Background: Dupilumab provided clinical improvement and favorable safety in patients with moderate-to-severe atopic dermatitis (AD) not controlled with topical medications. Objectives: To evaluate the effect of long-term dupilumab in adult Koreans with moderate-to-severe AD. Methods: LIBERTY AD OLE (NCT01949311) is a phase 3 study assessing 300mg weekly open-label dupilumab in adults with moderate-to-severe AD who previously participated in controlled (parent) studies of dupilumab. Results: 96 Korean patients were treated (65.6% male; mean age 30.2 years). At Week 124 (n=40), 57.5% of patients had Investigator’s Global Assessment ≤1, and 85.0% of patients had ≥75% improvement in Eczema Area and Severity Index (EASI-75); 46.9% had EASI-75 by Week 2 (n=96). Mean percent change of weekly averaged Peak Pruritus Numerical Rating Scale (PP-NRS) from parent study baseline to Week 124 (n=35) was -59.31%. In the total OLE population (n=2,677), 9.6% patients had serious treatment-emergent adverse events (TEAE). Most common TEAEs were nasopharyngitis (28.1%), AD exacerbation (16.4%), upper respiratory tract infection (13.1%), conjunctivitis (9.7%), headache (8.1%), oral herpes (7.0%). There was no increase in TEAE rates or new safety signals compared with the parent studies. Conclusion: Dupilumab treatment provided a sustained, multidimensional control in Korean patients with moderate-to-severe AD with the safety profile supporting dupilumab as long-term treatment option.

<P>BACKGROUND: Whether triple antiplatelet therapy is superior or similar to dual antiplatelet therapy in patients with acute ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention in the era of drug-eluting stents remains unclear. METHODS AND RESULTS: A total of 4203 ST-segment elevation myocardial infarction patients who underwent primary percutaneous coronary intervention with drug-eluting stents were analyzed retrospectively in the Korean Acute Myocardial Infarction Registry (KAMIR). They received either dual (aspirin plus clopidogrel; dual group; n=2569) or triple (aspirin plus clopidogrel plus cilostazol; triple group; n=1634) antiplatelet therapy. The triple group received additional cilostazol at least for 1 month. Various major adverse cardiac events at 8 months were compared between these 2 groups. Compared with the dual group, the triple group had a similar incidence of major bleeding events but a significantly lower incidence of in-hospital mortality. Clinical outcomes at 8 months showed that the triple group had significantly lower incidences of cardiac death (adjusted odds ratio, 0.52; 95% confidence interval, 0.32 to 0.84; P=0.007), total death (adjusted odds ratio, 0.60; 95% confidence interval, 0.41 to 0.89; P=0.010), and total major adverse cardiac events (adjusted odds ratio, 0.74; 95% confidence interval, 0.58 to 0.95; P=0.019) than the dual group. Subgroup analysis showed that older (>65 years old), female, and diabetic patients got more benefits from triple antiplatelet therapy than their counterparts who received dual antiplatelet therapy. CONCLUSIONS: Triple antiplatelet therapy seems to be superior to dual antiplatelet therapy in patients with ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention with drug-eluting stents. These results may provide the rationale for the use of triple antiplatelet therapy in these patients.</P>

<P><B>Abstract</B></P> <P>The seawater samples from the Chukchi and East Siberian Seas were collected along a shelf-slope-basin gradient and analyzed for chromophoric and fluorescent DOM (i.e., CDOM and FDOM, respectively). Unexpected high protein-like FDOM (0.35±0.40 and 0.24±0.34 RU for peaks B and T, respectively) levels were identified, which corresponded to 1–2 orders of magnitude higher than those documented by previous reports. This unique phenomenon could be attributed to a fall phytoplankton bloom. The seawater chl-<I>a</I> data, estimated from in situ fluorescence measurements and satellite remote sensing data, showed the subsurface chl-<I>a</I> maximum of up to 1.52mgm<SUP>−3</SUP> at ~25–70m depths and the surface monthly average values (August 2015) up to 0.55 to 0.71mgm<SUP>−3</SUP>, which fall in the range of ~0.5–2.0mgm<SUP>−3</SUP> during fall phytoplankton blooms in this area. Meanwhile, the depth profile of DOM parameters revealed subsurface maxima of protein-like fluorescence peaks along the shelf-slope gradient. The positive correlations between the protein-like peaks and biological index implied the lateral transport of DOM and nutrients from the shelf to the slope and basin. pH-Triggered Transition of Silk Fibroin from Spherical Micelles to Nanofibrils in Water

Many natural proteins self-assemble in complex ways, either to fulfill their biological function or introduce particular properties, such as high strength and toughness. We report the morphological transition in water from a spherical to rod-like shape of Bombyx mori silk fibroin by reducing the pH. Transmission electron microscopy, scanning electron microscopy, and dynamic light scattering were used to characterize the dilute solutions of silk fibroin in an aqueous environment, and provide direct visualization of the transformation of spherical micelles at pH 6.8 to nanofibrils at pH 4.8. This change in morphology occurred as a result of the stretching entropy due to the formation of $\beta$-sheets, which was analyzed using circular dichroism spectroscopy. This study demonstrates the self-assembly of silk fibroin as a function of pH.
Ranks of complex skew symmetric operators and applications to Toeplitz operators

<P>We study the rank of complex skew symmetric operators on separable Hilbert spaces. We prove that a finite rank complex skew symmetric operator can't have an odd rank. As applications, we show that any finite rank commutator of two Toeplitz operators on the pluriharmonic Bergman space of the ball can't have an odd rank. We also show that for any positive even integer N, there are two Toeplitz operators whose commutator is exactly of rank N. Also we obtain the similar result for certain truncated Toeplitz operators. We study commutants of Toeplitz operators acting on the Dirichlet space of the unit disk and prove that an operator in the Toeplitz algebra commuting with a Toeplitz operator with a nonconstant polynomial symbol must be a Toeplitz operator with an analytic symbol.
