본 연구는 경기도 사회서비스원에서 위탁받아 운영하는 국공립어린이집 원장이 어린이집을 운영하면서 경기도사회서비스원 만의 수준 높은 서비스를 제공하는 데 필수적으로 알아야 할 운영 내용을 담아 국공립어린이집을 처음 운영하거나 이미 운영 중인 원장에게도 운영의 효율성을 높이고, 모호한 부분을 더욱 명백하게 하고, 국공립 원장의 어려움과 심적 부담을 경감시켜주고자 하였다. 또한, 국공립어린이집 원장들이 해당 제도를 정확히 이해하고 영유아에게 적절한 최상의 서비스를 제공할 수 있도록 필요한 운영의 기본과 심화, 전문지식, 기술을 제공하는데 목적이 있다. 이러한 목적을 달성하기 위해 문헌검토와 해당 분야 학계, 현장 전문가들의 의견과 포커스그룹 인터뷰에 참여한 어린이집 원장의 심층인터뷰를 시행하였으며, 수집된 내용을 종합적으로 분석·도출하여 경기도사회서비스원 위탁 국공립어린이집 운영체계 및 기준, 운영관리 방법 및 내용, 권리 존중 및 청렴운영의 4가지 매뉴얼 모듈을 구성하였다. 본 연구는 사회서비스원 위탁 국공립어린이집 원장으로서 갖춰야 할 기본적 역량을 함양하기 위한 주제로 구성함으로써 원장이 본 연구 결과를 활용하여 적확하고 투명한 운영을 할 수 있는 지침과 최상의 보육서비스를 제공할 수 있는 기초를 제공하였다. The aim of this study was to provide the high-quality service at the Gyeonggi Provincial Social Service Center by operating the nursery school, which is entrusted to the Gyeonggi Provincial Social Service Center. In addition to enhancing the efficiency of operations for the first time, this study helped the director, who was already operating the national daycare center, clarify the ambiguous parts of the operations of the daycare center. The purpose of the present invention was to provide basic and in detail operations, expertise, and technology to provide the best service. To achieve this purpose, in-depth interviews of the nursery director who participated in the interviews with academics and field experts in the field and a focus group and a literature review were comprehensively analyzed and derived to form four manual modules for the operation system and standards, operation management method and contents, respect for rights and integrity operation of the Gyeonggi Provincial Social Service Center based on the national level childcare project guide. It is expected that the director will be able to contribute to the realization of the principle of providing the best childcare service and the best benefits for infants and young children through systematic operation of national and public daycare centers, integrity operation, respect for the rights of childcare staff, and helping the infants and young children with confidence and leisure by constructing the theme to cultivate basic competencies to be done.

Guidelines for prescribing opioids for chronic non-cancer pain in Korea

As the treatment of chronic non-cancer pain gradually increases, clinicians have more opportunities to encounter opioid prescription. However, guidelines for prescribing opioids for chronic non-cancer pain have never been published in Korea. The present guidelines were prepared by reviewing various research data. In cases in which the data were insufficient, recommendations were presented following discussion among experts affiliated with the Opioids Research Group in the Korean Pain Society. The present guidelines may need to be continuously revised and amended as more clinical evidence is acquired. (Korean J Pain 2017; 30: 18-33)

Glucose transporter 3 gene variant is associated with survival outcome of patients with non-small cell lung cancer after surgical resection

<P><B>Abstract</B></P> <P>This study was conducted to explore whether polymorphisms of glucose transporter 3 (<I>GLUT3</I>) gene affect the prognosis of patients with non-small cell lung cancer (NSCLC) after surgical resection. Four single nucleotide polymorphisms (SNPs) in <I>GLUT3</I> were investigated in a total of 782 patients with NSCLC who underwent curative surgery. The association of the SNPs with overall survival (OS) and disease free survival (DFS) was analyzed. Among the four SNPs investigated, <I>GLUT3</I> rs7309332C>T was significantly associated with OS and DFS in multivariate analyses. The SNP was associated with significantly worse OS (adjusted hazard ratio [aHR] = 1.62, 95% confidence interval [CI] = 1.04–2.53, <I>P</I> = 0.03, under recessive model), and worse DFS (aHR = 1.64, 95% CI = 1.18–2.29, <I>P</I> = 0.003, under recessive model). When stratified by tumor histology, the association between the <I>GLUT3</I> rs7309332C>T and OS/DFS was not limited to either squamous cell carcinoma (SCC) or adenocarcinoma (AC), although the significant association remained only in AC for OS (<I>P</I> = 0.40 for SCC and <I>P</I> = 0.04 for OS) and only in SCC for DFS (<I>P</I> = 0.03 for SCC and <I>P</I> = 0.08 for OS). When AC patients were stratified according to <I>EGFR</I> mutation status, the SNP was significantly associated with DFS in patients with <I>EGFR</I> mutant tumors (aHR = 2.47, 95% CI = 1.15–5.30, <I>P</I> = 0.02, under recessive model), but not in those with <I>EGFR</I> wild-type tumors. This study suggests that genetic variation in <I>GLUT3</I> may be useful in predicting survival of patients with early stage NSCLC.</P> <P><B>Highlights</B></P> <P> <UL> <LI> <I>GLUT3</I> variant was significantly associated with survival of NSCLC after surgery. </LI> <LI> The association was not limited to either squamous cell carcinoma or adenocarcinoma. </LI> <LI> The SNP may help identify patients at high risk of poor outcome in early stage NSCLC. </LI> </UL> </P>

의료기기산업은 다양한 제품군으로 점차 복잡화되고 다양화되며 정부의 의료정책 및 관리제도의 영향도가 높고, 의료기관 등 수요가 한정되어 있다. 의료기관에서의 의료 기기 구매는 사용부서와 구매부서, 최고경영층 등 여러 이해관계자들의 의사결정 과 정을 통해 이루어진다. 사용부서는 잠재적인 공급업체의 탐색과 적격요건들의 결정을 하게 되는 의료기기 도입의 주도적인 역할을 해야 하지만 현실적으로 구매계획을 위 한 정보탐색은 비전문적이고 단순하다. 전문 의료기기의 경우 온라인 쇼핑에 대한 유 통인프라가 잘 구축되어 있지 않고, 주된 정보의 획득은 영업사원이 제공하는 정보나 타 기관 사용부서의 의견을 공유하는 정도이다. 의료기기를 필요로 하는 소비자는 구 매행정에 비전문가로 정보획득에 대한 전문노하우가 부족할 수밖에 없고 정보의 비대 칭으로 인해 선택의 어려움을 겪는다. 따라서 본 연구에서는 선행연구 자료를 분석하 고 식품의약품안전처와 건강보험심사평가원 등에서 제공하는 공공데이터를 기반으로 의료기기정보를 한눈에 비교하고, 검색할 수 있는 웹 사이트를 제작하고자 한다. 전 문가 자문을 통해 소비자에게 제공 가능한 합법적이고 유용한 의료기기 정보의 범위 를 정해 공공데이터 및 의료기기 업체 정보를 활용한 의료기기 정보를 업로드하고, 사용자 게시판을 통해 자유로운 정보공유가 가능하도록 한다. 본 연구는 웹사이트를 통해 소비자와 공급자 간의 정보 비대칭 문제를 완화하여 소 비자가 필요로 하는 정보들을 최대한 구현하려고 하였다. 바람직한 의료기기 활용을 위해서는 의료기기 사용자나 최종 소비자가 합리적인 선택이 가능한 환경이 필요하 다. 그러기 위해서 정부는 소비자와 공급자의 중간매개자 역할을 함과 동시에 지속적 인 웹사이트의 관리와 개발에 대한 지원이 필요하다. 정부지원을 통해 의료기기 정보 검색 웹사이트가 지속적으로 활성화 된다면 의료기기업체의 공정한 경쟁과 온라인 마 케팅 활용은 물론 소비자의 다양하고 적극적인 정보탐색이 가능하며 나아가 복잡한 유통구조 개선을 통해 투명한 의료기기 산업의 관리가 가능해질 것으로 기대된다.

류마티스관절염 환자에서 백내장 수술 후 발생한 주변부각막궤양 1예

Purpose: To report a case of unilateral peripheral corneal ulcer after cataract extraction in a patient with rheumatoid arthritis. Case summary: A 57-year-old woman who had been taking immune modulating medication since being diagnosed with rheumatoid arthritis previously presented with a chief complaint of visual disturbance and ocular pain 4 days after cataract extraction was performed on her left eye. Slit lamp examination showed a peripheral corneal ulcer around the nasal cataract incision site. Systemic administration of steroid agents for three days reduced the corneal lesion, and then the oral steroid was tapered. However, the peripheral corneal ulcer was found to be aggravated several days later, and oral cyclosporine was added as an immune modulating treatment. After 2 weeks of treatment, thinning of the cornea and the peripheral corneal ulcer stopped and corneal re-epithelialization was found. Conclusions: Treatment with steroids and immune modulating agents may be effective in the management of peripheral corneal ulcers after cataract extraction surgery in patients with rheumatoid arthritis.

Effects of Catechins and Wheat Bran on the Beef Color in the Late Fattening Period of Hanwoo Steers

This study was conducted to evaluate the effects of dietary supplementation of catechins and wheat bran on growth performance, carcass characteristics and meat color using forty-eight Hanwoo steers. Each steer was randomly assigned to one of four treatments. Treatments were as follows: control, catechins, wheat bran, and catechins+wheat bran (CW). At the end of the feeding trial, blood samples were collected for analysis of hemoglobin and iron concentration, and then steers were slaughtered at a commercial abattoir. Catechins, wheat bran, and CW had no effect on growth performance and carcass characteristics. Percentage of myoglobin in M. longissimus dorsi was not affected by treatments. Percentage of oxymyoglobin was increased (p<0.05) by CW and maintained at the highest level (p<0.05) for CW during meat display time up to 24 h. Percentage of metmyoglobin was the lowest (p<0.05) for CW between 5 and 24 h during display time. Although blood iron concentration was not affected by treatments, total concentration of muscle iron was the lowest (p<0.05) for CW compared with that of other treatments. Therefore, the results of the present study suggest that CW may provide good protection against oxidation of myoglobin, and might be an effective dietary supplement for improving meat color in the late fattening period of Hanwoo steers.

Spontaneous bacteriaritonitis is one of the important complication of childhood nephrotic syndrome which occurs not infrequently but this complication in adults with nephrotic syndrome is, however, very rare. The fact that ascites formation is more frequently seen in childhood nephrotic syndrome and that minimal change disease, the commonest pathology found in childhood is associated with impairment in both cellular and humoral immunities may be an explanation for the discrepancy. We have experienced two cases of spontaneous bacterial peritonitis complicated in adults with nephrotic syndrome. The age of patients is 23 and 68 years respectively. The serum IgG level of younger patient is markedly decreased (375 mg/dL) and that of elderly patient is lower normal limit (765 mg/dL). Peritonitis was complicated during relapse in the younger patient. Both of the patients were receiving steroid therapy before complicated by peritonitis. Both patients recovered from peritonitis by antibiotic therapy but, elderly patient died from nosocomial pneumonia. (Korean J Med 62:302-306, 2002)

제조사와 통신사의 통합적 품질이 소비자 만족에 미치는 영향 - 한국 스마트폰 시장을 중심으로

As we can use the wireless Internet and various applications with smart phones, the factors, which affect customers’ satisfaction with mobile phones, are changing accordingly. This paper analyzes the factors that have an effect on consumer satisfaction with telecommunication companies and manufacturers. For the estimation process, we use a framework of the combination of SERVQUAL and Critical Incident Technique. The results show that price, service, wireless Internet and applications are the main factors which satisfy the smart phone users rather than voice service quality. In addition, the manufacturers’ brand name positively affects the satisfaction level with telecommunications companies. 스마트폰의 등장으로 인해 기존의 휴대폰 시장은 변화를 겪고 있다. ‘인터넷도 되는 전화기’ 였던 기존 휴대폰의 개념은 ‘전화도 되는 복합적 단말기’로 변모하였으며 이러한 변화는 소비자 만족에 미치는 요소가 달라질 것임을 암시한다. 또한, 기존 휴대폰 시장에서 가장 중요한 요소로 꼽혔던 음성품질의 수준이 보장됨에 따라 소비자 선택에 있어 제조사의 영향력이 클 것으로 보인다. 따라서 본 논문에서는 기존에 통신사에 초점을 맞추어 소비자의 만족에 미치는 요소를 측정하던 과거의 방식과는 다르게, 제조사 및 통신사의 요소를 모두 반영하여 소비자 만족을 측정 하였다. 소비자에게 직접 만족 혹은 불만족에 영향을 미친 사항을 적게 하는 결정적 사건 방법론(Critical Incident Technique)과, 제품 사용 전 후의 만족도 차이에 따라 최종적 만족도를 측정하는 SERVQUAL을 합하여 프레임워크를 구성하였다. 본 연구에 따르면 제조사와 통신사의 요소가 복합적으로 소비자 만족에 영향을 미치며, 특히 통신사에 대한 만족도가 제조사를 선택하는 데에 영향을 미치는 것으로 조사되었다. 이와 더불어, 제조사의 브랜드네임은 통신사 만족도를 결정하는 데에 영향을 미쳤다. 또한 소비자들은 만족도를 결정하는 데에 있어 통신사의부정적 경험과 제조 사의 긍정적인 경험에 더 큰 영향을 받는 것으로 조사되었다.
