        Original Articles : Clinical significance of occult hepatitis B virus infection in chronic hepatitis C patients

        Background/Aims: We investigated the frequency of occult hepatitis B virus (HBV) infection in anti-hepatitis C virus (HCV)-positive individuals and the effects of occult HBV infection on the severity of liver disease. Methods: Seventy-one hepatitis B virus surface-antigen (HBsAg)-negative patients were divided according to their HBV serological status into groups A (anti-HBc positive, anti-HBs negative; n=18), B (anti-HBc positive, anti-HBs positive; n=34), and C (anti-HBc negative, anti-HBs positive/negative; n=19), and by anti-HCV positivity (anti-HCV positive; n=32 vs. anti-HCV negative; n=39). Liver biopsy samples were taken, and HBV DNA was quantified by real-time PCR. Results: Intrahepatic HBV DNA was detected in 32.4% (23/71) of the entire cohort, and HBV DNA levels were invariably low in the different groups. Occult HBV infection was detected more frequently in the anti-HBc-positive patients. Intrahepatic HBV DNA was detected in 28.1% (9/32) of the anti-HCV-positive and 35.9% (14/39) of the anti-HCV-negative subjects. The HCV genotype did not affect the detection rate of intrahepatic HBV DNA. In anti-HCV-positive cases, occult HBV infection did not affect liver disease severity. Conclusions: Low levels of intrahepatic HBV DNA were detected frequently in both HBsAg-negative and anti-HCV-positive cases. However, the frequency of occult HBV infection was not affected by the presence of hepatitis C, and occult HBV infection did not have a significant effect on the disease severity of hepatitis C.         眞智王의 卽位에 대한 再檢討

        There has been little substantial progress in the study of King Jin Ji (眞智王). This is due to a shortage of records regarding his reign that resulted from the political turmoil leading up to his dethronement four years after taking the throne. Negative impressions implanted in the process of evaluating and recording his administrative achievements in comparison to King Jin Pyeong(眞平王) are another reason for this dearth of records. The background information that made it possible for him to become King, however, highlights both his personal ability and internal and external circumstances unique to the era of his accession. In terms of ability, his age was considered an important fact qualifying him to be King, as was his renown as a wise statesman with a keen and accurate sense of judgment. Taking into account the internal and external circumstances both before and after he became king, one must consider the Shilla Kingdom (新羅) suffering frequent attacks mounted by the Paekje Kindgom(百濟) externally given the inroads that King Jin Heung(眞興王) made into the Hang River(漢江) basin. The ideology of Cakravartirajan(轉輪聖王) late emerged as an internal factor, in terms of how it was closely connected to the Buddhist politics of King Jin Heung(眞興王). It was Geo Chil-bu(居柒夫) who played a pivotal role in bringing Jin Ji to the throne. He had been characterized as a symbolic figure that made great contributions to the expansion of Shilla Kingdom(新羅) territory by becoming a member of inner Buddhist circles somewhat earlier than others. In this context he was able to assist Jin Ji's accession to the throne in the following ways: Safeguarding the Shilla Kingdom territory from external threats was always an imminent proposition, while the internal politics of the Kingdom involved a pursuit of inheriting the Buddhist policies of King Jin Heung(眞興王). In addition to the Geo Chil-bu's(居柒夫) loyalty played in the coronation of King Jin Ji(眞智王), the role Kim Mu-ryeok(金武力) played is also worthy of attention. It has been inferred from what few records are available, that the military stronghold Kim Mu-ryeok had in cooperation with Geo Chil-bu(居柒夫) was a major factor in Jin Ji's accession to the throne. The relationship between King Jin Ji(眞智王) and Kim Mu-ryeok(金武力) also played had a significant influence on their kinsfolk. It has, therefore, been suggested that as a result King Yong-chun(金龍春) was nudged toward ties with Kim Seo-hyeon(金舒玄), and Kim Chun-chu(金春秋) affiliated himself with Kim Yu-sin(金庾信). It was perhaps these Saryun(舍輪系) and Kaya(加耶系) clique relationships, upon which a new aristocracy that has come to be known as the spread of ‘the middle ages(中代)’ was based.

      • A Study on the Clearance Decision of Fine Blanking Tool for Eco-Al Special Parts with Various Inner Corner Shapes

        <P>Fine blanking is a press-working process that permits the production of precise finished components which are cleanly sheared through the whole cutting surface. It can be eliminated secondary operations, such as milling, grinding, etc. Recently, many studies on the weight reduction of automobile for fuel saving were underway. Especially, there are many examples in which light metal like Al were applied for automotive parts. Eco-Al is the new material which is replaced Mg of aluminum alloy with Mg+Al2Ca, therefore Eco-Al material has improved mechanical properties and formability. The clearance between the punch and die of a fine blanking tool is an important design factor that affects the sheared surface of a product. In a fine blanking process, the clearance is typically assigned a 0.5% material thickness. If the clearance is too big, a fractured surface would occur in the product while if it is too small, bulging would occur. In this study, a setting for optimum clearance was proposed for inner corner shapes by checking shear characteristics and bulging effects according to various clearances. After designing a special part with various corner shapes possessing Eco-Al5052 (thickness: 4 mm), a fine blanking tool was constructed and fine blanking experiments were conducted. The result could be usefully applied in fine blanking processes for Eco-Al automobile parts with corner shapes in the future.</P>

      • Korean Youth with Comorbid Allergic Disease and Obesity Show Heightened Psychological Distress

        <P><B>Objective</B></P> <P>To explore psychological distress in Korean adolescents having allergic disease comorbid with obesity.</P> <P><B>Study design</B></P> <P>A total of 703 869 adolescents who completed the Korean Youth Risk Behavior Web-based Survey between 2007 and 2016 were analyzed. Participants were divided into 4 groups—healthy control, allergic disease only, obesity only, and comorbidity of allergic disease and obesity—and compared them to determine whether they showed differences in mental health.</P> <P><B>Results</B></P> <P>Adolescents with both atopic dermatitis and obesity had significantly greater odds of experiencing unhappiness (OR, 1.17), stress (OR, 1.32), and suicidal ideation (OR, 1.25) than those without both conditions. The same was true of adolescents with obesity and allergic rhinitis (OR, 1.21, 1.37, and 1.27, respectively) or bronchial asthma (OR, 1.37, 1.39, and 1.37). The comorbidity groups also showed significantly greater odds of stress and suicidal ideation than the allergic disease-only (atopic dermatitis with obesity, 1.21 and 1.15, respectively; allergic rhinitis with obesity, 1.11 and 1.09; bronchial asthma with obesity, 1.17 and 1.14) and obesity-only groups (atopic dermatitis with obesity, 1.13 and 1.09; allergic rhinitis with obesity, 1.18 and 1.10; bronchial asthma with obesity, 1.18 and 1.21).</P> <P><B>Conclusions</B></P> <P>Allergic disease and obesity negatively and additively influence mental health in adolescents.</P>

        <P>The aim of this study was to investigate the effect of alendronate released from chitosan scaffolds on enhancement of osteoblast functions and inhibition of osteoclast differentiation in vitro. The surface and cell morphologies of chitosan scaffolds and alendronate-loaded chitosan scaffolds were characterized by variable pressure field emission scanning electron microscope (VP-FE-SEM). Alendronate was released in a sustained manner. For evaluating osteoblast functions in MG-63 cells, we investigated cell proliferation, alkaline phosphatase (ALP) activity, and calcium deposition. Furthermore, for evaluating inhibition of osteoclast differentiation in RAW 264.7 cells, we investigated tartrate-resistant acid phosphatase (TRAP) activity, TRAP staining, and gene expressions. The in vitro studies revealed that osteoblasts grown on alendronate-loaded chitosan scaffold showed a significant increment in cell proliferation, ALP activity, and calcium deposition as compared to those grown on chitosan scaffolds. In addition, the in vitro study showed that osteoclast differentiation in RAW 264.7 cells cultured on alendronate-loaded chitosan scaffolds was greatly inhibited as compared to those cultured on chitosan scaffolds by the results of TRAP activity, TRAP staining, and gene expressions. Taken together, alendronate-loaded chitosan scaffolds could achieve the dual functions of improvement in osteoblast functions and inhibition of osteoclast differentiation. Thus, alendronate-eluting chitosan substrates are promising materials for enhancing osteoblast functions and inhibiting osteoclast differentiation in orthopedic and dental fields.</P>

The result could be usefully applied in fine blanking processes for high-strength steel automobile parts with corner shapes in the future.</P>

        Temperature Characteristics of Traditional Indirect Moxibustion and Electronic Moxibustion

        Background: Electronic moxibustion (EM) was developed to minimize the side effects of traditional moxibustion, such as burns, and to overcome therapeutic compliances such as smoke or smell. Objectives: To investigate distributions and thermal stimulation of EM at various depths using silicon phantom and to compare this methodology to traditional indirect moxibustion (TIM). Methods: A silicon phantom composed of polydimethylsiloxane was heated and immersed in a hot plate containing warm water to set the phantom’s temperature to that of biological tissue. K-type thermocouples were inserted into the phantom at depths of 0, 2, 5, 7, and 10 mm to measure temperature changes with thermal stimulation of EM or TIM placed on top of the phantom. Results: At the surface of the phantom, the peak temperature after applying TIM (55.04 ± 0.92℃ [Δ23.79 ± 0.96℃]) was significantly higher than after EM (43.25 ± 1.95℃ [Δ13.00 ± 2.23℃]), with both interventions reaching the highest temperature after 2 minutes. The temperature increase for TIM was also statistically significant compared to EM when measured at a depth of 2 mm. For the experimental setting with TIM, after reaching peak surface temperature, a rapid decrease was observed at the surface and 2 mm while EM showed a much more gradual decline. There was no significant difference in temperature change between the groups at depths of 5, 7, and 10 mm. Conclusion: TIM resulted in a higher temperature rise compared to EM at the surface and at a 2 mm depth reaching over 50℃, which creates risk of burns. Thermal stimulation with EM had a lower risk of burns with temperature increment not being statistically different from TIM below the depth of 5 mm.

        A Case Report of Temporomandibular Bilateral Osseous Ankylosis Treated by Total Joint Replacement in Ankylosing Spondylitis

        Ankylosing spondylitis (AS) is a chronic autoimmune disease mainly involving the axial skeleton. The pathology of the disease is usually found at the sacroiliac joint, and half of the patients experience cervical spine invasion, but eventually, the whole spine is affected. The involvement of the temporomandibular joint (TMJ) in AS has not been investigated very well. A review of the literature revealed that there are only a few studies of TMJ involvement in AS that combined clinical and radiographic examinations. These studies show widely different results, ranging between 4% and 32%. We experienced Bilateral osseous ankylosis of the jaw treated by total alloplastic joint replacement in AS, and offer a case report.
