        근막 결손의 복원과 자궁천골인대 고정을 통한 전자궁적출술 후 질탈출의 성공적 교정

        목적 : 본 연구는 전자궁적출술 후 질탈출을 부위-특이적인 결함의 개념에 입각하여 치골경부 근막과 직장질 격막의 연결을 통한 골반내 근막의 복원과 자궁천골인대 고정술을 이용한 질탈출의 성공적인 수술적 교정의 방법과 결과를 보고하고자 한다. 연구 방법 : 1999년부터 2001년 8월까지 자궁적출술 후 질궁탈출 증상으로 문화병원 산부인과를 내원, 탈출된 질의 근막을 재건시킨 다음, 이를 자궁천골인대에 고정시킴으로써 질의 부위-특이적인 교정술을 받은 3명의 환자를 대상으로 하였다. 3명중 1명은 개복술을 하였고 2명은 내시경 수술을 하였다. 결과 : 환자의 연령 분포는 49-67세였으며, 수술시간은 평균 113분 (105-125)이었다. 세 환자 모두 수술 후 질의 상태가 해부학 및 기능적으로 정상이었으며, 9일째 특이사항 없이 퇴원하였다. 수술 후 HRT를 시행한 1명을 포함한 3명 모두 각각 24개월, 12개월, 7개월간의 추적 검사에서도 특이한 합병증이나 재발 없이 좋은 경과를 보였다. 결론 : 골반 근막 결손의 복원과 자궁천골인대 고정술은 전자궁적출술 후 질탈출의 교정에 매우 안전하고 효과적이며, 기능적인 방법으로 사료되며, 자궁천골인대를 질탈출 교정에 이용한 연구는 본 연구가 국내에서 처음이다. Objective : To report the method and efficacy of the site specific-defects repair in the endopelvic fascia and uterosacral ligament suspension for the correction of posthysterectomy vaginal vault prolapse. Methods : This study was performed in 3 patients with posthyterectomy vaginal vault prolapse who underwent the surgery for the reconstruction of endopelvic fascia and the correction of vault prolapsed vagina by using uterosacral ligament suspension at the department of Obstetrics and Gynecology in Moon-Hwa Hospital. Among them, 1 patient was treated by laparotomy, and 2 patients were treated by laparoscope. Results : The age of patients ranged from 49 to 67 years. The mean operation time was 111 minutes (105-120 minutes). The status of postoperative vagina was normal in all 3 patients, who were discharged 9 days after operation without symptoms. No complication or recurrence were found in them despite long-term follow-up for 24 months, 12 months, 7 months, respectively after operation. One patient has been treated with HRT therapy. Conclusion : The reconstruction of the defect of endopelvic fascia and uterosacral ligament fixation may be very safe, effective, and functional in the correction of vaginal vault prolapse. This study reports the treatment of vaginal vault prolapse using uterosacral ligament within Korea for the first time.

      • 벼 褐色葉枯病에 關한 硏究 : 벼 品種間抵抗性에 對하여 Rice Vrietal Resistance to Rice leaf Scald

        This experiment was conducted to obtain basic information on the breeding for the disease resistnat rice varieties by clarifying the degree of varietal resistance to Rhynchosporium oryzae which was known as a new disease of the rice variety "Tongil" in Korea. The results obtained are as follows; 1. The varietal difference of resistance to this fungus showed some similarity in the spraying inoculation at the seedling stage, but the ratio of lesion enlargement in the soaking inoculation at the heading tsage showed remarkable difference. 2. Indicatype and Indica x Japonica type varieties were relatively more susceptible to this fungus than Japonica type varieties. 3. Among the Japonica type varieties tested, upland rice varieties were more susceptible than paddy rice varieties, and Korean developed varieties such as Jaekeun, Nongbaek, Palkeum, Pungkwang, etc., were more resistant than Korean native and introduced varieties. The most susceptible varieties among the tested varieties were Suweon 264, 230, 265, 262, Milyang 29, Iri 326, etc., which were Indica Japonica crosses.

        This is the second part of the paper series about Jagyeungnoo, 'The Self Time Announcing Clepsydra', restoration project. The content of the second part is about the constant flow rate keeping system to the time measuring vessels. This system consists of three vessels. They are reservoir, compensating, and regulating vessels. In Jagyeungnoo system overflow mechanism was adopted to achieve constant flow rate regardless inflow rate variations. Through extensive parametric and experimental study based on extensive trial-error based on the mathematical model, optimal values of two parameters, i.e, initial height of regulating vessel and the diameter of the pipe exit orifice were determined and discussed improving method of accuracy. For the application of this result to Jagyeungnoo system, the eleven 24 solar seasonal time-ruler and its floating accessories were restored based upon Nujutongui, i.e. Clepsydra Operation Manual published in King Sejong's era.

        A thermolabile alkaline phosphatase has been purified through steps of osmotic shock, ammonium sulfate salting-out, and DEAE-cellulose chromatography from the cultured broth of the marine Vibrio sp. M-96 strain. The optimal temperature for the enzyme activity was 35℃. The optimal pH was pH 11.0, and the range of pH stability was pH 10.4 to 12.0. Thermal inactivation occured within 6 minutes at 60℃. The enzyme was considerably inactivated by 0.1mM concentrations of Hg^2+, Ni^2+ and Zn^2+, whereas activated up to 234% by 1mM of Mn^2+. The activation energy and deactivation energy by the Arrhenius equation were 4.02 Kcal/mol and 9.09 Kcal/mol, respectively. The Km and Vmax values of the enzyme for p-nitrophenylphosphate were found to be 0.0465mM and 0.001335mM/min. respectively. Active from of the enzyme had a molecular weight of 57,000 dalton determined by the Sephadex G-200 gel filtration method.

        Validity of Tono-pachymetry for Measuring Corrected Intraocular Pressure in Non-surgical and Post-photorefractive Keratectomy Eyes

        Purpose: To assess the validity of central corneal thickness (CCT) and corrected intraocular pressure (IOP) values obtained by tono-pachymetry in non-surgical and post-photorefractive keratectomy (PRK) eyes. Methods: For the study, 108 young healthy participants and 108 patients who had PRK were enrolled. Measurements were randomly performed by tono-pachymetry, ultrasonic (US) pachymetry, and Goldmann applanation tonometry (GAT). CCT measurement by tono-pachymetry was compared to that of US pachymetry. The corrected IOP value obtained by tono-pachymetry was compared to that obtained by US pachymetry and GAT. The corrected IOP from US pachymetry and GAT was calculated using the identical compensation formula built into the tono-pachymetry. Bland-Altman plot and paired t-test were conducted to evaluate the between- method agreements. Results: The mean CCT measurement using tono-pachymetry was significantly greater by 7.3 μm in non-surgical eyes (p < 0.001) and 17.8 μm in post-PRK eyes (p < 0.001) compared with US pachymetry. Differences were significant in both Bland-Altman plotand paired t-test. The mean difference of corrected IOP values obtained by tono-pachymetry and calculated from measurements by US pachymetry and GAT was 0.33 ± 0.87 mmHg in non-surgical eyes and 0.57 ± 1.08 mmHg in post-PRK eyes. The differences in the Bland-Altman plot were not significant. Conclusions: The CCT measurement determined using tono-pachymetrywas significantly thicker than that of US pachymetry. The difference in CCT was greater in post-PRK eyes than in non-surgical eyes. However, the corrected IOP value obtained by tono-pachymetry showed reasonable agreement with that calculated from US pachymetry and GAT measurements.

        Jagyeungnoo, 'The Self Time Announcing Clepsydra', used as the national standard clock system from the first day of July in 1434 was invented by Jang Yeong-sil under the royal command of King Sejong in 1433. It was famous for its elaborated time announcing mechanism and its time keeping accuracy from the beginning. However this original system was destroyed during Japanese invasion started 1592. At present day only a few parts of rebuilt one in 1536 in King Jungjong's era were survived and later assigned as the National Treasure # 229. These remains are two cylindrical time measuring vessels with a dragon sculptured on the outer surface and a few relic parts associated with this system. Recently the Jagyeongnoo restoration project was conducted by National Palace Museum and Konkuk University. The whole restoration procedure followed along the written record Borukaki in King Sejong's Veritable Record, i.e, Sejong Silok and relics survived from 1536 system. This series of papers are associated with Jagyeungoo restoration project since the written record was delicate and sometime need engineering explanation. The first part of this series is about detailed reproduction processes of the water vessels of Jagyeungnoo system. This includes measuring dimensions of relics and casting process of the vessels.
