Purpose Despite widespread adoption of open abdominal management (OAM), there is currently no threshold criterion for OAM duration for non-trauma patients. Moreover, there is a positive relationship between morbidity and the duration of OAM, but an uncertain relationship with patients’ age. Therefore, a novel clinical index for the duration of open abdominal management (IDOM) was developed based on the patient’s age and risk of severe complications following OAM to indicate the maximum tolerable number of days of OAM based on the individual’s age. The utility of this new index was evaluated. Methods This retrospective study included 65 non-trauma patients managed with an open abdomen (OA) from August 2015 to August 2018. The IDOM was developed based on the patient’s age. The result indicated the maximum number of OA days. Patients’ demographic and operative variables were examined and patient data was assigned to one of two groups according to whether the actual number of OA days was above or below the calculated IDOM. Prevalence of complications between these groups was compared. Measures of validity were employed to assess the utility of the IDOM for patient complications. Results Sixty-five patients were included. The above-the calculated IDOM group exhibited a significantly longer OA and higher rates of wound complications and postoperative respiratory complications compared with the below the calculated IDOM group. The IDOM predicted the incidence of OA-related complications with a sensitivity of 72.4%, and a specificity of 80.6%. Conclusion The IDOM is a potentially useful tool for appropriate duration at the outset of OA.
In Search of an Organizational Decline Mechanism

We try to explain why the excellent organization decline. We develop Hirschman(1970)'s model to clarify internal organizational mechanism of the declining organization. Hirschman used the concept exit and voice to explain that organization recover from the decline. He focused on the relation between a firm and customers in the market. We assume a firm provides goods to its customers. When the quality of the goods deteriorates, the customers have two options. One option is that they switch to other goods of same kind, or that they stop buying it. This option is exit. The other option is that customers request the improvement of the quality to the firm. This option is voice. We develop these concepts into internal organization. In addition, we change a modeling assumption. we assume the organization members can act independently. In consequence, we offer some fresh insight to organization decline.

Study Design: A prospective cohort study.Purpose: This study aimed to reveal physical functions and comorbidity affecting collapse at ≥4 weeks after hospital admission of patients with osteoporotic vertebral fracture.Overview of Literature: Only a few studies have investigated the influence of physical function and activity on collapse in patients with osteoporotic vertebral fractures.Methods: This prospective cohort study analyzed patients with osteoporotic vertebral fractures admitted to the hospital between March 2018 and October 2019. Logistic regression analysis was performed to explore the predictors of vertebral collapse at >4 weeks after admission. Model 1 used basic medical information and physical functions at admission; model 2 used basic medical information and physical function and activity at >4 weeks after admission.Results: In the model 1 results of logistic regression analysis, cardiovascular disease (odds ratio [OR], 12.27; 95% confidence interval [CI], 1.28–117.91) was extracted as a factor affecting vertebral collapse at ≥4 weeks after admission. In the model 2 results of logistic regression analysis, cardiovascular disease (OR, 34.57; 95% CI, 2.53–471.74), movement control during one leg standing at 4 weeks (OR, 7.25; 95% CI, 1.36–38.71), and Pain Catastrophizing Scale score at 4 weeks (OR, 1.11; 95% CI, 1.01–1.21) were extracted as factors affecting vertebral collapse at ≥4 weeks after admission.Conclusions: Our results indicate that physical functions and comorbidity affect collapse at ≥4 weeks after admission in patients with osteoporotic vertebral fractures.

One of the major problems in strawberry production is difficulty in diagnosis of anthracnose caused by Colletotrichum acutatum or Glomerella cingulata in latent infection stage. We here developed a diagnostic tool for the latent infection consisting of initial culturing of fungi, DNA extraction, synthesis of PCR-amplified probes and microtube hybridization (MTH) using a macroarray. The initial culturing step is convenient to lure the fungi out of the plant tissues, and to extract PCR-inhibitor-free DNA directly from fungal hyphae. For specific detection of the fungi, PCR primers were designed to amplify the fungal MAT1-2 gene. The subsequent MTH step using the PCR products as probes can replace the laborious electrophoresis step providing us sequence information and high-throughput screening. Using this method, we have conducted a survey for a few thousands nursery plants every year for three consecutive years, and finally succeeded in eliminating latent infection in the third year of challenge.

<P>The proton-conductive spots on the membrane surface of sulfonated poly(arylene ether) multiblock copolymer were successfully imaged by current-sensing atomic force microscopy under hydrogen atmosphere at various temperatures and humidities. These spots should be connected to the proton-conductive paths inside the membrane. The average diameter of the spots was approximately 12 nm, consistent with the size of hydrophilic domains observed by transmission electron microscopy. The size of the proton-conducting spots was almost unchanged regardless of the temperature and humidity, whereas the number of the spots increased at higher humidity; the total area of the proton-conducting spots increased accordingly on the membrane surface.
