HEALTH DELIVERY REFORM IN NEW ZEALAND: WHAT DOES IT MEAN TO POLICY ACTORS?

Purpose: To discover the experiences of primary health managers who have dealt with frequent changes in the health delivery framework. The changes have sometimes moved in contradictory directions. The effects of such changes on health managers have not been properly examined. A better understanding of this phenomenon would allow policy makers to encourage managerial commitment to emerging policies. Design/Methodology/Approach: A critical hermeneutic approach is employed to interpret evidence from government policy documents, and semi-structured interviews with health managers. Findings: Reforms have brought constant changes in public health governance, as well as its structure. Although some changes were major, the need for the reforms has generally been well accepted by the health policy actors; they have believed the reforms necessary to improve the efficiencies of the health systems. However, in terms of policy implementation, the health policy actors have expressed their bewilderment and concern over the policy changes and the way in which many of the reforms were implemented. Discussions: Although reforms were expected to bring about improvement to the quality of public health and government financial statements, in many instances the policy actors were caught up in the complexity of health structure and policy; this prevented the full potential benefits from materialising.
E-Health Grid Network Topology Based on Province in Indonesia

Information and Communication Technology (ICT) has enhanced human life including in health care. Consequently, professional workers in the field of health services are required to improve productivity, accuracy, efficiency and service. The use of ICT for health purposes is known as e-Health. The development of e-Health can use the grid technology for resource sharing and interoperability. The technology can process a very large data for parallel computation. It can also be used to facilitate health practitioners and researchers in the field of health, including to find a new virus, discovery new drugs, disease, an image of organs and to determine the actions for a patient. As an archipelago country where many health services centers distributed in the country, Indonesia needs an appropriate model of e-Health Grid. AS preliminary study, this paper proposes an e-Health Grid network topology based on the province in Indonesia.
Analysis of Indonesia e-Government Grid Services Simulation Based on Population

As the fourth most populous country in the world, Indonesia has many challenges in e-Government development because of its many scattered island. The country needs Information and Communication Technology for communication and fast access to information to create good governance and clean government. Grid technology could be a solution for resource sharing and interoperability of e- Government systems. We have proposed the Indonesia e-Government Grid service topology model in which is an adjustment based on the framework of existing e-Government and also the form of government in the country. We have designed and simulated the topology of Indonesian e-Government Grid services based on function group from e-Government application solution map to connect the ministry/agency/department /institution and also based on the province and population in the country. In this paper we simulate and analyze the result of the e-Government services topology based on population using Friedman Test. The results shows that scenario involves three types of routers that are configured hierarchically which consist of leaf routers, edge routers and core routers using Self-Clocked Fair Queuing (SCFQ) scheduling algorithm give the best results. The setting of time delay make the waiting time for packets in Grid simulation queue become shorter.
