<B>Background.</B><P>The prevalence of community-associated methicillin-resistant<I>Staphylococcus aureus</I>(CA-MRSA) strains causing bloodstream infection (BSI) has not been studied in Korea.</P><B>Objective.</B><P>We sought to determine the prevalence of CA-MRSA strains among isolates recovered from patients with MRSA BSIs and to explore epidemiological changes in Korea. We also sought to evaluate clinical characteristics relevant to the development of healthcare-associated BSIs.</P><B>Methods.</B><P>We prospectively collected consecutive MRSA isolates from patients with BSI at 4 hospitals from July 1 through November 30, 2007, and we also included MRSA isolates recovered from culture of blood samples collected during a previous year (October 1, 2004 through September 30, 2005) at a different hospital. Molecular typing studies were performed, including pulsed-field gel electrophoresis (PFGE), multilocus sequence typing,<I>Staphylococcus</I>protein A (<I>spa</I>) typing, and staphylococcal cassette chromosome<I>mec</I>(SCC<I>mec</I>) typing. We compared the clinical characteristics and outcomes of patients with healthcare-associated BSI due to CA-MRSA strains with those of patients with healthcare-associated BSI due to healthcare-associated MRSA (HA-MRSA) strains.</P><B>Results.</B><P>There were 76 cases of MRSA BSI, of which 4 (5.3%) were community-associated and 72 (94.7%) were healthcare-associated. Among the 72 HA-MRSA BSIs, 18 (25%) were community onset, and 54 (75%) were hospital onset. PFGE type D-ST72-<I>spa</I>B-SCC<I>mec</I>type IVA MRSA, the predominant genotype of CA-MRSA in Korea, accounted for 19 (25%) of all 76 MRSA BSIs, including 17 (23.6%) of 72 HA-MRSA BSIs and 11 (20.8%) of 53 hospital-onset HA-MRSA BSIs. Patients with healthcare-associated BSIs due to CA-MRSA strains carrying SCC<I>mec</I>type IVA tended to have fewer healthcare-associated risk factors, compared with patients with healthcare-associated BSIs due to HA-MRSA strains carrying other SCC<I>mec</I>types. The presence of a central venous catheter or other invasive device was the only independent factor differentiating patients infected with hospital-associated genotype strains from patients infected with other strains. Clinical outcomes were similar between both groups.</P><B>Conclusions.</B><P>CA-MRSA strains are emerging as a major cause of BSI in healthcare settings in Korea. This changing epidemiology of MRSA poses a challenge to public health and infection control in hospital settings.</P>

<P><B>ABSTRACT</B></P><P>Animal models facilitate evaluation of vaccine efficacy at relatively low cost. This study was a comparative evaluation of the immunogenicity and protective efficacy of a new 13-valent pneumococcal conjugate vaccine (PCV13) with a control vaccine in a mouse model.</P><P>After vaccination, anti-capsular antibody levels were evaluated by pneumococcal polysaccharide (PnP) enzyme-linked immunosorbent assay (ELISA) and opsonophagocytic killing assay (OPA). Also, mice were challenged intraperitoneally with 100-fold of the 50% lethal dose of <I>Streptococcus pneumoniae</I>.</P><P>The anti-capsular IgG levels against serotypes 1, 4, 7F, 14, 18C, 19A, and 19F were high (quartile 2 >1,600), while those against the other serotypes were low (Q2 ≤ 800). Also, the OPA titres were similar to those determined by PnP ELISA. Comparative analysis between new PCV13 and control vaccination group in a mouse model exhibited significant differences in serological immunity of a few serotypes and the range of anti-capsular IgG in the population. Challenge of wild-type or neutropenic mice with serotypes 3, 5, 6A, 6B, and 9V showed protective immunity despite of induced relatively low levels of anti-capsular antibodies. With comparison analysis, a mouse model should be adequate for evaluating serological efficacy and difference in the population level as preclinical trial.</P>

<P>We attempted to apply fluorescence resonance energy transfer technology to nucleic acid sequence-based amplification (FRET-NASBA) on the platform of the LightCycler system to detect <I>Aspergillus</I> species. Primers and probes for the <I>Aspergillus</I> 18S rRNA were newly designed to avoid overlapping with homologous sequences of human 18s rRNA. NASBA using molecular beacon (MB) showed non-specific results which have been frequently observed from controls, although it showed higher sensitivity (10<SUP>−2</SUP> amol) than the FRET. FRET-NASBA showed a sensitivity of 10<SUP>−1</SUP> amol and a high fidelity of reproducibility from controls. As FRET technology was successfully applied to the NASBA assay, it could contribute to diverse development of the NASBA assay. These results suggest that FRET-NASBA could replace previous NASBA techniques in the detection of <I>Aspergillus</I>.</P>

최근 Panton-Valentine leukocidin (PVL) 유전자 양성인 지역사회관련 메티실린내성 포도알균이 전세계적으로 보고되고 있지만 국내에서는 아직 드물다. 저자들은 76세 여자환자에서 PVL양성 지역사회관련 메티실린 내성 포도알균에 의한 광범위한 항문주위 농양을 진단하고 성공적으로 치료하였다. 분자역학연구를 통해 본 균주는 미국에서 유행하는 USA300주와 매우 유사하고, staphylococcal cassette chromosome mec mec (SCCmec) type IV, multilocus Sequence typing ST8, spa type type 1, accessory gene regulator locus (agr) group I이 관련되어있었다. 문헌조사 결과 동아시아에서 보고된 PVL 양성 지역사회관련 메티실린 내성 포도알균은 분자역학적 특성이 다양하고, 미국 및 유럽 증례와 비교해 PVL 유전자가 비교적 드물었다. Community-associated methicillin resistant Staphylococcus aureus (CA-MRSA) with Panton-Valentine leukocidin (PVL) genes have recently emerged worldwide, but infections due to PVL carrying CA-MRSA strains have never been reported in Korea. We report a case of extensive perianal abscess due to PVLU CA-MRSA in a 76-year-o1d Korean female patient, of which genetic background was very close to USA300. It belonged to staphylococcal cassette chromosome mec element (SCCmec) type IV, ST8 of multilocus sequence typing (MLST), type 1 spa type, and accessory gene regulator locus (agr) group I. Comprehensive literature reviews from the Far East showed molecular characteristics were diverse and PVL genes were infrequently found than in western countries.
A Perspective of the Wearing-off Phenomenon in a Patient with Parkinson’s Disease: A Case Report

Dysphagia is a major disability in patients with Parkinson’s disease. Unlike typical motor symptoms, dysphagia is relatively unresponsive to dopaminergic drugs. However, occasionally, swallowing difficulties are much improved by dopaminergic agonists and significantly affected by on/off periods. In such cases, it is difficult to assess the severity of dysphagia accurately if swallowing function evaluation is performed without considering the wearing-off phenomenon in patients with PD. Here, we report a case of dysphagia in a female patient with Parkinson’s disease that was severely affected by the wearing-off phenomenon in response to prolonged use of levodopa. The patient presented with severe oral phase delay with choking symptoms during fluid intake. A videofluoroscopic swallow study performed during an ‘off-period’ showed massive aspiration with severely impaired swallowing function. However, when swallowing evaluation was performed during the ‘on-period’, almost no abnormal function was observed. After adopting an on/off period-tailored diet prescription, sufficient nutrition was possible without aspiration. Consideration of the wearing-off phenomenon is essential when evaluating swallowing function in patients with Parkinson’s disease. Patient-specific swallowing evaluations and diet prescriptions are needed to establish optimal therapeutic strategies.
Evaluation of Time-Based Arc Flash Detection with Non-contact UV Sensor

An arc fash is caused by electrical faults or mechanical non-contact such as insulation failure, short circuit, partial disconnection, and poor contact, emits high energy and strong light, so it can be detected by optical methods with relatively simple structures. The optical detection methods using the wavelength of light may malfunction depending on the surrounding environment, the sunlight and lighting. In addition, with such methods, it is difcult to measure the detection distance to the point where the arc fash occurs, measure the detection range of the detection sensor, and monitor how amounts of harmful from the arc fash. This paper aims to examine the techniques for quantitatively detecting arc fash caused by electrical faults. To this end, a system for detecting the time when an arc fash occurs and the techniques for calibrating this system were analyzed, respectively. A feld test was also conducted to detect the arc fash caused by mechanical non-contact between the pantograph of electric railway vehicles and the catenary. In the feld test, 9 number of arc fashes were detected using the percentage (%) technique for quantitative detection. The technique of detecting the time when an arc fash occurs is a new quantitative evaluation method, which is expected to be widely used to prevent negligent accidents in the electrical feld as well as being able to determine in real time whether there is a harmful arc.
