      • 불안정 지지면을 이용한 운동 프로그램이 뇌졸중 환자의 균형에 미치는 효과

        본 연구의 목적은 뇌졸중 환자의 재활을 위하여 불안정 지지면을 이용한 운동 프로그램이 균형 능력과 운동 기능 능력에 미치는 효과를 알아보고자 실시하였다. 불안정 지지면의 이용한 치료는 임상적으로 뇌졸중으로 인한 편마비 환자뿐만 아니라 스포츠 손상 및 노인들의 질환 등 다양한 병인을 가진 환자들의 치료에 이용되고 있다. 대부분의 뇌졸중 환자들은 재활 훈련을 할 수 있는 병원이나 기관에서 치료를 받을 때에 지지면의 차이를 제공함으로써 보다 더 안정한 지지면에서 적응할 수 있는 방법이 요구되고 있다. 하지만, 아직도 불안정 지지면을 이용한 구체화된 치료적 운동 방법과 객관적 평가에 대한 연구가 미비한 실정이다. 이러한 문제점을 해결하기 위하여 최근 뇌졸중 환자의 불안정 지지면과 이론적 근거를 바탕으로 한 운동 프로그램에 대한 연구가 더욱 필요하다. 본 연구에서는 실험군과 대조군이 같은 운동 프로그램을 적용하였을 때, 두 그룹 간에서 나타나는 균형 향상과 운동 기능 향상에 어떠한 효과를 주는지 살펴보고자 하였다. The purpose of this article was to study the effect of the exercise programme that uses the unstable supporting surface to rehabilitate cerebrovascular patients on balance ability and exercise function ability. The therapy is being clinically used for the patients with diverse pathological factors such as hemiplegia caused by CVA, sports injury, and diseases in the elderly. when most patients with CVA are treated in the hospital or institute that can provide rehabilitation training, they need to be given the way to adjust more stable surpporting surface by being provide with various stages of surface. However, the actual therapeutic exercises is still needed and the study for objective assessments wants conducting. To solve these problems, the study of unstable surface exercise programmes based on the evidences recently need to be made for the patients with CVA. in this study, we intend to look into what effects on the balance improvement and the exercise function are different from the experimental group to the control group with the same programme applied to the two groups.

        Mirizzi's syndrome is a rare complication of the gallstone disease, characterized by narrowing of the common bile duct (CBD) due to chronic extrinsic compression of an impacted gallstone in the cystic duct or the neck of the gallbladder. The impacted gallstone may erode into the bile duct, causing cholecystocholedochal fistula. The patients generally have history of repeated attacks of jaundice and abdominal pain. But there is no report about asymptomatic Mirizzi syndrome with cholecystocholedochal fistula which were found incidentally. So herein we present our clinical experience with a case of asymptomatic Mirizzi syndrome with cholecystocholedochal fistula in a young female.

        Necrotizing pancreatitis develops in about 10-20% of patients with acute pancreatitis, and pancreatic abscesses develop in 3% of them. The mortality of pancreatic abscess has been reported up to 40% in one study. Recently, there are several reports that show good results with n on-surgical therapy, such as percutaneous drainage and endoscopic drainage, but if the non-surgical drainage fails, surgical therapy are recommended. However, there is no definite therapeutic method which is suggested yet so far after the failure of the surgical drainage. So we report the case in which we experienced successful treatment of pancreatic abscess by endoscopic removal of necrotic tissue through the gastrocystostomy, after the failure of the surgical debridement and internal drainage through gastrocystostomy.

      • 림프종으로 오인된 위 매독 1 예

        Syphilis is a chronic systemic infectious disease caused by Treponema pallidum. Syphilitic involvement of the stomach can occur via the blood flow in the primary or secondary period of syphilis, but its incidence is very rare. Gastric syphilis should be considered in patients at risk of sexually transmitted disease who complains of abdominal pain, nausea and vomiting and in whom unusual endoscopic or radiographic features are found. Because gastric syphilis has no pathognomic clinical findings and it can simulate gastric cancer or lymphoma, it’ s not so easy to diagnose. If diagnosed, however, gastric syphilis can be easily treated with proper antibiotics. We report on a case of gastric syphilis that manifested epigastric pain, anorexia and weight loss. It was initially thought to be lymphoma, but it was correctly diagnosed by serologic testing and progress.

        노인 인구에서 신경인지기능저하와 혈청 지질농도와의 상관성

        Objectives : In this study, the authors evaluated the correlation between eurocognitive impairments and serum lipids levels among Korean elderly over the age of 65. Methods : A total of 609 elderly individuals participated in this study. Screening for cognitive impairments were carried out using the Mini-Mental Status Examination-Korean version (MMSE-KC). There were 197 subjects above 1.5SD Of MMSE- KC and they were evaluated using the Korean versions of the Consortium to Establish a Registry for Alzheimer's Disease (AD) (CERAD-KC) and Geriatric Depression Scale (GDS-K). Results : There was a correlation between old age/low levels of education and AD (p<0.05, P<0.01). There was an inverse correlation between the serum level of total cholesterol and the word list recognition test scores. The serum levels of LDL cholesterol were inversely correlated with the scores on the constructional praxis test, word list recall test and word list recognition test. Inverse correlations between the serum level of triglyceride and scores on the word list recall test and word list recognition test were also observed. Conclusion : There were inverse correlations between the serum levels of lipids and language/memory function in subjects with AD.

      • 뇌졸중 환자의 상지 재활 치료에 있어서 건측 억제 유도 기법의 적용 가능성

        본 연구의 목적은 뇌졸중 편마비 환자의 재활을 위하여 강제유도치료(Constraint Induced movement Therapy: CIMT)의 잠재성을 조사하는 것이다. 강제유도치료는 학습된 불용과 피질 재배치 현상을 근거로 개발된 운동 중재로 임상적으로 뇌졸중으로 인한 편마비 환자뿐만 아니라 다양한 병인을 가진 환자들의 치료에 이용되고 있다. 최근 회복시기에 집중적인 치료를 받지 못하는 뇌졸중 환자들을 위한 상지 기능의 향상과 학습된 비사용 현상을 감소시키기 위한 치료 방법으로 강제 유도 운동 치료가 추천되고 있다. 본 연구에서는 강제 운동 유도치료의 정의, 적용분야, 이론적 배경, 신경 생리학적 기전 및 장·단점과 뇌졸중 환자의 상지 재활 치료에 있어서 그 적용 가능성에 대하여 살펴보고자 한다. The purpose of this review was to research the potential of CIMT(Constraint Induced movement Therapy) on stroke hemiplegic patient rehabilitation. Constraint Induced movement Therapy that is exercise intervention developed on the basis of learned non-use and cortex relocation, is used to treat not only storke hemiplegic patient but also the patients with various other pathogenesis. Constraint Induced movement Therapy is recently recommended as a treatment to improve upper extremity function and reduce learned non-use symptoms in stroke patient who is not received intensive therapy during the recovery time. In this review, definition of constrait induced movement therapy, the applications, the theoretical background, the neurophysiological mechanisms and its feasibility of advantages and disadvantages of the upper extremities rehabilitation were discussed.

        Thermoelectric properties of Te-doped In0.9Si­0.1Se with enhanced effective mass

        Metal chalcogenides have attracted attention as potential thermoelectric materials due to their intrinsically low thermalconductivity arising from their layered structure with weak van der Waals atomic bonding. InSe, one of the post transitionmetal chalcogenides, also has low thermal conductivity and doping of InSe with elements such as Sn, Si, and As is knownto improve the electronic transport properties. Herein, we investigated Te doping in Si-doped InSe (In 0.9 Si 0.1 Se) and reportenhanced thermoelectric properties, mainly the increased Seebeck coeffi cient due to the increase in eff ective mass. Dueto the increase in eff ective mass, the magnitude of the Seebeck coeffi cient systematically increased with Te doping from234 μV/K to 405 μV/K. Eventually, the zT at 700 K was enhanced from 0.040 for the pristine sample to 0.069–0.096 for theTe-doped samples.

      • Glucosamine-Induced Reduction of Integrin β4 and Plectin Complex Stimulates Migration and Proliferation in Mouse Embryonic Stem Cells

        <P>We investigated the role of glucosamine (GlcN) on the integrin β4/plectin complex and its role in the regulation of mouse embryonic stem cell (mESC) migration and proliferation. GlcN significantly decreased integrin β4 mRNA/protein expression, whereas plectin protein expression did not change. Also, decrease of integrin β4 expression caused reduction of integrin β4/plectin complex formation, and then increased cell migration. GlcN increased intracellular calcium influx and protein kinase C (PKC) phosphorylation followed by integrin β4 serine phosphorylation and reduction of the integrin β4/plectin complex. GlcN entered into the cell through glucose transporter 1 and then increased O-GlcNAc transferase (OGT) and the level of glycosylation (CTD110.6). Inhibition of OGT (OGT inhibitor; ST045849) increased integrin β4/plectin complex opposite with decreased cell migration. Moreover, GlcN increased O-GlcNAc-specificity protein 1 (Sp1) and nuclear translocated p-Sp1 stimulated calmodulin (CaM) expression, which combined with plectin. In addition, GlcN increased Akt glycosylation and glycogen synthase kinase-3β (GSK-3β) phosphorylation, and then Snail1 glycosylation. Snail small interfering ribonucleic acid (siRNA) reversed the reduction of integrin β4/plectin complex and dissociation of cell junctions (tight and adherent junction). GlcN increased cell migration, cell cycle regulatory proteins [cyclinD1, cyclin-dependent kinase 4 (CDK4), cyclinE, and CDK2], and the percentage of S phase cells, which were inhibited by a PKC inhibitor, CaM siRNA, or Snail1 siRNA. Additionally, GlcN maintained the undifferentiation status of ESCs. In conclusion, GlcN contributed to migration and proliferation of mESCs through integrin β4/plectin complex reduction via Ca2?/PKC, as well as the Sp1/CaM and Akt/GSK-3β/Snail1 signaling pathway.</P>

        Glucosamine-Induced OGT Activation Mediates Glucose Production Through Cleaved Notch1 and FoxO1, Which Coordinately Contributed to the Regulation of Maintenance of Self-Renewal in Mouse Embryonic Stem Cells

        <P>We aimed to study the relationship between glucosamine and FoxO1/Notch in gluconeogenesis and maintenance of mouse embryonic stem cell (mESC) self-renewal. Glucosamine (GlcN) increased glucose production and gluconeogenic enzyme (G6Pase and PEPCK) expression. GlcN also increased the percentage of cells in S phase, number of cells, and the protein expression of cell cycle regulatory proteins that were blocked by 3-mercaptopicolinic acid (gluconeogenesis inhibitor) or glucose transporter (GLUT) 1 neutralizing antibody. GlcN increased the O-GlcNAc transferase (OGT)-dependent protein O-GlcNAc level. Moreover, inhibition of OGT (by ST045849) decreased glucose production. GlcN enhanced the expression of OGT-dependent O-GlcNAcylated Notch1 and then increased the translocation of cleaved Notch1 to the nucleus. Moreover, GlcN stimulated the translocation of O-GlcNAcylated FoxO1 to the nucleus. GlcN increased the binding between cleaved Notch1 and FoxO1 with CSL, a transcription factor, which was blocked by L-685,458 (관-secretase inhibitor) or ST045849, respectively. Simultaneous blockage of cleaved Notch1 and FoxO1 also decreased the expression of G6Pase and PEPCK more significantly than that by inhibition of cleaved Notch1 alone or FoxO1 alone. In addition, GlcN maintained the undifferentiation status while depletion of Notch1 and FoxO1 for 3 days decreased Oct4 and SSEA-1 expression and alkaline phosphatase activity or increased the mRNA expression of GATA4, Tbx5, Cdx2, and Fgf5. In conclusion, GlcN-induced OGT activation mediated glucose production through cleaved Notch1 and FoxO1, which contributed to the regulation of maintenance of self-renewal in mESCs.</P>
