Nasopharyngeal carcinoma (NPC) is one of the infrequently occurring malignant tumors in childhood. It is different from the adult form of the disease by its close association with Epstein- Barr virus infection, lymphoepithelial histology, and the high incidence of locoregional advancement of the disease. However, these tumors are associated with higher cure rates as they are very sensitive to radiation. We report two cases of pediatric nasopharyngeal carcinoma with locoregional advancement.

Epiglottic abscess may result from a coalescent epiglottic infection or secondary infection of an epiglottic mucocele. The lesion has the possibility of compressing the potential airway and thus warrants rapid assessment and treatment. The authors experienced three cases of the epiglottic abscess and report them with a review of literature. (Korean J Otolaryngol 2006;49:236-40)

Background and Objectives:Patients with diabetes mellitus (DM) have suppressed immunity, and the clinical course are always more severe and may cause life-threatening complications. This article investigates the differences in clinical manifestation of deep neck infections between diabetic patients and nondiabetic patients and to take this into account for their treatment. Subjects and Method:The present study reviews the experience of 158 patients with deep neck infections from 1995 to 2004. Cases of peritonsillar abscess, superficial infection, and infections related to external neck wound and head and neck tumors were excluded. Among 158 patients, 27 patients were presented with DM. To find out the characteristics of diabetic patients, the author compared these patients with nondiabetic patients using student t-test and chi-square test. Results:Compared with the nondiabetic patients, the unique features of deep neck infections in diabetic patients were as follows:1) tendency to have Klebsiella pneumoniae as a pathogen, 2) prolonged hospitalization, and 3) higher complication rate (p<0.05). There were no significant differences in sex, duration of symptom, fever higher than 38.0℃, white blood cell count, etiology, and involved spaces between two groups (p>0.05). Conclusion:Patients with DM were found to have Klebsiella pneumoniae pathogen, higher complication rate, and longer hospitalization. Doctors should keep close observation, appropriately control the diabetes, detect life-threatening complications early, perform aggressive surgical treatment if fluctuations or complications occurs, and take into account the preponderance of Klebsiella pneumoniae when administering empirical antibiotics. This article evaluates and suggests ways to improve the deduction of charitable contribution under the Corporate Tax Law. The problems of the current system are as follows; First, there is a bias in favor of charitable contributions by corporation compared to charitable contributions by individual shareholders. Second, the agency cost problem exists where a manager decides charitable contributions of a corporation that achieves the private interests of the manager by enjoying the prestige benefits. Third, there is an awkward dichotomy between business expenses and charitable contributions. The suggestions for the remedy are as follows; First, a corporation should not be allowed to deduct charitable contributions which are not related to the corporation's business at all, instead, the deduction should go to the individuals such as shareholders, managers or employees. Second, charitable contributions which are directly or indirectly related to the corporation's business by contributing to the enhancement of present or future business should be treated as a normal business expenses or capital expenditures.
Predisposing Factors of Complicated Deep Neck Infection: An Analysis of 158 Cases

Both the introduction of antibiotics and improvements in oral hygiene have made deep neck infections occur less frequently today than in the past. Nevertheless, the complications from these infections are often life-threatening. The purpose of this article was to review the clinical findings of deep neck infections and identify the predisposing factors of these complications. The present study reviewed 158 cases of deep neck infections between the years of 1995 to 2004, 23 of which had life-threatening complications. Cases were excluded if they had peritonsillar abscesses, superficial infections, infections related to external neck wounds, or head and neck tumors. The authors used multiple linear regression and the logistic regression analysis in order to determine the clinical parameters that are associated with longer hospitalizations and complicated deep neck infections, respectively. The multiple linear regression showed that patients with a large number of involved spaces, diabetes mellitus, and complications required longer hospitalizations (p<0.05). The logistic regression showed that patients with more than two involved spaces were more likely to have complicated deep neck infections (p<0.05). Patients with odontogenic causes had negative correlation (p<0.05). We recommend that high-risk groups, such as diabetic patients and/or patients with more than two involved spaces, should be more closely monitored throughout their hospitalization.
만성 요통증 환자에 대한 록소닌 (Loxoprofen sodium)의 임상적 효과에 대한 연구

We conducted a clinical effectiveness and safety of Loxoprofen sodium, a nonsteroidal antiinflammatory drug(NSAID) for the treatment of chronic low back pain. Loxoprofen sodiun 180 mg/day(60 mg three time daily) was administered to 30 patients with chronic low back pain associated with a range of arthritic disorders, for a period up to 4 weeks, and clinical efficacy was evaluated by objective and subjective methods. 4 weeks therapy, loxoprofen showed subjective improvements of 100%, 64.7% and 63.6% in patients with mild to moderate, moderate to severe, and severe lumbar pain etc, respectively. Objective and subjective improvements were significant after only 1 week therapy in patients with mild to moderate pain and after 4 weeks, over all intensities of pain were improved. The incidence of adverse effects over this study population was extremely low, affecting only 3.3% of patients, and were mild in nature and edematous change. Loxoprofen is thus a well-tolerated NSAID in the treatment of chronic lumbar pain.
