Background and Aim: Alpha-fetoprotein (AFP), Lens culinaris agglutinin- reactive fraction of AFP (AFP-L3), and protein induced by vitamin K absence or antagonist-II (PIVKA-II) are widely used as tumor markers for diagnosis of hepatocellular carcinoma. This study aimed to perform a head-to-head comparison of the diagnostic value of AFP, AFP-L3 and PIVKA-II as single or in combination to find the best biomarker or panel. Methods: Seventy nine patients with newly diagnosed HCC and 77 non-HCC liver cirrhosis control patients were enrolled. Plasma AFP, AFP-L3, PIVKA-II levels were measured by ELISA, and receiver operating characteristic curve analyses were performed for each biomarker and for every combination of the markers. Results: Among three single biomarkers, AFP showed the highest area under the curve (0.751, 95 % confidence interval0.683-0.818). The diagnostic performance (sensitivity/specificity) of each single biomarker was 68.35 %/81.82 % with AFP (AFP>40 ng/mL), 70.89 %/70.13 % with PIVKA-II (PIVKA-II > 40 mAU/mL), and 50.63 %/83.12 % with AFP-L3 (AFP-L3>10 %), respectively. In case of combination of the biomarkers, ‘PIVKA-II > 40 mAU/mLand AFP>40 ng/mL’ showed the highest AUC (0.765, 95 % CI:0.708-0.823), and its sensitivity and specificity were 55.70 % and 97.40 % each. Compared with combination of ‘PIVKA-II > 40mAU/mL and AFP>40 ng/mL and AFP-L3>10 %’ (sensitivity 40.51 %, specificity 98.70 %, AUC 0.696), it had better sensitivity (55.70 % vs. 40.51 %, p=0.001) and AUC (0.765 vs. 0.696, p=0.001) and similar specificity (97.40 %vs. 98.70 %, p=0.001). The combination of ‘PIVKA-II > 40 mAU/mLor AFP>40 ng/mL or AFP-L3>10 %’ showed the highest sensitivity (sensitivity 84.81 %, specificity 51.95 %, AUC 0.684), but compared to combination of ‘PIVKA-II > 40 mAU/mLor AFP>40 ng/mL’(sensitivity 83.54 %, specificity 54.55 %, AUC 0.690), there is no statistical significant difference ( p=0.549). Conclusions: AFP showed the best diagnostic performance as a single biomarker for diagnosing HCC. Furthermore, when AFP combined with PIVKA-II, its diagnostic value could be maximized. However, AFP-L3 did not contribute to distinguish between HCC and non-HCC liver cirrhosis.

<P>Background and objectives The notion that oral intestinal sorbent AST-120 slows renal disease progression has not been evaluated thoroughly. In this study, we investigated the long-term effect of AST-120 on renal disease progression (doubling of serum creatinine, eGFR decrease >50%, or initiation of RRT) in patients with advanced CKD. Design, setting, participants, & measurements We prospectively recruited 579 patients (CKD stage 3 or 4) from 11 medical centers in Korea from March 4, 2009 to August 31, 2010 and randomized them into an AST-120 arm and a control arm. Patients in the AST-120 arm were given 6 g AST-120 in three divided doses per day, and those in the control arm received only standard conventional treatment (open-label design) for 36 months or until the occurrence of primary outcomes. Results Levels of serum and urine indoxyl sulfate and beta 2-microglobulin decreased throughout the study period in both treatment arms; however, there was not a significant difference in change in uremic toxins in the AST-120 and control arms. The two arms were not different in the occurrence of composite primary outcomes (100 events in 272 individuals in the AST-120 arm and 100 events in 266 individuals in the control arm; hazard ratio, 1.12; 95% confidence interval, 0.85 to 1.48; log-rank P=0.45). The decline in eGFR and change in proteinuria were similar in the two treatment arms over time (Prandomization-time=0.64 and Prandomization-time=0.16, respectively). There was no difference in mortality (nine deaths in the AST-120 arm and 11 deaths in the control arm; log-rank P=0.73) or unplanned hospitalizations (102 in the AST-120 arm and 109 in the control arm; log-rank P=0.76) in the two treatment arms. There was no significant difference of the health-related quality of life score between the two arms. Conclusions Long-term use of AST-120 added to standard treatment did not change renal disease progression, proteinuria, mortality, and health-related quality of life in patients with advanced renal dysfunction.</P>

<P>Conclusions: A large majority of Korean hypertensive CKD patients had uncontrolled BP and abnormal dipping patterns. Furthermore, better BP control and dipping status changes were associated with better renal function and proteinuria, and also less cardio-cerebrovascular damages.</P>

<P><B>Abstract</B></P> <P>The effect of Korean red ginseng (KRG; <I>Panax ginseng</I> Meyer) against doxorubicin (DOX)-induced testicular inflammation was investigated. Male Sprague-Dawley rats were treated with DOX (1 mg mL<SUP>−1</SUP> kg<SUP>−1</SUP> week<SUP>−1</SUP>; intra-peritoneally) for 8 weeks, and KRG water extract (EX1; 100 and EX2; 200 mg/kg/day; orally) was administered for 9 weeks starting one week before DOX exposure. The expression levels related to spermatogenesis, inflammatory and autophagy markers were evaluated using western blotting and reverse transcriptase polymerase chain reaction. DOX-induced a significant (<I>p</I> < .05) alteration in the expression of antioxidation spermatogenesis and sex hormone receptors genes when compared with control groups, which were attenuated with KRG-WE treatment significantly (<I>p</I> < .05 ∼ <I>p</I> < .01). KRG-WE also ameliorated the DOX-induced alteration in pro-inflammatory cytokines via the mitogen-activated protein kinases/nuclear factor kappa-B pathway and autophagy in the testes of rats. In conclusion, KRG might be used as a functional food for prevention of chemotherapy-induced testicular inflammation.</P> <P><B>Highlights</B></P> <P> <UL> <LI> KRG attenuated mRNA expression of spermatogenesis in DOX-induced rat testis. </LI> <LI> KRG inhibited the DOX-induced inflammatory activation in rat testis. </LI> <LI> KRG ameliorated the changes in DOX-induced mTORC expression in rat testis. </LI> </UL> </P>

The valve for high-pressure cleaner used to clean engine blocks after machining process consists of one set of 2-way vales, which play a role of change of water flow direction. In the 2-way valve, there are lots of surging effects occurred in operation at high pressure condition, so that the 3-way reversing valve can be used to reduce the surging. Since the 3-way reversing valve provides a rapid change of water flow direction, the surging is reduced significantly. However, a complicated internal trim shape of the 3-way reversing valve causes frequent cavitations which occur at sudden pressure drop and velocity change. In this study, the design of internal trim shape of the 3-way reversing valve was carried out via shape optimization considering minimization of the cavitation. (RIC 2012-0592)
