        Topical ALA-Photodynamic Therapy for Acne Can Induce Apoptosis of Sebocytes and Down-regulate Their TLR-2 and TLR-4 Expression

        Background: Although photodynamic therapy (PDT) is widely performed for acne, little is known about its exact therapeutic mechanism. Objective: We aimed to estimate the efficacy and safety of PDT on acne and to discover its mode of action. Methods: We performed PDT on 12 patients with mild to moderate acne. The clinical efficacy was assessed by counting the acne lesions and measuring the sebum secretion before and after PDT. In addition, we took biopsy samples from the peri-lesional areas before and after 3-session of PDT. To examine the degree of apoptosis of the sebaceous follicles, TUNEL assay was performed. To investigate the changes of toll-like receptor (TLR)-2 and TLR-4 expression after PDT, immunohistochemical stainings were also carried out. Finally, we performed TUNEL assay using the cultured sebocytes to confirm the apoptosis of sebocytes in vitro after PDT. Results: There was a significant reduction in the number of inflammatory acne lesions after PDT, compared to baseline (p<0.05). Sebum excretion significantly decreased 2 weeks after the first PDT session except for one patient (p<0.05). The TUNEL positive cells in the peri-lesional sebaceous glands after PDT markedly increased, compared with those of before PDT. A decrease in TLR-2 and TLR-4 expression by sebaceous glands and epidermis after PDT was 50% and 30%, respectively. Conclusion: Our results demonstrate that apoptosis of the sebaceous glands is associated with improvement of acne by PDT. PDT has shown to down-regulate TLR-2 and TLR-4expression in the sebaceous glands and epidermis of acne patients. (Ann Dermatol 23(1) 23∼32, 2011)

        The Effect of Consumers’ Loss Aversion on Pioneering Advantage

        The present study provides a theoretical investigation on pioneering advantage based on reference dependence and loss aversion effect under prospect theory (Kahneman and Tversky, 1979). Behavioral explanations for pioneering advantage are provided from two different perspectives: one based on the prototypicality and the other on the utility uncertainty of the option. A pioneer brand creates the product category and makes a strong impression in customers’ mind, and thus becomes the most representative or prototypical option of the category. In addition, the pioneer brand becomes the first option to be experienced by the majority of consumers in the product category, thus has the lowest level of utility uncertainty compared with the late movers. This study integrates the previous accounts for pioneering advantage by showing that consumers have higher preferences for the most prototypical and the least uncertain option based on loss aversion and reference dependence effect. This study suggests that firms should carefully analyze the consumers’ loss aversion and perceived uncertainty and prototypicality of their products in order to develop effective market entry strategies.

        서브퀄(SERVQUAL)과 서비스블루프린트의 통합모델 연구

        산업화 시대를 벗어나 산업이 지식과 서비스 중심으로 전환되면서 서비스사업의 경쟁이 더욱 치열해 지고 있다. 이런 흐름에 발 맞춰 서비스가 사용자에게 얼마나 만족을 주는지 파악하는 방법론 또한 많은 연구를 통해 이루어지고 있다. 그 중 대표적인 방법론인 서브퀄(SERVQUAL)과 서비스블루프린트는 서비스마케팅과 서비스디자인을 대표하는 방법론이다. 그러나 서비스마케팅과 서비스디자인은 서비스의 만족도를 알아보는 공통의 목적을 갖고 있지만 그 둘의 성격과 지향하는 바가 다르며 각각의 장·단점이 존재한다. 이러한 서비스마케팅과 서비스디자인의 경계에서 이 두 개념을 적절히 통합하고 서로 보완한다면 이 두 개념의 장점을 극대화하는 새로운 개념의 통합모델을 만들 수 있을 것이라는 가설에 의해 이 논문은 시작되었다. 먼저 이 논문에서는 서비스마케팅과 서비스디자인이 가지고 있는 성격을 이론적 고찰을 통해 파악 한 후 그 하위 개념인 서브퀄(SERVQUAL)과 서비스블루프린트를 구성하고 있는 속성과 구조를 분석할 것이다. 그리고 두 개의 방법론이 통합될 수 있는 계통체계로 정리 하여 기존의 서비스블루프린트에 서브퀄(SERVQUAL)을 보완 발전시킨 통합모델을 제안 할 것이다. 그리고 그 모델을 적용시킨 사례를 통해 향후 기대효과와 가능성을 제안하고자 한다. As the current industrial framework is transforming into a knowledge and service based paradigm instead of the past industrial age, the service business sector is facing intense competition. Keeping pace with the rapidly changing market trends, diverse methodological researches have been conducted to comprehend how service products can satisfy consumer needs. SERVQUAL and service blueprint are main representative methodologies for service marketing and service design respectively. Service marketing and service design have the common ground that both activities aim at estimating customer satisfaction level. The two areas are, however, different from each other in characteristics and purposes while showing their own merits and demerits separately. This thesis starts from a hypothesis; balancing between service marketing and service design, a conceptually new integrated model can be created based on convergence and mutual supplementation of the two concepts. First of all, this dissertation analyses characteristics of service marketing and service blueprint through theological consideration, and assesses structures and properties of their subordinate concepts: SERVQUAL and service blueprint. Then, this research suggests an integrated model in which the existing service blueprint is developed and supplemented with SERVQUAL by organizing the two methodologies into a comprehensive system. The final objective of this thesis is to demonstrate the feasibility and expected outcomes of this study with practical examples of the implemented model.

        Synergistic effect of anti-platelet and anti-inflammation of drug-coated Co–Cr substrates for prevention of initial in-stent restenosis

        <P><B>Abstract</B></P> <P>Antiplatelet and antithrombotic therapies are systematically considered to prevent restenosis following coronary stent implantation. Currently, patients receiving medicated stents are prescribed to orally take anticoagulants and antiplatelet drugs such as aspirin (ASP) and prasugrel (PRAS). Propolis (PROP) known as a natural organic compound was recently evaluated for its antiplatelet activity, antibiotics and immunomodulatory activities. In this study, antiplatelet drug-coated Co–Cr substrates were prepared with biodegradable poly(<SMALL>D</SMALL>,<SMALL>L</SMALL>-lactide) (PDLLA) containing ASP, PRA, or PROP using electrospray and the blood compatibility of the different substrates was investigated by measuring protein adsorption and platelet adhesion. In addition, the anti-inflammatory properties of the modified Co–Cr surfaces were assessed by measuring IL-8 and IL-6 expression levels in human endothelial cell cultures. Drug-coated surfaces were found to resist the adsorption of fibrinogen when compared to bare Co–Cr or PDLLA-coated Co–Cr. Interestingly, ASP- and PROP-containing substrates not only showed reduced adhesion of platelets and delayed coagulation time, but also drastically reduced the expression level of IL-8 and IL-6. Such results are supported that ASP- or PROP-coated Co–Cr can be potentially used as a stent material to mitigate early stage of restenosis.         Quantification of hepatic macrosteatosis in living, related liver donors using T1‐independent, T2*‐corrected chemical shift MRI

        <P><B>Abstract</B></P><P><B>Purpose:</B></P><P>To evaluate the diagnostic implications of the iterative decomposition of water and fat using echo‐asymmetry and the least‐squares estimation (IDEAL) technique to detect hepatic steatosis (HS) in potential liver donors using histopathology as the reference standard.</P><P><B>Materials and Methods:</B></P><P>Forty‐nine potential liver donors (32 male, 17 female; mean age, 31.7 years) were included. All patients were imaged using the in‐ and out‐of‐phase (IOP) gradient‐echo (GRE) and IDEAL techniques on a 1.5 T MR scanner. To estimate the hepatic fat fraction (FF), two reviewers performed regions‐of‐interest measurement in 15 areas of the liver seen on the IOP images and on the IDEAL‐FF images. The magnetic resonance imaging (MRI) and pathology values of macrosteatosis were correlated using the Pearson correlation coefficient. We analyzed the diagnostic performance of IOP imaging and IDEAL for detecting HS.</P><P><B>Results:</B></P><P>The results of the hepatic‐FF estimated on IDEAL were well correlated with the histologic degree of macrosteatosis (γ = 0.902, <I>P</I> < 0.001). IDEAL showed 100% sensitivity and 91% specificity for detecting HS, and IOP imaging showed 87.5% sensitivity and 97% specificity, respectively.</P><P><B>Conclusion:</B></P><P>IDEAL is a useful tool for the preoperative diagnosis of HS in potential living liver donors; it can also help to avoid unnecessary biopsies in these patients.         가성통풍으로 인한 수근관 증후군

        가성통풍으로 인한 수근관 증후군은 드물게 발생한다. 저자들은 수근관 내 종괴로 인한 급성 정중신경 압박 증세로 내원한 65세 여자환자에게 종괴 절제술과 횡수근 인대 절개술을 시행하고 조직검사를 통해 가성통풍으로 진단된 증례를 경험하여 문헌 고찰과 함께 보고하고자 한다. Carpal tunnel syndrome (CTS) caused by pseudogout is an uncommon disease. The authors report a 65-year-old female who complained of sudden pain and neurological symptoms on her left hand. Surgical decompression was performed. In the histologic study, a calcium pyrophosphate dihydrate crystal deposit was confirmed. Her pain and neurological symptoms were relieved. Because CTS caused by pseudogout is rare, it is difficult to differentiate it from other diseases. This paper reports an uncommon case of CTS caused by pseudogout.
