      • Gemcitabine And Cisplatin Followed by Chemo-Radiation for Advanced Nasopharyngeal Carcinoma

        Concurrent chemo-radiation (CRT) has been established as the standard of care for non-metastatic loco-regionally advanced nasopharyngeal carcinoma (NPC) but recently the addition of induction chemotherapy in the already established regimen has presented an attractive multidisciplinary approach. This retrospective study was carried out to evaluate the efficacy of induction chemotherapy (IC) followed by CRT for the management of loco-regionally advanced NPC. Between July 2005 and September 2010, 99 patients were treated with cisplatin based IC followed by CRT. Induction chemotherapy included a 2 drug combination; intravenous gemcitabine $1000mg/m^2$ on day 1 and 8 and cisplatin $75mg/m^2$ on day 1 only. Radiotherapy (RT) was given as a phase treatment to a total dose of 70 Gy in 35 fractions. Concurrent cisplatin ($75mg/m^2$) was administered to all patients on days 1, 22 and 43. All patients were evaluated for tumor response and adverse effects after IC and 6 weeks after the completion of the treatment protocol. Statistical analysis was performed using SPSS version 17 and Kaplan Meier estimates were applied to project survival. Median follow-up duration was 20 months. The 5-year overall survival (OS), loco regional control (LRC) and relapse free survival (RFS) rates were 71%, 73% and 50%respectively. Acute grade 4 toxicity related to induction chemotherapy and concurrent chemo-radiation was 4% and 2% respectively, with only 3 toxicity-related hospital admissions. We conclude that induction gemcitabine and cisplatin followed by chemo-radiation is a safe and effective regimen in management of nasopharyngeal carcinoma, meriting further investigation in randomized clinical trials.

      • Ten Year Experience with Surgery and Radiation in the Management of Malignant Major Salivary Gland Tumors

        Background: Despite being rare in incidence, malignant tumors of major salivary glands show diverse histological variation. There are limited data on major salivary gland tumor management and outcome from Pakistan. The objective of this study was to share our experience with management of malignant tumors of major salivary glands. Materials and Methods: Patients who received treatment at Shaukat Khanum Cancer Hospital and Research Center from July 2002 to June 2011 with an underlying diagnosis of a major salivary gland malignancy were included. Patient characteristics and treatment modalities were assessed. Local, regional and distant failures were determined. Disease free survival (DFS) and overall survival (OS) were calculated using Kaplan Meier curves and the Log rank test was used to determine statistical significance. Univariate and multivariate analyses were performed using Cox proportional hazard regression. Results: The parotid gland was the primary site of origin in 104 (80%) patients. Mucoepidermoid carcinoma (43%) and adenoid cystic carcinoma (24%) were the most common histological types. Surgery followed by adjuvant radiation remained the mainstay treatment modality with 81 (62%) patients. Nineteen (15%) patients were treated with surgery alone and 30 (23%) patients with locally advanced surgically inoperable tumors received radiation only. Forty one (32%) patients failed the treatment (local 12, regional 11, locoregional 5, distant 13). The expected 5 year DFS and OS were 65% and 74% respectively. On multivariate analysis, grade was the only independent predictor of DFS and nodal involvement was the only independent predictor of overall survival. Conclusions: Employing existing standards of treatment, comparable survival can be achieved in Pakistani population with major salivary gland malignancies as elsehwere in the world.

        Collaborative Position Control of Pantograph Robot Using Particle Swarm Optimization

        This article presents the design and real-time implementation of an optimal collaborative approach to obtain the desired trajectory tracking of two Degree of Freedom (DOF) pantograph end effector position. The proposed controller constructively synergizes the Proportional Integral Derivative (PID) and Linear Quadratic Regulator (LQR) by taking their weighted sum. Particle Swarm Optimization (PSO) algorithm is proposed to optimally tune the gains of PID, weighting matrices of LQR, and their ratio of contributions. Initially, the PID and LQR controller parameters are optimally tuned using PSO. In order to enhance the control effort and to provide more optimal performance, the weightages of each controller are optimally tuned and are kept constant. The collaborative position control strategy is tested against the PID and the LQR controllers via hardware in loop trials on a robotic manipulator. Experimental results are provided to validate the accurate trajectory tracking of the proposed controller. Results demonstrate that the optimal combination renders a significant improvement of 10% in steady-state response and about 37% in transient response over the PID and LQR schemes.

        Adaptive Backstepping Based Sensor and Actuator Fault Tolerant Control of a Manipulator

        The purpose of this research is to propose and design fault tolerant control (FTC) scheme for a robotic manipulator, to increase its reliability and performance in the presence of actuator and sensor faults. To achieve the said objectives, a hybrid control law relying on observer and hardware redundancy-based technique has been formulated in this paper. Non-linear observers are designed to estimate the unknown states. The comparison of actual states and observed states lead to fault identifcation, this is followed by fault tolerance accomplished with redundant sensors. For actuator fault tolerance, fault estimation and controller reconfguration techniques are applied in addition to nominal control law. Fault estimation is based on adaptive back-stepping technique and it is further used to construct actuator fault tolerant control. The proposed method is applied to a six degree of freedom (DOF) robotic manipulator model and the efectiveness of this technique is verifed by LabVIEW simulations. Simulation results witnessed the improved tracking performance in the presence of actuator and sensor failures

      • Outcomes of Triple-Negative Versus Non-Triple-Negative Breast Cancers Managed with Breast-Conserving Therapy

        Background: Triple negative breast cancer is associated with aggressive behavior and high risk of local and regional failure. Aggressive surgical intervention is considered suitable. This makes role of breast conserving therapy (BCT) debatable in these patients. The objective of this study was to compare outcome of BCT for triple negative versus non-triple negative breast cancer. Materials and Methods: Medical records of patients who underwent breast conserving therapy from 1999 to 2009 at Shaukat Khanum Cancer Hospital and had complete receptor status information were extracted. Patients were divided into triple negative breast cancer (TNBC) and non-TNBC. Patient characteristics, medical treatment modalities and adverse events were compared. Expected five year locoregional recurrence free, disease free and overall survival was calculated. The Cox proportional hazard model was used to identify independent predictors of outcome. Results: A total of 194 patients with TNBC and 443 with non-TNBC were compared. Significant difference was present for age at presentation (p<0.0001), family history (p=0.005), grade (p<0.0001) and use of hormonal therapy (p<0.0001). The number of locoregional failures, distant failures and mortalities were not significantly different. No significant difference was present in 5 year locoregional recurrence free (96% vs 92%, p=0.3), disease free (75% vs 74%, p=0.7) and overall survival (78% vs 83%, p=0.2). On multivariate analysis, tumor size, nodal involvement and hormonal treatment were independent predictors of negative events. Conclusions: Breast conserving therapy has comparable outcomes for triple negative and non-triple negative breast cancers.

      • Synthesis and Biological Evaluation of 3‐thiazolocoumarinyl Schiff‐base Derivatives as Cholinesterase Inhibitors

        <P>On the basis of the observed biological activity of the coumarins, a new set of 3‐thiazolocoumarinyl Schiff‐base derivatives with chlorine, hydroxy and methoxy functional group substitutions were designed and synthesized. These compounds were tested against acetylcholinesterase from <I>Electrophorus electricus</I> and butyrylcholinesterase from horse serum and their structure–activity relationship was established. Studies revealed them as the potential inhibitors of cholinesterase (acetylcholinesterase and butyrylcholinesterase). The <B>3f</B> was found to be most potent against acetylcholinesterase with <I>K</I><SUB>i</SUB> value of 1.05 ± 0.3 μ<SMALL>m</SMALL> and <B>3l</B> showed excellent inhibitory action against butyrylcholinesterase with <I>K</I><SUB>i</SUB> value of 0.041 ± 0.002 μ<SMALL>m</SMALL>. The synthesized compounds were also docked into the active sites of the homology models of acetylcholinesterase and butyrylcholinesterase to predict the binding modes of these compounds. It was predicted that most of the compounds have similar binding modes with reasonable binding affinities. Our docking studies have also shown that these synthesized compounds have better interaction patterns with butyrylcholinesterase over acetylcholinesterase. The main objective of the study was to develop new potent and selective compounds, which might be further optimized to prevent the progression of the Alzheimer’s disease and could provide symptomatic treatment.</P>

        Robust Integral Sliding Mode Control Design for Stability Enhancement of Under-actuated Quadcopter

        In this paper, a robust backstepping integral sliding mode control (RBISMC) technique is designed for the flight control of a quadcopter, which is an under-actuated nonlinear system. First, the mathematical model of this highly coupled and under-actuated system is described in the presence of dissipative drag forces. Second, a robust control algorithm is designed for the derived model to accurately track the desired outputs while ensuring the stability of attitude, altitude and position of the quadcopter. A step by step mathematical analysis, based on the Lyapunov stability theory, is performed that endorses the stability of both the fully-actuated and under-actuated subsystems of the aforementioned model. The comparison of proposed RBISMC control algorithm, with fraction order integral sliding mode control (FOISMC), affirms the enhanced performance in terms of faster states convergence, improved chattering free tracking and more robustness against uncertainties in the system.

        Estimation of Surgical Needle Insertion Force Using Kalman Filter

        This paper presents a novel low-cost technique to measure the insertion force of a surgical needle on a testing surface to check needle strength. A combination of a load cell with a current sensor for a linear DC motor has been used to estimate the insertion force based on Kalman Filter (KF). The custom-designed and in-house fabricated system to estimate the insertion force comprises of the motor coupled with a vertically articulated arm. The needle to be tested is mounted at the end of the arm. Movement of the arm has been controlled electronically to produce the insertion force by the needle to a testing surface. The sensory measurement data generated during this process has been collected using Arduino based embedded electronic hardware. The KF based proposed strategy has been validated using the developed system by conducting various tests with diferent needles. Results in the form of penetration force and friction force have been experimentally observed and are then compared with standard force meters. Comparative analysis witnesses efciency of the proposed approach.

        Fuzzy Fault-tolerant Controller With Guaranteed Performance for MIMO Systems Under Uncertain Initial State

        It is always problematic that the initial value of the trajectory tracking error must be inside the area included in the prescribed performance constraint function. To overcome this problem, a novel fault-tolerant control strategy is designed for a second-order multi-input and multi-output nonlinear system (MIMO-NLS) with unknown initial states, actuator faults, and control saturation. Firstly, a predefined time convergence (PTC) stability criterion is theoretically proven. Then, an error conversion function is introduced to convert the trajectory tracking error to a new error variable with an initial value of zero, and an adaptive fuzzy system is designed to approximate the compound interference composed of actuator fault, parameter perturbation, control saturated overamplitude, and external disturbance. Based on the backstepping control method, prescribed performance control method, and predefined time convergence stability theory, an adaptive fuzzy fault-tolerant controller for the new error variable is designed and theoretically proven for the predefined time convergence of the closed-loop system. The numerical simulation results of the guaranteed performance trajectory tracking control for industrial robots with actuator faults demonstrate that the adaptive fuzzy fault-tolerant control algorithm has strong fault tolerance to actuator faults and anti-interference capabilities. The convergence time and performance of trajectory tracking errors can be preset in advance, and the parameter settings of the prescribed performance constraint function are not affected by the initial state values.

        Some Pyridyl- and Thiophenyl- Substituted 1,2,4-Triazolo[3,4-b]1,3,4-thiadiazole Derivatives as Potent Antibacterial

        The target compounds 6-11a-e were synthesized by condensing 4-amino-5-aryl-3H-1,2,4-triazole-3-thiones 5a-f with various aromatic carboxylic acids in the presence of phosphorous oxychloride. The structures of newly synthesized compounds were characterized by IR, 1H NMR, 13C NMR, elemental analysis and mass spectrometric studies. All the synthesized compounds were screened for their antibacterial activity. Almost all the tested compounds were potent against four different strains of bacteria when compared with that of reference drug ciprofloxacin. Compounds 6c, 6e, 8d, 9b, 9e, 11a and 11b showed nearly equal or lower MIC values than standard drug, against all four tested bacterial strains but rest of the compounds showed excellent antibacterial activities.
