Heat stroke, a medical emergency, occurs when the body's thermal regulation is upset and unable to dissipate adequate amounts of heat with a rise in body temperature. It is characterized by hyperpyrexia, with a core temperature of 40˚C or more. hot dry skin, and central nervous system disturbance and usually results in rhabdomyolysis and multiorgan failure. Our case, a 43-year-old healthy male, was caused by a marathon, half course, on a sunny day in late summer. He suddenly fell down on the road and was delivered to a nearby hospital, where a seizure developed. He was transfered to our hospital and then displayed Central Nervous System disturbance, hot dry skin, acute liver failure, rhabdomyolysis, anurlc acute renal failure, and disseminated intravasculular coagulopathy. He was treated with general supportive care and hemofiltration. Despite the aggressive management, he died of shock on the fifth day after admission.
Effects of Apolipoprotein A-I on Apoptosis and Cytokine Production in Human Neutrophils

아포지방단백 A-I (apoA-I)은 항염증 및 항산화 작용을 가지고 있는 것으로 알려져 있다. 본 논문에서는 인간혈액의 호중구를 이용하여 in vitro 상태에서 세포사멸과 시토카인의 분비 과정에 미치는 apoA-I의 영향을 조사하였다. 인간 호중구에 apoA-I을 처리한 결과 세포사멸의 정도가 감소되었다. 또한, apoA-I을 함유하는 고밀도 지방단백(HDL)에 의하여도 세포사멸은 현저히 감소되었다. HDL이 결합하는 수용체인 scavenger 수용체 B-1에 대한 항체에 의하여도 세포사멸은 억제되었다. ApoA-I을 20 시간 처리한 세포의 배양액내 인터루킨-8, 인터페론-유도 단백질 10(IP-10) 및 종양괴사인자-α의 분비가 현저히 증가하였다. 호중구에 인터페론 감마와 apoA-I을 동시 처리하였을 때 시토카인 중에서 IP-10과 종양괴사인자-α의 분비가 단독으로 처리한 경우에 비해 현저히 증가되었다. 이러한 결과들은 인간 혈액 호중구에 apoA-I을 처리한 경우 호중구가 활성화되고 호중구의 세포사멸이 지연된다는 것을 제시하는 것으로 apoA-I이 호중구에 대하여 염증성 인자로 작용할 것으로 추정된다. Apolipoprotein A-I (apoA-I) has anti-inflammatory and anti-oxidative properties. This study was designed to investigate whether apoA-I affects apoptosis and cytokine production of human blood neutrophils in an in vitro culture system. Spontaneous apoptosis of neutrophils was significantly delayed by apoA-I. In addition, high density lipoprotein containing apoA-I also delayed apoptosis of neutrophils. Apoptosis of neutrophils was inhibited by anti-scavenger receptor type B-I antibodies. The amounts of interleukin-8, interferon (IFN)-inducible protein 10 (IP-10), and tumor necrosis factor-α (TNF-α) in the supernatants of cultured neutrophils treated with apoA-I were significantly increased. Combined treatment of neutrophils with IFN-γ and apoA-I produced higher amounts of IP-10 and TNF-α than did treatment with IFN-γ or apoA-I alone. The present study reveals that apoA-I activates neutrophils to produce cytokines and delays spontaneous apoptosis of neutrophils. These findings suggest that apoA-I, although a well-known negative acute-phase protein, has a pro-inflammatory effect in neutrophils.

Spontaneous spinal epidural hemorrhage(SSEH) represent 0.3%∼0.9% of spinal epidural-space-occupying lesions. The therapeutic outcome seems to be determined by the accuracy of the diagnosis and by the time interval between the onset of symptom and surgical decompression. Thus, SSEH is a rare spinal emergency and a diagnostic challenge. We experienced two such cases. In one case, the symptoms were confused with those for a ureter stone, aortic aneurysm, spinal cord infarction. That patient completely recovered spontaneously within 2 hours. In the other case, which was initially diagnosed incorrectly as a cerebral infarction, surgical decompression was performed. That patient recovered completely within 1 month.
RFLP Analysis of cag7 Gene of Helicobacter pylori

The cag7 gene of Korean H. pylori strains was analyzed by RFLP to develop a discriminatory tool for genotyping clinical isolates. For this study, a total of 82 H. pylori strains were isolated from the patients; 27 strains from the patients with chronic gastritis, 26 from duodenal ulcer, and 29 from gastric cancer. Genomic DNA was isolated and subjected to PCR targeting entire ORF or the repeat regions I and II of cag7 gene. PCR products from entire ORF or repeat region I of cag7 gene were divided into two types. However, there was no difference in the length of PCR products from the repeat region II. By the PCR genotyping of the entire cag7 gene, genotypes A and B were established, which showed approximately 5,100 and 5,500 bp PCR products, respectively. The repeat region I showed approximately 600 or 1,000 bp DNA fragments by PCR. The length of cag7 gene was determined by the size variation in the repeat region I. In addition, RFLP analysis of the PCR products of cag7 gene showed 11 subtypes, based on the major bands. These findings illustrate that the genetic diversity of the repeat region I would serve a reliable target for the genotyping of the cag 7 gene.

CVD 중년남성의 복합운동 참여가 호흡 순환계에 미치는 영향

This study aimed to analyze the effect of complex exercises on the cardiorespiratory system of middle aged men with CVD (Cardiovascular Disease), and support them to lead more healthy life by providing data for exercise treatment after analyzing proper exercise intensity and making them participate in exercises aggressively to improve risky factors. We were diagnosed as CVD patients in the health screening test, to proceed with this study. We performed submaximum exercise stress test using modified Bruce protocol for the patients and set exercise intensity for each patient based on this test. We stopped exercises at once at this time, when 2 conditions corresponded to out of 3 conditions during submaximum exercise stress test : blood pressure is higher than 180/120 ㎜Hg, ST fragments falls below -2㎜, the heart rate is higher than 160times/min. We decided the point to stop exercise as 100% of individual’s exercise intensity, and performed aerobic exercise at the intensity of 60% for each patient, and proceeded with resistance exercises (warm up exercise: 10min, complex exercise : 80min, cooling down exercise : 10min) at the arms 40%RM, at the legs 60%RM after measuring dynamic maximum strength of each individual (1-RM) for 8 weeks, 100min/time, 3times/week, to achieve the purpose of this study, we processed data measured using the statistical program SPSS 12.0 version. We calculated mean and standard deviation by each item of measured data came from pre/post test, performed paired t-test to verify differences by exercise type between before/after exercise. The results of this study is as follows. The middle aged men with CVD, who participated in the complex program, showed a significant improvement in blood pressure, diastolic blood pressure, heart rate, ST segment, maximum oxygen uptake, ventilation/min.
