        Understanding the Influence of Two Types of Ruminations on Posttraumatic Growth

        The main purpose of the current study was twofold: (a) to examine the effects of intrusive rumination and deliberate rumination on posttraumatic growth and (b) to test the mediated moderating effect of psychological acceptance on posttraumatic growth. For this study, 334 South Korean adults completed questionnaires which included demographic information, K-ERRI, K-PTGI, and K-AAQ-II. After data cleaning, a total of 292 responses were analyzed by structural equation modeling. As a result, the initial model which hypothesized that deliberate rumination would predict present intrusive rumination was rejected. In addition, psychological acceptance had a moderating effect on the relationship between past intrusive rumination and present intrusive rumination. The implications for counselors and counseling researchers are discussed.

      • BODY MASS INDEX AND SELF-EMPLOYMENT IN SOUTH KOREA

        <B>Summary</B><P>This study assesses differential labour performance by body mass index (BMI), focusing on heterogeneity across three distinct employment statuses: unemployed, self-employed and salaried. Data were drawn from the Korean Labor and Income Panel Study. The final sample included 15,180 person-year observations (9645 men and 5535 women) between 20 and 65 years of age. The findings show that (i) overweight/obese women are less likely to have salaried jobs than underweight/normal weight women, whereas overweight/obese men are more likely to be employed in both the salaried and self-employed sectors than underweight/normal men, (ii) overweight/obese women have lower wages only in permanent salaried jobs than underweight/normal weight women, whereas overweight/obese men earn higher wages only in salaried temporary jobs than underweight/normal weight women, (iii) overweight/obese women earn lower wages only in service, sales, semi-professional and blue-collar jobs in the salaried sector than underweight/normal weight women, whereas overweight/obese men have lower wages only in sales jobs in the self-employed sector than underweight/normal weight women. The statistically significant BMI penalty in labour market outcomes, which occurs only in the salaried sector for women, implies that there is an employers’ distaste for workers with a high BMI status and that it is a plausible mechanism for job market penalty related to BMI status. Thus, heterogeneous job characteristics across and within salaried versus self-employed sectors need to be accounted for when assessing the impact of BMI status on labour market outcomes.</P>

        Barriers to Safe and Effective Medication for Individuals with Sensory Impairment: A Systematic Literature Review

        Visual impairment (VI) and hearing impairment (HI) are common disabilities whose prevalence increases with age, potentially presenting a major challenge in the future. Identifying the types of obstacles which they encounter during drug administration is essential to improve health outcomes. This systematic literature review aimed to identify types of medication barriers encountered by individuals with VI or HI to improve their health outcomes. Combination of keywords indicating VI or HI, barrier, challenge, or difficulty, and drug or medication were used to identify potential studies up to May 31, 2021, from Ovid Medline, Embase, and Cochrane. Overall, 20 and 17 articles met the predefined eligibility criteria for VI and HI, respectively. Individuals with VI were found to encounter difficulties in the entire process of pharmaceutical therapy, including drug identification, medication management, access to pharmacies, medication instructions, drug information (labeling), and communication. Individuals with HI were found to experience challenges in communication, health literacy, and negative experiences and emotions toward doctors/pharmacists. The drug management needs of individuals with VI and HI remain unmet. For safe and effective medication in individuals with VI and HI, it is essential to establish practical solutions, such as providing identification tools for tablets for those with VI, deploying sign language interpreters at designated healthcare institutions and pharmacies for individuals with HI, utilizing novel technologies, including audio prescription labeling systems or mobile applications, and providing education for both healthcare providers and individuals with disabilities to improve their partnership in drug therapy.

        Causal Analyses of Statin to Prevent Liver Disease Progression: A Nationwide Study Using Superlearning Targeted Maximum Likelihood Estimation

        Many studies have shown that statins reduce the risk of progression to liver cirrhosis (LC) and hepatocellular carcinoma (HCC) among at-risk populations. However, causality has not been proved. This study examined whether statins could prevent LC and HCC in patients with progressive and worsening chronic liver disease, using a robust methodology for causality. Between 2002 and 2013, 52,145 patients with chronic liver diseases were identified from the National Health Insurance Service database in South Korea. The inverse probability weighting (IPW) and superlearning targeted maximum likelihood estimation (TMLE) were used to assess the causality of statin use on the risk of LC and HCC, adjusting for sex, age, comorbidities, and co-medications. Multivariable superlearning TMLE revealed that statin use was associated with reduction in the incidence risk of LC (Marginal odds ratio (MOR) 0.59, 95% confidence interval [CI] 0.50-0.65) and HCC (MOR 0.59, 95% CI 0.50-0.67). Such a protective effect was more evident with atorvastatin and lipophilic statin. This population-based observational study indicated the benefit of statin use, particularly atorvastatin and lipophilic statin, for causally reducing the risk of LC and HCC.

      • Development of a high-energy-density portable/mobile hydrogen energy storage system incorporating an electrolyzer, a metal hydride and a fuel cell

        <P><B>Abstract</B></P> <P>A hydrogen energy storage system for portable/mobile applications such as personal power sources and unmanned underwater vehicles is developed. An application-oriented design and system integration strategy are newly suggested to maximize energy density while incorporating conventional technologies for the electrolyzer (Ely), the metal hydride (MH), and the polymer electrolyte membrane fuel cell (PEMFC). To improve both the energy density and usability, the systems for charging and discharging are separated. The charging component is composed of a water Ely (0.5 Nm<SUP>3</SUP> h<SUP>−1</SUP>) and an MH cooling device as one system. The discharging component consists of an MH (900 NL H<SUB>2</SUB>), a PEMFC stack (50 W), and a power conditioning system (PCS) as a single system. The MH material and engineering properties are investigated to find an MH that is suitable for the target system. The hybrid design and operating strategy of the PEMFC and PCS are developed to maximize energy density. The prototype system provides a nominal power output of 31.5 W at 12 V for 38 h with one recharging. We find it significant that the discharging component shows an energy density of 410 Wh L<SUP>−1</SUP>, which is twice that of conventional energy storage systems at the 2.9-L level.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The core components are a water electrolyzer, a metal hydride, and a PEMFC. </LI> <LI> AB<SUB>5</SUB>- and AB<SUB>2</SUB>-type metal hydrides are investigated as energy storage media. </LI> <LI> The hybrid design and operating strategy of the PEMFC and PCS are developed. </LI> <LI> A 2.9-L H<SUB>2</SUB> energy storage system with a 410 Wh L<SUP>−1</SUP> energy density is developed. </LI> <LI> The system provides 1200 Wh of electricity with a nominal power of 31.5 W at 12 V. </LI> </UL> </P>

      • Endoscopic Variceal Ligation Induced Ulcer Bleeding: What Are the Risk Factors and Treatment Strategies?

        Aims: To determine the risk factors of endoscopic variceal ligation (EVL) induced ulcer bleeding Methods: A total of 430 patients who had undergone EVL in Chonnam National University Hospital from January 2014 to October 2016 were reviewed. EVL was performed for prophylaxis or acute hemorrhage. The patients were classified into two groups: a bleeding group (n=33) and a non-bleeding group (n=397). The patients who had endoscopically confirmed EVL induced ulcer bleeding were included in the bleeding group. The patients without any bleeding sign after the EVL were included in the non-bleeding group. Data including baseline clinical characteristics, endoscopic findings, treatment methods for EVL induced ulcer bleeding, and clinical outcomes were collected and analyzed. Results: EVL induced ulcer bleeding occurred in 7.7% (n=33) of the patients. There were more patients with hepatitis C virus infection (21.3% vs. 13.1%, P=0.005), Child-Pugh classification C (36.4% vs. 13.9%, P=0.008), and concomitant gastric varix form 3 (GV F3) (30.3% vs. 11.8%, P=0.012) in the bleeding group. Initial prothrombin time (PT index, %), Child-Pugh score, Model for end-stage liver disease (MELD) score were associated with EVL induced ulcer bleeding (P=0.026, P< 0.05, and P=0.022, respectively). More bleeding related death was observed in the bleeding group (27.3% vs. 3.6%, P=0.005). In multivariate analysis, MELD score > 10 [OR: 3.42, 95% CI: 1.10-10.64], concomitant GV F3 [OR: 14.1, 95% CI: 2.84-71.43], and detachment of o-ring bands on follow-up endoscopy [OR: 8.06, 95% CI: 2.55-25.64] were independent predictive factors of EVL induced ulcer bleeding. In regarding to treatment methods of EVL induced ulcer, various endoscopic modalities were tried for hemostasis [EVL in eight cases (24.2%), endoscopic variceal obturation (EVO) with cyanoacrylate in 6 cases (18.2%), argon plasma coagulation (APC) in one case (3%), Sengstaken-Blakemore (SB) tube in 3 cases (9.1%), and proton pump inhibitor (PPI) only in 15 case (45.5%)]. Conclusions: MELD score> 10, concomitant gastric varices F3, and detachment of o-ring bands on follow-up endoscopy are risk factors for EVL induced ulcer bleeding.

        Deoxypodophyllotoxin Inhibits Cell Growth and Induces Apoptosis by Blocking EGFR and MET in Gefitinib-Resistant Non-Small Cell Lung Cancer

        As one of the major types of lung cancer, non-small cell lung cancer (NSCLC) accounts for the majority of cancer-related deaths worldwide. Treatments for NSCLC includes surgery, chemotherapy, and targeted therapy. Among the targeted therapies, resistance to inhibitors of the epidermal growth factor receptor (EGFR) is common and remains a problem to be solved. MET (hepatocyte growth factor receptor) amplification is one of the major causes of EGFR-tyrosine kinase inhibitor (TKI) resistance. Therefore, there exists a need to find new and more efficacious therapies. Deoxypodophyllotoxin (DPT) extracted from Anthriscus sylvestris roots exhibits various pharmacological activities including anti-inflammation and anti-cancer effects. In this study we sought to determine the anti-cancer effects of DPT on HCC827GR cells, which are resistant to gefitinib (EGFR-TKI) due to regulation of EGFR and MET and their related signaling pathways. To identify the direct binding of DPT to EGFR and MET, we performed pull-down, ATP-binding, and kinase assays. DPT exhibited competitive binding with ATP against the network kinases EGFR and MET and reduced their activities. Also, DPT suppressed the expression of p-EGFR and p-MET as well as their downstreat proteins p-ErbB3, p-AKT, and p-ERK. The treatment of HCC827GR cells with DPT induced high ROS generation that led to endoplasmic-reticulum stress. Accordingly, loss of mitochondrial membrane potential and apoptosis by multi-caspase activation were observed. In conclusion, these results demonstrate the apoptotic effects of DPT on HCC827GR cells and signify the potential of DPT to serve as an adjuvant anti-cancer drug by simultaneously inhibiting EGFR and MET.

        테오필린에 대한 약물동력학 자문서비스의 비용-편익분석

        Background : Economic evaluation of clinical pharmacokinetic consultation services for theophylline, which is being widely used recently, is considered in patients for both proper care and cost efficiency. Mathods : This is a cost-benefit analysis of clinical pharmacokinetic consultation service for theophylline. Trial groups were chosen from 2 general hospitals which was performing clinical pharmacokinetic consultation- services in 1998. Control group was chosen from another one general hospital. The analysis includes 25 patients (sample patients) for trial group and 17 patients tor-control group. Results : On the basis of incremental analysis, it is estimated that the total (direct and indirect) annual costs of the clinical, pharmacokinetic services of theophylline for the patients in the trial group was about ₩65 million, whereas total annual benefits from those services was estimated to be about ₩551 million. The net benefits incurred to the sample patients, thus calculated, was about ₩485 million per year. In the analysis, we assumed that indirect benefits accruing to those services were non-existent. If that amount was included, the estimated net benefits would be much greater than the calculated one. Conclusion : We found that clinical pharmacokinetic consultation services for theophylline could produce more marginal benefits than marginal costs by those services from the social point of view. More controlled prospective trial in the future would be helpful for affirmation of the results of this study.

      • Development of Measurement Tool for Execution Function through Behavioral Data in Virtual Bookshelves Environment: focused on the Shifting Ability

        Objective: This study aims to develop a measurement tool for the shifting ability as an Executive Functions (EF) in Virtual Environment (VE) based on the theory of embodied cognition. Background: Most of conventional EF measurement tools show an 18-20% difference between measured value through the questionnaires and ability of daily practices, which leads to dysfunctional problems in everyday life. Method: We designed a shifting measurement tool in VE. A measurement tool was developed by creating the scenario for organizing books on a bookshelf according to specific rules through an expert-based evaluation in a VE. (Color, Shape, Category) Using this measurement tool, we gained the tasks score data, duration, and the behavioral data of hand and head from 12 healthy adults(20-30s) and analyzed the data. Results: Using this Shifting measurement tool, we gain the behavioral data of hands and head, and tasks scores data from 12 participants. It is compared with the behavioral data of hand and head movement according to the task score. Conclusion: Findings of this study could be used to evaluate existing measurements and enhance a newly developed shifting measurement tool.
