The comparison of operative outcomes between “Energyless” myomectomy and the conventional myomectomy

Objective: Advance in surgical energy devices facilitates hemostasis and tissue dissection in various surgical fields including myomectomy to preserve the fertility in young patients. However, the use of energy devices can cause the thermal injury to myometrium or endometrium during myomectomy, which may adversely affect the subsequent pregnancy or the fertility. Here, we performed myomectomy without energy device and assessed its perioperative outcomes. Methods: Retrospective non-inferior study was conducted with 253 patients who underwent myomectomy between December 2016 and December 2021 at the Department of Obstetrics and Gynecology, Ulsan University Hospital. 188 Patients underwent conventional myomectomy, and 65 patients did ‘energyless myomectomy in which uterine incisions were made using scissors and bleeding was controlled by vasopressin injection and suture. Perioperative outcomes were evaluated according to several indices such as expected blood loss (EBL), operation time (OP time), perioperative change in the level of hemoglobin, and the day of hospitalization. Results: The demographic characteristics between the two groups showed no difference in age, body mass index, and gravida. Analysis did not reveal any differences in EBL, OP time, and hospitalization day. However, the perioperative change in the level of hemoglobin was significantly lower in the energyless myomectomy than in the conventional operation (P < 0.01). Conclusion: Energlyless myomectomy is a feasible method and can have benefits due to no increase in the perioperative complications. It also has low risk of thermal injury to the myometrium or the endometrium. Thus, we expect it to improve the fertility preservation and the outcome of the subsequent pregnancy, but further analysis of the long-term prognosis is needed.

<P>The development of sulfonated poly (ether ether ketone)-based (SPEEK) membranes by the incorporation of layered double hydroxide (LDH) as inorganic nanofiller has been achieved and its potential application in proton exchange membrane fuel cells (PEMFCs) has been investigated. The physical properties and proton conductivity of SPEEK matrix incorporated layered double hydroxide (LDH) were analyzed and found that the incorporation of LDH to SPEEK improves the thermal stability, water uptake and proton conductivity compared to the virgin SPEEK. The proton conductivity of the SPEEK/LDH composite membrane containing 3 wt% LDH has been found to be 229.9 mS/cm where as the virgin SPEEK shows the proton conductivity value 147.6 ms/cm at 80 °C and 100 %RH.</P>

This paper proposes a path planning and obstacle avoidance algorithm for USV(Unmanned Surface Vessel). Path planning for autonomous driving is largely classified into global path generation and regional path generation. First, the A<SUP>*</SUP> algorithm is used when creating the global path. At this time, when applying the simple A<SUP>*</SUP> algorithm, since the rotation radius and dynamic characteristics of USV are not taken into account, a path generated from the A<SUP>*</SUP> algorithm is modified by applying a cardinal spline. Next, the P-DWA (Predictive-Dynamic Window Approach) algorithm was applied to avoid the fixed/moving obstacles of the local path and follow the waypoints generated from the global path.

This study investigated the problem of the existing rural development plan where the opinions of local residents are excluded due to the lack of expert knowledge and limited participation and sought the village development plan driven by the voluntary participation of local residents by applying a rural forum targeting rural villages to solve the problem. As a result of the study, ① the problem of the current village development plan should be made based on the participation of local residents, and ② this problem has remained as an important challenge. Therefore, the village improvement activities that have reflected the opinions of local residents can be said to be ③ the process that seeks the change into the rural planning process led by local residents, and this study derived the ways to ensure the voluntary behavior of local residents, strengthen local residents` capacity to create on-going villages, and expand local residents` participation opportunities in the entire process of rural planning through the application of the rural forum. ④ Although it is generally difficult for local residents to propose opinions directly and exert influence in the rural planning and design process and it is not easy to reduce the difference in the perspective between the professionals and local residents, significance lies in the fact that local residents voluntarily determine the future of their region and derive the plan. Therefore, the experience of decision-making and consensus process through the rural forum can promote the sustainability of the village creation participated by local residents and the increase of local residents` role. This study took one village as an example to reflect the results applied to the rural forum, and it is difficult to determine that it has been fully verified with the method of residents` participation. Therefore, future studies to verify the effectiveness of village creation and seek the empirical utilization measures are needed more, and the operation of education programs and various support plans suitable to the capacity and level of local residents should be promoted rather than the traditional lecture-style stereotyped education in the future for sustainable rural development.
