Purpose To compare the per-patient diagnostic performance of simulated abbreviated MRI (AMRI) to that of conventional MRI (CMRI) with full-sequence dynamic gadoxetic acid (GA) enhancement for early-stage hepatocellular carcinoma (HCC) screening in high-risk patients. Materials and Methods A total of 201 consecutive patients at high-risk for HCC, who underwent 3T liver MRI, were included in this retrospective study. The AMRI protocol comprised T2-weighted imaging, hepatobiliary phase imaging after GA injection, and diffusion-weighted imaging. For each patient, two AMRI and CMRI image sets were independently reviewed by two radiologists. Inter-reader agreement was assessed using Cohen’s kappa value. A composite reference standard was used to determine the diagnostic performance of each image set for each reader. Results A total of 93 HCCs were detected in 79 patients. The inter-reader agreement was almost perfect for both image sets (κ = 0.839, 0.948). In AMRI, the per-patient sensitivity and negative predictive values (NPV) were 94.9% and 96.4%, respectively. In CMRI, the per-patient sensitivity and NPV were 96.2% and 97.5%, respectively. Conclusion AMRI, using only three sequences, had a comparable diagnostic performance to CMRI in screening early-stage HCC. AMRI could be an alternative HCC screening tool for highrisk HCC patients.

Prescription Patterns and Burden of Pediatric Asthma in Korea

<P><B>Purpose</B></P><P>This study aimed to estimate the prevalence, prescription pattern and burden of pediatric asthma in Korea by analyzing the National Health Insurance (NHI) claims data.</P><P><B>Methods</B></P><P>We retrospectively analyzed the insurance claim records from the Korean NHI claims database from January 2010 to December 2014. Asthmatic patients were defined as children younger than 18 years, with appropriate 10th Revision of the International Classification of Diseases codes (J45 or J46) and a prescription for 1 or more asthma maintenance medications at the same date. Hospitalization and emergency department visits for asthma were defined as use of short-acting beta<SUB>2</SUB>-agonists during hospital visits among asthmatic patients.</P><P><B>Results</B></P><P>There were 1,172,807 asthmatic children in 2010, which increased steadily to 1,590,228 in 2014 in Korea. The prevalence showed an increasing trend annually for all ages. The mean prevalence by age in those older than 2 years decreased during the study period (from 39.4% in the 2–3 year age group to 2.6% in the 15–18 year age group). In an outpatient prescription, leukotriene receptor antagonists were the most commonly prescribed medication for all ages. Patients older than 6 years for whom inhaled corticosteroids were prescribed comprised less than 15% of asthmatic patients. The total direct medical cost for asthma between 2010 and 2014 ranged from $376 to $483 million. Asthma-related medical cost per person reached its peak in $366 in 2011 and decreased to $275 in 2014.</P><P><B>Conclusions</B></P><P>The prevalence of pediatric asthma increased annually and decreased with age. Individual cost of asthma showed a decreasing trend in Korean children.</P>

Video platforms, including YouTube, have a structure in which the number of video views is directly related to the publisher’s profits. Therefore, video publishers induce viewers by using provocative titles and thumbnails to garner more views. The conventional technique used to limit such harmful videos has low detection accuracy and relies on follow-up measures based on user reports. To address these problems, this study proposes a technique to improve the accuracy of filtering harmful media using thumbnails, titles, and audio data from videos. This study analyzed these three pieces of multimodal information; if the number of harmful determinations was greater than the set threshold, the video was deemed to be harmful, and its upload was restricted. The experimental results showed that the proposed multimodal information extraction technique used for harmfulvideo filtering achieved a 9% better performance than YouTube’s Restricted Mode with regard to detection accuracy and a 41% better performance than the YouTube automation system.

Background: Human coronaviruses (HCoV) cause mild upper respiratory infections; however, in 2019, a novel coronavirus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), emerged, causing an acute respiratory disease pandemic. Coronaviruses exhibit marked epidemiological and clinical differences.Purpose: This study compared the clinical, laboratory, and radiographic findings of children infected with SARS-CoV-2 versus HCoV.Methods: SARS-CoV-2 data were obtained from the Korea Disease Control and Prevention Agency (KDCA) registry and 4 dedicated coronavirus disease 2019 (COVID-19) hospitals. Medical records of children admitted with a single HCoV infection from January 2015 to March 2020 were collected from 10 secondary/tertiary hospitals. Clinical data included age, sex, underlying disease, symptoms, test results, imaging findings, treatment, and length of hospital stay.Results: We compared the clinical characteristics of children infected with HCoV (n=475) to those of children infected with SARS-CoV-2 (272 from KDCA, 218 from COVID-19 hospitals). HCoV patients were younger than KDCA patients (older than 9 years:3.6% vs. 75.7%; P<0.001) and patients at COVID-19 hospitals (2.0±2.9 vs 11.3±5.3; P<0.001). Patients with SARS-CoV-2 infection had a lower rate of fever (26.6% vs. 66.7%; P<0.001) and fewer respiratory symptoms than those with HCoV infection. Clinical severity, as determined by oxygen therapy and medication usage, was worse in children with HCoV infection. Children and adolescents with SARS-CoV-2 had less severe symptoms.Conclusion: Children and adolescents with COVID-19 had a milder clinical course and less severe disease than those with HCoV in terms of symptoms at admission, examination findings, and laboratory and radiology results.

Semi-invasive or invasive aspergillosis occurring after chemotherapy with sunitinib is a rare condition with unknown incidence and prognosis. We report a case of 59 year-old man who had underlying stable aspergilloma and was newly diagnosed with metastatic renal cell carcinoma. After surgical resection for renal cell carcinoma and receiving adjuvant chemotherapy with sunitinib for 8 months, the patient presented with hemoptysis. Chest computed tomography revealed an increased soft tissue mass and air crescent sign of the previous aspergilloma, combined consolidation and bronchial artery hypertrophy around the lesion. The patient underwent bronchoscopy with biopsy from the lesion, and was eventually diagnosed with chronic necrotizing pulmonary aspergillosis, which was progressed from a stable aspergilloma. This case shows that clinicians should be aware of the risk for opportunistic conversion to invasive fungal infections from a stable aspergilloma in patients undergoing sunitinib treatment.

Objectives: The purpose of the study was to examine the expected role and competency of future public health dental hygienist. Methods: This study was cross-sectional design. One dental hygienist per 16 cities and provinces was selected from the list of Korea Society of public dental hygienists. After signing in the informed consent, the interview was carried out. Transcripts were made after each interview. Six researchers recorded meaningful contents in the transcripts. They classified and integrated the information that they had recorded commonly or not. Their opinions were converged by conference. Two supervisors verified results whether they were derived from actual recordings for the reliability of the results. The records were confirmed once again and corrected into common message. Results: The main difficult subjects in public health official examination were public health, English, and medical legislation to pass the examine. The preliminary certificates included public health related certificate, computer certificate, and driver`s license. In the personality aspect, creativity, activeness, cooperation and good responsibility were very important trait to public health official. Among 16 interviews, 9 were positive and 5 were negative about the future prospects of a public dental hygienist. Conclusions: The future of public health dental hygienist will be positive. Throughout effort to prepare for the public health official, the public health dental hygienist will be expanded and in many ways the dental hygienists will take the charge of the important parts of the public health administration in the near future.
