<P>Impairment in macroautophagy/autophagy flux and inflammasome activation are common characteristics of nonalcoholic steatohepatitis (NASH). Considering the lack of approved agents for treating NASH, drugs that can enhance autophagy and modulate inflammasome pathways may be beneficial. Here, we investigated the novel mechanism of ezetimibe, a widely prescribed drug for hypercholesterolemia, as a therapeutic option for ameliorating NASH. Human liver samples with steatosis and NASH were analyzed. For in vitro studies of autophagy and inflammasomes, primary mouse hepatocytes, human hepatoma cells, mouse embryonic fibroblasts with Ampk or Tsc2 knockout, and human or primary mouse macrophages were treated with ezetimibe and palmitate. Steatohepatitis and fibrosis were induced by feeding Atg7 wild-type, haploinsufficient, and knockout mice a methionine- and choline-deficient diet with ezetimibe (10mg/kg) for 4wk. Human livers with steatosis or NASH presented impaired autophagy with decreased nuclear TFEB and increased SQSTM1, MAP1LC3-II, and NLRP3 expression. Ezetimibe increased autophagy flux and concomitantly ameliorated lipid accumulation and apoptosis in palmitate-exposed hepatocytes. Ezetimibe induced AMPK phosphorylation and subsequent TFEB nuclear translocation, related to MAPK/ERK. In macrophages, ezetimibe blocked the NLRP3 inflammasome-IL1B pathway in an autophagy-dependent manner and modulated hepatocyte-macrophage interaction via extracellular vesicles. Ezetimibe attenuated lipid accumulation, inflammation, and fibrosis in liver-specific Atg7 wild-type and haploinsufficient mice, but not in knockout mice. Ezetimibe ameliorates steatohepatitis by autophagy induction through AMPK activation and TFEB nuclear translocation, related to an independent MTOR ameliorative effect and the MAPK/ERK pathway. Ezetimibe dampens NLRP3 inflammasome activation in macrophages by modulating autophagy and a hepatocyte-driven exosome pathway.</P>

Background : Post-transplant lymphoproliferative disorder (PTLD) represents a spectrum of clinically and morphologically heterogeneous group of lymphoid proliferations of varying clonal composition seen following hematopoietic stem cell transplantation (HSCT) or solid organ transplantation. However overt peripheral or bone marrow (BM) involvement by PTLD is uncommon as early lesion. Case: A 38-year-old woman was diagnosed with Philadelphia chromosome positive acute lymphoblastic leukemia (Ph+ALL) in October 2013. Complete remission was achieved after chemotherapy using hyperfractionated CVAD regimen with imatinib, and she received haploidentical HSCT from mother in January 2014. Conditioning chemotherapy consisted of BU/fiU/ATG, and G-CSF mobilized peripheral graft with 6.08 x 10^6 CD34+ cells/kg and 2.62 x 10^8 CD3+ T cells/kg was transplanted. Engraftment and complete donor chimerism were confirmed by BM study and STR test on day 14, respectively. On day 32, WBC was suddenly increased, and up to 217.30 x 10^3/uL on day 37. BM study, donor chimerism study, and RQ-PCR for Bcr/Abl were performed on day 34, however, there were no evidences of leukemic relapse or loss of complete donor chimerism except polymorphic proliferation in B cell lineage expressing EBV latency-associated RNA in BM. Quantitative PCR for EBV-DNA on peripheral blood was also increased as high as 38,825,576 copies/mL on day 39. Pneumonia and septic shock were developed from day 38, and she finally died of pneumonia with multi-organ failure on day 51 in spite of intensive treatments. Conclusion: PTLD has to be considered as one of treatment related complications in HSCT. Although peripheral blood and/or bone marrow involvement of PTLD after HSCT is uncommon as early lesion, early peripheral lymphocytosis can be initial presenting manifestation of PTLD like in this case.

This study is a descriptive survey research to identify factors affecting the nursing intention of nursing college students who will nurse COVID-19 patients. This study was analyzed through convenience sampling for freshmen, sophomore, junior, senior attending the Department of Nursing at E University in Gyeonggi-do from May 17 to August 11, 2022. As a result, it was found that the intention to care for COVID-19 patients increased by 20% every time the knowledge score on COVID-19 increased by 1 point . If the average score for each item in the moral sensitivity question increased by 1 point, the nursing intention increased significantly by 5.21 times, and it was found that the nursing intention for COVID-19 patients increased significantly by 6.93 times each time the average score for each behavioral belief item increased by 1 point. Based on the results of this study, it was confirmed that the higher the knowledge, moral sensitivity, and behavioral beliefs of nursing students, the higher the intention to care for new infectious disease patients. Based on the results of this study, it was confirmed that It is necessary to develop and apply educational and psychological intervention programs and integrated nursing ethics programs.
