Objective : The aim of this study was to evaluate the efficacy of laparoscopic uterosacral nerve ablation (LUNA) in infertile women with chronic pelvic pain and endometriosis. Method : Prospective randomized study was performed in 20 infertile women with endometriosis and chronic pelvic pain, who had undergone of laparoscopic uterosacral nerve ablation at Samsung Cheil hospital between April and September, 1998. Statistical analysis was perfomed using by DBSTAT (ver. 2.0). Result : The pelvic pain score of pre-operation was 3.72±1.07 (mean±SD) and it was significantly decreased to 1.83±0.85 at 1 month later and 1.93±0.92, 1.69±0.60, 1.56±0.62, 1.56±0.81 each in second, third, fourth and fifth month after operation. There was no correlation between pelvic pain and stage of endometriosis. Pregnancy rate was 65% after LUNA (twelve deliveries, one pregnancy is ongoing at IUP 24 weeks). Conclusion : Laparoscopic uterosacral nerve ablation would be effective for infertile women with endometriosis and pelvic pain.
조산 예측을 위한 IGFBP-1 (Insulin-like Growth Factor-binding Protein-1)의 유용성에 대한 연구

Objective : This study is performed to assess whether detection of phosphorylated insulin like growth factor binding protein-1 (phIGFBP-1) in cervical secretions by a bed side test could be used to prediction preterm delivery in patient with regular uterine contraction. Methods : In our prospective study, 42 women between 20.2 and 36.4 weeks gestation with regular, persistent contraction (2-3/min for 30 min). and 27 women between 22.0 and 37.0 weeks of gestation without symptoms of preterm labor were assessed for presence of cervical phIGFBP-1. Dacron swabs were applied to the cervix and assayed in 5min by using immunochromatography, actim partus test (Medix Biochemica). Results : Of the 42 patients with regular uterine contractions, 22 had a positive actim partus test and 20 had a negative test. Among the 22 patients with positive test, 5 delivered preterm and other 17 patients delivered term (<37 weeks). Among the the 20 women with a negative test, 2 delivered preterm (NS). The mean concentration of phIGFBP-1 in 22 positive patients is 83.8±86.5 ㎍/L and that in 20 negative patients is 2.1±5.0 ㎍/L (P<0.0001) (Table 4). Sensitivity, specificity, false positive, false negative, positive predictive, and negative predictive values were 18.5, 59.6, 40.3, 28.6, 16.7, and 94.9%, respectively. Conclusion : The absence of cervical phIGFBP-1 is seems to be a valuable test in excluding preterm delivery in patients who have regular uterine contractions. The high negative predictive value in this study may enable physicians to prevent over treatment of patients with uterine contractions. Therefore, many unwanted side-effects and complications of potentially hazardous tocolytic therapy can be prevented.

Buckwheat sprout is used as vegetable, and also flour for making noodles, and so on. Currently, information about tissue culture in buckwheat is limited and restricted to micropropagation. We carried out somatic embryogenesis and plant regeneration using hypocotyl segments as explant of the cultivated buckwheat species Fagopyrum esculentum, differs from existing studies in the growth regulator combinations used. Maximum callus regeneration was induced on MS medium containing 2,4-D(2.0 mg/L) and benzylaminopurine BAP (1.0 mg/L) and 3% sucrose. Friable callus was transferred to solidified MS media containing BAP (1.0 mg/L) and at various concentrations for the induction of embryogensis. The optimum concentrations of additives were IAA (2 mg/L), KIN(1.0 mg/L), BAP (1.0 mg/L), and 3% (w/v) sucrose. Only 2,4-D did not show any significant effect on callus induction or embryogenesis. Regeneration of embryonic callus varied from 5 % to 20%. Whole plants were obtained at high frequencies when the embryogenic calluses with somatic embryos and organized shoot primordia were transferred to MS media with 3% sucrose. Regenerated plants after acclimation will transfer to green house. The main objective of this research was to develop a efficient protocol for plant regeneration for common buckwheat, and to apply in future for genetic transformation.
고압에 의한 장파열 : 1례 보고 A Case Report

Rupture of intestine caused by direct pressure from compressed air nozzle is a rare abdominal emergency. Recently, using of compressed gas has been increased and the risk of accident due to improper use of this gas was also increased. We experienced a case of rupture of the colon due to compressed gas delivered through anus as a result of prank. The patient was a 33-year-old man and admitted to the emergency department with severe painful, distended abdomen, and respiratory distress. The patient showed tachypnea, tachycardia, and mild irritability. Supine X-ray film of the abdomen showed marked pneumoperitoneum and elevated diaphragm. After an insertion of the nasogastric tube insertion, decompressive paracentesis to relieve respiratory difficulty was done. After initial management, explo-laparotomy was performed. There was a longitudinal perforation sized by 10cm at antemesenteric border of the sigmoid colon and mucosal and a submucosal tearing of rectal wall sized by 15cm. Primary repair of the rectum and sigmoid colostomy at the site of ruptured portion were performed. The patient was recovered without complication and now waiting for repair of colostomy.

The present work was designed to evaluate the enzyme histochemical and immunohistochemical characteristics of various cells that constitute lymph node. Lymph nodes were obtained at surgery for the evaluation of lymphadenopathy and were embedded in glycerol methacrylate(plastic) through the complexing tissue processing procedures. Enzyme histochemical examinations('5-nucleotidase, adenosine triphosphatase, acid phosphatase, alkaline phosphatase, nonspecific esterase, and peroxidase) were performed on thin sectioned(2m) tissues. And immunohistochemical stains for IgG and S-100 protein were also performed.

Author evaluated specimens of the uterine cervix submitted in Chonbuk National University Hospital laboratory from Sept. 1983 to Aug. 1986. The specimens were diagnosed hitologically under the light microscope previously. The results were as follows ; 1. The analyzed cervical tissues were 982 cases, e.g. 681 cases by punch biopsy, 166 cases by polypectomy, 32 cases by conization, 51 cases by total hysterectomy, 51 cases by radical hysterectomy, and one cases by endocervical curettage. 2. On the hitological classification of the tissue, chronic cervicities and chronic cervicitis with wquamous metaplasia were 483 cases, cervical polyps 164 cases, dyplasia 123 cases, carcinoma in situ 22 cases, microinvasivee carcinoma 6 cases, invasive squamous cell carcinoma 157 cases, adenocarcinoma 13 cases, condyloma accuninaturn 3 cases and verrucous carcinoma was one case. 3. The peak incidence of cervical lesions occurs 5th decade (365cases), and the average was 44.2 year of age. The youngest and oldest cases were 16 and 79 year of age. 4. The average age of chronic cervicitis was 40.6 years, chronic cervicitis with squamous metaplasia was 40.7 years, cervical polyp was 42.9 years, dysplasia was 42.2 years, carcinoma in sity was 42.8 years, microinvasive carcinoma was 49.8 years, invasive squamous cell carcinoma was 48.4 years, and adenocarcinoma was 50.8 years. 5. Clinical manifestation of the patients with performed biopsy of the uterine cervix were leukorrhea(289 cases), vaginal spotting(279 cases), contact bleeding(229 cases), and irregular vaginal bleeding(221 cases). And other many symptoms were complained. 6. Clinical manifestations of the chronic cervicitis and chronic cervicitis with squamous metaplasia were leukorhea, vaginal spotting and contact bleeding, dysplasia was contact bleeding and leukorrhea, and invasive carcinoma was irregular vaginal bleeding, vaginal spotting, and contact bleeding. 7. The main histological diagnosis of the cervical tissues obtained by punch biopsy(681 cases) were chronic cervicitis and squamous metaplasia(442 cases), invasive carcinoma(128 cases), and dysplasia(85 cases). The tissues by polypectomy(166 cases) were diagnosed with polyp(160 cases) mostly, and conebiopsy tissues(32 cases) were disclosed with dysplasia(16 cases) and chronic cervicitis & squamous metaplasia(10 cases). The specimens by total hysterectomy specimens(51 cases) were confirmed with invasive carcinoma(38 cases) mainly.

Recently, bacterial angular leaf spot disease, caused by Xanthomonas fragariae, causes severe damage in strawberry production and its’ export to other countries, since the pathogen has been classified as an A2 quarantine pathogen. Typical the Angular Leaf Spot (ALS) disease represent that water-soaked angular spots symptoms, bacteria ooze exudate under relatively high humidity condition and later the spot become reddish brown on the leaf surface. The pathogen disseminated by irrigation water, infected mother plant and farmer’s hand. In this study, we reported that rubbing inoculation method showed more effective in the pathogen dissemination than infiltration with needles, regardless the strawberry cultivars. Additionally, Dichloroisocyanurate (NaDCC) treatment in commercial strawberry fields provided reliable efficiency to reduce the bacterial angular leaf spot disease incidency and severity.
