Purpose: To analyze the clinical results of total knee arthroplasty In elderly patients who had arthritic knees and intertrochanteric fractures. Materials and Methods: From fan 1992 to March 1999, 27 cases of the patients with arthritic knees and associated intertrochanteric fractures were treated by total knee arthroplasties and we evaluated the patients by Knee Score of American Knee Society. Results: Flexion contracture was improved from 10.3 degree to 5.2 degree, KSS score was improved from 57.4 points to 78.6 points, authors' evaluation system score was improved from 50.3 points to 73.2 points. Conclusion: In elderly patients with arthritic knees and associated intertrochanteric fractures, total knee arthroplasty seemed to decrease the rehabilitation time and allow earl weight bearing, so provide an improvement in quality of life.
증기발생기 취출계통으로부터의 방사성 폐기물 방출에 관한 연구

증기발생기의 냉각수 취출에서 발생하는 방사선의 영향을 조사하기 위해서 설계상 방출량 10GPM일 때 외부에 나타나는 방사선 준위를 계산하였다. 이 결과 액체 취출물의 누적 총방사선 량은 12.574Ci/yr이며 기체로 방출되는 것은 주로 I-131 및 I-133으로 나타났다. To investigate the radioactive effects produced from the steam generator blowdown system, off-site radioactive levels are calculated for the design base of coolant effluents with 10 GPM. As a result, total accumulated radioactivity in the liquid from blowdown system is 12.574 Ci/yr and main constituents of discharge gages are I-131, I-133.
Effects of Buckwheat, Potato and Rice on Glycemic Indices in Healthy Subjects

We compared the long-term metabolic effects of equal amounts of carbohydrate from potato, rice and buckwheat on glycemic indices and blood lipids in healthy subjects. Nine healthy volunteers-2 men and 7 women were studied. All subjects ate diets based on the same-7-day rotating menu differing only in that the major source of carbohydrate (about 50% of daily total calories) came either from buckwheat, rice or potato. The study was conducted with a triple crossover design over three 7 day periods. On the morning of the 8th day, fasting blood was drawn from each subject to determine serum glucose, insulin, triglycerides, total and HDL-cholesterol. Subjects were then asked to eat breakfast with their respective carbohydrate within a 20 min period. Blood samples were drawn at 30, 60, 120 and 180 min after the start of breakfast to determine glucose and insulin levels. At 30 min the glucose response to the rice meal(7.15mmol/L) and potato meal(6.71mmol/L) were greater than the response to the buckwheat meal(5.855mmol/L) (P < 0.05). The mean area under the glucose response to the curve following the rice meal was greater than that following the buckwheat meal(P < 0.05). The insulin responses to the potato and rice meals at 30 and 60 min were greater than those to the buckwheat meal (P < 0.05). The mean area under the serum insulin response curve after the rice meal was greater than of buckwheat. Blood lipids, uric acid and glycosylated hemoglobin were not affected by the three meals. The study shows that the buckwheat meal has more beneficial effects on glycemic indices than either the rice meal or potato meal in healthy subjects.
The Exposure Status and Biomarkers of Polycyclic Aromatic Hydrocarbons in Shipyard Workers

Because shipyard workers are involved with various manufacturing process in shipyard industry, and they are exposed to many kinds of hazardous materials. Especially, painting workers were exposed polycyclic aromatic hydrocarbons (PAH). This study was conducted to assess the exposure status of PAH based on job-exposure matrix. We investigated the effect of genetic polymorphism of xenobiotic metabolism enzymes involved in PAH metabolism on levels of urinary metabolite. A total of 93 shipbuilding workers were recruited in this study. Questionnaire variables were age, sex, use of personal protective equipment, smoking, drinking, and work duration. The urinary metabolite was collected in the afternoon and corrected by urinary creatinine concentration. The genotypes of CYP1A1, CYP2E1, GSTM1, GSTT1 and UGT1A6 were investigated by using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods with DNA extracted from venous blood. Urinary 1-OHP levels were significantly higher in direct exposured group (spray and touch-up) than indirect exposed group. Urinary 1-OHP, concentration of the high exposure with wild type of UGT1A6 was significantlyhigher than that of the high exposure with other UGT1A6 genotype. In multiple regression analysis of urinary 1-OHP, the regression coefficient of job grade was statistically significant (p<0.05) and UGT1A6 was not significant but a trend (p<0.1). The grade of exposure affected urinary PAH concentration was statistically significant. But genetic polymorphism of xenobiotics metabolism enzymes was not statistically significant. Further investigation of genetic polymorphism with large sample size is needed.
Development of a Tip-Tilt Mechanism for a 1 m large mirror

A tip-tilt mechanism is one of the key technologies in satellite payloads for meteorology and remote sensing. The same technology is also used for ground telescopes to enhance the image quality. Giant Magellan Telescope is a next generation giant telescope, which contains a tip-tilt secondary mirror. A consortium led by Korea Astronomy and Space science Institute (KASI) is developing a prototype of the tip-tilt secondary mirror. The prototype is a full-size segment of the secondary mirror system, consisted of a dummy mirror, a mirror cell, three axial supports, a lateral support, and a vacuum device. Design, analysis, manufacturing, and assembly have been performed, and system test will be followed. In this paper, development of the prototype is presented and a sample test is shown. The tip-tilt technology will be beneficial to many fields related to precision control.

Background and Objectives: Impaired recovery from left ventricular (LV) dysfunction is a major prognostic factor after myocardial infarction (MI). Because P2Y12 receptor blockade inhibits myocardial injury, ticagrelor with off-target properties may have myocardial protection over clopidogrel. In animal models, ticagrelor vs. clopidogrel protects myocardium against reperfusion injury and improves remodeling after MI. We aimed to investigate the effect of ticagrelor on sequential myocardial remodeling process after MI. Methods: High platelet inhibition with ticagrelor to improve LV remodeling in patients with ST-segment elevation MI (HEALING-AMI) is an investigator-initiated, randomized, open-label, assessor-blinded, multi-center trial done at 10 sites in Korea. Patients will be enrolled if they have ST-segment elevation MI (STEMI) treated with primary percutaneous coronary intervention and a planned duration of dual antiplatelet treatment of at least 6 months. Screened patients will be randomly assigned (1:1) using an internet-based randomization with a computer-generated blocking with stratification across study sites to either ticagrelor or clopidogrel treatment. The co-primary primary endpoints are LV remodeling index with three-dimensional echocardiography and the level of N-terminal prohormone B-type natriuretic peptide (NT-proBNP) at 6 months representing post-MI remodeling processes. Changes of LV end-systolic/diastolic volume indices and LV ejection fraction between baseline and 6-month follow-up will be also evaluated. Analysis is per protocol. Conclusions: HEALING-AMI is testing the effect of ticagrelor in reducing adverse LV remodeling following STEMI. Our trial would show the benefit of ticagrelor vs. clopidogrel related to the recovery of post-MI LV dysfunction beyond potent platelet inhibition. DNA Ligase4 as a Prognostic Marker in Nasopharyngeal Cancer Patients Treated with Radiotherapy

Background: The capability for DNA double-strand breaks (DSBs) repair is crucial for inherent radiosensitivity of tumor and normal cells. We have investigated the clinicopathologic significance of DNA repair gene expression in nasopharyngeal (NP) carcinoma. Materials and Methods: A total of 65 NP cancer patients who received radiotherapy were included. The immunopositivity to Ku 70, DNA-PKcs, MRN, RAD50, XRCC4, and LIG4 were examined in all tumor tissues. Results: The patients comprised 42 males and 23 females, with a median age of 56 years (range, 18-84). The expression levels of RAD50 (0,+1,+2,+3) were 27.7%, 32.3%, 21.5%, and 18.5%. LIG4 (${\pm}$) were 43.1% and 56.9% respectively. The 5-year OS rate of patients with LIG4 (${\pm}$) were 90% and 67.9%, respectively (p=0.035). The 5-year TTP rate of patients with LIG4 (${\pm}$) were 75.9%, 55.5%, respectively (P=0.039). Conclusions: Our results suggest the possibility of predicting the radiosensitivity of NP cancer by performing immunohistochemical analysis of LIG4.
