        Comparison of the effects of muscle stretching exercises and cupping therapy on pain thresholds, cervical range of motion and angle: a cross-over study

        Objective: Computers and smartphones have become a necessity for modern people, and the use of these things in an inappropriate position has increased the number of people who complain about neck problems. The purpose of this study was to compare the changes of cervical angle, range of motion (ROM) and pain threshold according to the McKenzie stretching and dry cupping therapy. Design: Cross-over design. Methods: We included 12 male and 6 female college students in their twenties, and conducted a pre- and post-test to evaluate the changes of each variable after the application of the McKenzie stretching and dry cupping therapy. Results: Neither the cervical spine angle nor the turtle neck angle showed any change in both the McKenzie stretching and the dry cupping treatment. In the McKenzie stretching, the pain threshold decreased, and the ROM of the cervical spine increased in all directions but there was no significant difference. The pain threshold was increased in the dry cupping treatment, and the ROM of the cervical spine was significantly increased in all directions (p<0.05). Comparisons of the McKenzie stretching and cupping treatment showed that the cupping treatment produced significantly greater pain thresholds and improvements in ROM of the cervical spine than the McKenzie stretching technique (p<0.05). Conclusions: Cupping treatment is more effective in improving ROM of the cervical spine and pain thresholds than the McKenzie stretching technique. In the future, cupping treatment will be one of the treatment options for pain and ROM impairments of the cervical spine.

      • A Necessary New Health Care System in Korea : A Theoretical Review

        Korea is currently faced with severely declining birth rates and an increasing aging population. In particular, the decrease of the productive population is expected to have a huge effect on health insurance finances, and demands from different industries to revise the health insurance policy paradigm based on the past fairness are growing. This article aims at diagnosing Korea’s health care environment theoretically, by analyzing the critical juncture. It also presents future goals and directions for Korea’s health insurance policies by comparing them to this newly constructed paradigm. Results of the literary analysis show that first, in the new paradigm, efficiency and productivity are pursued as the core values. The essence of the insurance premium levying system, improvements of the operating system and wage structure, and allowing exemptions of combined examinations are designed to overcome problems with internal inefficiency and institutional productivity. Second, instead of the treatment-oriented approach of the old paradigm, an attempt was made in the new paradigm to focus on disease prevention and management. This is a preventive approach that promotes disease prevention and customizes health improvement projects using large scale data in order to achieve sustainable medical welfare. Third, the new paradigm is being converged into a global level health insurance policy paradigm. This reflects the OECD recommendations well and is expected to develop at an international level through exchange and cooperation.

        공공병원의 효율성 및 생산성 분석

        Despite its contribution to the Korean medical service industry the question of how efficiently the Korean public hospitals have operated has been unresolved. This study gauges and analyzes the overall efficiency and the Malmquist productivity index in the industry over the period 1992 through 2001. In addition to cost efficiency, we also measure technical, allocative, pure technical and scale efficiencies. Furthermore, the Malmquist index is decomposed into efficiency and frontier changes. We identify several important factors which seem to have strong relationship with various inefficiency estimates. The results indicate that on average the public hospitals has wasted a significant amount of resources and costs over the period. Unlike many other industries, the low level of cost efficiency of the public hospital industry is mainly due to allocative inefficiency rather than technical inefficiency. The Malmquist productivity indices seem both due to the frontier change and efficiency change, but with more effect by the former. The results also indicate that the turnover of hospital beds has played an important role in determining efficiency and productivity of this important industry.

        The correlation between the physical power of golf players and the Titleist Performance Institute Level 1 test

        Objective: This was a correlation study for predicting the power to improve the distance of golf players and developing an effective power program. Design: Cross-sectional study. Methods: The subjects of this study were 24 healthy males and females in their 20s and 30s who were golf athletes living in Seoul and Gyeonggi-do. In the Titleist Performance Institute (TPI) Level 1 test, the twelve swing characteristics include components of stability, mobility, body conditioning ability, and balance. The power test consisted of a vertical jump (VJ) in order to examine the power of the lower body, the seated chest pass (SCP) was used to examine upper body power, and the sit up and throw (SUAT) was used to examine the power of the upper body and core for the TPI Level 2 test. Results: The results of this study showed that the 90/90 was correlated with VJ, SUAT, and SCP scores (p<0.05). Single leg stance was correlated with VJ, SUAT, over head deep squat, and bridge with leg extension were correlated with VJ (p<0.05). The toe touch was correlated with SUAT results (p<0.05). Conclusions: The results of this study showed that the TPI Level 1 test and power test were correlated with the performance of the golf players. Future studies have shown that fitness program development and further research are needed.

      • Selective Functional Movement Assessment(SFMA)-based Therapeutic Corrective Exercises Reduces Knee Joint Pain in a Patient with Patellofemoral Pain Syndrome after Pregnancy (Case study)

        The aim of this study was to determine the effect of selective functional movement assessment (SFMA)-based therapeutic corrective exercise on knee joint pain in a patient with patellofemoral pain syndrome (PFPS) after pregnancy. PFPS is one of the most common causes of knee pain. The present study was conducted in a 35-year-old woman with chronic PFPS following pregnancy. She was seen every other day, 3 times a week for 8 weeks. Physical therapy was performed for tight muscles and fascia in the front, rear, left, and right following mat relaxation, with use of a form roller and corrective exercises suitable for SFMA after simple manual therapy. Corrective exercises were performed with pattern assist, unload, pattern assist with load, and load, based on a 4 × 4 matrix and the patient's status and exercise capacity. For each corrective exercise, photographic materials were also provided so that the patient could carry out the corrective exercises at home. The treatment time per session was 50 minutes. Multi-Segmental Flexion showed improvement from Dysfunctional Non-painful (DN) to Functional Non-painful (FN), Multi-Segmental Extension showed improvement from DP to FN, Single Leg Stance (both right and left) showed improvement from DN to FN, and Overhead Deep Squat showed improvement from DP to FN. The Straight Leg Raising (SLR) test showed an increase on the right from 82° to 90° and from 63° to 81° on the left. The Kujala questionnaire score increased from 63 to 94. The visual analogue scale (VAS) score decreased from 8 to 3. SFMA-based therapeutic corrective exercise was effective for chronic PFPS after pregnancy in this patient

        Effect of Sprinter Pattern Bridging Exercise using Theraband on Activation of Lower Extremity and Abdominal Muscle

        Objective: The purpose of this study was to investigate the effect of sprinter pattern bridging exercise using theraband on activation of lower extremity and abdominal muscle and to find out postures that can effectively improve abdominal and lower extremity muscle strength and increase abdominal stability. Methods: This study was designed as a cross-sectional study. The following research was done with applicants attending S university in Seoul to compare the difference in muscle activity between one-leg-Support bridging exercise and sprinter-pattern bridging exercise using theraband. For 48 study participants, we first measured their MVC. Then, we applied one-leg-support bridging exercise and sprinter-pattern bridging exercise at random order. These data were expressed as the percentage of maximal voluntary contraction (%MVC).Electromyography analysis was performed by measuring the external obliques, internal obliques, biceps femoris, and gluteus maximus. Results: There was a statistically significant increment of muscle activity in external and internal oblique muscle(p<0.001)by sprinter-pattern bridging exercise using theraband. On the lower body, statistically significant increment of muscle activity in biceps femoris and gluteus maximus was found(p<0.05). On the other hand, on erector spinae, there was statistically significant decrease in muscle activity(p<0.05). Conclusions: Efficient treatment is expected when sprinter-pattern bridging exercise using theraband is applied clinically.For patients with chronic knee and ankle pain who have difficulty bearing weight, including low back pain and internal rotation of the femur, starting with a low weight bearing, we think it will be helpful in planning systematic training aimed at progressively strengthening the lower extremities.

        Acute effect of self-myofascial release using a foam roller on the plantar fascia on hamstring and lumbar spine superficial back line flexibility

        Objective: The purpose of this study was to investigate the immediate effect of applying self-myofascial release (SMR) to the plantar fascia using a foam roller on hamstring and lumbar spine superficial back line (SBL). Design: Randomized controlled trial. Methods: Thirty-one healthy adults agreed to the method and purpose of the study. Selection and exclusion criteria were screened, and baseline measurements for the Toe Touch test and passive straight leg raise (PSLR) test were obtained. The participants were then randomly assigned to the SMR group or the sham group. After group assignment, the SMR group rolled the surface of the foot from the heel to the metatarsal head using a foam roller for 5 minutes. The sham group received passive mobilization of the ankle joint in the supine position. Afterwards, the Toe Touch test and the passive straight leg-raise test were re-assessed. Results: In the SMR group, the Toe Touch test results showed significant improvement (p<0.05). Left and right PSLR test results showed a significant increase (p<0.05). In the sham group, there was no significant difference between pre and post-test results. The SMR group showed a significant difference in the PSLR test and Toe Touch test compared to the sham group (p<0.05). Conclusions: The results of this study showed that SMR on the plantar fascia was immediately effective for improving the flexibility of the SBL of the lumbar spine and hamstring.

      • The Effects of Masticatory Exercise Using Chewing Gum on Cognitive Function and Stress

        Masticatory function affects energy expenditure, heart rate, working memory, choice reaction time, and psychological arousal. In other words, masticatory function is closely related to cognitive processing ability. It also influences concentration and stress. The purpose of this study was to determine whether masticatory exercise training using chewing gum can improve cognitive function and stress. The participants were recruited from Sahmyook University and were divided into a masticatory exercise group (n = 15) and a control group (n = 15) by having each of the subjects take out one card from a box containing two types of cards representing the study groups. This study used sugar-free chewing gum with xylitol for the masticatory exercise. Cognitive function and stress were assessed before and after the masticatory exercise. The Memorizing Numbers (MN), Finding a Picture by Name (FPN), and Match Card (MC) components of the COMCOG® test were used to assess cognitive function, which is closely linked to memory, attention, and concentration. Stress was measured using the SA-3000P® system. COMCOG® component scores and stress resistance (SR) and stress index (SI) scores were assessed before and after the training. The groups significantly differed in MN, FPN, MC, SR, and SI scores (p < 0.05). These findings suggest that masticatory exercise using chewing gum improves cognitive function and reduces stress.

        The Effect of a Telephone-Based Self-management Program Led by Nurses on Self-care Behavior, Biological Index for Cardiac Function, and Depression in Ambulatory Heart Failure Patients

        Purpose: This study was conducted to examine the effects of a telephone-based self-management support program led by nurses on self-care behavior, biological index for cardiac function, and depression. Methods: This study is a quasi-experiment in nonequivalent control group design. Thirty-eight heart failure patients underwent medical treatment at the hospital (18 heart failure patients in the experimental group and 20 heart failure patients in the control group). The experimental group (n = 18) received the telephone-based self-management support program, which included a 30-minute face-to face education session and four telephone consultation and education sessions. The face-to-face education session was conducted at the first visit to the outpatient clinic. Thereafter, weekly telephone consultations and education sessions were performed for 4 weeks. Data were analyzed using descriptive statistics, Chi-square test, Fisher's exact test, independent t test, paired t test, and repeated measures analysis of variance using the SPSS/WIN 21.0. Results: The participants in the experimental group showed significantly increased self-care behavior scores (t = 6.65, p < .001), decreased N-terminal pro-brain natriuretic peptide level (U=-2.28, p = .022), improved left ventricular ejection fraction values (t = 2.24, p = .032), and decreased depression scores (t =-3.49, p = .001) compared with the control group. Conclusion: The findings indicate that the telephone-based self-management program is an effective intervention to improve self-management in heart failure patients.
