Background: Understanding the risk factors and outcomes of intraoperative periprosthetic femoral fractures (IPFF) during hip arthroplasty is crucial for appropriate perioperative management. Previous studies have identified risk factors for IPFF in total hip arthroplasty patients, but data for hip hemiarthroplasty (HA) is lacking. The aim of this study was to determine the age associated with increased rates of IPFF in patients undergoing HA. Methods: We retrospectively reviewed patients aged 65 years and above who underwent a cementless HA for a displaced femoral neck fracture and had a minimum of 1-year follow-up. Patients were stratified into five age groups (65–79, 80–84, 85–89, 90–94, and ≥ 95 years) and further divided into two subgroups (under 95 years and 95 years or older). The presence, location, and treatment of IPFF, as well as the effect of IPFF on the postoperative weight-bearing status, were compared between groups. A multivariate logistic regression was also performed. A total of 1,669 met the inclusion criteria and were included in the study. Results: The rates of IPFF were significantly higher for patients 95 years or older (p = 0.030). However, fracture location (greater trochanter fractures, p = 0.839; calcar fractures, p = 0.394; and femoral shaft fractures p = 0.110), intraoperative treatment (p = 0.424), and postoperative weight-bearing status (p = 0.229) were similar between the groups. While mortality and nonorthopedicrelated readmissions were significantly higher for patients 95 years or older, orthopedic-related readmissions (p = 0.148) and revisions at the latest follow-up (p = 0.253) were comparable between groups. In a regression analysis, age over 95 years (odds ratio, 2.049; p = 0.049) and body mass index (odds ratio, 0.935; p = 0.016) were independently associated with IPFF. Conclusions: The findings of this study suggest that age over 95 years is a significant, independent risk factor for IPFF in patients undergoing cementless HA. Although we were unable to show an impact on perioperative outcomes and orthopedic complications, when operating on patients 95 years or older, surgeons should be aware of the increased risk of IPFF and consider the use of stem designs and fixation types associated with decreased IPFF rates.

Background: Reverse oblique intertrochanteric fractures (ROFs) are unstable extracapsular hip fractures that present a mechanical challenge. These fractures are classified as AO/Orthopaedic Trauma Association (OTA) 31-A3 according to the Trauma Association classification system and can further be subclassified into 3 subtypes based on their specific characteristics. The study aimed to evaluate and compare the radiographic and clinical outcomes of the 3 subtypes of ROFs. Methods: A retrospective study was conducted at a single high-volume, tertiary center, where data were collected from electronic medical records of consecutive patients who underwent surgical fixation of AO/OTA 31-A3 fractures. Patients with less than 1-year follow-up, pathological fractures, and revision surgery were excluded. The subtypes of fractures were classified as 31-A3.1 (simple oblique), 31-A3.2 (simple transverse), and 31-A3.3 (wedge or multi-fragmentary). The operation was done using 4 different fixation methods, and radiological evaluation was performed at routine intervals. Results: The final population consisted of 265 patients (60.8% women) with a mean age of 77.4 years (range, 50–100 years) and the mean follow-up time was 35 months (range, 12–116 months). The incidence of medical complications was similar across the groups. However, there was a trend toward a higher incidence of orthopedic complications and revision rates in the 31-A3.2 group, although this was not statistically significant (p = 0.21 and p = 0.14, respectively). Conclusions: Based on the findings of this study, no significant differences were observed between the groups, indicating that the subclassifications of AO/OTA 31-A3 fractures do not have a significant impact on surgical outcomes or the occurrence of postoperative complications.

Psychodynamic Psychotherapy for Functional (Psychogenic) Movement Disorders

Objective: As the literature for the treatment of functional (psychogenic) movement disorders (FMD) is sparse, we assessed clinical outcomes in patients with FMD who underwent treatment with psychodynamic psychotherapy (PDP). Methods: A retrospective analysis of the data of patients with FMD who were referred for PDP from 2008−2014 at Emory University Medical Center was performed. Results: Thirty patients were included, mean age at presentation was 50 years (SD 13.9) and majority were female (27/30). Most common movement disorder was involuntary shaking/jerky movements (50%) and tremor (43%). Mean duration of symptoms was 3.2 years and mean number of PDP visits was 4.9. PDP lead to good outcomes in 10, modest in 8, and poor in 9. Three patients lost to follow up. Mean duration of symptoms between two groups (good vs. poor) was not statistically significant (p = 0.11), mean number of PDP visits showed a trend towards significance (p = 0.053). In all cases of good outcomes precipitants of the movement disorder were identified and a majority (60%) was receptive of the diagnosis and had good insight. Conclusion: PDP lead to improvement in 60% of the patients which is encouraging as the treatment is challenging. This study supports heterogeneous causes of FMD including varied roles of past/recent events and demonstrates importance of psychological approaches such as PDP. Treatment with PDP should be considered in some patients with FMD but predicting who will respond remains a challenge. Further long term prospective studies with large sample size and placebo control are needed

Background: Polycomb group protein enhancer of zeste homolog 2 (EZH2) controls cell proliferation and differentiation and is frequently up-regulated in hepatocellular carcinoma. Genetic inactivation of Ezh2 did not produce obvious phenotypic differences in knockout livers suggesting a functional redundancy with Ezh1 homolog. Methods: To investigate the role of both Ezh1 and Ezh2 in liver homeostasis, we generated double knockout mice (E1/2KO) by crossing Ezh1 null and albumin promoter- driven Ezh2 conditional-null mice.The combined loss of EZH1 and EZH2 in mouse hepatocytes caused a global and gene-specific depletion of H3K27me3 marks including genes associated with hepatocyte homeostasis and regeneration. Mice mutant for both Ezh1 and Ezh2 remained viable and overtly normal by 3 months of age. Thereafter, E1/2KO mice exhibited progressive liver abnormalities manifested by development of regenerative nodules and concomitant periportal fibrosis, infiammatory infiltration and activation of A6-positive hepatic progenitor cells (HPCs). Results: In response to chronic treatment with CCl4, E1/2KO mice showed increased hepatic degeneration associated with liver dysfunction, cholestasis, and reduced ability to proliferate. CCl4-induced hepatotoxicity triggered an activation of HPCs, which did not prevent early lethality of E1/2KO mice. After 2/3ds partial hepatectomy, E1/2KO livers showed an increased liver injury and a blunted regenerative response. RNA-seq analysis revealed dysregulation of genes related to regulation of cell survival, fibrosis, and proliferation including Cdkn2a and Cdkn2b. Conclusions: This work demonstrates a critical role of EZH1 and EZH2in maintaining hepatic homeostasis and ability to regenerate.
Telemedicine in an Academic Movement Disorders Center during COVID-19

ObjectiveaaTelemedicine has rapidly gained momentum in movement disorder neurology during the coronavirus disease (COVID-19) pandemic to preserve clinical care while mitigating the risks of in-person visits. We present data from the rapid implementationof virtual visits in a large, academic, movement disorder practice during the COVID-19 pandemic. MethodsaaWe describe the strategic shift to virtual visits and retrospectively examine elements that impacted the ability toswitch to telemedicine visits using historical prepandemic in-person data as a comparator, including demographics, distancedriven, and diagnosis distribution, with an additional focus on patients with deep brain stimulators. ResultsaaA total of 686 telemedicine visits were performed over a five-week period (60% of those previously scheduled for inofficevisits). The average age of participants was 65 years, 45% were female, and 73% were Caucasian. Men were more likely tomake the transition (p = 0.02). Telemedicine patients lived farther from the clinic than those seen in person (66.47 km vs. 42.16km, p < 0.001), age was not associated with making the switch, and patient satisfaction did not change. There was a significantshift in the distribution of movement disorder diagnoses seen by telemedicine compared to prepandemic in-person visits (p <0.001). Patients with deep brain stimulators were more likely to use telemedicine (11.5% vs. 7%, p < 0.001). ConclusionaaTelemedicine is feasible, viable and relevant in the care of movement disorder patients, although health care disparitiesappear evident for women and minorities. Patients with deep brain stimulators preferred telemedicine in our study. Furtherstudy is warranted to explore these findings.
