만성 콩팥병 환자에서 추정 사구체 여과율에 따른 25-Hydroxyvitamin D의 특성

Background/Aims: Accumulating data suggest that vitamin D deficiency is prevalent in patients with chronic kidney disease (CKD). However, comprehensive data are lacking for Koreans. The aim of this study was to survey vitamin D deficiency among patients with CKD in Korea and to identify the relationships among various factors. Methods: We conducted a retrospective cohort study of 444 patients who were divided into four subgroups by estimated glomerular filtration rate (eGFR) for comparisons of mean 25-hydroxyvitamin D [25(OH)D] level and other parameters. In addition, non-dialyzed patients were categorized into four groups based on 25(OH)D levels (<10, 10-19, 20-29, and ≥ 30 ng/mL), and risk factors for severe vitamin D deficiency (<10 ng/mL) were investigated. Results: Of patients with an eGFR ≥ 60 mL/min/1.73 m2, 43% (34/79) showed severe 25(OH)D deficiency, and the mean 25(OH)D level was 11.7±5.3 ng/mL. In CKD3 group, 53.2% (41/77) showed severe 25(OH)D deficiency, with a mean level of 11.3±7.2 ng/mL. In CKD4 group, 53.3% (49/92) had severe 25(OH)D deficiency, with a mean level of 11.0±6.2 ng/mL. Approximately 71% (139/196) of patients in CKD5 group showed severe deficiency, and the mean level was 9.2±5.9 ng/mL. Severe 25(OH)D deficiency was affected by winter season, renal function, diabetes, and low-density lipoprotein cholesterol. The serum parathyroid hormone level was inversely correlated with the 25(OH)D level, such that 25(OH)D < 20 ng/mL were associated with a steep increase in parathyroid hormone. Conclusions: Vitamin D deficiency is highly prevalent in the Korean population. Few patients met a sufficient 25(OH)D concentration, even in the early stages of CKD. Our data suggest that 25(OH)D level of 20 ng/mL is a threshold for a rapid increase in parathyroid hormone levels. (Korean J Med 2012;83:740-751)

Objective Spatial normalization is an essential process for comparative analyses that heavily depends on the standard brain template used. Brain morphological differences are observed in different populations due to genetic and environmental factors, causing mismatches in regions when the data are normalized to different population templates. Recent studies have indicated differences between Caucasian and East Asian populations as well as within East Asian populations, suggesting the necessity of population-specific brain templates. Thus, this study aimed to construct a Korean young adult age-specific brain template utilizing an advanced method of template construction to update the currently available Korean template. Methods The KOR152 template was constructed via affine and nonlinear iterative procedures based on prior studies. We compared the morphological features of different population templates (MNI152, Indian_157, and CN200). The distance and volumetric changes before and after registering the data to these templates were calculated for registration accuracy. Results The KOR152 global brain features revealed a shorter overall length than the other population templates. The registration accuracy by distance and volumetric change was significantly lower than that of the other population templates, implying that the KOR152 was more accurate than other templates for the young adult Korean population. Conclusion This study provided evidence for the need for a population-specific template that may be more appropriate for structural and functional studies in Korean populations.

The dental characteristics created by acquired dental treatments can be used as age estimators. This pilot study aimed to analyze the correlation between the number of teeth observed for dental characteristics and chronological age and to develop new non-invasive age estimation models. Dental features on panoramic radiographs (420 radiographs of subjects aged 20-89 years) were classified and coded. The correlation between the number of teeth for each selected code (codes V, X, T, F, P, and L) and age was observed, and multiple regression was performed to analyze the relationship between them. Eleven regression models with various combinations of dental sextants were presented. The model with the data from both sides of the posterior teeth on both jaws showed the best performance (root mean square error of 14.78 years and an adjusted R2 of 0.461). The model with all teeth was the second-best. Based on these results, we confirmed statistically significant correlations between certain dental features and chronological age. We also observed that some regression models performed sufficiently well to be used as adjunctive methods in forensic practice. These results provide valuable information for the design and performance of future full-scale studies.

The site-selective and metal-free C–H nitrationreaction of quinoxalinones and pyrazinones as biologicallyimportant N -heterocycles with t -butyl nitrite is described. Awide range of quinoxalinones were effi ciently applied in thistransformation, providing C7-nitrated quinoxalinones withoutundergoing C3-nitration. From the view of mechanisticpoint, the radical addition reaction exclusively occurred atthe electron-rich aromatic region beyond electron-defi cientN -heterocycle ring. This is a fi rst report on the C7–H functionalizationof quinoxalinones under metal-free conditions. In contrast, the nitration reaction readily takes placeat the C3-position of pyrazinones. This transformation ischaracterized by the scale-up compatibility, mild reactionconditions, and excellent functional group tolerance. Theapplicability of the developed method is showcased by theselective reduction of NO 2 functionality on the C7-nitratedquinoxalinone product, providing aniline derivatives. Combinedmechanistic investigations aided the elucidation of aplausible reaction mechanism.

대한민국의 고령화 진행에 따라, 고령층 및 교통약자의 대중교통 접근성 향상을 위한 웹 서비스를 개발하고자 한다. 음성 기반 UI를 통해 목적지 및 버스 노선 검색을 가능하게 하였으며 과정을 단순화하여 접근성을 향상하였다. 또한 노년층을 위한 UI 기술을 적용하여 화면을 간소화하고 상호작용을 줄여 손쉬운 목적지 검색과 직관적인 UI를 달성하였다. In response to South Koreas aging population, a web service has been developed to enhance the accessibility of public transportation for the elderly and those with mobility challenges. The service incorporates voice-based user interfaces (UI) to simplify destination and bus route searches, improving accessibility. Additionally, UI technologies tailored for the elderly have been applied to streamline the interface and minimize interactions, making it easier for users to search destinations and providing an intuitive UI experience.
A Case of Immunoglobulin G4-Related Tubulointerstitial Nephritis with Extrarenal Involvement

Immunoglobulin G4-related sclerosing disease is a novel clinicopathological disease entity known to involve various organs including the pancreas, bile ducts, gall bladder, retroperitoneum, kidney, salivary gland, lung and prostate. The most common organ involved is the pancreas and cases without pancreatic involvement are uncommon. Positive response to steroids is an important characteristic of this disease and this enables early diagnosis, which is required for good prognosis. We demonstrate a case of immunoglobulin G4-related tubulointerstitial nephritis in a 59-year-old male accompanied by sclerosing cholangitis and sialadenitis without any evidence of pancreatic infiltration. The patient was treated with prednisolone and was fully recovered in 6 months.
