        Arthroscopic Treatment of Elbow Osteoarthritis and Arthroscopic Ulnar Nerve Decompression

        Although arthroscopic surgery has been used conventionally, it has not been widely adopted yet due to the risks of complications, including nerve damage, technical difficulties, and limited indications. As shown in other joints, however, the use of an arthroscope will gradually increased in the elbow joint (‘Arthroscopy always wins’). Herein, arthroscopic treatments and arthroscopic ulnar nerve decompression will be discussed in cases of elbow osteoarthritis.

        Superconducting MgB2 Wire Drawing Considering Anisotropic Hardening Behavior and Hydrostatic Effect

        Numerical modeling was conducted to investigate the deformation behavior of powder mixture during multi-pass drawingprocesses for multi-filamentary MgB2wire. A modified Drucker-Prager Cap (DPC) model with an elliptical cap surface usingthe new material characterization method was developed to capture the anisotropic hardening behavior and hydrostatic effectof the powder mixture. A number of uniaxial die compaction, cold isostatic pressing, diametrical compression, and uniaxialcompression tests were conducted using different powder densities to characterize the modified DPC model. A commercialfinite element software ABAQUS with a user subroutine was used to simulate the drawing of the MgB2wire. The densityand area fraction of the powder mixture during the wire-drawing process were verified with experimental results. The differencein packing density at the inner and outer filaments of the MgB2wire was successfully captured by simulation. Inaddition, the effect of the initial packing density on the superconducting properties of MgB2wire was numerically studied. It is shown that the increase in the superconducting area, which results from a high initial packing density, should be moreeffective compared to the increase in the grain connectivity in enhancing the critical current properties for the MgB2wirewhen the final packing density is saturated after a number of drawing processes.

        Cystic Medullary Thyroid Carcinoma: A Case of Undergoing Endoscopic Thyroid Lobectomy

        On ultrasonography, medullary thyroid carcinoma (MTC) shows hypoechogenicity, an irregular margin, a predominantly solid composition, and microcalcifications, similar to those observed in papillary thyroid carcinoma (PTC). MTC presenting as a cystic lesion is rare, and endoscopic thyroidectomy can be performed for benign thyroid masses and early stage PTC, however it is inappropriate for MTC regardless of cystic change. The authors report a case of cystic MTC found after endoscopic thyroid lobectomy and provide a review of the literature on this topic.

        산업장 근로자의 스트레스에 대한 한의계 연구경향

        Objectives : The purpose of this study is to explore the trends of Korean medical study on stress in the industrial worker. Methods : We received numbers of the studies about stress in the industrial worker from 6 Korean web databases, using key words such as ‘Industry AND Stress’, ‘Oriental medicine AND Stress’, ‘Worker AND Stress’, ‘Workplace AND Stress’. We received 12 research papers and they were analyzed and the journals, the authors, the contents of 3 classification of those research papers. Results : Among them, we had 6 reviews of Dept. of preventive medicine and Dept. of third medicine, college of Korean Medicine, Won-kwang University, and the rest of them were 6 reviews. We had 4 reviews of Korean Journal of Oriental Preventive Medical Society, 2 reviews of the Journal of Traditional Korean Medicine, 2 reviews of Journal of Oriental Neuropsychiatry and so on. According to the contents of reviews, we had 7 reviews of correlation of industrial worker and stress, 4 reviews of cause on the industrial worker’s stress and 1 case report. Conclusions : We reviewed the trends of Korean medical study on stress in the industrial worker. The stress in the industrial workers were related with the type of occupation, social psychology, drinking, smoking, the satisfaction of job, the harmony of organization.And we knew that the musculoskeletal pain was related to the industrial worker’s stress. In the future, abundant case studies, follow-up trials and RCTs should be done about the industrial worker’s stress.

        우황청심원 관련 최근 연구 동향 : 국내 논문에 대한 고찰

        Objectives : Woohwangchungsim-won has been used for acute diseases such as cerebrovascular disease (stroke), heart disease. The aim of this study was to determine the effects, significance & necessity of Woohwangchungsim-won in acute stages of these disease by reviewing Korean articles about Woohwangchungsim-won. Methods : Article searches were performed on 8 major Korean web article search engine from January 1980 to August 2010. There were no restrictions on the types of publication, including grey articles. Results : Forty-three articles were included. Twenty one were efficacy tests, thirteen were comparative efficacy tests, eight were toxicity tests, and one was new method development. Eighteen articles were multicenter studies and twenty five were single center studies. Thirty two articles were animal testing, only ten articles were clinical articles. Thirty seven articles presented the composition and quantity of Woohwangchungsim-won, but six did not. Conclusions : The results of Korean articles about Woohwangchungsim-won can not strongly support the effects, significance & necessity of Woohwangchungsim-won. More rigorous studies are required for determination of the effects of Woohwangchungsim-won.

        An Experimental Study on Dryout Pattern of Two-Phase Flow in Helically Coiled Tubes

        Experimental results are presented for the effects of coil diameter, system pressure and mass flux on dryout pattern of two-phase flow in helically coiled tubes. Two tubes with coil diameters of 215 and 485 mm are used in the present study, Inlet system pressures range from 0.3 to 0.7 MPa, mass flux from 300 to 500 kg/㎡s, and heat flux from 36 to 80 kw/㎡. A partial dryout region exists because of the geometrical characteristics of the helically coiled tube. The length of the partial dryout region increases with coil diameter and system pressure. On the other hand, it decreases with increasing mass flux. The critical quality at the tube top side increases with mass flux, but decreases with increasing system pressure. This tendency is more notable when the coil diameter is larger. When the centrifugal force effect becomes stronger, dryout starts at the top and bottom sides of the tube. However, when the gravity effect becomes stronger, dryout is delayed at the tube bottom side. In some cases when the mass flux is low, dryout occurs earlier at the outer side than at the inner side of the tube because of film inversion.

        Cortical Neuronal Loss after Chronic Prenatal Hypoxia : A Comparative Laboratory Study

        Objective : The purpose of this study was to investigate the prenatal hypoxic effect on the fetal brain development. Methods : We used the guinea pig chronic placental insufficiency model to investigate the effect of hypoxia on fetal brain development. We ligated unilateral uterine artery at 30-32 days of gestation (dg : with term defined as -67 dg). At 50 dg, 60 dg, fetuses were sacrificed and assigned to either the growth-restricted (GR) or control (no ligation) group. After fixation, dissection, and sectioning of cerebral tissue from these animals, immunohistochemistry was performed with NeuN antibody, which is a mature neuronal marker in the cerebral cortex. Results : The number of NeuN-immunoreactive (IR) cells in the cerebral cortex did not differ between the GR and control groups at 50 dg. However, the number of NeuN-IR cells was lesser in GR fetuses than in controls at 60 dg (p<0.05). Conclusion : These findings show that chronic prenatal hypoxia affect the number of neuron in the cerebral cortex of guinea pig fetus at 60 dg. The approach used in this study is helpful for extending our understanding of neurogenesis in the cerebral cortex, and the findings may be useful for elucidating the brain injury caused by prenatal hypoxia.

        Inactivation Mechanism of Bacillus subtilis Spores by Ethanol Extract of Torilis japonica Fruit

        To confirm the antimicobial mechanism of Torilis japonica, antimicrobial profile was observed on various spore conditions by combining 0.1% (3 mM) torilin with antimicrobial activity and 0.27% water fraction with germinants. A 75% ethanol extract of T. japonica fruit reduced Bacillus subtilis ATCC 6633 spore counts by 3 log cycles and reduced the vegetative cells to undetectable level (by about 6 log cycles) (both in terms of CFU/㎖). Further fractionating the ethanol extract into n-hexane and water fractions revealed that the former reduced the spore count by 1 log cycle whereas the latter had no effect. The antimicrobial active compound was isolated and purified from the hexane layer, and identified as torilin (C₂₂H₃₂O?). The water fraction of the ethanol layer did not show antimicrobial activity, whereas the antimicrobial effect of 0.1% (3 mM) torilin was significantly enhanced in the presence of the water fraction (0.27%). This result can be explained by synergistic effects of the water fraction containing considerable amounts of germinants such as L-alanine and K? ions that triggered germination.

        Introduction and Background To evaluate the altered gene and protein expression patterns in the rotator cuff muscles of the patients with rotator cuff tears who got frequent corticosteroid joint injection compared with those who had no history of corticosteroid injection. Material and Method Twenty-four samples of rotator cuff muscle from 12 patients with frequent corticosteroid joint injection history (steroid (+) group: 7 men and 5 women; mean age 63.0±7.2 years) and age- and sex-matched 12 control patients without steroid injection history (steroid (-) group: 7 men and 5 women; mean age 62.8±7.7 years) with medium-sized tears were acquired during arthroscopic surgery. As a statistical method, the propensity score matching technique was used to select control group by 1:1 matching for age and gender. Inclusion criteria were patients that underwent arthroscopic repair for medium-sized full-thickness rotator cuff tears and had history of frequent steroid injection (more than 5 times of steroid joint injection including more than once within recent 3 months). Patients lacking medium-sized tears, isolated subscapularis tear, preoperative stiff shoulder, acute traumatic tear, current heavy smoker (>20 pack year), uncontrolled diabetes, or previous surgery on the same shoulder, or those that declined to participate were excluded. Alterations in the expression of genes and proteins associated with adipogenesis, myogenesis, inflammation, and muscle fibrosis were compared between groups with reverse-transcription quantitative polymerase chain reaction, western blotting, and immunohistochemistry. Results The mRNA expression levels of adipogenic CCAAT/enhancer-binding protein alpha (C/EBPα; P=0.037) and muscle atrophy related gene (Atrogin; P=0.034) were significantly higher, and myogenic differentiation 1 (MyoD; P=0.032), inflammatory interleukin 6 (IL6; P=0.005) and high mobility group box 1 (HMGB1; P=0.043) were significantly lower in samples from steroid (+) group than from steroid (-) group. The fibrogenic alpha-smooth muscle actin (α- SMA; P=0.157), and collagen 1 and 3 (COL1 and 3; P=0.475 and 0.060, respectively) did not show statistical differences. A correlation was observed between gene and protein expression of C/EBPα (p=0.048), Atrogin (p=0.016), MyoD (p=0.028) and IL6 (p=0.013). The immunohistochemistry also showed increased expressions of C/EBPα and Atrogin, and decreased expressions of MyoD and IL6 in the steroid (+) group compared with the steroid (-) group. Conclusions Patients with rotator cuff tears who had frequent steroid injection history showed large fat infiltration and muscle atrophy as well as less inflammation in rotator cuff muscle, which is associated with the increased expression of C/EBPα and Atrogin, and decreased expressions of MyoD and IL6.
