Background As the number of Alzheimer's disease (AD) patients steadily increases, many pharmaceutical industries and researchers are making efforts and investments in developing AD treatments. However, numerous AD new drug candidates are frustrated with clearing the hurdles in clinical trials, and few AD treatments have been approved by regulatory agencies worldwide. In order to develop AD treatments, it is essential to consider the characteristics of AD itself and AD patients, and to devise evaluation methods deriving significant clinical safety and efficacy. Area covered This review focused on pharmacometrics, the latest mechanism-based PK/PD modeling and simulation approaches among strategies for developing AD treatments. Including the basic concept of pharmacometrics, several cases of applying pharmacometrics to AD treatments in the approval or development stage were categorized as PK, PD, and PK-PD models and reviewed. Expert opinion Unlike traditional PK/PD models, the population model-based PK/PD evaluations could apply to sparse data collected from a heterogeneous population. Various covariate factors in patients receiving AD treatments were investigated and quantified through the developed and validated population models. Investigating potential covariates of AD treatments could be employed to optimize the dosage regimen for AD patients. Consequently, a summary of the population PK/PD model will be useful information for clinical pharmacometricians and contribute to the development of AD treatments in the future in decision-making such as dosage optimization.
Pharmacotherapeutic Problems and Pharmacist Interventions in a Medical Intensive Care Unit

Background: Interest in pharmacist participation in the multidisciplinary intensive care team is increasing. However, studies examining pharmacist interventions in the medical intensive care unit (MICU) are limited in Korea. The aim of this study was to describe the current status of pharmacist interventions and to identify common pharmacologic problems requiring pharmacist intervention in the MICU. Methods: Between September 2013 and August 2014, a retrospective, observational study was conducted in the 22-bed MICU at a university hospital. Data were obtained from two trained pharmacists who participated in MICU rounds three times a week. In addition to patient characteristics, data on the cause, type, related drug, and acceptance rate of interventions were collected. Results: In 340 patients, a total of 1211 pharmacologic interventions were performed. The majority of pharmacologic interventions were suggested by pharmacists at multidisciplinary rounds in the MICU. The most common pharmacologic interventions were adjustment of dosage and administration (n = 328, 26.0%), followed by parenteral/enteral nutritional support (n = 228, 18.1%), the provision of drug information (n = 228, 18.1%), and advice regarding pharmacokinetics (n = 118, 9.3%). Antimicrobial agents (n = 516, 42.6%) were the most frequent type of drug associated with pharmacist interventions. The acceptance rate of interventions was 84.1% with most accepted by physicians within 24 hours (n = 602, 92.8%). Conclusions: Medication and nutritional problems are frequently encountered pharmacotherapeutic problems in the MICU. Pharmacist interventions play an important role in the management of these problems.

Guar gum is an edible starch extracted from Cyamopsis tetragonolobus and used as an auxiliary ingredient to enhance the viscosity and adhesiveness of food and improve its emulsification stability. Here, characteristics of fish surimi mixture with various added amounts (0, 0.23, 0.45, 0.67, and 0.89 wt%) of guar gum were analyzed. In unheated surimi with guar gum, the pH decreased as the content increased. In the color values analysis, only the b<sup>*</sup> (yellowness) increased depending on the amount of added guar gum. The firmness increased with increasing guar gum content, and no significant difference in adhesiveness was observed. In heated surimi, a<sup>*</sup> (redness) and b<sup>*</sup> decreased in a content-dependent manner, and L<sup>*</sup> (lightness) was higher than that of unheated surimi. Regarding physical properties, the highest values of hardness, gumminess, and chewiness were observed in the heated surimi with 0.67 wt% of guar gum. However, no significant change was observed in springiness and cohesiveness. Results of sensory evaluation indicated that with higher guar gum content, only the hardness increased slightly, and significant differences were absent in the remaining parameters. Thus, surimi with 0.67 wt% of guar gum is considered to be optimal for 3D printing.
