        소아마취의 임상통계학적 고찰

        Midazolam maleate, a water soluble banzodiazepine unlike diazepam, was synthetized by Walser and Fryer in 1975. In order to ealuate the effectiveness as an induction agent for general anesthesia, 22 elective surgical patients belonging to ASA classification 1 and 2 were selected. And ten patients (male 7, female 3, average 33.5 years old) were induced with midazolam maleate 0.2mg/kg and twelve patients (mdale 7, female 5, average 32.5 years old) with diazepam 0.3mg/kg. The results were summarized as follows. 1) Anesthesia was induced in all of midazolam group by one intravenous injection, but only 6 patients of diazepam group could induced by one injection and the other 6 patients needed 2 or more injections. 2) The changes of blood pressure and pulse rate after induction and intubation were almost similar in the two groups, but greater individual variation was seen in diazepam group. 3) A buring sensation following intravenous injection was noted in 11 cases of diazepam group, but none in midazolam group and no phlebitis in either group. From the above results, diazepam and midazolam are almost similar in nature but midazolam is definitely effective as an induction agent and has less side effects. Therefore midazolam maleate is regarded as a more effective agent than diazepam as an induction for general anesthesia.

        소장내용물의 위내역류 및 위내용물 배출시간에 관한 연구

        Radionuclide enterogastric scintigraphy was done with (99m)Tc-DISIDA and (111)In-DTPA in 35cases; control group 5, gastric ulcer group 6, subtotal gastrectomy with Billroth g group 5, and functional gastrointestinal disorder group with or without grossly mixed bile in the gastric juice 19 cases. The results were: 1) Enterogastric reflux was very significantly increased in the subtotal gastrectomy with Billroth II group (p$lt;0, 001), 2) Gastric emptying was significantly delayed in the functional gastrointestinal disorder groups with or without grossly mixed bile (p$lt;0.05, p$lt;0. 001). 3) Enterogastric reflux was not significantly increased in the gastric ulcer group. 4) Enterogastric reflux was not significantly different between the functional gastrointestinal groups with or without grossly mixed bile in the gastric juice. 5) Inverse correlation between the gastric emptying indices and the enterogastric reflux indices was not proved (r=-0.37).

        방전 플라스마에 의한 CFC-12(CCl2F2)의 분해

        Decomposition efficiency, power consumption, and applied voltage of CFC(Chlorofluorocarbon) were investigated by SPCY(surface induced discharge plasma chemical processing) reactor to obtain optimum process variables and maximum decomposition efficiencies. Decomposition efficiency of CFC-12 with various electric frequencies(5∼50㎑). flow rates (100∼1,000mL/min), initial concentrations(100∼1,000ppm), electrode materials(W, Cu. Al), electrode thickness(1, 2, 3mm) and reference gases(N₂, O₂, air) were measured and the products were analyzed with FT-IR. Experimental results showed that at the frequency of 10kHz, the highest decomposition efficiency of 92.7% for CFC-12 were observed at the power consumptions of 29.6W, respectively, and that decomposition efficiency decreased with increasing frequency above 20㎑ and decomposition efficiency per unit power were 3.13%/W for CFC-12. Decomposition efficiency was increased with increasing residence times and with decreasing initial concentration of pollutants. Decomposition efficiency was increased with increasing thickness of discharge electrode and the highest decomposition efficiency was obtained for the electrode diameter of 3mm. As the electrode material, decomposition efficiency was in order that tungsten(W), copper(Cu), aluminum (Al).

        경막외마취 확산의 비교관찰 제 1 보 Bupivacaine-Lidocaine

        Epidural anesthesia is widely practiced for lower abdominal operation and delivery in many hospital and its complication in minimal compared with spinal anesthesia. Lidocaine has been used extensively for epidural anesthesia with very satisfactory results. It has a very rapid onset of action, producing complete analgesia and has a reasonable duration of action(about 1-1½hr). A concentration of 1. 5% lidocaine causes effective sensory and autonomic blockade but it is unlikely to produce motor paralysis. More recently, bupivacaine(0.5%) has tended to supplement lidocaine as the drug of choice of epidural anesthesia. Firstly, the duration of action is longer. Secondly, it has better affinity for the tissue so that less will be absorbed into the circulation, thus reducing the risk of a toxic reaction. Thirdly, when used with a continuous technique, there is much chance of tachyphylaxis. Using a standardized anesthesia technic, we compared level of sensory anesthesia with a 0. 5% bupivacaine and 1. 5% lidocaine uder elective and emergency surgery. When equal volume of local anesthetic solution were injected(20 ml of 0.5% bupivacaine and l. 5% lidocaine) there were no statistically significant differences in sensory level in bupivacaine and lidocaine, but the duration of anesthesia in bupivacaine from 2 to 3 times longer than lidocaine.

        원심모형실험에 의한 굴착 흙막이벽의 안정 및 토압분포

        In this study, centrifugal model tests were performed to investigate the behavior of excavated earth retaining wall with the depth of excavation and different types of wall(aluminum, steel panel). Jumunjin standard sand was used for foundation soil. The raining method was adopted to form the required relative density of the model ground. The lateral earth pressure measured from tests were compared with estimated active earth pressure by Rankine's theory. The test results have shown that the earth pressure acting on the retaining wall and the rotation displacement of the wall are influenced by the depth of excavation and the type of wall. It was found from the test results that the deformation of the wall increases with the depth of excavation.

        Atropine과 Neostigmine이 심박수 및 심전도에 미치는 영향

        Four modes of administrating atropine and neostigmine to reverse pancuronium block were studied in thirty-eight anesthetized patients, with special referance to their effects on heart rate and electrocardiogram. At the end of operation in group 1 a mixture of atropine (0. 5 mg) and neostigmine(1. 0 mg) was rapidly injected together. In group 2 the same mixture was slowly injected in 3 minutes. In group 3 atropine was rapidly injected and 2 minutes later neostigmine was rapidly injected. In group 4 neostigmine was slowly injected in 1 minute and then immediately atropine was rapidly injected. The results were as follows. 1) In all groups a transient increase in heart rate was followed by a decrease which continued over 30 minutes. 2) The changes in heart rate were most pronounced when atropine was given before neostigmin(group 3) On the ther hand when atropine was given immediately after neostigmine the changes in heart rate seemed to be smallest. 3) A small number of cases showed transient ECG changes in each group. It is considered that one must be careful about arterial gas and ECG changes although the number of cases is small and certain correlation was not indicated but the ECG of the five of 38 patients are abnormal, four of these are among the abnormal cases in the gas study, two of the four abnormals are under PaO₂, 60 mmHg.

        인장 - 압축 하중 하의 균열선단의 탄 소성 응력해석

        In this study, theoretical stress field analysis near the crack tip under tension-compression loading was performed. The results of the theoretical stress analysis were compared to the results of Finite Element Method(FEM). From this study, generation of tensile residual stress at crack tip was proved after 1-cycle of tension-compression loading, and the fracture toughness and the fracture load of a structure can be decreased by the residual stress.

        흉막염환자에 있어서 흉막액 Adenosine Deaminase 활성도의 임상적 의의

        A clinical observasion was done on 145 cases of the various pleural effusions admitted to Yonsei University Hospital from Mar. 1984 to Mar. 1986, The pleural effusions were due to tuberculosis(76 cases), suspected tuberculosis(18 cases), malignancy(16 cases), suspected malignancy(22 cases), empyema or pneumonia(10 cases), miscellaneous (3 cases). We have measured the ADA activity of pleural fluids in each cases and the results were as follows. 1) In the tuberculous pleural effusion, the ADA activity of pleural fluid was significantly higher than that in the malignant pleural effusion. 2) There was no difference between the activities of pleural fluid ADA in the tuberculous pleural effusion and that in the suspected tuberculous pleural effusion. 3) If the cut-off value of pleural fluid ADA activity is 40U/L, the sensitivity was 90.4% and the specificity was 86.3% in the dignosis the tuberculous pleural effusion. It is suggested that the measurement of ADA activity in pleural effusion is of value in the differential diagnosis of patients with tuberculous pleurisy from those with malignant pleurisy and others, particularly when the results of other laboratory and clinical tests are negative, if the value is over 40 U/L.

        기계적 합금법으로 제조된 Al-Nb 합금 및 Al-Al3Nb 복합재료 압출재의 인장 특성

        Tensile behavior of mechanically alloyed(MA) Al-5 wt.%Nb alloy extrusion was examined at various temperatures and strain rates. The tensile ductility of MA Al-5wt.%Nb alloy extrusion was greatly reduced with increasing temperature and/or decreasing strain rate. The SEM observations of side and fracture surfaces of tensile fractured specimens demonstrated that the overall void density formed during deformation was considerably reduced, along with the sign of decreased plasticity around the dimples on the fracture surface. To understand the effect of grain size on the deformation behavior of submicron grain sized alloys, Al-5 vol. %Al₃Nb composite extrusions with different grain sizes were fabricated and tensile tested at different temperatures. The present study confirms that the reduction in tensile ductility of submicron grain sized alloys at elevated temperatures are undoubtedly related to the grain size. Fast dynamic recovery rates, due to the lack of dislocation substructures, in submicron grain sized alloys appears to be responsible for the elevated temperature reduction in tensile ductility.
