This case report describes a new method of pain management intervention: percutaneous foraminotomy using the Claudicare system (Seawon Meditech, Korea). In this case, a 77-year-old Asian man visited the hospital with motor weakness in his left foot. He was diagnosed with L4–5 grade three foraminal stenosis using Magnetic Resonance Imaging on both sides. A left L4–5 foraminal decompression was performed using percutaneous foraminotomy. The patient revisited the hospital after 17 months because the same symptoms recurred in his right foot. We observed that the symptoms on the left foot had disappeared completely. We confirmed the lesion on the right side and the postoperative change on the left side on the magnetic resonance imaging (MRI) image. Both the pre- and postoperative MRI images were compared by measuring the dimensions of the foraminal area (28.12 mm2 vs. 38.58 mm2, repectively). T1W images showed signs of increased epidural soft tissue after percutaneous foraminotomy.

We present a three-dimensional (3D) particle focusing channel using the positive dielectrophoresis (pDEP) guided by a dielectric structure between two planar electrodes. The dielectric structure between two planar electrodes induces the maximum electric field at the center of the microchannel, and particles are focused to the center of the microchannel by pDEP as they flow from the single sample injection port. Compared to the previous 3D particle focusing methods, the present device achieves the simple and effective particle focusing function without any additional fluidic ports and top electrodes. In the experimental study, approximately 90 % focusing efficiency were achieved within the focusing length of 2㎜, on both x-z plane (top-view) and y-z plane (side-view) for 2㎛-diameter polystyrene (PS) bead at the applied voltage over 15 Vp-p (square wave) and at the flow rate below 0.01 ㎕/min. The present 3D particle focusing channel results in a simple particle focusing method suitable for use in integrated microbiochemical analysis system.

Reinforced endotracheal tubes (ETTs) are designed to resist kinking or compression. However, these have a potential risk of being obstructed or severed by a patient’s bite. We report a case in which a reinforced ETT was severed by tube-bite while the patient was in the prone position during an intensive care unit stay. Bronchoscopic evaluation showed that the severed distal part of the tube had lodged in the patient’s right main bronchus, and it had to be surgically removed. The patency of reinforced ETTs should be carefully monitored in patients intubated in the prone position.
