        연취급 근로자들의 면역능의 변동에 관한 조사

        Lead induced immunological responses were examined in 16 workers who were occupationally exposed to lead, and 8 healthy meadical students who did not have previous history of lead exposure. The proportion of OKT3+ cell, OKT4+ cell, OKT8+ cell, B cell to the total lymphocytes in peripheral blood and the ratio of OKT4+ cell to OKT8+ cell(OKT4/OKT8) were used to assess the immunological effects. The results were summarized as follows ; 1. In the lead workers, blood lead (21.7 ±7.6㎍/100㎖), zinc protoporphyrin (53.1 ±29.9㎍/100㎖) and urinary lead (54.8 ±17.7㎍/ℓ) were significantly higher than those of the nonexposed subjects (p<0.01). 2. The proportion of OKT3+ cell, OKT4+ cell and OKT4+/OKT8+ were lower in lead workers compared to nonexposed subjects. But this difference was not statistically significant (p>0.1). 3. The proportion of B cell was slightly higher in lead workers compared to nonexposed subjects (P=0.15). 4. The proportion of OKT8+ cell of lead workers was comparable to that of nonexposed subjects.

      • 頭鍼이 中風患者의 단기간 NIHSS와 MBI에 미치는 영향

        Objective ; The aim of this study was to investigate the effect of Scalp acupuncture on recovery of motor disorders in stroke patients. Methods ; Twenty two patients with poststroke-hemiplegia were randomized into two groups. Ten patients(test group) treated by 2 methods-Scalp acupuncture and body acupuncture. The other twelve patients(control group) treated only by body acupuncture. The activity of daily living was measured with a National Institutes of Health stroke scale(NIHSS) and Modified Barthel Index(MBI). The therapy was performed one a day for 2 weeks. Results ; In terms of score of NIHSS, the test group showed statistically meaningful decrease after 2 week treatment, but the control group showed statistically meaningful decrease after 1 week(p<0.05). And in terms of score of MBI, the test group did not show statistically meaningful increase but the control group showed statistically meaningful increase after 1 week(p<O.05). There was no statistically meaningful difference after 1 and 2 week treatment between the groups. Conclusions ; These results support that test group has almost same effectness compared with control group in improvement of the activity of daily living of poststroke-hemiplegic patients.

        Rhabdomyolysis may be defined as a clinical and laboratory syndrome which releases muscle cell contents into the plasma, such as creatine phosphokinase, due to skeletal muscle injuries. It can be the result of many diverse causes including muscle disease, ischemia, genetic disorder, metabolic disorder, infectious disease, drugs, comatose state, trauma or vigorous exercise that can lead to acute renal failure as a complication. Since first reported in 1941, rhabdomyolysis-induced acute renal failure has been documented many times, and the major causes of this disease are carbon monoxide poisoning, snake bites, and etc in Korea Reports of acute renal failure after exercise-related rhabdomyolysis with myoglobinemia are rare. We report a case of acute renal failure caused by exercise-related rhabdomyolysis in a 20 year old man.

        삼중요관 제3형 1례

        Ureteral triplication is an extremely rare congenital anomaly of the urinary tract, which was first described in 1870 by Wrany. A review of the literature revealed only 92 cases published to date 1992. Herein we report a case of ureteral triplication, type Ⅲ in 54 years old woman. She was admitted to the hospital because of incidentally detected left hydroureteronephrosis on transabdominal ultrasonogram which was performed due to vague epigastric discomfort for 2 years. After radiologic study, under the impression of left incomplete duplicating system with nonfunctioning obstructive megaureter, left nephroureterectomy was performed.

        화상 환자에서 발생한 외과적 중재술을 요하는 감염성 심내막염 2예

        감염성 심내막염은 화상 후에 발생할 수 있는 치명적인 합병증이다. 임상 증상은 일반적으로 없는 경우가 대부분이고 지속되는 발열과 양성 혈액 배양 검사가 유일하다. 감염의 다른 확실한 원인 없이 발열과 양성 혈액 배양 검사가 있을 때는 감염성 심내막염을 강력하게 의심해야 하며 심장 초음파로 진단 가능하다. 감염성 심내막염은 사망률은 높지만 초기에 진단하면 항생제 정주를 지속하는 것으로 치료할 수 있다. 저자들은 화상환자에서 발생한 수술적 처치를 필요로 하는 감염성 심내막염 2예를 경험하였기에 보고하는 바이다. Infective endocarditis is a rare but fatal complication following burn injury. The clinical presentation is silent, but with persistent fever and positive blood culture. The manipulation of clinical care as well as the burn wound itself and immunosuppression caused by extensive bums puts the burn patients at risk of bacteremia. Staphylococcus aureus and Gram-negative bacilli are most common pathogens of infective endocarditis following burns. We report herein two cases of infective endocarditis in the burn patients who requires surgical intervention. The first case was caused by Pseudomonas aeruginosa with complication of multifocal pneumonia, and the second case by coagulase-negative stapylococcus with cerebral hemorrhage.

        Analyses of Single Nucleotide Polymorphisms and Haplotype Linkage of the Human ABCB1 (MDR1) Gene in Korean

        Single nucleotide polymorph isms (SNPs) in the MDR1 gene that are responsible for drug efflux can cause toxicity. Therefore, this study determined the SNPs of the Korean MDR1 gene, and analyzed the haplotypes and a linkage disequilibrium (LD) of the SNPs determined. The frequency of 9 SNPs from the MDR1 gene was determined by PCR-RFLP analyses of 100 to 500 healthy individuals. The frequcies of the SNPs were C3435T (47.7%), G2677T (37.6%), G2677A (4.4%), T1236C (21.7%), T129C (8%), A2956G (2.5%), T307C (1.5%), A41aG (9.2%), C145G (0%), and G4030C (0%). Analyses of the haplotype structure and an estimation of the LD of the combined polymorph isms demonstrated that the frequency of the 1236T-2677G-3435T haplotype is much higher in Koreans (14.1%) than in Chinese and western black Africans and the C3435T SNP in Koreans appears to have LD with T129C in Koreans for the first time. These results provide insight into the genetic variation of MDR1 in Koreans, and demonstrated the possibility of a new LD in this gene.

        Parametric Study for Assessment of Reaction Forces on Ship Docking Supports

        The docking analysis of a global ship structure is requested to evaluate its structural safety against the reaction forces at supports during docking works inside a dry dock. That problem becomes more important recently as the size of ships is getting larger and larger. The docking supports are appropriately arranged in a dock to avoid their excessive reaction forces which primarily cause the structural damages in docking a ship and, up to now, the structural safety has been assessed against the support arrangement by the finite element analysis (FEA) of a global ship structure. However, it is complicated to establish the finite element model of the ship in the current structural design environment of a shipyard and it takes over a month to finish the work. This paper investigates a simple and fast approach to carry out a ship docking analysis by a simplified grillage model and to assign the docking supports position on the model. The grillage analysis was considered from the motivation that only the reaction forces at supports are sufficient to assess their arrangement. Since the simplified grillage model of the ship cannot guarantee its accuracy quantitatively, modeling strategies are proposed to improve the accuracy. In this paper, comparisons between the proposed approach and three-dimensional FEA for typical types of ships show that the results from the present grillage model have reasonably good agreement with the FEA model. Finally, an integrated program developed for docking supports planning and its evaluation by the proposed approach is briefly described.

        SiO<sub>2</sub> doped Ge<sub>2</sub>Sb<sub>2</sub>Te<sub>5</sub> thin films with high thermal efficiency for applications in phase change random access memory

        <P>This study examined the various physical, structural and electrical properties of SiO<SUB>2</SUB> doped Ge<SUB>2</SUB>Sb<SUB>2</SUB>Te<SUB>5</SUB> (SGST) films for phase change random access memory applications. Interestingly, SGST had a layered structure (LS) resulting from the inhomogeneous distribution of SiO<SUB>2</SUB> after annealing. The physical parameters able to affect the reset current of phase change memory (<I>I</I><SUB>res</SUB>) were predicted from the Joule heating and heat conservation equations. When SiO<SUB>2</SUB> was doped into GST, thermal conductivity largely decreased by ∼ 55%. The influence of SiO<SUB>2</SUB>-doping on <I>I</I><SUB>res</SUB> was examined using the test phase change memory cell. <I>I</I><SUB>res</SUB> was reduced by ∼ 45%. An electro-thermal simulation showed that the reduced thermal conductivity contributes to the improvement of cell efficiency as well as the reduction of <I>I</I><SUB>res</SUB>, while the increased dynamic resistance contributes only to the latter. The formation and presence of the LS thermal conductivity in the set state test cell after repeated switching was confirmed. </P>

        Development of novel two-interconnected fluidized bed system

        Two-interconnected fluidized bed systems are widely used in various processes such as Fisher-Tropsch, hot gas desulfurization, CO₂ capture-regeneration with dry sorbent, chemical-looping combustion, sorption-enhanced steam methane reforming, chemical-looping hydrogen generation system, and so on. However, conventional two-interconnected fluidized bed systems are very complex, large, and difficult to operate because most of these systems require a riser and/or pneumatic transport line for solid conveying and loopseals or seal-pots for gas sealing, recirculation of solids to the riser, and maintaining of pressure balance. To solve these problems, a novel two-interconnected fluidized bed system has been developed. This system has two bubbling beds, solid injection nozzles, solid conveying lines, and downcomers. In this study, the effects of operating variables on solid circulation rate and gas leakage between two beds have been investigated in a cold mode two-interconnected fluidized bed system. The solid circulation rate increased as the hole diameter on the injection nozzle, the diameter of the injection nozzle, the solid height above the holes, and the number of holes on the injection nozzle increased. The gas leakage between the beds was negligible. Moreover, long-term operation of continuous solid circulation up to 60 hours was performed to check the feasibility of stable operation. The pressure drop profiles in the system loop were maintained steadily and solid circulation was smooth and stable.
