The long-term prognostic significance of maximal infarct transmurality evaluated by contrast-enhanced cardiac magnetic resonance (CE-CMR) in ST-segment elevation myocardial infarction (STEMI) patients has yet to be determined. This study aimed to see if maximal infarct transmurality has any additional long-term prognostic value over other CE-CMR predictors in STEMI patients, such as microvascular obstruction (MVO) and intramyocardial hemorrhage (IMH). The study included 112 consecutive patients who underwent CE-CMR after STEMI to assess established parameters of myocardial injury as well as the maximal infarct transmurality. The primary clinical endpoint was the occurrence of major adverse cardiac events (MACE), which included all-cause death, non-fatal reinfarction, and new heart failure hospitalization. The MACE occurred in 10 patients over a median follow-up of 7.9 years (IQR, 5.8 to 9.2 years) (2 deaths, 3 nonfatal MI, and 5 heart failure hospitalization). Patients with MACE had significantly higher rates of transmural extent of infarction, infarct size >5.4 percent, MVO, and IMH compared to patients without MACE. In stepwise multivariable Cox regression analysis, the transmural extent of infarction defined as 75 percent or more of infarct transmurality was an independent predictor of the MACE after correction for MVO and IMH (hazard ratio 8.7, 95% confidence intervals [CIs] 1.1-71; p=0.043). In revascularized STEMI patients, post-infarction CE-CMR-based maximal infarct transmurality is an independent long-term prognosticator. Adding maximal infarct transmurality to CE-CMR parameters like MVO and IMH could thus identify patients at high risk of long-term adverse outcomes in STEMI.
청열탕(淸熱湯)의 생물전환을 통한 항아토피피부염 효능(效能) 연구(硏究)

In order to improve efficacy of oriental medicines and to study the application of fermented oriental medicine in clinicals, the efficacy of CYT and CYTBH on atopic dermatitis were evaluated. The results and conclusions are as follows. CYT and CYTBH significantly improved the atopic dermatitis symptoms in NC/Nga mice by naked eye evaluation and significantly decreased clinical index in both groups. CYT and CYTBH both decreased the cell numbers of CD3+, CD11b+Gr-l+ cells in dorsal skin. Of the cells, CYT significantly decreased CD11b+Gr-1+ cells whereas CYTBH significantly decreased all immune cells. CYT and CYTBH both decreased the production rate of IL-4 and IFN-γ activated by CD3/CD28. In the case of CYTBH, significant decrease in all cases was observed. CYT and CYTBH decreased the production rate of IL-5, IL-13 and IL-17 in serum. Significant decrease of IL-5 in the case of CYT and IL-5 and IL-13 in the case of CYTBH were observed. CYT and CYTBH significantly decreased transcription of IL-5 mRNA and IL-13 mRNA in skin. Significant decrease in IgG1 and IgE immunoglobulins in serum were oberved in both groups. Significant decrease was only observed in the case of CYTBH. Both CYT and CYTBH significantly decreased the secretion of histamine. Both CYT and CYTBH suppressed erythema, hemorrhage, edema, excoriation, erosion of skin tissues of NC/Nga mice resulting in the decrease of thickness of epidermis. Significant decrease of infiltration of obese cells was also observed. The results above indicated that both CYT and CYTBH had significant efficacy in the treatment of atopic dermatitis through immune modulation. Animal studies showed that CYTBH had superior activity than that of CYT suggesting further and continuous studies on the changes in ingredients or absorption improvement by fermentation should follow.

ObjectiveaaIn this study, we explored the meaning attributed to the words “jungshinbunyeolbyung” (schizophrenia) and “jungshinbunyeol” (schizophrenic) in South Korean newspapers and news programs in the last 10 years. MethodsaaWe screened the websites of three national newspapers and the broadcasts of three nationwide television news programs from January 1, 2001, to December 31, 2010. We classified a total of 490 articles and 257 news segments by category and quantitatively and qualitatively analyzed them. The articles and news segments were assigned to one of the following categories based on their use of the term “schizophrenia”: 1) negative, 2) neutral or positive, 3) incidental, and 4) metaphorical. ResultsaaThe negative viewpoint accounted for 349 incidences (46.7%), while the neutral and positive viewpoints included 225 incidences (30.1%). Incidental uses accounted for 95 incidences (12.7%), and metaphorical uses accounted for 78 incidences (10.4%). The majority of the negative uses focused on violence or dangers posed by patients (137 mentions, 37.8%), while the metaphorical uses mainly focused on the idea of splitting (51 mentions, or 65%). ConclusionaaThis study showed that the South Korean news media do not provide balanced information about schizophrenia to the public. This study also showed that no significant move has been made toward a more positive use of the term since a previous study was conducted on the subject. Although the term schizophrenia has given way to “attunement disorder,” it will be difficult to establish the new term as the standard if the South Korean media continue to use the term “schizophrenic symptom.” Even though the term has been ch-anged, guidelines are necessary to encourage the mass media to provide balanced articles and reduce prejudice.

직접 접촉 방식의 기계적인 결합을 사용하는 기어는 접촉 마찰로 인한 소음, 진동, 발열 등의 문제가 지속적으로 제기되어왔기 때문에, 비접촉 자기(magnetic)결합을 이용한 마그네틱 기어(magnetic gear)가 제안되었다. 마그네틱 기어에 대한 연구 중 동일한 기어비에서 극수에 따라 손실의 차이가 발생함을 확인하였으며, 철손과 영구자석 와류손이 극수에 따라 일정한 경향성을 보일 것으로 예상되어 연구를 진행하였다. 본 논문에서는 마그네틱 기어의 극수에 따른 경향성을 확인하고 구체화하여 마그네틱 기어의 효율 개선 설계의 기반을 마련하였다. Mechanical gears driven by direct contact have problems including noise, vibration and heat. In order to, solve these problems, magnetic gears having a non-contact magnetic coupling have been proposed. Through various studies on magnetic gears, we found that losses are changed when the number of magnetic poles varies in the same gear ratio. For this reason, research team expect the iron loss of the magnetic gear and the Eddy current loss of the permanent magnet will have a certain tendency depending on the number of poles. This paper identified the magnetic gear’s loss tendency according to the number of poles, and laid the basis for efficiency improvement design.
선형탄성해석과 mα-tangent방법을 이용한 배관 한계하중 평가 적용성

본 연구에서는 선형탄성 유한요소해석 방법과 mα-tangent 방법으로 한계하중을 계산하고 이를 기존의 연구들에 의해 제시된 한계하중 식들과 비교하였다. 해석의 대상은 배관계의 대표 형상인 곡관과 분기관을 선정하였다. 곡관과 분기관의 형상 변수를 다양하게 바꿔가면서 mα-tangent 방법의 적용가능성을 살펴보았다. 하중은 내압하중과 면내굽힘하중을 각각 고려하였으며 곡관의 경우에는 기존의 한계하중식과 mα-tangent 방법이 비교적 잘 일치함을 확인하였다. 하지만 분기관의 경우 형상의 특수성으로 인해 뚜렷한 경향 없이 기존의 한계하중 식과 mα-tangent 방법이 일치하지 않는 것을 확인할 수 있었다. 이는 mα-tangent 방법이 불연속부의 응력집중현상에 의해 왜곡된 결과를 갖기 때문으로 보인다. In this study, the limit loads calculated by the mα-tangent method based on the linear finite element analysis are compared with the closed form solutions that are proposed by various authors. The objects of the analysis is to select the elbow and the branch pipe which are representative structure of piping system. The applicability of the mα-tangent method are investigated by applying it to cases with various geometries. The internal pressure and the in-plane bending moment are considered and the mα-tangent method is in good agreement with the existing solutions in case of elbows. However, the limit loads calculated by the mα-tangent method for branch junctions do not agree well with the existing solutions and do not show any tendency. The reason is a biased result due to the stress concentration of the discontinuous parts.

Various pixel structures have been proposed in the patterned vertical alignment (PVA) mode to improve transmittance and response time because collisions of the liquid crystal directors are inevitable near domain boundary region. One of them is well known as Z-shape structure, which shows multi-domain characteristic but low transmittance due to forming disclination lines at the domain boundary. This characteristic comes from the LC director that cannot tilt down to 45° with respect to the transmissive axis of crossed polarizers. In order to overcome this problem, we proposed a new pixel structure which improves transmittance while keeping the number of masks same as before. Consequently, the proposed structure improved transmittance up to 18% compared with conventional Z-shape structure.
