소는 금속염이 주성분인 공정조제가 가장 컸고, 고분자량의 지방산 알콜이나 에스터의 함량이 높은 공정조제가 낮았다. 공정조제의 함량증가에 따라 인장 모듈러스는 감소하고, 발열은 증가하는 경향을 나타내었지만, 극한물성인 인장강도와 파단신장율에는 그다지 큰 영향을 미치지 않았다. Effects of type and loading level of process aids on the rheological and mechanical properties of styrene-butadiene rubber (SBR) compound were investigated. Five commercial grades of process aids composed of fatty acids and their various derivatives such as metal salts, esters, alcohols and amides were selected .The reduction in Mooney and shear viscosities was higher for metal salt-type process aids, but lower for the process aids containing high molecular weight fatty acid alcohols and esters with increasing the loading of process aids. Tensile modulus generally decreased, while heat-build-up increased with increased process aids content. No considerable effect was observed for ultimate properties such as tensile strength and elongation at break.
