        실시간 광유도 성대주입술의 개발과 임상적 적용

        Vocal fold injection (VFI) is widely accepted as a first line treatment in treating unilateral vocal fold paralysis and other vocal fold diseases. Although VFI is advantageous for its minimal invasiveness and efficiency, the invisibility of the needle tip remains an essential handicap in precise localization. Real-time light-guided vocal fold injection (RL-VFI) is a novel technique that was developed under the concept of performing simultaneous injection with precise placement of the needle tip under light guidance. RL-VFI has confirmed its possibility of technical implementation and the feasibility in injecting the needle from various directions through ex vivo animal studies. Further in vivo animal study has approved the safety and feasibility of the procedure when various transcutaneous approaches were applied. Currently, RL-VFI device is authorized for clinical use by the Ministry of Food and Drug Safety in South Korea and is clinically applied to patients with safe and favorable outcome. Several clinical studies are currently under process to approve the safety and the efficiency of RL-VFI. RL-VFI is expected to improve the complication rate and the functional outcome of voice. Furthermore, it will support laryngologists in overcoming the steep learning curve by its intuitive guidance.

        Low Current Driving of a Heat-assisted Magneto-optic Spatial Light Modulator Using an Amorphous DyFe Film with a Compensation Composition

        We developed a magneto-optic spatial light modulator (MOSLM), for all visible wavelengths, using amorphous rare earth-transition metal (RE-TM) film. The magneto-optical elements (pixels) were made of amorphous DyFe films with perpendicular magnetic anisotropy. The the amorphous DyFe film was determined to have 22 atom percent Dy, and the Curie temperature (Tc) was found to be 343 K. For magnetic switching in this MOSLM, heating to the Curie temperature (Tc) by using a laser radiation density of 5 mJ/cm2 and a driving current of 1 mA at all visible wavelengths were needed. In addition, we confirmed the switching speed of pixels for various laser radiation densities.

        리조트 방문객 만족속성의 유형화 - 고객만족 세(三)요인 모델의 적용 -

        리조트 방문객의 만족도 연구에서 리조트의 만족속성이 전반적 만족도에 미치는 영향은 대칭적 관계를 기대하는 연구들이 대부분이다. 하지만 최근 상품/서비스 만족 속성이 전반적 만족도에 비대칭적인 관계로 영향을 미친다는 연구결과들이 증가하고 있다. 따라서 본 연구는 리조트 방문객의 전반적 만족도에 영향을 미치는 리조트 방문의만족속성을 고객만족 세요인 모델을 이용하여 분류하였다. 본 연구의 분석결과 세요인중 기본요인에는 ‘접근성’, 실행요인은 ‘숙박시설속성’, ‘자연자원속성’으로 분류되었고, 감동요인에 속하는 만족속성은 없는 것으로 분석되었다. 또한 리조트 방문의 만족속성중 전반적인 만족도를 증가시키는 속성요인과 불만족을 유발하는 속성요인을 파악하였다. 이러한 연구결과는 이론적인 측면 뿐 만 아니라 실무적인 측면에서도 중요한 시사점을 제시한다. In most studies on resort visitor’s satisfaction with resort , the impact of satisfaction attributes on overall satisfaction is expected to be symmetrical. However, according to recent studies, the evaluation attributes of a product or a service asymmetrically affect overall satisfaction. Therefore, based on recent studies, this study offers a categorization of satisfaction attributes of resort visitors by applying the three-factor model of customer satisfaction. According to the results, “accessibility attribute” is identified as basic factors; “accommodation facilities attribute,” and “natural resource attribute” are performance factors. And it is analyzed that there is no satisfaction attributes belonging to the excitement factors. It is important for resort managers and operators to recognize which satisfaction attribute leads to an increase in overall satisfaction and a decrease in overall dissatisfaction. This study not only makes theoretical contributions to the study of resort but also offers practical and useful suggestions for resort management.

        난시와 시력장애를 동반한 소아에 대한 침치료 1례 보고

        Objectives : To estimate clinical effect of acupuncture treatment for astigmatism and visual impairment. Methods : Acupuncture treatment was performed on a young child, diagnosed as astigmatism and visual impairment from 23th May 2008 through 23th July 2008. Visual acuity and astigmatism were measured before and after treatment. Results : Visual acuity and astigmatism were improved after acupuncture treatment. Conclusions : This case demonstrates that acupuncture treatment has clinical possibilities for improving astigmatism and visual impairment and future studies will be required.

        양측 폐상엽 진균구에 의한 객혈의 순차적 수술 치험 1례

        저자들은 양측 폐상엽 국균증으로부터 유발된 객혈 1 례를 경험하였다. 환자는 66세 남자로 간헐적으로 재발된 객혈을 가진 환자로 다량의 객혈로 전원되어 X-ray 검사상 양측 상엽에 공동과 진균구를 관찰되었다. 환자는 불량한 전신상태와 호흡기능을 가져 술후 합병증을 최대한 예방하기 위해 정중 흉골절개에 의한 동시 절제보다는 순차적 절제를 계획해 우측병변을 먼저 절제하고 후에 좌측병변을 절제하였다. 조직 검사상 폐결핵성 병변은 없는 폐국균증으로 확진되고 큰 합병증 없이 회복되어 외래 추적치료중이며 객혈이나 진균증의 재발소견은 없었다. We experienced a case of hemoptysis caused by bilateral upper lobe aspergilloma. He was 66 years old and had a history of old unclear pulmonary tuberculosis with irregular medication 20 years ago and intermittent hemoptysis for several years. In x-ray study, there was a bilateral upper lobe aspergilloma with cavity. He received bilateral wedge resection through thoracotomy with some interval to reduce postoperative complications. We selected the priorty of operation through the bronchoscope in the operation room. Both sides had the same pathology of aspergilloma and he was discharged after an uneventful postoperative course.

        The cautious blasting works had been used with emulsion explosion electric M /S delay caps. Drill depth was from 3m to 6m with Crawler Drill 70mm on the calcalious sand stone (soft-moderate-semi hard Rock) . The total numbers of feet blast were 88. Scale distance were induces 15.52-60.32. It was applied to Propagation Law in blasting vibration as follows .Propagtion Law in Blasting Vibration V=k(D/W/sup b/)/sup n/ where V : Peak partical velocity(cm/sec) D : Distance between explosion and recording sites(m) W ; Maximum Charge per delay -period of eight milliseconds or more(Kg) K : Ground transmission constant, empirically determind on the Rocks, Explosive and drilling pattern ets. b : Charge exponents n : Reduced exponents Where the quantity D/W/sup b/ is known as the Scale distance. Above equation is worked by the U.S Bureau of Mines to determine peak particle velocity. The propagation Law can be catagrorized in three groups. Cabic root Scaling charge per delay Square root Scaling of charge per delay Site-specific Scaling of charge delay Charge and reduction exponents carried out by multiple regressional analysis. It's divided into under loom and over loom distance because the frequency is varified by the distance from blast site. Empirical equation of cautious blasting vibration is as follows. Over 30m--under 100m----V=41(D/ W)/sup -1.41/-----A Over l00m---------V=121(D/ W)/sup -1.56/-----B K value on the above equation has to be more specified for furthur understand about the effect of explosives. Rock strength, And Drilling pattern on the vibration levels, it is necessary to carry out more tests.

        On the Seoul Metropolitan Subway Construction of No. 3, 4 Line, the total length is 57 Km and it is now undergoing almost 55% progress. The working method is classified into Open Cut of 70% and the rest of 30% tunnelling method in the 48 job site. Above tunnelling method is execute by American Steel Support System and the rest of 10 job site carried out by New Austria Tunnelling Method. This paper describes Blasting Standardizations works on the above Tunnelling ' Open Cut Method under big slogan, first safety, second execution. As a superintendent, I strived standardization of works with Better powder, Better Drills ' Better Pattern. Geological structure of Seoul area is composed by Jurassic Granite and also the above rockgroup are over burden by Alluviums as a Unconformity. First of all, I carried out the standard amount of powder and burden through experimental standard blasting by each powder as following Blasting works in the subway construction is surrounding shop Building, under pass the city river and also under pass highest building basement floor. I made allowable Blasting Vibration Value by West-Germany Vornorm DIN 4150, Teil 3 and should measure each blasting works as fellows all of powder is used basically Low-Gravity and Low Velocity such as Slurry, Ammonium Nitrate ' Finex I, II. for Smooth Blasting Instead of Gelatin Dynamite. Electric Detonation Cap is used basically M/S Delay Cup instead of Electric delay ' Simultaneous cap. I applied following formula V=KW3/4 $D^{-2}$ V=Particle Velocity (Cm/sec) K=Ginh Huh's Value W=Delay Charge (Kg) D=Distance(m) In the Open Cut, within 1m distance from H-pile I made to use the Concrete breaker, as following V=7W/$^{0.5}$V/$^{-1.75}$ On the Concentrate Building area, I advise to use Light class drill ø36m Bit and advance 1.1m per round blasting the three boom jumbo drill over ø45mm used only suburb of city.e Light class drill ø36m Bit and advance 1.1m per round blasting the three boom jumbo drill over ø45mm used only suburb of city.
