다한증 환자에서 클립을 이용한 교감신경 교통가지 차단술 사체 연구 및 임상적용

배경: 다한증 환자의 가장 효과적인 치료방법은 흉강경 최소절개를 이용한 교감신경 차단술로 알려져 있다. 하지만 수술 후 발생하는 보상성 다한증과 무한증이 수술의 만족도를 감소시키는 중요한 요인이므로 환자에게 적극적인 수술 적응을 하기가 어렵다. 합병증을 줄이기 위한 방법으로 교감신경의 선택적인 교통가지 차단술을 시행하였으나 시술의 어려움과 혈관 손상으로 인한 출혈의 위험성으로 보편화되지 못하였다. 본원에서는 사체를 이용하여 새로운 수술기법을 연구하고 이를 임상에 적용하여 좋은 결과를 얻었기에 보고한다. 대상 및 방법: 포르말린 처리를 하기 전 사체 3구를 대상으로 실험하였다. 앙와위에서 전흉부를 모두 절제한 후 양측 흉부 교감신경을 노출시켰다. 교감신경의 분지와 교통가지의 위치를 확인하였고 2번째와 3번째 교통가지에 클립을 이용한 차단술을 시행하였다. 교통가지 차단술 후 교감신경절을 포함한 교감신경을 절제하여 교통가지의 차단 정도를 관찰하였다. 임상적용은 25명의 환자에서 시행하였으며 평균 연령은 21세였다. 수술은 전신마취와 양측 폐 환기를 하고 반 좌위 자세에서 시행하였다. 4번째 늑간에 2 mm 흉강경을 삽입하였고 겨드랑이에 5 mm 트로카를 삽입하고 내시경용 클립을 이용하여 2번째와 3번째 교감신경절 교통가지를 차단하였다. 수술 후 전화 설문 조사를 통하여 합병증의 유무, 만족도, 보상성 다한증의 발생 정도를 관찰하였다. 결과: 사체 3구에서 모두 교감신경 교통가지의 차단을 완벽하게 시행할 수 있었다. 임상환자에서 모두 수술로 증세의 호전이 있었으며 기존의 교통가지 절제술에 비하여 수술 시간을 단축할 수 있었고 수술로 인한 합병증은 발생하지 않았다. 수술 후 수부 다한증은 모든 환자에서 호전되었으나 4명의 환자는 수술의 만족도가 감소하였다. 보상성 다한증은 15명의 환자(60%)에서 중증 이상으로 발생하였고 6명의 환자는 없거나 잘 느끼지 못하는 정도로 발생하였다. 미각다한증은 2명의 환자에서 발생하였으나 정도는 심하지 않았다. 결론: 본 연구의 수술방법은 2개의 피부 절개와 하모닉 스칼펠, 클립을 사용하여 쉽게 교감신경의 교통가지를 차단할 수 있었고 교통가지 절제술에 비하여 수술시간 및 출혈의 위험성을 감소시킬 수 있었다. 또한 교감신경 절제술에 비하여 비슷한 정도의 수술 성공률을 보이면서 보상성 다한증의 정도나 발생 빈도를 줄일 수 있어 우수한 수술 방법이라 생각한다.

Government R&D Support for SMEs: Policy Effects and Improvement Measures

Government R&D grants for SMEs have risen to three trillion Korean won a year, placing Korea second among OECD nations. Indeed, analysis results have revealed that government support has not only expanded corporate R&D investment and the registration of intellectual property rights but has also increased investment in tangible and human assets and marketing. However, value added, sales and operating profit have lacked improvement owing to an ineffective recipient selection system that relies solely on qualitative assessments by technology experts. Nevertheless, if a predictive model is properly applied to the system, the causal effect on value added could increase by more than two fold. Accordingly, it is important to focus on economic performance rather than technical achievements to develop such a model.

Expression of Luteinizing Hormone (LH) Subunit Genes in the Rat Ovary

흰쥐 난소에서의 Luteinizing hormonoe (LH) subunit 유전자 발현과 LH polypeptide의 존재를 조사하였다. 이를 위해 LH subunit들에 대한 역전사 중합효소연쇄반응 (RT-PCR)을 시행하고, 난소내 LH 함량을 방사면역측정법으로 정량하였다. 뇌하수체와 정소에서 공통적으로 존재하는 LH-$\beta$ subunit$(LH-\beta})$의 exon에 해당되는 primer를 사용하여 RT-PCR을 시행한 결과 흰쥐 난소에서도 뇌하수체, 정소와 같이 306 bp band가 확인되었고, 정소특이적인 exon에 해당되는 primer를 사용한 결과 정소와 난소에서 예상대로 428 bp band가 검출되었다. 또한 LH, FSH, TSH 그리고 hCG에서 공통적으로 발현되는 common $\alpha$-subunit $(C_\alpha)$의 전사물질도 PCRdml 의해 증폭되었다. 방사면역측정법에서는 LH standard curve와 난소추출물을 사용한 curve가 동일하게 sigmoid 형태를 보임으로서 흰쥐 난소내에 immunoreactive LH가 존재함이 증명되었다. 인위적으로 성적인 성숙을 유도한 PMSG 주사 동물에서 혈중 LH 수준은 주사 후 48시간에 preovulatory LH surge와 유사한 최고 수준을 나타냈으나, 난소내 LH 함량의 경우 주사 24시간 후부터 급격히 감소하여 주사 48, 72시간군까지도 낮은 수준이 유지되었다. 이 결과는 흰쥐 난소의 LH가 생리적으로 조절되고 그 조절방식이 뇌하수체에서와는 다를 가능성을 시사하는 것이다. 본 연구는 흰쥐의 난소에서 LH가 유전자가 발현됨을 최초로 보고한 것이며, LH의 경우 내분비적 경로 (endocrine; 뇌하수체로 부터의 LH)외에도 국부적 경로 (autocrine이나 paracrine; 난소내에서 합성되는 LH)를 통해 난소의 생리와 기능 조절을 담당함을 시사한다. The present study was performed to analyze the expression of LH genes in the rat ovary. Expression of LH subunit genes in the rat ovary was demonstrated by amplification of ovarian RNA by RT-PCR. The ovarian $LH_\beta$ transcripts contained at least two parts of the published cDNA structure, the pituitary exons 1, 2 and 3 and the part of testicular ex on 1 in the major trancripts form in rat testis. Using RIA, significant amount of LH-like molecules were detected in crude ovarian extracts, and the competition curves with increasing amount of tissue extracts were parallel with those of standard peptide, indicating that the ovarian immunoreactive LH-like material is similar to authentic pituitary LH molecule. The administration of PMSG to immature rats resulted in a sharp decrease of the ovarian LH contents after 24 h post-injection. In conclusion, these findings demonstrate that genes for LH subunits are expressed in the rat ovary, and suggest that LH can playa central role in regulation of female reproduction with both endocrine (by pituitary LH) and auto- and/or para-crine (by ovarian LH) manner.

김석철의 작품에서 나타나는 건축적 표현 특성에 관한 연구

In this study, I want to study the characteristics of Kim Suk Chul's architectural expression, which has a lot of interest in the design and planning of the city, as well as the works of architecture, both inside and outside of Korea.10 works of representative architectural works were selected and analyzed based on materials such as drawings and photographs of architectural works. Through this, architectural characteristics of architect Kim Seok - cheol are examined and the characteristics of expression expressed in his architectural works are derived. Kim Suk Chul's architectural activities were practiced on the offices of too korean famous architects Kim Joong-up and Kim Soo-keun. In 1988, his architectural expression became more prominent when he was elected to design the Seoul Arts Center competition against his teachers and famous architects. As a result of analyzing the selected 10 works selected from the above, his architectural expressive characteristics can be classified into four categories as traditional expression, organic expression, mixed expression, and symbolic expression. Kim 's architectural tendency shows many features of morphological tendency. The analysis of Kim 's morphological tendency suggests that many studies are needed in the future.
