We describe a 48-year-old man with family history of rheumatoid arthritis (RA) affected by chronic eosinophilic pneumonia (CEP) with severe peripheral eosinophilia. CEP might develop as a complication of longstanding active RA. The patient with 5 months history of seropositive RA and chronic respiratory symptoms, alveolar and blood eosinophilia, peripheral pulmonary infiltrates and pleural effusion on chest imaging. The lung may be involved as an extraarticular manifestation of RA. However, CEP is not recognized as a typical lung manifestation of RA, and the two diseases rarely coexist. The effusion was an eosinophil predominant exudates and was characterized by low pH, and glucose level and high lactic dehydrogenase. The patient responded rapidly to combination of steroids and disease modifying anti-rheumatic drugs.
췌장염 환자의 복통 호전을 위한 전침 및 이침 치료 예비 임상연구 프로토콜

The purpose of this study is to evaluate the feasibility of further acupuncture research as an effective alternative and safe treatment for abdominal pain control in patients with pancreatitis. Methods and Results : This study is an open-label, assessment-blind, parallel designed pilot clinical trial. Thirty participants will be assigned to the acupuncture group(n=15) and usual care group(n=15). All patients will receive the conventional standard-of-care(SOC) therapy, but only the experimental group will receive acupuncture therapy six times a week, and the duration of acupuncture therapy will be held up to 12 weeks or until the pains are to be resolved. For the conventional SOC therapy, painkiller will be given. In treatment group, the subjects will receive the identical SOC therapy in combination with electroacupuncture therapy on twelve acupuncture points(LI4, PC6, SP6, GB39, ST36, ST37), and auricular acupuncture therapy on five auricular acupuncture points(Sympathetic, Shen Men, Abdomen, Pancreas gall, and Spleen). The primary outcome will be measured using the visual analogue scale(VAS), and the secondary outcome will be measured using the painkiller demand, quality of life index and severity of pancreatitis by abdominal computed tomography(CT). Assessments will be made at baseline and at week 1, 4, 8 and 12. Results of abdominal CT will be evaluated at baseline and at week 12. Conclusions : The result of this trial will provide a basis for the effectiveness and safety of acupuncture treatment for abdominal pain in patients with pancreatitis.
다문화 가족 어머니의 문화적응 스트레스가 부모 양육행동을 매개로 초기 청소년 자녀의 학교생활적응에 미치는 종단적 영향

본 연구는 다문화 가족 결혼이주여성 어머니의 문화적응 스트레스와 초기 청소년자녀가 지각한 부모 양육행동, 학교생활적응 간의 관계를 자기회귀 교차지연을 활용하여 종단적으로 살펴보았다. 다문화청소년 패널조사 데이터 중 초기 청소년기에 해당하는 2차년도(초 5)에서 4차년도(중 1) 3개년 간 어머니와 청소년의 종단자료를 분석한 결과, 어머니의 문화적응 스트레스와 초기 청소년 자녀가 지각한 부모 양육행동, 학교생활적응은 이전 시점의 값이 이후 시점의 값을 유의하게 예측하고 시간 경과에 따른 안정성을 보였다. 또한, 어머니의 문화적응 스트레스는 이후 시점의 초기 청소년 자녀가 지각한 부모 양육행동과 학교생활적응 각각을 부적으로 예측하였으나, 초기 청소년 자녀가 지각한 부모 양육행동과 학교생활적응은 이후 시점 어머니의 문화적응 스트레스를 유의하게 예측하지 못하였다. 한편, 초기 청소년 자녀가 지각한 부모 양육행동과 학교생활적응은 종단적으로 서로 유의하게 예측하였다. 이러한 결과를 바탕으로 본 연구의 의의와 제한점 및 시사점을 논의하였다. The purpose of this study was to examine the longitudinal relationships among multicultural mothers’ acculturative stress, parental behaviors perceived by adolescent children, and school adjustment. Using three waves of the Multicultural Adolescents Panel Study, autoregressive cross-lagged models were estimated. Results indicated that mothers’ acculturative stress, parental behaviors perceived by adolescent children, and school adjustment were relatively stable over the 3-year period. Mothers’ acculturative stress negatively and subsequently predicted both parental behaviors and school adjustment. However, no significant effects were found in the associations between parental behaviors and school adjustment and mothers’ subsequent acculturative stress. Parental behaviors perceived by adolescent children were significantly and reciprocally associated with school adjustment over time. Based on these results, the implications and limitations of the current study were discussed.

Three different dutasteride extraction methods were compared and a method based on liquid-liquid extraction (LLE) using methyl tert-butyl ether and methylene chloride was proved to be more effective than others for the extraction of dutasteride and finasteride, the internal standard (IS), from rat plasma. Additionally, a method composed of the LLE extraction, liquid chromatography, and multiple reaction monitoring (MRM) to target dutasteride and IS was validated by assessing specificity, linearity ($r^2$ = 0.9993, 5 - 400 ng/mL), sensitivity (the limit of detection: 4.03 ng/mL; the limit of quantitation: 12.10 ng/mL), accuracy (intra-day: 89.4 - 105.9%; inter-day: 84.9 - 100.9%), precision (intra-day: 0.8 - 6.9%; inter-day: 2.9 - 15.9%), and recovery (84.7 - 107.8%). Since the validated method was successfully applied to a pharmacokinetic study of dutasteride, it can be useful for the pharmacokinetic evaluation of newly developed dutasteride formulations.

<P>This study evaluated bactericidal effects of chlorogenic acid (CGA), caffeic acid (CFA), coumaric acid (CMA), and ferulic acid (FA), against Pectobacteriwn carotovorum subsp. carotovorum (PCC) on Chinese cabbage using suspension and direct inoculation tests. Among suspension tests with four phenolic acids, CGA at 5-7 mM or CMA at 7-10 mM for 30min effectively eliminated PCC, whereas CFA and FA effectiveness increased on prolonging treatment time to 60 min. For cabbage, CGA at 5-7 mM or CMA at 10 mM for 60 min achieved a log(10) reduction of a 3.98 log(10) CFU/g. On comparison of soft rot development on Chinese cabbage after treatment with phosphate-buffered saline (negative control), 1% sodium hypochlorite (positive control), and 5 mM of CGA at 20 degrees C for 72 h, the first soft rot symptoms were observed after 12 h only for PBS treatment, and there were no significant differences between positive control and CGA treatment up to 36 h, after which the decay rate increased significantly. These results suggest that CGA at 5-10 mM for 60 min might allow significant shelf life extension of cabbage and prevent quality loss in kimchi by controlling the spread of the soft rot symptoms on cabbage.</P>

Diabetic nephopathy is one of the leading causes of end-stage renal disease and characterized pathologically by the glomerular mesangial expansion and increased extracellular matrix(ECM) formation. Glomerular hyperfiltration and increased vascular permeability observed in the early stage of diabetic nephropathy have been proposed to play a significant pathophysiologic role in the eventual development of glomerulosclerosis of dia-betic nephropathy. Some studies have suggested that this glomerular hyperfiltration is mediated by increased nitric oxide(NO) production via the constitutive nitric oxide synthase(cNOS) pathway present in endothelial cells under the high glucose environment. However, the exact role of the inducible NOS(iNOS) pathway present in mesangial cells in the pathogenesis of diabetic nephropathy is not clearly established. The present study was carried out to examine whether NO production via the iNOS pathway is mo-dulated in cultured rat mesangial cells exposed to the high glucose environment and underlying mechanism of this modulation. For this purpose, the production of the stable metabolite of NO(nitrite), intracellular cyclic gu-anosine monophosphate(cGMP), iNOS mRNA expression and iNOS protein synthesis were examined under different glucose concentrations. Rat mesangial cells cultured in high glucose concen- tration(30mM D-glucose) increased significantly nitrit#e/ nitrate production and intracellular cGMP levels upon stimulation with lipopolysaccharide(LPS) plus interfer-on-γ(IFN-γ) compared with control glucose concentration(5.6mM D-glucose). Mesangial iNOS mRNA expression and protein synthesis also increased significantly in response to high glucose. This enhanced iNOS mRNA expression induced by high glucose concentration was significantly suppressed by protein kinase C(PKC) inhibitor, calphostin C, and the aldose reductase inhibitor, 6-bromo-l, 3-dioxo-1H-benz[d, elisoquinoline-2(3H)-acetic acid. These results indicate that high glucose in combination with stimulation by LPS plus IFN-γenhances NO production from mesangial cells by the iNOS pathway, and that the activation of PKC and the polyol pathway may play a role in this enhancement.
