고엽제 환자 2세들의 질병현황과 고엽제와의 연관성에 관한 고찰

N/A Background : This is the studies about If the soldiers expose in the paraquat, how much have it influenced on themselves who participated in the Vietnam War and their second generation? and this is also researched about the factors that How differents do the symptoms according to their health conditions and a class of diseases?. First of all, We divided them into 3 groups as exposure duration and exposure degree of the paraquat and We gave marks against each steps, too. (< point 10: low group, point 11-19: middle group, > point 20: high group). We've focused in Does the high score really involves with their descendants who suffers from paraquat? as direct damages men during from June 1965 to February 1971 (72 persons) and their descendants (266 persons) who are in the Pusan veterans hospital and outpatients. Methods : It is completed by direct interview, telephone interview, army records, army history, and medical records with them. We've tried to minify sample bias as analyzing their information. We could contact only a few people among living in Pusan or Kyongnam province. Generally, someone including persons who couldn't get a damaging proofs from paraquat hardly joined us and the others strongly rejected the interview for this research. Results : Among the 72 participating soldiers in the Vietnam war, average age of patient is 53 years old (the youngest: 46 years old, the oldest: 64 years old), average of exposure score is 16 point (minimum: 1.9 point, maximum: 31.9 point), average of pregnant frequency is 5.2 persons, and average degree of smoking is 14.382 single cigarette (minimum: 4 single cigarette, maximum: 60 single cigarette). The second generation is suffering from abortion, skin disorder, still birth, congenital anomalies, weakness, visual disturbance, peripheral neuropathy in frequency. Conclusion : There's no relation between exposure score of paraquat and diseases of the descendants. (Korean J Med 58:221-226, 2000)

Clinical Practice Guidelines for the Endoscopic Management of Peripancreatic Fluid Collections

Endoscopic ultrasonography-guided intervention has gradually become a standard treatment for peripancreatic fluid collections (PFCs). However, it is difficult to popularize the procedure in Korea because of restrictions on insurance claims regarding the use of endoscopic accessories, as well as the lack of standardized Korean clinical practice guidelines. The Korean Society of Gastrointestinal Endoscopy appointed a Task Force to develop medical guidelines by referring to the manual for clinical practice guidelines development prepared by the National Evidence-Based Healthcare Collaborating Agency. Previous studies on PFCs were searched, and certain studies were selected with the help of experts. Then, a set of key questions was selected, and treatment guidelines were systematically reviewed. Answers to these questions and recommendations were selected via peer review. This guideline discusses endoscopic management of PFCs and makes recommendations on Indications for the procedure, pre-procedural preparations, optimal approach for drainage, procedural considerations (e.g., types of stent, advantages and disadvantages of plastic and metal stents, and accessories), adverse events of endoscopic intervention, and procedural quality issues. This guideline was reviewed by external experts and suggests best practices recommended based on the evidence available at the time of preparation. This will be revised as necessary to address advances and changes in technology and evidence obtained in clinical practice and future studies. (Gut Liver 2021;15:677-693)

급성출혈성 위염과 동반된 분선충증 1 예

A 69-year-old male was admitted to the neurosurgery department for traumatic intracra-nial hemorrhage in both frontal lobes. After 2 months, he complained of epigastric dis-comfort, nausea, vomiting, and loose stools. The gastric endoscopic examination found acute hemorrhagic gastritis and there were rhabditoid nematode larvae in the gastric fluid and biopsy sections. The filariform larvae of Strongyloides sp. were discovered from a fecal culture. The patient was treated with albendazole (200 mg, po bid, for 4 weeks). The epigastric discomfort disappeared and endoscopic findings improved after treatment.

안검 피지선암의 임상적 고찰

Malignant tumors of the eyelid pose a serious threat because of their proximity to the globe, brain & paranasal sinuses. Three types of carcinomas account for the vast majority of the malignant eyelid tumors: basal cell carcinoma, squamous cell carcinoma, and sebaceous gland carcinoma. In these malignancies, basal cell carcinoma is the most common. However, sebaceous gland carcinoma has a high recurrence rate and a moderately high 5-year mortality rate because of late diagnosis and incomplete surgical removal. This tumor may be clinically misdiagnosed frequently as a chronic unilateral conjunctivitis, chalazion, basal cell carcinoma, or squamous cell carcinoma in initial biopsy. Suspicious lid lesions should be managed with incisional biopsy initially. If they are positive for sebaceous gland carcinoma, wide excision and frozen section monitoring of surgical margin is the treatment of choice. The final diagnosis should be confirmed by special stain for lipid. We experienced 12 cases of malignant eyelid tumors including 3 cases of sebaceous gland carcinoma. In cases of sebaceous gland carcinoma, wide surgical excision with frozen section monitoring was performed and followed by eyelid reconstruction with intact eyelid tissue remained and adjuvant radiotheraphy. Guidelines for management are discussed.

PEGylated apoptotic protein-loaded PLGA microspheres for cancer therapy

<P>The aim of the current study was to investigate the antitumor potential of poly (D,L-lactic-<I>co</I>-glycolic acid) microspheres (PLGA MSs) containing polyethylene glycol (PEG)-conjugated (PEGylated) tumor necrosis factor–related apoptosis-inducing ligand (PEG-TRAIL). PEG-TRAIL PLGA MSs were prepared by using a water-in-oil-in-water double-emulsion method, and the apoptotic activities of supernatants released from the PLGA MSs at days 1, 3, and 7 were examined. The antitumor effect caused by PEG-TRAIL PLGA MSs was evaluated in pancreatic Mia Paca-2 cell-xenografted mice. PEG-TRAIL PLGA MS was found to be spherical and 14.4±1.06 μm in size, and its encapsulation efficiency was significantly greater than that of TRAIL MS (85.7%±4.1% vs 43.3%±10.9%, respectively). The PLGA MS gradually released PEG-TRAIL for 14 days, and the released PEG-TRAIL was shown to have clear apoptotic activity in Mia Paca-2 cells, whereas TRAIL released after 1 day had a negligible activity. Finally, PEG-TRAIL PLGA MS displayed remarkably greater antitumor efficacy than blank or TRAIL PLGA MS in Mia Paca-2 cell-xenografted mice in terms of tumor volume and weight, apparently due to increased stability and well-retained apoptotic activity of PEG-TRAIL in PLGA MS. We believe that this PLGA MS system, combined with PEG-TRAIL, should be considered a promising candidate for treating pancreatic cancer.</P>

Human Serum Albumin-TRAIL Conjugate for the Treatment of Rheumatoid Arthritis

<P>Albumin conjugation is viewed as an effective means of protracting short in vivo lifespans of proteins and targeting rheumatoid arthritis (RA). In this study, we present a human serum albumin (HSA) conjugate linked with tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) via a bifunctional PEG derivative (HSA-TRAIL). Prepared HSA-TRAIL was found to have a larger molecular size (∼240 kDa, 15.4 nm) than TRAIL (∼66 kDa, 6.2 nm), and its bioactivity (apoptosis, cytotoxicity, and antiproliferation) was well preserved in Mia Paca-2 cells and mouse splenocytes. The enhanced therapeutic efficacy of HSA-TRAIL was demonstrated in collagen-induced arthritis (CIA) mice. The incidence and clinical scores, expressed as degree of erythema and swelling in HSA-TRAIL-treated mice, were remarkably lower than those of TRAIL-treated mice. The serum levels of pro-inflammatory cytokines IFN-γ, TNF-α, IL-1β, and IL-2 in HSA-TRAIL-treated mice were significantly lower than those of TRAIL-treated mice. Furthermore, HSA-TRAIL accumulated in the hind paws of CIA mice, not in naïve TRAIL mice. Pharmacokinetic profiles of HSA-TRAIL were greatly improved in comparison to those of TRAIL (AUC<SUB>inf</SUB>: 844.1 ± 130.0 vs 36.0 ± 1.2 ng·h/mL; <I>t</I><SUB>1/2</SUB>: 6.20 ± 0.72 vs 0.23 ± 0.01 h, respectively). The HSA-TRAIL conjugate, which presents clear advantages of targeting RA and long systemic circulation by HSA and unique anti-inflammatory efficacy by TRAIL, has potential as a novel treatment for rheumatoid arthritis.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/bcches/2014/bcches.2014.25.issue-12/bc500427g/production/images/medium/bc-2014-00427g_0011.gif'></P>

원주천의 어류군집 동태

1998년 3월부터 1999년 2월까지 원주천에서 어류상과 어류군집 동태를 조사한 결과는 다음과 같다. 채집된 어류는 총 8과 24종이었으며 이중 한국특산종이 8종으로 전체종의 33.3%를 차지하였다. 원주천 전 조사지점에서 출현한 어종 중 Moroco oxycephalus(30.7%), Zacco platypus(29.6%), Carassius auratus(10%), Pseudogobio esocinus(9.4%), Rhinogobius brunneus(3.6%)가 우점하였다. Hemibarbus longirostris, Pungtungia herzi, Squalidus japonicus, Zacco temmincki, Cobitis rotundicaudata, Silurus asotus, Silurus microdorsalis 및 Micropterus salmoides 등은 상대풍부도가 0.1% 이하인 희소종 이었다. 각 조사지점별 우점종은 M. oxycephalus(St.1,2,3,5,6), Z. platypus(St.4), C. auratusa(St.7) 등 이었다. 군집분석 결과 St.3이 가장 다양한 상태를 나타내었다. Coreoleuciscus splendidus와 Coreopera herzi는 본 조사에서 출현하지 않았고 Microphysogobio yaluensis, Liobagrus andersoni, Cottus poecilopus, R.brunneus등은 급격한 감소 추세를 나타내었다. 증가 추세에 있는 대표적인 종은 C. carpio, C. auratus, P. esocinus, M. oxycephalus, Z. playypus 등이다.

Traditional maintenance planning is based on a constant maintenance interval for equipment life. In order to consider economic aspect for time based preventive maintenance, preventive maintenance is desirable to be scheduled by RCM(Reliability-Centered Maintenance) evaluation. The main objective of RCM is to reduce the maintenance cost, by focusing on the most important functions of the system and avoiding or removing maintenance actions that are not strictly necessary. So, Markov state model is utilized considering stochastic state in RCM. In this paper, a Markov state model which can be used for scheduling and optimization of maintenance is presented. The deterioration process of system condition is modeled by the stepwise Markov model in detail. Also, because the system is not continuously monitored, the inspection is considered. In case study, simulation results about RCM will be shown using the real historical data of combustion turbine generating unit in Korean power systems.
