Respiratory Failure Induced by Severe Hypothyroidism in a Korean Woman

A 76-year-old Korean woman visited to emergency room because of respiratory arrest and admitted to intensive care unit of the hospital. Severe hypothyroidism was diagnosed after repeated failure of weaning mechanical ventilator. Respiratory arrest and weaning failures were considered to be associated with hypoventilation due to hypothyroidism. She was recovered and weaned from mechanical ventilation after replacement of thyroid hormone. Severe hypothyroidism may be a cause of respiratory failure or weaning failure.

Lichenoid drug eruption is a rare form of delayed type drug eruptions. Among antituberculosis drugs, ethambutol is one of the most common causative drugs to induce lichenoid drug eruptions and cycloserine has been reported known as a rare causative drug of the lichenoid drug eruptions. The lymphocyte transformation test (LTT) is a safe and reliable diagnostic procedure for drug allergy, but it is rarely used for diagnosis of lichenoid drug eruptions. In the current case, we performed an LTT and successfully con. rmed cycloserine as the offending drug in the patient who were treated with many antituberculosis drugs. This suggests that the cycloserine should be considered as a possible causative drug of lichenoid drug eruption and an LTT could be an option for the diagnosis of lichenoid drug eruption due to cycloserine.

Acute transverse myelitis (ATM) is a heterogeneous syndrome characterized by acute spinal cord dysfunction resulting in paresis and sensory and autonomic impairment below the level of the lesion. The etiology of ATM includes parainfectious, paraneoplastic, drug-induced, systemic autoimmune disorders, and acquired demyelinating diseases. ATM is very rare in patients with rheumatoid arthritis (RA). A 79-year-old woman with RA, a recurring peptic ulcer, and congestive heart failure presented with acute weakness of both upper and lower extremities. She was diagnosed with ATM based on the results of laboratory and radiological tests. ATM is usually treated with high-dose glucocorticoid. However, we treated her with intravenous immunoglobulin (IVIG) considering the adverse events of high-dose glucocorticoid, and she improved. We report a case of ATM in a patient with RA successfully treated with IVIG. (Korean J Med 2015,89:604-608)
Very Early Onset of Amiodarone-Induced Pulmonary Toxicity

Amiodarone is a widely used antiarrhythmic agent. Among its various adverse effects, amiodarone-induced pulmonary toxicity (APT) is the most life threatening complication, which has been described mostly in patients who have been in treatment with high accumulative doses for a long duration of time. However, amiodarone therapy in short-term duration induced APT was rarely reported. We describe a case of a 54-year-old man who is presented with symptoms of APT after a few days of therapy for post-myocardial infarction ventricular ta-chycardia. For early diagnosis and successful treatment, awareness and high suspicion of this rare type of early onset APT is crucial in pa-tients with amiodarone therapy.

Background: Nanotechnology based therapy for ischemic disease may offer a novel therapeutic strategy. Methods: The nanoparticles were composed of hydroxybenzyl alcohol-incorporating copolyoxalate (HPOX) with average size of 450nm. HPOX, in the presence of H2O2, degrades completely into three known and safe compounds, cyclohexanedimethanol, hydroxybenzyl alcohol and CO2. To examine the effect of HPOX to blood flow in ischemic region, blood fl ow recovery was assessed by laser Doppler fl owmetry after femoral artery ligation in mice. Also, we analyzed anti-infl ammatory and angiogenesis factors to study the mechanism of increase blood fl ow in ischemic region. Results: % of blood fl ow in ischemic hindlimb were signifi cantly increased in HPOX group compared to saline group (55% vs 23%, p<0.05, 78% vs 29%, p<0.05, respectively) after 7 days and 14 days of surgery. The m-RNA expression of infl ammatory factors including TNF-a, MCP-1, and ICAM were signifi cantly lower in HPOX group than in saline group (60.7%, 48.2%, and 40.0% reduction. p<0.05,respectively). The mRNA expression of angiogenesis factors including VEGF, VEGF-2R, PCAM, and e-NOS were signifi cantly higher in HPOX group than in saline group (62.5%, 25.4%, 69.3%, 10.3% elevation. p<0.05, respectively). When comparing protein expression of p-AKT and p-AMPK, they were also signifi cantly higher in HPOX group than in saline group (33.2% and 74.7% elevation. p<0.05, respectively). To further examine test whether increased angiogenesis plays a role in increased blood flow in ischemic region by HPOX, we analyzed capillary density by CD31-antibody (+) cell in ischemic region (gastrocnemius muscle) 7 days after surgery. The capillary density in ischemic region was signifi cantly increase in HPOX group compared to saline group( p<0.05). Conclusions: HPOX nanoparticles significantly facilitate blood flow to hindlimb ischemia of mice via promotion of angiogenesis and anti-infl ammatory effects.
